Takayasu's arteritis after total proctocolectomy for ulcerative colitis: report of a case.

PURPOSE: Although coexistence of Takayasu's arteritis with ulcerative colitis is often reported, all these cases have been in patients whose colons were in situ. The aim of the present study was to describe a patient who developed Takayasu's arteritis after total proctocolectomy for ulcerative colitis. METHODS: The clinical course of a 42-year-old female who developed Takayasu's arteritis after total proctocolectomy for ulcerative colitis is described and the literature reviewed. RESULTS: Symptomatic arteritis developed in the left carotid artery four months after total proctocolectomy with

[Surgical treatment of ulcerous colitis].

Ulcerative colitis, being a typical medical disease, is encountered by the surgeon only in the stage of toxic megacolon or in case of sever complications during conservative treatment such as perforation, haemorrhage or obstruction. The authors operated six young women where they performed as the only choice total proctocolectomy with formation of an ileal reservoir which was sutured to the rectum stripped of the mucosa. Despite the serious condition at the time of operation, the patients survive the operation, although the percentage of postoperative complications is high, with the exception

[Anorectal localization of chronic inflammatory diseases of the intestines].

In Ulcerative Colitis (UC), the rectum is always involved, but the disease may be extended to the left colon (distal colitis) or the entire organ (pancolitis). Secondary perineal lesions (erythema, fissures, hemorrhoids) are limited to local irritation related to the diarrheic syndrome; specific ano-rectal lesions are related to the mucosal inflammatory process, but others pathological situations may be observed and are due to the postoperative status (stenotic ileo-anal anastomosis, pouch inflammation). Ano-rectal localisation of Crohn's Disease (CD), sometimes underestimated, is clinically m

Combined molecular and clinical approach for decision making for surgery in HNPCC patients: a report on three cases in two families.

Hereditary nonpolyposis colorectal cancer (HNPCC) is associated with highly penetrant germline mutations in mismatch repair genes. Due to a high lifetime risk in gene carriers for synchronous and for metachronous colorectal cancer and endometrial cancer in women, prophylactic and extended surgery are considered as options for gene carriers. A 54-year-old patient with a history of metachronous rectal cancer and a family history fulfilling the Amsterdam criteria presented with carcinoma of the cecum and highly dysplastic adenomas of the splenic flexure and descending colon. As a result of these

Endoscopic Treatment of Postoperative Bleeding, Bezoars, and Foreign Bodies.

Altered gastrointestinal anatomy is common in patients with inflammatory bowel disease, particularly in those who underwent bowel surgery. Commonly performed surgeries are bowel resection and anastomosis and strictureplasty for Crohn's disease; and restorative proctocolectomy with ileal pouch-anal anastomosis for ulcerative colitis. The area of anastomosis and suture line is at the greatest risk for the development of postoperative bleeding. Altered bowel anatomy, especially the presence of strictures, strictureplasty, or structural or functional pouch outlet obstruction, puts these patients a

Adenocarcinoma arising from along the rectal stump after double-stapled ileorectal J-pouch in a patient with ulcerative colitis: the need to perform a distal anastomosis. Report of a case.

Patients treated with restorative proctocolectomy for ulcerative colitis occasionally develop neoplasia from the rectal mucosal remnants. We report a case of a 65-year-old male who developed an adenocarcinoma from the rectal stump after a double-stapled ileorectal J-pouch for ulcerative colitis. We emphasize the need to perform the anastomosis either at the level of the dentate line or just cephalad to the anal transitional zone. Furthermore, when high-grade dysplasia at the rectum is evident, either an ileal pouch-anal anastomosis with mucosectomy or completion proctectomy with an end Brooke

Surgery for pediatric ulcerative colitis.

Restorative proctocolectomy with ileal pouch-anal anastomosis (IPAA) has become the "gold standard" operation for treatment of ulcerative colitis refractory to medical therapy. The J-pouch is the simplest to construct, produces the fewest complications, and is used most commonly. Long-term function with the IPAA is good in approximately 90% of cases, and patient satisfaction is high. Controversial aspects of the operation include 1) whether mucosectomy and hand-sewn IPAA is better than anal transition zone preservation and stapled anastomosis, 2) whether a temporary diverting ileostomy should

Functioning neorectum.

Seventeen patients (15 males, 2 females) underwent restorative proctocolectomy during the 4 year period 1983-87. Twelve patients had familial adenomatous polyposis. Two of these had superimposed malignancy--one each in the cecum and transverse colon. Five patients had ulcerative colitis. Ten pouch procedures were of the J type, six of the S type and one of W configuration. There was one death (mortality 6%). Mean stool frequency was 4.2 per day at one year after surgery and all patients were totally continent at this time. The ileal pouch-anal anastomosis provided a functioning neorectum with

Management of Acute and Chronic Pouchitis.

