Baseline multiparametric MRI combined with clinical factors for predicting pathological complete response of breast cancer after neoadjuvant chemotherapy.

PURPOSE: Early prediction of response to neoadjuvant chemotherapy (NAC) may facilitate treatment stratification in breast cancer. This study aimed to evaluate the association of baseline virtual MR elastography (vMRE) and intravoxel incoherent motion diffusion-weighted imaging (IVIM-DWI) parameters with pathological complete response (pCR) and assess their incremental value beyond pretreatment clinical factors. METHODS: Consecutive patients with histologically confirmed breast cancer who underwent baseline multiparametric MRI before NAC were retrospectively evaluated. vMRE parameters (shifted

Dynamic Changes in NPAR and the CALLY Score for Predicting Pathological Complete Response in Breast Cancer: A Simple Combined Clinical Score.

BACKGROUND: Pathological complete response (pCR) after neoadjuvant therapy is closely associated with improved survival in breast cancer, particularly in aggressive subtypes. In daily practice, treatment response is mainly evaluated using imaging modalities such as magnetic resonance imaging and ultrasonography; however, these methods may not always accurately reflect residual disease. Easily accessible and reliable laboratory-based markers are still lacking. Because systemic inflammation and nutritional status are closely linked to tumor biology, dynamic changes in composite indices may bette

Predictive value of rectal MRI variables for pathological complete response in locally advanced rectal cancer following neoadjuvant chemoradiotherapy.

PURPOSE: To evaluate the predictive value of MRI-determined variables for pathological complete response (pCR) in locally advanced rectal cancer (LARC) patients following neoadjuvant chemoradiotherapy (NCRT). METHODS: Clinical data were collected from patients who received NCRT between January 2019 and 2022. Patients with rectal adenocarcinoma, cT3-4N0, or TanyN1-2 were included. pCR was defined pT0N0. Patients were divided into pCR and non-pCR group. Logistic regression analysis was performed to identify factors associated with pCR. A nomogram model was constructed to validate its predictive

Structured Magnetic Resonance Imaging Assessment Improves Diagnosis of Pathological Complete Response in Rectal Cancer after Neoadjuvant Chemoradiation.

INTRODUCTION: Precise prediction of pathological complete response (pCR) following neoadjuvant chemoradiotherapy (nCRT) in rectal cancer may identify candidates for non-operative management. The optimal selection of diagnostic tools is, therefore, of major clinical importance. METHODS: Clinical, laboratory, endoscopic, and radiological data of patients with rectal cancer treated with nCRT and surgery at an academic medical center from 2010 to 2020 were retrospectively collected. Pre- and post-nCRT magnetic resonance imaging (MRI) was reviewed with a structured report template and assessed by

Deep learning-based prediction of axillary pathological complete response in patients with breast cancer using longitudinal multiregional ultrasound.

BACKGROUND: Noninvasive biomarkers that capture the longitudinal multiregional tumour burden in patients with breast cancer may improve the assessment of residual nodal disease and guide axillary surgery. Additionally, a significant barrier to the clinical translation of the current data-driven deep learning model is the lack of interpretability. This study aims to develop and validate an information shared-private (iShape) model to predict axillary pathological complete response in patients with axillary lymph node (ALN)-positive breast cancer receiving neoadjuvant therapy (NAT) by learning c

Pathological Complete Response in Locally-advanced MSI-H/dMMR Sigmoid Colon Cancer Treated With Pembrolizumab and Abdominal Reconstruction.

BACKGROUND/AIM: Microsatellite instability-high (MSI-H) and mismatch repair-deficient (dMMR) colorectal cancers have shown significant responses to immune checkpoint inhibitors such as pembrolizumab, presenting novel opportunities for potentially curative strategies in cases previously deemed unresectable. CASE REPORT: A 61-year-old Japanese man presented with abdominal pain and was diagnosed with locally advanced sigmoid colon cancer with abdominal wall invasion. The tumor also involved the small intestine and had formed a fistula extending to the skin. Imaging studies revealed no distant me

Analysis of different prognoses in two hepatocellular carcinoma patients with pathological complete response after conversion therapy: a case report.

BACKGROUND: Hepatocellular carcinoma is a common lethal malignancy, with most patients diagnosed at an advanced stage, precluding surgical resection. Conversion therapy combining locoregional and systemic treatments has emerged as an effective approach to downstage tumors and facilitate curative-intent surgery. However, the prognosis after conversion therapy varies significantly among patients. Case reports can reveal differences in clinical outcomes following conversion therapy and their underlying causes. CASE PRESENTATION: Here, we report two patients of advanced hepatocellular carcinoma w

Early Brain Metastasis despite Pathological Complete Response after Radical Resection for Esophageal Cancer following Chemotherapy and Pembrolizumab.

