Early versus delayed ureterolithotripsy following obstructive acute pyelonephritis treatment.

BACKGROUND AND OBJECTIVES: There is no recommendation on the timing of ureterolithotripsy after the treatment of obstructive acute pyelonephritis (APN). The effect of early and delayed ureterolithotripsy on postoperative urinary tract infection (UTI) and other complications was investigated. METHODS: Patients who underwent ureterolithotripsy after obstructive APN treatment between February 2017 and August 2021 were divided into two groups, those operated during hospitalization and those operated within 3 months after discharge. Two groups were compared in terms of stone-free status, postopera

Factors associated with absent microhematuria in symptomatic urinary stone patients.

INTRODUCTION: The aim of this study was to identify factors associated with absent hematuria in patients with symptomatic urinary stones. METHODS: This retrospective study analyzed the clinical and imaging findings of emergency department patients who underwent computed tomography (CT) for suspected ureteral colic over the past 2years. All patients also underwent a microscopic urinalysis, and the presence of 4 or more red blood cells/high-power field was defined as microhematuria. RESULTS: A total of 798 patients were included in this study. Of these patients, 750 (94.0%) presented with hema

Observational prospective study for surgical outcome and anesthetic feasibility of tubeless and totally tubeless supine PCNL: A single centre initial experience.

OBJECTIVE: To evaluate surgical outcome and spinal anesthetic feasibility of supine percutaneous nephrolithotomy (PCNL) by tubeless and totally tubeless method. MATERIAL AND METHODS: This observational study included a totally 53 patients. Initial diagnosis of renal stone was based on urinary ultrasonography, kidney, ureter and bladder X-ray, and later confirmed by computed tomography urography. PCNL was done with the patient in Galdakao-modified supine Valdivia position. Nephrostomy was not done using tubeless method, while in totally tubeless method instead of insertion of double j stent, u

Bilateral Renal Lymphangiomatosis.

Lymphangiomatosis is a rare and benign mesenchymal disorder where there is developmental malformation and dilatation of lymphatic channels. The most common sites for lymphangiomatosis are the cervical neck and axilla, but it can also occur in the mediastinum, retroperitoneum, mesentery, omentum, colon, and pelvis. Renal lymphangiomatosis is an even rarer multicystic disorder which may be hilar, pericalyceal, paracalyceal, peripelvic, or para pelvic. We report a case of 24-year-old female with bilateral perirenal lymphangiomatosis. Her complain was bilateral flank pain and bilateral perinephric

Necrosis of the duodenal wall with retroperitoneal perforation as a complication of perirenal abcess. Case report and management.

Duodenal perforation is a rare and severe acute surgical condition which commonly follows the complications of endoscopic and laparoscopic procedures. Small degree of damage in this mechanism and an early diagnosis allow for an effective primary management. The most difficult surgical challenge is an effective management of retroperitoneal duodenal perforation together with coexisting pathological changes of its wall. In this work we present a case of duodenal necrosis with excessive necrosis of a fragment of its wall due to perinephric abscess, with an effective method of management of a defe

[Castleman's disease: 6 cases report].

The authors present six cases of Castleman's disease. One was located in the left middle lung near the lung root, one in the right posterior mediastinum, one in the gallbladder fossa, one in the left perinephric space, and the last two in the mesenteric region. All the hyaline-vascular type showed good enhancement after contrast medium injection. The only one case of plasma cell type located in mesentery was not enhanced after contrast injection. Ultrasound of abdomen was performed in one of the six patients, which showed homogeneous and hypoechoic in echo-pattern.

Laparoscopic ablative and reconstructive surgeries in genitourinary tuberculosis.

BACKGROUND AND OBJECTIVES: Laparoscopy is the present standard of care for urologic diseases. Laparoscopy in renal tuberculosis (genitourinary tuberculosis) is difficult because of inflammation and fibrosis associated with the disease. We present the outcome of our experience of laparoscopy in genitourinary tuberculosis, both ablative and reconstructive. METHODS: The detailed data of patients with genitourinary tuberculosis who underwent laparoscopic surgeries between January 2011 and September 2012 were reviewed. Indications, type of surgery, duration, blood loss, intraoperative problems, po

Acquired cystic kidney disease.

ACKD is characterized by the development of many fluid-filled renal cysts and sometimes neoplasms in the kidneys of individuals with chronic renal failure but without a history of hereditary cystic disease. The condition is seen mainly in dialysis patients, but often begins in patients with ESRD before dialysis is started. Most patients with ACKD are asymptomatic, but the disorder may be associated with such serious complications as retroperitoneal hemorrhage and metastatic renal cell carcinoma. The diagnosis of ACKD and its complications is best achieved by CT scanning, although US and MR ima

Stone fragments migration into subcapsular renal hematoma post flexible ureterorenoscopy (unique presentation).

