Performance of Administrative Algorithms to Identify Interstitial Lung Disease in Rheumatoid Arthritis.

OBJECTIVE: To determine the performance of administrative-based algorithms for classifying interstitial lung disease (ILD) complicating rheumatoid arthritis (RA). METHODS: Participants in a large, multicenter RA registry were screened for ILD using codes from the International Classification of Diseases, Ninth Revision (ICD-9) and the ICD-10. Medical record review confirmed ILD among participants screening positive and a random sample of those screening negative. ICD and procedure codes, provider specialty, and dates were extracted from Veterans Affairs administrative data to construct ILD al

Rigid bronchoscopy and Y-stent for severe central malignant airway obstruction as a bridge to surgery requiring general anaesthesia.

A female patient in her 30s with a history of asthma presented with a chronic cough, worsening dyspnoea, dysphagia and a painless neck lump. Ultrasound revealed enlarged cervical lymph nodes and chest X-ray showed a large anterior mediastinal mass causing central airway obstruction. CT confirmed the mass partially encasing the superior vena cava. Due to concerns of airway collapse during surgery, interventional pulmonologists placed a silicone Y stent to restore airway patency before induction of anaesthesia for surgical lymph node biopsy. The excisional biopsy confirmed classic Hodgkin's lymp

Diagnostic value of nucleic acid amplification tests for tuberculous pleural effusion.

Diagnosing tuberculous pleural effusion (TPE) is challenging for pulmonologists and laboratory scientists. The gold standards for TPE diagnosis are pleural fluid Ziehl-Neelsen staining, Mycobacterium tuberculosis (Mtb) culture and pleural biopsy. These tools have limitations, including low sensitivity, long turnaround time and invasiveness. The nucleic acid amplification test (NAAT) is a rapid and minimally invasive tool for diagnosing TPE. This review summarizes the diagnostic accuracy of available NAATs for TPE, with a focus on the evidence from systematic reviews and meta-analyses. The NAAT

Acute Hemorrhagic Rectal Ulcer Complicated by Cytomegalovirus Enteritis following Steroid Pulse Therapy for Acute Exacerbation of Interstitial Pneumonia.

We herein report a rare case of acute hemorrhagic rectal ulcer (AHRU) complicated by cytomegalovirus enteritis following steroid pulse therapy for interstitial pneumonia. An 86-year-old woman underwent steroid pulse therapy for interstitial pneumonia. She was bedridden with dyspnea and suddenly developed melena. Colonoscopy revealed AHRU, which did not improve with conservative treatment, but did improve with ganciclovir administration for cytomegalovirus enteritis. This gastrointestinal complication has not received much attention by pulmonologists who perform steroid pulse therapy for inters

A 27-Year-Old Man With Intermittent Chest Pain During the Last 4 Years.

A 27-year-old man from Eritrea presented to the ED complaining about a progressively worse blunt chest pain in the anterior right hemithorax. Chest pain started 4 years ago and was intermittent. During the last 6 months, symptoms got worse, and the patient experienced shortness of breath in mild exercise. For this purpose, he visited another institution, where a chest radiograph was performed (Fig 1). He was advised to visit a pulmonologist for further evaluation, with the diagnosis of a loculated pleural effusion in the right upper hemithorax.

The Recognition-to-Practice Gap: Physicians' Approaches to Osteoporosis Assessment and Management in COPD.

BACKGROUND: Osteoporosis is a common yet frequently under-recognised comorbidity in patients with chronic obstructive pulmonary disease (COPD), yet its integration into routine COPD care remains poorly understood. This study evaluated physicians' perceptions, confidence, clinical practices, perceived barriers, and factors associated with routine osteoporosis risk assessment. METHODS: A national cross-sectional survey of general practitioners, pulmonologists, and critical care physicians practising in Saudi Arabia between October 2025 and March 2026 was conducted. The questionnaire assessed ph

Current Practices and Preferences Regarding Race and Spirometry Interpretation: A National Survey.

BACKGROUND: The American Thoracic Society recently recommended the use of race-neutral rather than race-specific equations for spirometry interpretation. The uptake of and clinicians' opinions about these practices remain unclear. RESEARCH QUESTION: What are clinicians' current practices and opinions around race-specific vs race-neutral reference equations? STUDY DESIGN AND METHODS: We conducted a national survey of pulmonologists identified through the American Lung Association and Pulmonary Fibrosis Foundation. Participants completed an electronic survey about their opinions and practices

The diagnostic trajectories of Danish patients with autoimmune rheumatologic disease associated interstitial lung disease: an interview-based study.

BACKGROUND: Autoimmune rheumatologic disease associated interstitial lung diseases (ARD-ILD) are rare conditions and the association between ARDs and respiratory symptoms often goes unrecognised by ARD patients and general practitioners (GPs). The diagnostic trajectory from the first respiratory symptoms to an ARD-ILD diagnosis is often delayed and may increase the burden of symptoms and allow further disease progression. The aim of this study was to 1) characterise the diagnostic trajectories of ARD-ILD patients and to 2) identify barriers for obtaining a timely ILD diagnosis based on the expe

The Mitral Valve and the Lungs.

TOPIC IMPORTANCE: Mitral valve disease has a significant impact on the respiratory system. Present-day pulmonologists must be aware of its myriad presentations and evolving treatment landscape. REVIEW FINDINGS: Mitral valve disease can lead to a variety of manifestations in the respiratory system. Elevated pressures in the pulmonary venous system can lead to pulmonary hypertension, hemoptysis, pulmonary edema, and parenchymal changes. Clinical presentations range from asymmetric pulmonary edema mimicking pneumonia to massive pulmonary hemorrhage. The management of mitral valve disease has cha

Medical thoracoscopy.

