Antiretroviral therapy maintenance among HIV-positive women in Ghana: the influence of poverty.

This study examines the role of poverty in the acquisition of and the adherence to antiretroviral therapy (ART) and prescribed clinical follow-up regimens among HIV-positive women. We conducted in-depth interviews with 40 women living with HIV (WLHIV) in Ghana and 15 stakeholders with a history of work in HIV-focused programs. Our findings indicate that financial difficulty contributed to limited ability to maintain treatment, the recommended nutrient-rich diet, and clinical follow-up schedules. However, enacted stigma and concurrent illness of family members also influenced the ability of the

Does Education Level Mitigate the Effect of Poverty on Total Knee Arthroplasty Outcomes?

OBJECTIVE: Total knee arthroplasty (TKA) outcomes are worse for patients from poor neighborhoods, but whether education mitigates the effect of poverty is not known. We assessed the interaction between education and poverty on 2-year Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) pain and function. METHODS: Patient-level variables from an institutional registry were linked to US Census Bureau data (census tract [CT] level). Statistical models including patient and CT-level variables were constructed within multilevel frameworks. Linear mixed-effects models with separat

Preventable Hospitalization Rates and Neighborhood Poverty among New York City Residents, 2008-2013.

Knowing which demographic groups have higher rates of preventable hospitalizations can help identify geographic areas where improvements in primary care access and quality can be made. This study assessed whether preventable hospitalization rates by neighborhood poverty decreased from 2008 to 2013 and whether the gap between very high and low poverty neighborhoods changed. We examined trends in age-adjusted preventable hospitalization rates and rate ratios by neighborhood poverty overall and by sex using JoinPoint regression. Prevention Quality Indicators (PQIs) developed by the Agency for Hea

The relationship between neighborhood poverty and alcohol use: estimation by marginal structural models.

BACKGROUND: Previous studies on the relationship of neighborhood disadvantage with alcohol use or misuse have often controlled for individual characteristics on the causal pathway, such as income-thus potentially underestimating the relationship between disadvantage and alcohol consumption. METHODS: We used data from the Coronary Artery Risk Development in Young Adults study of 5115 adults aged 18-30 years at baseline and interviewed 7 times between 1985 and 2006. We estimated marginal structural models using inverse probability-of-treatment and censoring weights to assess the association bet

Insulin-requiring diabetes in Ethiopia: associations with poverty, early undernutrition and anthropometric disproportion.

BACKGROUND/OBJECTIVES: Most insulin-requiring diabetes patients in Ethiopia have an atypical form of the disease, which resembles previous descriptions of malnutrition-related diabetes. As so little is known about its aetiology, we have carried out a case-control study to evaluate its social and nutritional determinants. SUBJECTS/METHODS: Men and women with insulin-requiring diabetes (n=107), aged 18-40 years, were recruited in two centres, Gondar and Jimma, 750 km northwest and 330 km southwest of the capital, Addis Ababa, respectively. Controls of similar age and sex (n=110) were recruited

Poverty, unstable housing, and HIV infection among women living in the United States.

Women who are HIV positive incur a higher risk of mortality than men who are HIV positive, a difference which is primarily based in the social context of poverty. Economic crises that lead to homelessness, unmet subsistence needs, and sex exchange often reorder priorities among women with HIV infection, de-emphasizing consistent medical care or the use of antiretroviral therapy. High rates of mental illness, drug use, and victimization further increase health and safety risks. HIV prevention messages highlighting education and behavior change insufficiently address the predicament of indigent

Poverty, affluence, and income inequality: neighborhood economic structure and its implications for health.

In this paper, we attempt to verify that neighborhood economic structure influences individual health over and above other individual characteristics, and to comparatively evaluate the effects of neighborhood concentrated affluence, concentrated poverty and income inequality in relation to individual health in the USA. We also explore physical environment, health-enhancing services, social hazards (crime) and social resources as mechanisms operating at the neighborhood level that may help to explain the influence of structural economic conditions on health. We use Hierarchical Ordinal Logit Mo

Living on the Edge: Investigating Experiences of Poverty Through the Lens of the Desperation Threshold Model.

The Desperation Threshold Model (DTM) seeks to explain conflicting findings about the risk propensity of people living in poverty, predicting risk aversion when they can still meet their basic needs, and risk taking when they cannot. The DTM makes assumptions: that people have a conception of basic needs, that their ability to meet these motivates their decisions, and that they modulate risky decisions depending on their ability to do so. The realism of these assumptions has not yet been investigated. To start filling this gap, we investigated experiences of poverty through the lens of the DTM

The double-edged sword of social capital: Impacts on women's health in extreme poverty.

