Melatonin vs. midazolam premedication in children: a double-blind, placebo-controlled study.

BACKGROUND AND OBJECTIVE: Unlike midazolam, melatonin premedication is not associated with cognitive impairment in adults despite its anxiolytic properties. The use of melatonin as a premedicant in children has not been reported. This randomized, double-blind, placebo-controlled study compared the perioperative effects of different doses of melatonin and midazolam in children. METHODS: Seven groups of children (n = 15 in each) were randomly assigned to receive one of the following premedicants. Midazolam 0.1, 0.25 or 0.5 mg kg(-1) orally, melatonin 0.1, 0.25 or 0.5 mg kg(-1) orally each mixed

10 microg CCK-4 premedication and 35% CO2 challenge in healthy volunteers.

1. The purpose of the study was to confirm whether a subthreshold dose of CCK-4 would enhance the vulnerability of healthy subjects to a 35% carbon dioxide challenge. 2. 40 subjects, with no prior or present psychiatric disorder and in good physical health were challenged with a vital capacity breath of a 35% carbon dioxide 65% oxygen mixture, immediately after an intravenous injection of 10 microg CCK-4 or placebo, according to a random order, double blind, separate group design. 3. Subjects reported significantly less anxiety and panic symptoms upon carbon dioxide after premedication with CC

Midazolam premedication and thiopental induction of anaesthesia: interactions at multiple end-points.

We have studied the effects of midazolam premedication on multiple anaesthetic end-points (hypnotic, loss of verbal contact (LVC); motor, dropping an infusion flex or bag (DF); analgesic, loss of reaction to painful stimulation (LRP); and EEG, attainment of burst suppression (BUR)) during induction by slow thiopental infusion at a rate of 55 mg kg-1 h-1. Patients received midazolam 0.05 mg kg-1 i.v. (group TM, n = 12) or no midazolam (group T0, n = 13). ED50 and ED95 values and group medians for times and doses at the end-points were measured. Midazolam premedication reduced significantly thio

Oral midazolam is an effective premedication for children having day-stay anaesthesia.

The effect of oral premedication was studied in a double-blind, randomised trial of 200 children undergoing day-stay anaesthesia. Midazolam 0.25 mg/kg, midazolam 0.5 mg/kg, diazepam 0.5 mg/kg or a placebo was given orally one hour prior to anaesthesia. Patient state was assessed at nine stages, from administration of the premedication up to and including induction of anaesthesia, using a four-point behavioural scale. Patient state was also assessed postoperatively in the recovery area and the day-stay ward. There was no difference between the four groups until induction of anaesthesia. At this

Postoperative sore throat. A comparison after premedication with papaveretum/hyoscine or temazepam.

A randomised double-blind trial was conducted to study the use of two commonly used premedication regimens and the subsequent incidence of sore throat. Fifty adult patients who underwent routine surgery were anaesthetised in a standard fashion after premedication with papaveretum and scopolamine or temazepam, and interviewed after operation to assess the presence of sore throat. A significantly higher incidence of sore throat was associated with the use of papaveretum and scopolamine.

[2 cases of brief asystole following premedication--a possible connection with Thalamonal?].

Two cases of cardiac arrest about 30 min after intramuscular administration of premedication are analyzed. The patients were two young men without serious diseases in the history. The premedication consisted of Thalamonal (Innovar) and atropine. The cardiac arrest responded readily to treatment and there were no further problems. The indication for Thalamonal for premedication is critically analyzed; our conclusion is that it should be used only with caution.

Comparison of oral slow release morphine (MST) with intramuscular morphine for premedication.

Oral morphine sulphate slow-release (MST) 40 mg and intramuscular morphine sulphate 10 mg, each administered with intramuscular atropine 0.6 mg, were compared in a randomised double-blind trial as premedication agents in 30 patients undergoing abdominal hysterectomy. Both formulations produced sedation but no anxiolysis in the anaesthetic room, as measured by 10 cm, horizontal linear analogue scales. There was no significant difference between the preparations in terms of postoperative pain, recorded either by the linear analogue scales or using a patient questionnaire. The usage of analgesics

Evaluating the Efficacy of Premedication in Preventing Hypersensitivity Reactions to Nonionic Contrast Agents.

Iodine-based contrast agents can induce various acute hypersensitivity reactions ranging from mild itching or vomiting shortly after administration to severe hypotension or loss of consciousness. In Japan, steroid premedication is commonly used to prevent acute hypersensitivity reactions. However, little clear evidence supporting its efficacy is available. In this study, we evaluated the effectiveness of premedication for acute hypersensitivity reactions induced by nonionic iodine contrast agents using propensity score matching. The participants included patients who were administered nonionic

[Premedical treatment of children and adolescents prior to painful procedures and general anaesthesia].