As opposed to 'video-assisted thoracoscopic surgery' which requires general anesthesia, double-lumen tracheal intubation and single lung ventilation, medical thoracoscopy (or 'pleuroscopy') is frequently performed in the respiratory endoscopy suite using local anesthesia. It can be done by well-trained physicians, either pulmonologists or thoracic surgeons, and its main indication is related to diagnosis and treatment of pleural effusions. Also, pneumothorax can be managed in most cases using medical thoracoscopy. This article focuses in particular on technical aspects of medical thoracoscopy,

Anesthetic Challenges in a Pregnancy Case With Congenital Kyphoscoliosis.

Kyphoscoliosis is a well-known spinal deformity. The abnormal curvature in both the coronal and sagittal planes presents unique challenges during pregnancy. This case discusses the management of a 27-year-old primigravida with thoracolumbar kyphoscoliosis, who underwent an emergency cesarean section at 39.3 weeks of gestation. An interdisciplinary team consisting of an obstetrician, pulmonologist, orthopedic surgeon, anesthesiologist, and physiotherapist collaborated in her care. In such cases, successful outcomes require a tailored approach that prioritizes maternal-fetal well-being and minim

Tissue Eosinophils Threshold and its Association With Adult-Onset Asthma in Chronic Rhinosinusitis.

INTRODUCTION: Tissue eosinophil counts (TEC) might serve as a biomarker linking chronic rhinosinusitis (CRS) and the presence of adult-onset asthma. This study aimed to determine if TEC in sinus mucosa/polyps in CRS patients is an independent indicator of asthma and to identify its optimal cut-off point. METHODS: This cross-sectional study was conducted on primary CRS patients scheduled for surgery. All patients were assessed by a pulmonologist for asthma diagnosis. Tissues were collected during surgery and evaluated for TEC. Logistic regression and receiver operating characteristic analysis

Persistent Dyspnea in a 74-Year-Old Man With Normal Spirometry and Lung Volumes.

A 74-year-old man was referred to a pulmonologist for evaluation of a 1-year history of nonproductive cough and progressive exertional dyspnea. He was initially evaluated by his primary care physician, where he had spirometry that was negative for any obstructive or restrictive lung disease. An echocardiogram showed a normal left ventricular ejection fraction, with no wall motion abnormality or valvular heart disease. He had an outpatient chest radiograph performed (Fig 1), and he was subsequently treated empirically for a COPD exacerbation with 5 days of oral prednisone and azithromycin. He w

A 10 years audit of cardiothoracic referrals from a pulmonology service in a developing country: impact of patient human immunodeficiency virus status.

BACKGROUND: There are few studies that have documented the clinical presentation, complication rate and outcome of cardiothoracic referrals from a Pulmonology Service, particularly in developing countries. Furthermore, the impact of human immunodeficiency virus (HIV) status on such referrals and their outcomes is not well documented. METHODS: This was a retrospective audit of combined pulmonologist/cardiothoracic consultations, undertaken among the tertiary academic hospitals attached to the University of the Witwatersrand, Johannesburg, South Africa, over a 10 years period (2001-2010) in ord

Dysplasias in the Child's Spine.

Many of the skeletal dysplasias impact the spinal column causing scoliosis, kyphosis, stenosis, and/or instability at various segments of the spine. Understanding how a particular dysplasia is likely to manifest is important to ensure proper screening and treatment decisions. Equally important is understanding the natural history of specific spine abnormalities, such as awareness of the resolution of the cervical kyphosis common to infants with diastrophic dysplasia. Many patients with rarer dysplasias are best cared for by teams of providers including neurosurgeons, orthopedic surgeons, genet

A 3-Year-Old Girl With a Mediastinal Mass.

A 3-year-old girl was referred to a pediatric pulmonologist for dyspnea and recurrent upper respiratory tract infections (RTIs). The patient was born full term to unrelated Dutch parents after an uneventful pregnancy and birth. The year before presentation, she had suffered from pneumonia and > 10 upper RTIs. Apart from the recurrent RTIs, which started in infancy, her medical history was not significant and did not include allergies or eczema. An adenotonsillectomy was performed at the age of 2 years, and she was treated with multiple antibiotic regimens and inhalation therapy with salbutamol

[Laboratory diagnostics of pleural effusion].

Pleural effusion is a frequent reason for a pulmonologist's investigation. Many pulmonary and extrapulmonary causes of pleural effusion exist. Heart failure, pneumonia and malignancies are the most frequent among them. Laboratory examination of pleural liquid is a corner stone of diagnostics. We use various biochemical, microbiological, cytologic and other methods. The first step is a differentiation between transudate and exudate. If the laboratory examinations are unsuccessful, we can use invasive procedures - pleural biopsy and thoracoscopy. Despite all modern diagnostic methods the causes

Basic assessment of chronic cough in primary care and referral pathways of patients to different specialists: plain language summary of publication.

People with chronic cough (a cough lasting more than 8 weeks) are often referred to different specialists and undergo numerous diagnostic tests, but clear guidance is lacking. This work summarizes a consensus (an agreement) among medical specialists who are involved in managing people with chronic cough: primary care physicians (family doctors), pulmonologists (doctors who specialize in lung conditions), allergists (medical professionals specializing in allergies) and ear, nose and throat (ENT) specialists. They discussed how to perform a basic assessment of people with chronic cough in primar
