Immunoregulatory role for a public IgM idiotype in the induction of autoimmune diseases in Mycoplasma pneumoniae infection.

Mycoplasma pneumoniae (MP) infection is associated with the emergence of various autoimmune disorders and autoantibody production. The most common autoantibodies induced are of anti-I specificity and express cold agglutinin (CA) activity. However, the mechanisms by which the microbial infection triggers the appearance of these autoantibodies are still unknown. To investigate these mechanisms, we used BALB/c mice as experimental models. In this paper, we show that BALB/c mice polyclonal antisera to MP react with human CA IgMs, and reciprocally, that BALB/c mice polyclonal antisera to human IgM

Parainfectious Transverse Myelitis in a Pregnant Woman: A Rare Clinical Entity.

Transverse myelitis (TM) is an inflammatory disorder of the spinal cord characterized by motor, sensory, and autonomic impairments. Parainfectious transverse myelitis (PITM) represents a post-infectious, immune-mediated entity. Its occurrence during pregnancy is rare and presents with unique diagnostic and therapeutic challenges. We report a 31-year-old primigravida at 32 weeks of gestation who presented with acute-onset bilateral lower limb weakness, sensory loss, and bladder involvement following a febrile illness. MRI spine revealed longitudinally extensive transverse myelitis (C3-T2). The

Glossopharyngeal neuralgia after SARS-CoV-2 infection: A case report.

Glossopharyngeal neuralgia (GN) is a rare neuropathic disorder characterized by sudden, unilateral, electric shock-like pain in the areas innervated by the glossopharyngeal nerve. Its diagnosis is frequently delayed because of its clinical overlap with odontogenic and otorhinolaryngological conditions. In the context of the COVID-19 pandemic, different cranial neuropathies have been reported, suggesting possible post-infectious mechanisms. We describe the case of a 54-year-old male dentist, without relevant medical history, who developed recurrent episodes of intense pain in the right pharynx

Intimomedial mucoid arterial degeneration, a rare arterial disorder of forensic significance.

The body of a 43-year-old African woman with a history of aortic aneurysm and hypertension was forensically investigated after her sudden death. The cause of death was related to a cardiac tamponade due to a ruptured aneurysm of the ascending aorta. Post-mortem gross examination showed an abnormal whitish discoloration of the intima with fibrous thickening of the aortic wall. Several arteries (left main and circumflex coronaries, carotid, renal and iliac arteries) showed similar features. Upon histological examination, the aortic aneurysm as well as the other arteries sampled showed mucoid deg

[Post-transplantation lymphomas and Epstein-Barr virus].

Post-transplant lymphoproliferative disorders (PTLD) are recognized as a devastating complication of organ transplantation. Their occurrence appears to vary according to the transplanted organ and the type and the level of administered immunosuppressive therapy. In kidney transplants the incidence is estimated between 1 and 2%. PTLD represent a clinically and morphologically heterogeneous group of lymphoid proliferations. They have been closely associated with Epstein-Barr virus (EBV), and the spectrum of manifestations after transplantation range from an infectious mononucleosis-like syndrome

[A case of post-rubella Guillain-Barré syndrome associated with ulcerative colitis].

A 56-year-old female with post-rubella Guillain-Barré syndrome (GBS) associated with ulcerative colitis (UC) was described. The patient dramatically recovered with plasmapheresis. It is very rare to encounter the neurological complication following rubella, especially polyradiculoneuropathy as observed in the present case. Significantly high titer of anti rubella antibody at admission and subsequent decrease in the titer suggest that rubella infection is likely to involve the development of GBS. Although GBS and UC are currently regarded as autoimmune disorders, association of GBS and UC is ex

[Benign encephalitis of the brain stem].

INTRODUCTION: The secondary encephalitis disorders are due to an immunological mechanism which causes demyelinating lesions of the central and peripheral nervous systems, with very variable clinical features. The pathogenesis and localization of benign encephalitis of the brain stem and the Miller Fisher syndrome (MFS) are still subject to debate. It is suggested that they may both belong to different extremes of the same nosological spectrum known as the ophthalmoplegia-ataxia-areflexia syndrome. CLINICAL CASE: We report the case of an 11 year old boy with encephalitis of the brain stem who

Overview of Epstein-Barr virus-associated diseases in Japan.

