Legionnaires' Disease Outbreak at a Long-Term Care Facility Caused by a Cooling Tower Using an Automated Disinfection System--Ohio, 2013.

On July 9, 2013, an outbreak of Legionnaires' disease (LD) was identified at Long-Term Care Facility A in central Ohio. This article describes the investigation of the outbreak and identification of the outbreak source, a cooling tower using an automated biocide delivery system. In total, 39 outbreak LD cases were identified; among these, six patients died. Water samples from a cooling tower were positive for Legionella pneumophila serogroup 1, reactive to monoclonal antibody 2, with matching sequence type to a patient isolate. An electronic control system turned off cooling tower pumps during

Testing for directed influences among neural signals using partial directed coherence.

One major challenge in neuroscience is the identification of interrelations between signals reflecting neural activity. When applying multivariate time series analysis techniques to neural signals, detection of directed relationships, which can be described in terms of Granger-causality, is of particular interest. Partial directed coherence has been introduced for a frequency domain analysis of linear Granger-causality based on modeling the underlying dynamics by vector autoregressive processes. We discuss the statistical properties of estimates for partial directed coherence and propose a sig

[Optimized fitting of a midface implant to anchor a magnetic nasal prosthesis using 3D printing].

BACKGROUND: Plate-based anchorage systems for craniofacial prostheses offer advantages over extraoral solitary titanium implants in terms of the flexible choice of mounting points and higher stability. Disadvantages become apparent in the complex individual intraoperative adaptation of the plate-based systems to the usually poorly accessible bone. The current article presents a method to overcome these disadvantages and make greater use of the advantages of plate-based systems. MATERIALS AND METHODS: The bony midface of a patient who had undergone rhinectomy for cancer of the nasal entrance w

Enhancing Outcome Measurement in Oncology Clinical Trials Through Artificial Intelligence: A Scoping Review.

The phased framework of oncology trials is designed to ensure patient safety and conserve resources by advancing only promising therapies from early- to late-phase testing. Despite decades of refinement, overall trial success rates-defined by the proportion of studies ultimately supporting regulatory approval-remain low, with failures increasingly occurring in late-phase studies. These failures are often contributed to by methodological shortcomings, including suboptimal end point selection, restrictive eligibility criteria, and inefficient trial designs. Although traditional approaches to bio

Dissecting PANoptosis in the Nervous System: A Unified Cell-Death Mechanism Driving Neuroimmune Activation and Chronic Neuroinflammation.

PANoptosis, a collective form of programmed cell death that includes apoptosis, necroptosis, and pyroptosis, is turning out to be a key player in the neuroimmune activation and sustaining chronic neuroinflammation in the nervous system. PANoptosis, in contrast to single cell death mechanisms, is a web of events coordinating neuronal death, glial cell changes, and inflammatory signals, being implicated in the initiation and progression of neurodegenerative and neuroinflammatory diseases. This review compiles current knowledge of the molecular pathways of PANoptotic signaling, its interaction wi

Practical considerations for using concentrated U200 insulin in automated insulin delivery systems.

The use of automated insulin delivery systems (AID) is standard of care for people with type 1 diabetes. However, the limited capacity of insulin pump cartridges, which can hold 1.6-3.0mL or the equivalent of 160-300 units of U100 insulin, can be a barrier to AID use for individuals with high total daily insulin (TDI) requirements. With the rising prevalence of obesity, expansion of AID use to type 2 diabetes, and trends towards smaller cartridge volumes to decrease the size of devices, practical solutions to reduce barriers to AID use for those with high TDI requirements are needed. U200 conc

Helicobacter cinaedi bacteremia in a returning traveler.

Of the non-Helicobacter pylori Helicobacter (NHPH) species, Helicobacter cinaedi is an emerging cause of infection in humans. Here we report a novel clinical presentation of H. cinaedi infection: a case of fever in a returning traveler. A 31 year old previously fit and well male presented with onset of fever 24 h after returning from travel in Singapore and Indonesia. Associated symptoms consisted of sore throat, mild shortness of breath, generalized myalgia and arthralgia, headache, and four episodes of loose stools. The patient recovered spontaneously without treatment and was discharged. Af

Cardiac sympathetic degeneration correlates with olfactory function in Parkinson's disease.

Autonomic and olfactory dysfunctions are considered markers for preclinical diagnosis in Parkinson's disease (PD), because pathological changes in these systems can start before motor symptoms develop. We investigated whether cardiac sympathetic function and olfactory function are associated in PD. Participants comprised 40 nondemented patients with idiopathic PD, and age-matched controls. Cardiac sympathetic function was evaluated by (123) I-metaiodobenzylguanidine (MIBG) uptake, in terms of the heart to mediastinum (H/M) ratio in both early and delayed images, and the washout rate (WR). An Atypical Presentation of VEXAS Syndrome: A Diagnostic Conundrum.