BACKGROUND: Restorative proctocolectomy with IPAA is the procedure of choice when colectomy is needed for medically refractory ulcerative colitis. Pouchitis is one of the most common complications among patients who have undergone IPAA and represents a spectrum of disease varying in both phenotype and clinical course. OBJECTIVE: This study aimed to assist clinicians and surgeons in the treatment of both acute and chronic pouchitis, including newer therapies and future directions. DIAGNOSIS AND MANAGEMENT: Diagnosis is made by endoscopy of the pouch with biopsy because other conditions may pr

Construction of J- and S-Pouches.

BACKGROUND: Pelvic pouch surgery evolved under the late Dr. Victor Fazio's influence. OBJECTIVE: This article aimed to describe construction of J- and S-pouches according to Fazio's teachings. TECHNICAL POINTS: There are several key points to consider for pelvic pouch construction, starting with the decision of performing a 2- or 3-stage procedure and handling of the rectal stump. At time of pouch construction, ileal reach must be assessed early in the operative course, and mesenteric lengthening maneuvers are deployed as warranted. If these maneuvers still do not allow sufficient length, al

Measurement of Disease Activity of Pouchitis.

BACKGROUND: Pouchitis is the most common inflammatory complication in ulcerative colitis patients undergoing postoperative construction of an IPAA. Pouchitis refers to a spectrum of diseases, and as such, it lacks a universally accepted definition as well as validated instruments to measure disease activity and treatment response. Assessing pouchitis activity is challenging, and methods for diagnosis and classification of severity of pouchitis are not universally agreed upon. CLINICAL FEATURES: Pouchitis is characterized by a constellation of clinical symptoms, including increased stool frequ

Relationship Between Stages of Ileal Pouch-Anal Anastomosis, Timing of Restoration of Fecal Continuity, and Pouchitis.

BACKGROUND: The most common complication following ileal pouch-anal anastomosis (IPAA) in patients with ulcerative colitis (UC) is pouchitis. AIMS: We aimed to investigate whether a shorter period between pouch creation and restoration of fecal flow through an IPAA was associated with an increased risk of development of pouchitis within the first 2 years after IPAA. METHODS: We performed a retrospective cohort study evaluating patients undergoing colectomy with IPAA for UC between January 1, 2004 and December 31, 2016. We used Kaplan-Meier testing and Cox Proportional Hazards Modeling to eva

Laparoscopic-assisted total colectomy with pouch reconstruction.

Whether in adults or children, benign diseases of the colon are ideally suited for laparoscopic-assisted resection. In children, the most frequent diseases leading to colectomy and pull-through include Hirschsprung's disease, ulcerative colitis, and familial polyposis. Laparoscopic proctocolectomy with J-pouch reconstruction is an ideal approach for children and adolescents who are generally stable but suffer from unremitting symptoms of ulcerative colitis. An early experience with this technique at the Children's Hospital in Alabama is described for children with ulcerative colitis.

[Indications for restorative rectal resection in carcinoma--complete vs. partial and local resection].

Rectal continence preservation became feasible in treatment of cancer of the lower third of the rectum based upon technical evolutions and better understanding of tumor biology. Absolutely necessary precondition is correct preoperative staging. Operative strategy is determined by tumor stage, localization, grade and continence function. In most cases the total mesorectal excision leads to tumor-free circumferential resection margins. Distal safety margins are assured by intraoperative frozen sections. Restorative approaches are contraindicated in case of sphincter infiltration. If continence f

Proctocolectomy and ileostomy to pouch surgery for ulcerative colitis.

The development of continence-preserving and sphincter-preserving procedures for operation of ulcerative colitis has a long and interesting history. Reported clinical results on the continent ileostomy (Kock pouch) and the pelvic pouch procedure have often been enthusiastic; and when confronted with the options patients have mostly been in no doubt in selecting "the best operation." However, even if the continent ileostomy and subsequently restorative proctocolectomy were great innovations, it is by no means obvious that they should be recommended as the first choice for all patients. For pati

The surgical management of constipation.

The anal physiology laboratory plays a very important role in the selection of patients for surgical treatment for constipation. Any report which does not include reference to these methods of evaluation will not be helpful since there are several causes of constipation. The current recommended treatment for slow transit constipation is still total abdominal colectomy with ileorectal anastomosis. Treatment of pelvic floor outlet obstruction seems to be best accomplished using muscle/sensory retraining techniques since this is a functional disorder rather than an anatomical or physiological dis