INTRODUCTION: Brain metastasis from esophageal cancer is rare. In particular, recurrent brain metastasis following multimodal treatment, such as preoperative chemotherapy and surgical resection, is extremely uncommon. We present a case of early brain metastasis from esophageal cancer despite achieving pathological complete response (pCR). CASE PRESENTATION: A middle-aged man presented with dysphagia. Upper gastrointestinal endoscopy revealed stenosis caused by an ulceroinfiltrative tumor located in the middle of the thoracic esophagus. Targeted biopsies confirmed squamous cell carcinoma. Contr

Frequency of axillary nodal complete pathological response of breast cancer patients in neoadjuvant chemotherapy setting: A cross-sectional study.

OBJECTIVES: Pathological complete response (pCR) occurs in about 20-30% of patients undergoing systemic neoadjuvant therapy. This leads to the idea of sparing the patient the morbidity associated with axillary surgery. "Wait and watch" policy for cancers which achieve complete pathological response on neoadjuvant systemic therapy is a well-established practice in various cancers like the esophagus, rectum and larynx. This has led to organ preservation protocols being practiced worldwide for these cancers without affecting the overall survival of the patient. We believe patients undergoing a co

Esophageal Neuroendocrine Carcinoma With Pathological Complete Response After Neoadjuvant Chemotherapy Followed by Robot-assisted Surgery: A Case Report.

BACKGROUND/AIM: Malignant esophageal neuroendocrine carcinoma (ENEC) is rare, and its prognosis is poor. Therefore, a standard strategy for treating ENEC remains controversial, and few reports have described a pathological complete response of ENEC to neoadjuvant chemotherapy (NAC) followed by surgery. CASE REPORT: A 74-year-old woman presented with an esophageal tumor, discovered as a result of dysphagia, that was clinically diagnosed as ENEC (small-cell type). The clinical stage was cT2N1M0 stage II (Union for International Cancer Control, eighth edition). Two cycles of carboplatin and etop

[A Case of Locally Advanced Rectal Cancer with Invasion to Other Organs in Which Pathological Complete Response Was Obtained with Neoadjuvant Chemotherapy].

A 67-year-old man was admitted with melena. A colonoscopy detected advanced rectal cancer, and a CT scan revealed invasion of the seminal vesicle and prostate. Given the wild-type RAS status of the tumor, we administered 6 courses of XELOX plus cetuximab as neoadjuvant chemotherapy. After treatment, the tumor had shrunk, and the rectum had narrowed. Later, following a diagnosis of coronary artery disease, colostomy was performed. The patient was treated for the coronary artery disease for 2 months. Following treatment, tumor progression was detected, and hence, the patient was treated with the

A pathological complete response by chemotherapy with S-1 and oxaliplatin for a locally advanced duodenal adenocarcinoma in Lynch syndrome: a case report.

BACKGROUND: Although primary duodenal adenocarcinoma (DA) is a rare malignancy representing ~ 0.5% of all gastrointestinal cancers, the incidence of DA is more frequent in Lynch syndrome. Because of its rarity, treatment strategies or optimal chemotherapeutic regimens have not been clearly defined for advanced DA. CASE PRESENTATION: A 72-year-old woman with Lynch syndrome visited our hospital with a right upper abdominal pain. Computed tomography (CT) showed wall thickness with enhancement in the second portion of the duodenum and adjacent abdominal wall, which suggested direct tumor invasion

[A Case of Complete Pathological Response of Unresectable Synchronous Colorectal Hepatic Metastasis with Icterus after FOLFOX plus Panitumumab Therapy].

A 35-year-old man was diagnosed with initially unresectable synchronous liver metastases from sigmoid colon cancer. We started FOLFOX plus panitumumab therapy. After 3courses of chemotherapy, the liver metastases became resectable. Sigmoidectomy, extended left lobectomy, and partial hepatectomy were performed. On the final histopathological analysis, all hepatic lesions were fibrotic without viable cancer cells. Nineteen months after the surgery, CT revealed isolated residual liver recurrence in segment 5. After partial hepatectomy, the patient is alive without recurrence. We report a case of

[A Case of Synchronous Liver Metastasis of Ascending Colon Cancer with Pathological Complete Response to S-1/Oxaliplatin (SOX) and Bevacizumab].

A 77-year-old man was diagnosed with ascending colon cancer with synchronous liver metastasis. Per our policy we first only performed a right hemicolectomy (pSSN2H2M0, stage IV). We then planned S-1 and oxaliplatin (SOX) plus bevacizumab (Bmab) chemotherapy as a neoadjuvant for the resection of liver metastasis. After 4 courses, enhanced CT and EOB-MRI findings showed the liver tumor had significantly decreased in size with no side effects, and we performed a partial liver resection for the S7 lesion. Postoperatively, histopathological analysis revealed only a fibrotic lesion and no cancerous