Epstein-Barr virus (EBV) associated diseases and studies performed in Japan are reviewed. Infectious mononucleosis is a common disease in Japanese infants. Chronic and severe EBV-infections include severe chronic active EBV-infection (SCAEBV), EBV-associated hemophagocytic syndrome, and mosquito allergy with granular lymphocyte proliferative disorder (GLPD). Autoimmune lymphoproliferative syndrome (ALPS), a disease caused by a defect in the Fas-Fas ligand pathway of cell-death, may develop into lymphoproliferative disease after early exposure to EBV. More than ten cases of X-linked lymphoproli

Streptococcus equi subsp. zooepidemicus finding in confirmed feline infectious peritonitis cat patient.

BACKGROUND: Feline infectious peritonitis (FIP) is a fatal immune-mediated disease in cat, caused by mutated feline coronavirus (FCoV). Due to its difficulties in diagnosis, FIP is sometimes underdiagnosed. Therefore, several laboratory procedures were performed to gain high index suspicion of FIP. However, through several laboratory findings, not only FIP but also SEZ infection was confirmed in this case. CASE DESCRIPTION: A-year-old male, domestic cat was admitted to Veterinary Medicine Clinical Pathology Laboratory, Universitas Gadjah Mada, for further effusion examination due to its high

Etiology of Acute Leukemia: A Review.

Acute leukemias constitute some of the most common malignant disorders. Despite significant progress made in the treatment of these disorders, their etiology remains unknown. A large and diverse group of genetic and environmental variables have been proposed. The role of a variety of factors, including pre-existing and acquired genetic mutations, exposure to radiation and various chemicals during preconception, pregnancy and throughout life, have been explored. The effects of inherited genetic variations and disorders, pre-existing diseases, infectious agents, hobbies, occupations, prior treat

Post-tuberculosis pulmonary function and noninfectious pulmonary disorders.

Tuberculosis (TB) is among the top infectious causes of morbidity and mortality worldwide and is associated with frequent pulmonary damage despite microbiological cure. Patients with treated TB may remain lifelong sufferers of disabling structural and functional sequelae of the disease, which subsequently impair quality of life. Long-term follow-up studies have revealed that many patients with treated pulmonary TB show signs of permanent impairment of their lung function. Impairment is variable in pattern and severity, ranging from none to severe, and shows restrictive, obstructive, or mixed p

Duodenal implications in the pathophysiology of functional dyspepsia.

Functional dyspepsia (FD) is a heterogeneous disorder associated with diverse pathophysiologic mechanisms. Studies have shown duodenal implications in the pathophysiology of FD. Duodenal hypersensitivity to acid, increased duodenal acid exposure, and abnormal responses to duodenal lipids or released cholecystokinin have been observed in patients with FD. Moreover, there is evidence indicating duodenal immune activation in FD. Alterations in the number of duodenal eosinophils or intraepithelial lymphocytes have been reported in a subset of FD patients, particularly in patients with post-infecti

Neuro-Inflammatory Mechanisms in Developmental Disorders Associated with Intellectual Disability and Autism Spectrum Disorder: A Neuro- Immune Perspective.

Intellectual disability (ID) and autism are present in several neurodevelopmental disorders and are often associated in genetic syndromes, such as Fragile X and Rett syndromes. While most evidence indicates that a genetic component plays an important role in the aetiology of both autism and ID, a number of studies suggest that immunological dysfunctions may participate in the pathophysiology of these disorders. Brain-specific autoantibodies have been detected in the sera of many autistic children and autoimmune disorders are increased in families of children with autism. Furthermore, cytokine

Detection of 65 kD heat shock protein in cerebrospinal fluid of tuberculous meningitis patients.