VEXAS syndrome is a rare and recently identified autoinflammatory disorder recently, characterized by the involvement of multiple organ systems, with manifestations in dermatology, hematology, and rheumatology. The syndrome results from a somatic mutation in the UBA1 gene, leading to defective ubiquitin-mediated protein degradation, which triggers a cascade of inflammatory responses. We report the case of a 76-year-old male who presented with a recurrent rash initially suggestive of Sweet's syndrome. Following a comprehensive diagnostic workup, including genetic testing, the patient was diagno

Salivary anti-Ro60 and anti-Ro52 antibody profiles to diagnose Sjogren's Syndrome.

Simple and non-invasive saliva-based diagnostics may be useful for the identification, understanding, and monitoring of autoimmune and infectious diseases. Previously, Luciferase Immunoprecipitation Systems (LIPS) were used for sensitive detection of patient serum autoantibodies in Sjögren's Syndrome (SjS), a chronic autoimmune disease affecting the salivary and lacrimal glands. Here we explored the ability of LIPS to diagnose SjS based on IgG autoantibodies in patient saliva. From LIPS testing, anti-Ro60 autoantibodies were detected in the saliva of 70% (19/27) of SjS patients with 96% specif

[Pituitary macroadenoma associated with pulmonary and cutaneous sarcoidosis--a clinical case].

Sarcoidosis is a granulomatous disease of unknown etiology that can affect all organs and systems, including the central nervous system in approximately 5% of patients. Although the hypothalamo-hypophyseal system may be one of the areas affected by neurosarcoidosis, pseudotumoral lesions are very rare. This paper reports the case of a 42-year-old male Caucasian patient followed by the Pneumology and Dermatology services for pulmonary and cutaneous sarcoidosis. Corticotherapy was initiated five months from diagnosis and the patient showed clear clinical and imagiological signs of improvement. I

Assessing Gamma Index Sensitivity to Selected Linear Accelerator Mechanical Errors Using Various Dosimetric Systems.

Background: Volumetric modulated arc therapy (VMAT) for stereotactic body radiotherapy (SBRT) involves highly modulated dose distributions and steep dose gradients, increasing the importance of patient-specific quality assurance (QA). Gamma index analysis is widely used for comparing measured and treatment planning system (TPS)-calculated dose distributions; however, its sensitivity to small systematic mechanical errors remains controversial, particularly across different detector systems and normalization methods. This study aimed to investigate the sensitivity of various gamma index criteria

Engagement of Social Support Systems in Patients Being Considered for and Following Advanced Heart and Lung Surgical Therapies: An ISHLT Expert Consensus Statement.

Social support systems, particularly caregivers, play a critical role in the evaluation, treatment, and recovery of patients undergoing heart or lung transplantation or left ventricular assist device (LVAD) implantation. However, definitions, assessment practices, and expectations for social support vary considerably across programs, and evidence guiding optimal caregiver engagement remains limited. This International Society for Heart and Lung Transplantation expert consensus statement summarizes current evidence and provides recommendations for assessing, engaging, and supporting social supp

Development of continuous warning system for timely prediction of septic shock.

As delayed treatment of septic shock can lead to an irreversible health state, timely identification of septic shock holds immense value. While numerous approaches have been proposed to build early warning systems, these approaches primarily focus on predicting the future risk of septic shock, irrespective of its precise onset timing. Such early prediction systems without consideration of timeliness fall short in assisting clinicians in taking proactive measures. To address this limitation, we establish a timely warning system for septic shock with data-task engineering, a novel technique rega

Role of the physician in prehospital management of trauma: European perspective.

Advanced prehospital trauma life support is challenged as a whole. Formerly well-accepted basic principles for stabilizing vital functions of the severely injured patient like volume resuscitation, airway protection, and immobilization have been questioned. In prehospital management of trauma, the role of not only the physician but also the paramedic must be redefined. In the absence of evidence about the effectiveness of advanced trauma life support training for paramedic crews, the needs of trauma victims and capacities of emergency medical systems must be re-evaluated. Assessment of patient

Eligibility-Identification Difficulty and Self-Reported Preventive Care Implementation Among Japanese Primary Care Physicians.

BACKGROUND: Primary care is an important setting for prevention, but implementation gaps may arise before clinicians can identify patients eligible for preventive services. We described self-reported preventive care implementation among Japanese primary care physicians and examined whether lower implementation co-occurred with eligibility-identification difficulty. METHODS: We conducted a nationwide web-based cross-sectional survey of physician members of the Japan Primary Care Association from May to June 2025. Respondents reported recommendation consistency for 30 preventive services, diffi

From biomarker expansion to equitable implementation in mild cognitive impairment.

The expanding role of cerebrospinal fluid (CSF) biomarkers in Alzheimer's disease diagnosis represents a major shift toward biologically driven dementia care. In response to the article by Poli et al., we discuss important considerations regarding the broader implementation of CSF biomarker testing in patients with mild cognitive impairment (MCI), particularly in the era of emerging anti-amyloid therapies. While biomarker-guided approaches may improve identification of atypical or non-amnestic Alzheimer's disease presentations, widespread adoption remains limited by disparities in diagnostic i