Preoperative anxiety and pain in children and adolescents are associated with postoperative complications. Despite relevant non-pharmacological treatment, the addition of premedical treatment may be needed. Traditionally, oral midazolam has been a favourite premedical treatment. However, several alternatives are available. Additionally, simultaneous administration of the medications to achieve synergy may be advantable. The choice of premedical treatment and method of administration depends on the individual child, the situation, and the preferences of the medical staff.

Comparison of Dexmedetomidine/Morphine and Xylazine/Morphine as Premedication in Isoflurane-Anesthetized Sheep.

In this study we investigated the sedative, anesthetic, and pulmonary histopathologic effects of dexmedetomidine/morphine (DM) and xylazine/morphine (XM) in sheep. We hypothesized that DM would provide profound sedation and better maintain physiologic parameters under anesthesia than XM in sheep undergoing laparoscopic surgery. Nineteen male sheep were premedicated with either DM (dexmedetomidine [0.006 mg/kg] and morphine [0.3 mg/kg]) or XM (xylazine [0.1 mg/kg] and morphine [0.3 mg/kg]). After DM or XM administration, 3 blinded veterinarians evaluated sedation scores (0 [no sedation], 1 [mil

Premedication in the Outpatient Setting: IV Cetirizine or IV Diphenhydramine? A Literature Review.

BACKGROUND: Premedication regimens to prevent infusion-related reactions typically include steroids and first-generation antihistamines, primarily diphenhydramine. With the recent approval of intravenous cetirizine, a second-generation antihistamine, for acute urticaria and angioedema, reevaluating premedication standard regimens is warranted. This review explores whether cetirizine can improve efficiency and reduce side effects in outpatient infusion settings. METHODS: A literature review was conducted using the PICO question: "In adult and pediatric outpatient infusion patients receiving pr

Impact of Premedication De-Escalation on Incidence of Infusion-Related Reactions With Daratumumab.

PURPOSE: Daratumumab is an anti-CD38 monoclonal antibody used to treat multiple myeloma and light chain amyloidosis. Because daratumumab may cause reactions after intravenous or subcutaneous (SC) administration, the manufacturer labeling recommends administration of premedications before every dose. Given incidence of infusion reactions appears to be rare after cycle 1, in April 2022, the Mayo Clinic implemented a practice change in which premedications were omitted from all daratumumab order sets after completion of cycle 1. This study investigated the safety of this practice. METHODS: A ret

Dexmedetomidine in combination with ketamine for pediatric procedural sedation or premedication: A meta-analysis.

OBJECTIVE: To evaluate effectiveness of combinational use of dexmedetomidine and ketamine (DEX-KET) for pediatric procedural sedation or premedication. METHODS: Relevant studies were identified after a literature search in electronic databases and study selection was based on precise eligibility criteria. Meta-analyses of mean differences were performed to examine differences in sedation onset and recovery times between DEX-KET and comparators. Changes from baseline in heart rate (HR), respiratory rate, oxygen saturation, and mean arterial pressure (MAP), were pooled. Meta-analyses of proport

Use of premedication with intravenous immune globulin in Kawasaki disease: A retrospective review.

BACKGROUND: Kawasaki disease (KD) is a significant febrile illness in children and is the leading cause of acquired pediatric heart disease in developed countries. Its recommended treatment is high-dose intravenous immune globulin (IVIG) plus aspirin. However, IVIG-related adverse events are seen frequently in this population. Premedication is commonly used to reduce this risk, but evidence supporting this practice is conflicting. Ultimately, practices vary among institutions and no standard guidelines regarding IVIG premedication currently exist. METHODS: Electronic medical records for pedia

Nebulized dexmedetomidine-lidocaine inhalation as a premedication for flexible bronchoscopy: a randomized trial.

BACKGROUND: Dexmedetomidine plus opioid infusion after topical anaesthesia with nebulized lidocaine for cough suppression is a commonly used method for flexible bronchoscopy. Recently, the use of dexmedetomidine as an additive to local anaesthetics has been reported to have several advantages over conventional intravenous administration. However, there are no data regarding the use of nebulized dexmedetomidine-lidocaine for topical anaesthesia as a premedication for flexible bronchoscopy. Therefore, this study compared the tolerability and safety of nebulized dexmedetomidine with that of conve