This is a case report of 45-year-old female patient who presented with right flank pain. Abdominopelvic CT scan showed right renal pelvis stone measuring 20 mm. Right flexible ureterorenoscopy with holmium laser was performed in our institute. Postoperatively, she was febrile and pale. Immediate Abdominopelvic CT scan was obtained which revealed a large right subcapular hematoma. Conservative management was maintained for a week. Two months later, repeated Abdominopelvic CT scan showed regression of right subcapsular renal hematoma with stone fragments migration into the perinephric space as a

Sonographic renal findings in 20 dogs with leptospirosis.

Abdominal ultrasound examinations of 20 dogs with confirmed leptospirosis were reviewed retrospectively for renal abnormalities. Three dogs had a normal ultrasound examination. The remaining 17 dogs had sonographic abnormalities of the kidneys. These abnormalities, seen either alone or in combination, included renalmegaly (n=10), pyelectasia (n=9), increased cortical echogenicity (n=15), perinephric effusion (n=5), and a medullary band of increased echogenicity (n=6). At our institution, the medullary band of increased echogenicity has only been seen in dogs with leptospirosis and may therefor

Computed tomographic evaluation of blunt renal injuries.

Adequate radiologic demonstration of the extent of renal injury following blunt abdominal trauma is an important guide to therapy. Traditionally, the evaluation has been based on urographic and/or angiographic studies. This report details the results of computed tomographic (CT) evaluation of suspected renal injuries in ten patients. Renal injuries were demonstrated by CT in nine of these patients. Abnormalities demonstrated were subcapsular/perinephric hematomas, intrarenal hematomas, renal lacerations, segmental infarcts, and contrast material extravasation. This study suggests that CT is va

CT-guided antegrade pyelography and percutaneous nephrostomy.

Percutaneous antegrade pyelography and percutaneous nephrostomy can be easily performed using CT guidance. CT provides cross-sectional images which facilitate accurate needle placement into the renal pelvis. CT is capable of detecting subtle density differences within the tissues so that even renal pelvises which do not contain contrast material and are not dilated can be accurately punctured. In addition to these procedures, CT accurately displays the perinephric space so that assessment of complications is possible. Four of five attempted percutaneous nephrostomies and two antegrade pyelogra

Imaging of Urologic Trauma.

Urologic trauma encompassed a wide range of injuries affecting the urologic tract, resulting from various traumatic events or iatrogenic procedures. This review explores the clinical presentation, diagnostic strategies, and management approaches of urologic trauma, emphasizing the critical role of imaging, particularly computed tomography, in accurately assessing and guiding treatment decisions. Renal, ureteral, bladder, and urethral trauma are comprehensively discussed, including mechanisms of injury, classification systems, and therapeutic interventions. In addition, we discuss potential com

Extensive emphysematous pyelonephritis in a renal allograft treated conservatively: case report and review of the literature.

Emphysematous pyelonephritis (EPN) is a rare occurrence in renal allografts. An aggressive approach resulting in transplant nephrectomy is viewed as the standard of care. Over the recent years, treatment with percutaneous drainage (PCD) of the renal and perinephric collections and appropriate antibiotics has been reported with good success in lesser grades of this infection. Only 4 cases of extensive EPN disease with Escherichia coli, treated with conservative management, are reported in the English-language literature. We present a case of severe EPN caused by Klebsiella pneumoniae, successfu

Renal emphysema in diabetic patients: CT evaluation.

Renal emphysema is a rare, fulminant, suppurative infection of pelvicaliceal system, renal parenchyma, perinephric tissues, and retroperitoneum. It is characterized by formation of gas. Invariably this condition is associated with diabetes mellitus and carries high mortality (40-90%). Renal emphysema can be classified into two distinct clinical entities: emphysematous pyelitis and emphysematous pyelonephritis. This classification has important prognostic and therapeutic implications. Herein we describe the computed tomographic findings in five unilateral cases of renal emphysema (two cases of

Embolization of postbiopsy and postnephrostomy complications in transplanted kidney: a case report.

Complications such as arteriovenous, arteriocalyceal fistula, pseudoaneurysm, or perinephric hematoma occur in allografted and native kidneys after interventional procedures. When these complications are not detected and treated immediately, they might be associated with a poor allograft prognosis. Reoperation may produce parenchymal injuries, therefore, endovascular embolization may be a preferred treatment modality. Herein we report an arteriocalyceal fistula and an arteriovenous fistula in a transplant kidney that were detected after biopsy and nephrostomy, respectively, they were treated w

Extrarenal retroperitoneal angiomyolipoma.

Angiomyolipoma of the retroperitoneum is extremely rare with only 7 cases reported to date in the literature. Spontaneous hemorrhage and shock is the most common presentation that requires emergency intervention. We report a case of large extrarenal retroperitoneal angiomyolipoma that illustrates a different form of presentation for such a rare tumor mimicking a large locally advanced renal parenchymal tumor. The diagnosis was made by histopathology after performing radical nephrectomy. Although rare, angiomyolipoma of the perinephric fat may have variable presentations and should be considere
