Pneumoperitoneum induced by pneumatosis cystoides intestinalis associated with volvulus of the stomach.

A rare association of pneumoperitoneum induced by pneumatosis cystoides intestinalis with chronic mesenteroaxial volvulus of an irreducible stomach in a paraesophageal hiatus hernia is described. The importance of accurate radiologic work-up is emphasized. The possible significance of this association to explain the pathogenesis of pneumatosis cystoides intestinalis is discussed.

An ectopic gastric duplication arising from the pancreas and presenting with a pneumoperitoneum.

This case report describes a large gastric duplication, which had no attachment to the stomach, and it was found in the pancreas. The second unusual feature of this duplication was that it had eroded into the splenic flexure of the colon, resulting in a large pneumoperitoneum.

Tension Pneumoperitoneum: A Comprehensive Review of Etiologies, Diagnosis, and Management Strategies.

INTRODUCTION: Tension pneumoperitoneum (TPP) is a rare but life-threatening condition characterized by free intraperitoneal air causing intra-abdominal hypertension and hemodynamic compromise. It shares physiologic features with abdominal compartment syndrome and requires timely recognition and intervention. This systematic clinical review evaluates the presentation, diagnostic strategies, management approaches, and outcomes associated with TPP. METHODS: A comprehensive literature search was conducted across PubMed, EMBASE, and Google Scholar for studies published between 2004 and 2024. Searc

Navigating Asymptomatic Idiopathic Pneumoperitoneum: A Case Report and a Literature Review.

Pneumoperitoneum refers to the presence of free air in the abdominal cavity, typically indicating viscus perforation requiring urgent surgical intervention. Occasionally, pneumoperitoneum occurs without organ perforation, termed 'spontaneous' or 'non-surgical' pneumoperitoneum. We present the case of a 65-year-old male referred to the emergency department after a seizure episode. The patient reported no abdominal pain or fever, and examination revealed no other signs of peritonitis. An erect chest X-ray showed air under the diaphragm, and a subsequent computed tomography (CT) scan confirmed fr

Dirty Mass sign of abdominal imaging: A case of silent pneumoperitoneum.

Pneumoperitoneum is a common complication after penetrating abdominal trauma, gastric ulcer, or colitis in which free air is present in the peritoneal or retroperitoneal space. Sole pneumoretroperitoneum, which refers to gas in the retroperitoneal space, is a rare entity, and when significant, results in a characteristic radiographic sign known as "Dirty Mass." Common causes include penetrating trauma or perforation of the retroperitoneal portions of the gastrointestinal tract (duodenum, ascending colon, descending colon, and rectum). Our case describes a 59-year-old female admitted for sudden

Safe insertion of Veress needle for the induction of pneumoperitoneum: a technical note.

Techniques for the induction of pneumoperitoneum for laparoscopic surgery remain varied as complication risk remains with all techniques. Veress needle is used for the induction of pneumoperitoneum as a technique of preference or when an open technique is considered challenging as in obese patients. We present a novel safe technique for insertion of Veress needle by measuring the depth of the anterior abdominal wall prior to insertion. Accurate measurements help in the safe insertion of the Veress needle for inducing pneumoperitoneum and hence reduce the incidence of intra-abdominal injuries.

Influence of pneumoperitoneum and head-down maneuver on the cerebral microvasculature in rabbits.

BACKGROUND: With recent advances in robot-assisted techniques, an increasing number of surgeries are being performed with pneumoperitoneum and head-down maneuver (HDM) that may affect the cerebral microcirculation. For the first time, this study investigated the direct influence of pneumoperitoneum and HDM on the cerebral microvasculature in rabbits. METHODS: Adult male rabbits were randomly allocated to the following groups (n = 7 each): control, pneumoperitoneum alone (P), and pneumoperitoneum with HDM (P + HDM) for 120 min. A closed cranial window was installed above the parietal bone to v

Distinctive clinical features of spontaneous pneumoperitoneum in neonates: A retrospective analysis.