Social capital is extremely important in determining health outcomes yet relatively little is known about how different types of social capital impact on individuals' health outcomes, particularly for the poorest in society. We use secondary data generated from a quasi-experimental study conducted among extremely poor women in rural Bangladesh and apply Propensity Score Matching (PSM) technique to create comparable treatment and control groups. Using a comparative analytical approach, we examine how social capital effects vary and investigate how different social capital measures are associate

Efficacy of group counseling in enhancing social adaptation of adolescents relocated for poverty alleviation.

BACKGROUND: Adolescents relocated for poverty alleviation are prone to social maladaptation, which may adversely affect their personal development and social stability. Enhancing their social adaptation is therefore of considerable significance. This study designed and implemented a group counseling program to examine its effects on the social adaptation of these adolescents. METHODS: This study recruited a total of 300 relocated adolescents experiencing social adaptation difficulties. These participants came from urban districts, county seats, and towns in Z City, Guizhou Province, China. The

Persistent Poverty and Its Interaction With Race: Their Influence on Prostate Cancer-Specific Mortality.

BACKGROUND: Prostate cancer mortality is shaped by both racial and socioeconomic disparities, yet the combined effect of long-term economic hardship and race remains underexplored. OBJECTIVE: To investigate the association between persistent poverty and race on prostate cancer-specific mortality in U.S. counties. METHODS: We analyzed age-adjusted prostate cancer mortality rates from the Surveillance, Epidemiology, and End Results (SEER) program (1999-2020) and linked them to U.S. Department of Agriculture county-level persistent poverty classifications. Persistent poverty was defined as ≥ 20

Critical Windows for Perinatal Health: Evaluating the Impacts of Timing of Poverty Alleviation Benefits.

INTRODUCTION: The earned income tax credit, the largest U.S. poverty alleviation program for families with children, provides tax refunds to low-income working families and has been shown to improve perinatal outcomes and reduce racial/ethnic inequities. Variation in when earned income tax credit refunds are received during pregnancy allows for a natural experiment of the timing of income receipt on perinatal health. METHODS: National birth certificate data from 2010 to 2019 and a difference-in-differences study design were used to examine whether the trimester of earned income tax credit dis

Poverty, Neglect, and Socioemotional Outcomes: An Examination of Child Care as a Promotive and Protective Factor.

According to the family stress model (FSM), poverty increases parent's psychological distress and maladaptive parenting behaviors, which contribute to children's behavioral difficulties. Poverty is one of the strongest predictors of neglect, but there is less evidence of neglect as a mediator in the FSM. Most families in poverty do not engage in neglect and some children exposed to poverty demonstrate adaptive socioemotional outcomes. One way to enable child and family well-being could be access to child care, but there remain questions regarding its benefits. This study examined associations

Cancer Incidence and Trends in Persistent Poverty Areas of California by Race/Ethnicity and Sex.

BACKGROUND: Although previous studies have observed the relationship between living in persistent poverty areas (PPAs) and adverse cancer outcomes, the relationship between residing in PPAs and disparities in cancer incidence rates and temporal trends by race/ethnicity and sex in California is unknown. METHODS: We used California Cancer Registry data to identify patients diagnosed with 16 common cancers between 2006 and 2020. We calculated age-adjusted incidence rates (IRs), incidence rate ratios (IRRs), and average annual percent changes (AAPCs) to compare incidence and temporal trends in PP

The relationship between living in poverty and youth COVID-19 testing in underserved populations.

BACKGROUND: Children living in poverty face particular risk for pandemic-related adverse health events. Place-based pandemic-related health inequities may vary for children living in poverty due to social and environmental factors. We aimed to examine the association between living in low-income households and COVID-19 testing in youth across several regions of the United States. METHODS: This cross-sectional study drew data from three Rapid Acceleration of Diagnostics-Underserved Populations projects (2021-2023). The association between living in low-income households and COVID-19 testing wa

Investigating the impact of poverty on mental illness in the UK Biobank using Mendelian randomization.

It is unclear whether poverty and mental illness are causally related. Using UK Biobank and Psychiatric Genomic Consortium data, we examined evidence of causal links between poverty and nine mental illnesses (attention deficit and hyperactivity disorder (ADHD), anorexia nervosa, anxiety disorder, autism spectrum disorder, bipolar disorder, major depressive disorder, obsessive-compulsive disorder, post-traumatic stress disorder and schizophrenia). We applied genomic structural equation modelling to derive a poverty common factor from household income, occupational income and social deprivation.

Adapting to poverty and dealing with gender: a comparative perspective on teenage pregnancy in the Peruvian Amazon.

Teenage pregnancy rates in the Peruvian Amazon are double the national average and among the highest in Latin America. Peruvian women living in rural, underserved and Amazon areas are more likely to become teenage mothers but the factors contributing to this socio-demographic trend are unclear. Thirty-one interviews and ethnographic observations of teenage and adult mothers living in the Peruvian Amazon were conducted to examine how social class and gender impacted their motherhood experiences. Despite preconceptions concerning the undesirability of teenage pregnancy, results show that teenage