BACKGROUND: Diagnosis of tuberculous meningitis (TBM) is difficult. Rapid confirmatory diagnosis is essential to initiate required therapy. There are very few published reports about the diagnostic significance of 65 kD heat shock protein (hsp) in TBM patients, which is present in a wide range of Mycobacterium tuberculosis species and elicits a cellular and humoral immune response. In the present study we have conducted a prospective evaluation for the demonstration of 65 kD hsp antigen in cerebrospinal fluid (CSF) of TBM patients, by indirect ELISA method using monoclonal antibodies (mAb) aga

Swyer-James-MacLeod Syndrome: A Rare Cause of Unilateral Hyperlucent Lung in a Two-Year-Old Male Child.

Swyer-James-MacLeod syndrome (SJMS) is an uncommon post-infectious bronchiolitis obliterans-related pulmonary disorder characterized by a unilateral hyperlucent lung with associated hypovascularity on chest radiography and CT, and markedly reduced perfusion on ventilation-perfusion scintigraphy. Although typically identified in older children or adolescents due to its insidious progression, diagnosis in early childhood remains rare and diagnostically challenging. This report describes a two-year-old male child with recurrent lower respiratory tract infections who was ultimately diagnosed with

Multiple intracranial space-occupying lesions in a renal transplant recipient from an area endemic for tuberculosis (TB): TB vs. toxoplasmosis.

Renal transplant recipients may present with intracranial space-occupying lesions (SOLs) due to infections as well as a post-transplant lymphoproliferative disorder (PTLD). Here, we discuss a renal transplant recipient who presented with neurologic symptoms and magnetic resonance imaging (MRI) of the brain showed multiple focal SOLs. Tuberculosis (TB), toxoplasmosis, nocardiosis, fungal infections, and PTLD were considered in the differential diagnosis. MRI spectroscopy was suggestive of an infectious cause, such as toxoplasmosis or TB. Serologic tests using Toxoplasma were negative. A brain b

Non-communicable diseases in the Western Area District, Sierra Leone, following the Ebola outbreak.

Background: Non-communicable diseases (NCDs) are the leading causes of morbidity and mortality in the world. During infectious disease outbreaks, such as the Ebola virus disease outbreak in West Africa from 2014-2015, the health system is often strained, and diagnosis, management and care of NCDs may be compromised. This study assessed numbers and distribution of NCDs in all health facilities in the Western-Area District, Sierra Leone, in the post-Ebola period (June-December 2015) comparing findings with the pre-Ebola (June-December 2013) and Ebola outbreak (June-December 2014) periods. Somatic mutation, a cause of biliary atresia: A hypothesis.

Despite many years of research, the causes of biliary atresia still remain elusive. Infection, immune disorder, toxins or maternal microchimerism have been cited as potential triggers of biliary atresia. This is a rare disease with a stable incidence over the years although with sizeable ethnic variations. This stability suggests that environmental factors have in fact only a slight influence. During the search for etiologies, twin studies have often helped disentangle the genetic from the environmental. For this condition, twin studies have mainly demonstrated a lack of concordance between tw

Prediction of neutrophil nadir and recovery following paediatric haematopoietic cell transplantation with busulfan conditioning.

AIMS: In haematopoietic cell transplantation (HCT), neutropenia resulting from myelosuppression is an expected endpoint following busulfan-based conditioning. However, if prolonged, neutropenia can lead to complications like serious infection and death. The routine use of pharmacokinetic (PK)-guided busulfan dosing has decreased toxicities; however, patients still have serious, sometimes fatal, complications of drug-induced neutropenia. We sought to investigate whether the time-course of neutropenia after HCT could be predicted for paediatric patients following busulfan-based conditioning. Suc

Antimicrobial Stewardship Program for Patients in the Hematological Department Receiving Carbapenem Therapy: A Single-Center and Interrupted Time Series Analysis.

As antibiotic resistance has become a global problem, the intervention of an antimicrobial stewardship team (AST) is warranted. In hematological disorders, infectious complications are crucial owing to abnormal neutrophil function and decreased cell-mediated immunity. Despite the widespread implementation of AST intervention, the effectiveness of stewardship practices for immunocompromised patients remains uncertain. We determined the effect of AST interventions on carbapenem therapy in the department of hematology. Patients admitted to the department and undergoing carbapenem therapy were enr