BACKGROUND: Spontaneous pneumoperitoneum (SP) without gastrointestinal perforation rarely occurs in neonates, with most SP cases being idiopathic. Although SP usually follows a benign clinical course with favorable prognosis, it can become life-threatening in certain situations. In these cases, urgent surgical intervention may be required. Therefore, it may be difficult to decide when or how to perform prompt interventions. AIM: To demonstrate the distinct clinical features of SP to guide appropriate management by comparing characteristics between SP and typical pneumoperitoneum secondary to

Idiopathic asymptomatic pneumoperitoneum in a patient with ureteric calculus and Crohn's disease.

A 46-year-old male referred to the Emergency Department with pain in the left flank. The patient suffered from Crohn's disease. He had not experienced any fever, and a urine strip showed signs of blood. A ureteric calculus was suspected and a computed tomography scan was performed, which surprisingly showed free intraperitoneal air (FIA) located along the ascending colon. The patient had no signs indicative of peritonitis and the spontaneous pneumoperitoneum was suspected. Pneumoperitoneum is often associated with severe intraabdominal pathology, such as perforation of the gastrointestinal tra

The Use of Ceus Software with No Contrast Media Administration in the Diagnosis of Pneumoperitoneum.

BACKGROUND: Pneumoperitoneum is defined by the presence of free air in the abdominal cavity; gastrointestinal perforation is an important cause of this pathological condition. In emergency situations, radiology is considered vital in the early detection and identification of the site and cause of the perforation, which is critical for proper surgical planning. AIM: The aim of our study was to evaluate a new diagnostic US tool, based on the US contrast-specific software generally used during contrast-enhanced US examination (CEUS), without the administration of sonographic contrast media, and

Pneumatosis Intestinalis, Pneumoperitoneum, and Ascites Secondary to Scleroderma: A Case Report.

Pneumatosis intestinalis (PI), pneumoperitoneum, and ascites are radiographic findings that may be incidental or associated with severe bowel compromise. Asymptomatic patients with benign PI, pneumoperitoneum, or ascites are often observed or treated conservatively. However, these findings are concerning in symptomatic patients and often require surgical consultation and urgent surgical intervention Approximately 15% of PI cases are idiopathic, and 85% are secondary due to an underlying pathology including but not limited to pulmonary disease, autoimmune disease, drug-induced sources, gastroin

Case Report: Is Pneumoperitoneum the Only Indication for Surgery in Necrotizing Enterocolitis?

Objective: This study aims to explore whether pneumoperitoneum is the only surgical indication for neonates with necrotizing enterocolitis (NEC) and to analyze when early surgical intervention should be considered. Methods: A retrospective case series study was conducted including six neonates with stage 2a-2b NEC who received surgeries without absolute indication but with failed conservative treatment. In the meantime, seven infants who received surgery due to pneumoperitoneum and 32 infants treated with conservative treatment were also included for comparison. Results: Our results indicated

An Unusual Pneumoperitoneum in an Extremely Low Birth Weight Preterm Newborn.

Pneumoperitoneum (PP) is a radiological diagnosis, characterized by the presence of air under the diaphragm, that in 90% of cases results from a perforated viscus while in 10% of cases it is not associated to perforation, the so-called non-surgical or spontaneous PP. Spontaneous PP is rare at any paediatric and neonatal age. In the neonatal population, sporadic cases of spontaneous PP have been described, almost invariably following mechanical ventilation. We presented the case of an extremely low birth weight infant (ELBW) with spontaneous PP secondary to pneumomediastinum who has never under

The enigma of asymptomatic idiopathic pneumoperitoneum: A dangerous trap for general surgeons.

INTRODUCTION: Idiopathic pneumoperitoneum is an extremely rare condition that can easily be misdiagnosed as an acute abdomen. Awareness of this phenomenon can help avoid unnecessary surgical intervention and the potential associated morbidities. PRESENTATION OF CASE: A 76 year old man presented to hospital with dyspnoea and productive cough and was diagnosed with an infective exacerbation of COPD. He subsequently had a CTPA which showed a significant amount of free intraperitoneal gas in the upper abdomen. He was completely asymptomatic from this with no abdominal pain, distension, or signifi
