Personality disorders and the 3-year course of alcohol, drug, and nicotine use disorders.

CONTEXT: Little is known about the role of a broad range of personality disorders in the course of substance use disorder (SUD) and whether these differ by substance. The existing literature focuses mostly on antisocial personality disorder and does not come to clear conclusions. OBJECTIVE: To determine the association between the 10 DSM-IV personality disorders and the persistence of common SUDs in a 3-year prospective study of a national sample. DESIGN: Data were drawn from participants in the National Epidemiologic Survey on Alcohol and Related Conditions (NESARC) who had alcohol dependen

Personality disorders evident by early adulthood and risk for anxiety disorders during middle adulthood.

Data from the Children in the Community Study, a prospective longitudinal investigation, were used to investigate the association of personality disorder (PD) traits, evident by early adulthood, with risk for development of anxiety disorders by middle adulthood. Individuals without a history of anxiety disorders who met diagnostic criteria for >or=1 PD by early adulthood were at markedly elevated risk for agoraphobia, generalized anxiety disorder, obsessive-compulsive disorder, and panic disorder by middle adulthood. Antisocial, avoidant, borderline, dependent, depressive, histrionic, obsessiv

Personality disorders do not influence the results of cognitive behavior therapies for anxiety disorders.

Two studies are presented in which the presence of personality disorders (PDs) was assessed using a structured clinical interview (Structured Clinical Interview for DSM-III-R Personality Disorders [SCID-II] by an independent rater before the start of treatment. The therapy effect was measured by change scores. In the first study, patients (N = 31) with panic disorder (with or without agoraphobia) received standardized cognitive behavioral treatment protocols. The effect of treatment as assessed by questionnaires and panic frequency was comparable in the groups with and without PD. In a second

DSM-III personality disorders among patients with major depression, anxiety disorders, and mixed conditions.

Of 298 mainly nonpsychotic psychiatric outpatients between 19 and 59 years of age, a group of patients having either pure major depression, major depression in combination with anxiety disorders, or pure anxiety disorders was extracted. The anxiety disorders were further differentiated in panic and nonpanic anxiety disorders. The groups were compared as to differences in frequency of personality disorders assessed by means of the Structured Interview for DSM-III Personality Disorders. The mixed major depression/anxiety disorder group appeared to be the most deviant with more severe personality

[Trauma-focused and personality disorder treatment for posttraumatic stress disorder and comorbid cluster C personality disorder].

BACKGROUND: Posttraumatic stress disorder (PTSD) is associated with high rates of avoidant, dependent and obsessive-compulsive personality disorders (cluster C-PD). It is unknown whether concurrent treatment for PTSD and comorbid cluster C-PD leads to superior treatment effects, compared to standalone trauma-focused treatment. AIM: To test the efficacy of adding 52-58 sessions of PD treatment (group schema therapy; GST) to 12-18 sessions of individual trauma-focused treatment (imagery rescripting; ImRs). METHOD: 130 adult outpatients were randomized to receive either ImRs or ImRs+GST. The ma

Personality disorders in a non-Western context: A comprehensive meta-analysis of personality disorder prevalence in Turkiye.

Personality disorders (PDs) are severe mental health conditions associated with significant distress and functional impairment. However, the epidemiology of personality disorders in non-Western populations is underrepresented. This meta-analysis aimed to estimate the prevalence of any personality disorder in both clinical and community samples in Turkiye. A systematic search was conducted in Web of Science, PubMed, Scopus, TRDizin, and the Turkish Psychiatry Index. The meta-analysis was performed in RStudio following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISM

An Emotional Neglect-Personality Disorder Approach: Quantifying a Dimensional Transdiagnostic Model of Trauma-Related and Personality Disorders.

Are personality disorders (PDs) associated with emotional neglect? Draijer (2003) developed a dimensional model of trauma-related disorders and PD. The first dimension consists of the severity of the trauma endured. The second dimension consists of emotional neglect, which is assumed to be related primarily to personality pathology. In this article, we investigate whether an association between retrospective reports of emotional neglect and the presence and severity of PD exists. A sample of 150 patients was systematically assessed. Results indicate that there is little evidence to support a l

Matricide by person with bipolar disorder and dependent overcompliant personality.

Matricide is an infrequent form of homicide. This paper is to present a case of matricide with typical characteristics of the act but interesting particularities as well. The perpetrator was a 43-year-old man, respected member of his community, with over compliant characteristics, eagerness in serving people and caring his parents, good social adaptation before and after the crime. He abandoned his family and work in order to better serve his old, disabled but over demanding mother who frequently insulted and humiliated him. Suddenly he came to a state of "mental confusion" and strangled her.

The prevalence of personality disorder in mental state disorder.

PURPOSE OF REVIEW: This review examines the prevalence of personality disorder in those with mental state disorder. Some challenges disentangling these psychopathologies are delineated and advances in understanding broadly are reviewed. RECENT FINDINGS: Recent taxonomic changes to a dimensionally based classification system in the ICD-11 and DSM of mental disorders-V's alternative model of personality disorder include requiring cut offs to examine clinical outcomes akin to those in hypertension. These new criteria affect personality disorder prevalence where it is comorbid with mental state d

Comorbidity of personality disorder with intermittent explosive disorder.

There is ambiguity in how recurrent anger and aggression are accounted for by psychiatric nosology. One area of uncertainty is the extent to which Intermittent Explosive Disorder (IED) overlaps with and is distinct from Personality Disorder (PD). Accordingly, we conducted a study of individuals with IED and PD in order to understand the nature of comorbidity relationships seen across these two areas of psychopathology. One-thousand-five-hundred-twenty-one adults were studied (441 Healthy Controls (HC), 430 Psychiatric Controls (PC), and 650 IED subjects) and assessed for DSM-5 psychiatric diso

Distinguishing Mood Temperaments from Personality Disorders: Diagnostic and Clinical Implications.

Temperament refers to enduring, biologically rooted affective dispositions that influence how individuals experience and regulate emotion. Although central to classical psychiatry, from Kraepelin's manic-depressive illness to Akiskal's affective styles, temperaments have been largely excluded from modern nosology and psychoanalytic theory. This omission has contributed to diagnostic confusion, particularly in differentiating cyclothymic temperament from borderline personality disorder and hyperthymic temperament from narcissistic personality disorder. While these conditions share surface simil

Moving beyond Personality Disorders: A Challenge for the DSM-6.

The authors argue that as a diagnostic category "personality disorders" falls short in offering helpful counsel to patients with difficulties in self and interpersonal functioning. The article begins with a discussion of symptom versus disorder in general medicine before critiquing the impulse of psychiatrists and humans more broadly to categorize and sort. The authors summarize well-described limitations to categorical personality disorder diagnoses, including their questionable clinical utility, unsteady empirical support, and potential to reify myriad forms of stigma. These limitations emph
