[Clinical situations in detention settings].

Infectious diseases, substance use disorders, and psychiatric conditions are more prevalent in prisons than in the general population. There is relatively limited evidence regarding the medical management of non communicable diseases in prison settings. The clinical cases described in this article highlight specific dimensions to be considered when practicing medicine in detention settings : multidisciplinary work ; regular voluntary screening for infectious diseases that are prevalent in this population ; management of non communicable diseases ; availability of medical protocols for specific

Adult newcomers' perceptions of access to care and differences in health systems after relocation from Syria.

BACKGROUND: In Canada, approximately 13% of the population lives with multiple chronic conditions. Newcomers, including refugees, have the same or higher risk of developing chronic diseases as their host population. In 2015-2016, Canada welcomed almost 40, 000 newcomers from Syria. This study aimed to (1) understand adult newcomer health needs for self-management of non-infectious chronic conditions; and (2) identify strategies to improve access to health care services to meet these needs. METHODS: This study used a qualitative descriptive design. Interviews and focus groups were conducted wi

Ambient Air Pollution Health Impacts in High-Income Countries, Part II: A Case Study in the United Arab Emirates.

Ambient air pollution poses major global public health risks, particularly in rapidly developing regions, including the Arabian Gulf. This study estimates the disease burden attributable to anthropogenic air pollution (particulate matter [PM] and ozone) in Abu Dhabi, United Arab Emirates, for 2019-2022 and compares findings to previous estimates for 2008. Burdens were calculated using updated air quality, population, and health incidence data for 2019-2022 along with concentration-response functions from recent epidemiologic evidence. Results show that Abu Dhabi's investments in air quality ma

Facilitating entry into drug treatment among injection drug users referred from a needle exchange program: Results from a community-based behavioral intervention trial.

We evaluated a case management intervention to increase treatment entry among injecting drug users referred from a needle exchange program (NEP). A randomized trial of a strengths based case management (intervention) versus passive referral (control) was conducted among NEP attenders requesting and receiving referrals to subsidized, publicly funded opiate agonist treatment programs in Baltimore, MD. Logistic regression identified predictors of treatment entry within 7 days, confirmed through treatment program records. Of 247 potential subjects, 245 (99%) participated. HIV prevalence was 19%. O

[An epidemic of meningococcal meningitis in the region of Savanes in Togo in 1997: research and control strategies].

Neisseria meningitidis is responsible for high levels of morbidity and mortality in the developing countries of the African meningitis belt. There are frequent meningococcal meningitis epidemics in this region affecting almost 1,000 people in every 100,000 (1%). Epidemics generally occur during the dry season but the interval between epidemics is variable (between 2 and 25 years). The reasons for these recurrent epidemics are unclear. There is a safe and effective polysaccharide vaccine against meningococci A and C. Unfortunately, the immunity it provides decreases with time, especially in you

Cardiovascular disease in HIV patients: recent advances in predicting and managing risk.

INTRODUCTION: Cardiovascular disease (CVD) is one of the leading causes of mortality in virally suppressed people living with HIV (PLWH) and with an aging population, is likely to become one of the leading challenges in maintaining good health outcomes in HIV infection. However, factors driving the risk of CVD in PLWH are multiple and may be different from those of the general population, raising challenges to predicting and managing CVD risk in this population. AREAS COVERED: In this review, we examine the relevant data regarding CVD in HIV infection including CVD prevalence, pathogenesis, a

Safeguarding our nation's children: the diagnosis, management, and containment of smallpox in infants and children.

Smallpox continues to be a major national health concern as it poses the most serious bioterrorist threat to the US population at this time. Due to similarities in clinical presentation, smallpox may easily be confused with varicella (chickenpox) in young children. Management of a large-scale outbreak of smallpox in young children would require an intensive health care response. In addition, the current debate concerning potential revisions to the Centers for Disease Control and Prevention (CDC) interim guidelines for vaccination against smallpox has significant health implications for high-ri
