Of the newer laparoscopic procedures for colorectal cancers the abdominoperineal resection of the rectum was previously communicated in this journal. We report herein the case of a 38-year-old man found to have a rectal arteriovenous malformation (AVM). The patient was admitted to our hospital for investigation of fresh anal bleeding and general malaise. Barium-enema examination showed a slightly elevated lesion in the rectum, and a selective superior rectal angiogram subsequently revealed an AVM in the peripheral region of the superior rectal artery, which was presumed to be the cause of the anal bleeding. Injection of sclerosing solutions into the rectal submucosa or into the perirectal space is one of the many therapies designed to correct rectal prolapse. The posterior approach to the low and mid-rectum is a good and anatomical way to treat high fistulas' problems, perirectal tumors and villous adenoma. It is also possible to treat by this method some rectal adenocarcinomas in special situations and in high-risk patients. A good evaluation of the disease if possible by rectal endosonography, has to be done. Coccygectomy is necessary but the transsphincteric route is not mandatory and has never been used here. 9 cases are reported with relatively good results.

Digital rectal examination, measurement of prostatic-specific antigen, and transrectal ultrasound, with or without prostate biopsy, are the urologist's primary tools for diagnosing carcinoma of the prostate. However, in the patient who has previously undergone abdominoperineal resection of the rectum such procedures are not possible. We therefore describe a simple, cost-effective method for evaluating the prostate in the patient without a rectum, via transperineal ultrasound and prostate biopsy. This procedure provides the urologist with a solution to a heretofore difficult problem.

In the period from 1.8.89 to 1.7.92 140 adenomas and 63 carcinomas were locally resected from the rectosigmoid area with the instruments of TEM. The mortality was 0.5%, the rate of dehiscence of the suture 9.8%. 75% of these were treated conservatively. One of the 29 patients with locally resected pTl-low risk-carcinoma developed a recurrence so far. The rate of recurrent adenomas was 1.4%. Three hundred and seventy-two patients with colorectal tumours treated by curative resection between January 1982 and January 1992 were reviewed in order to determine the role of colonoscopy and the outcome of patients with multiple tumours. The posterior transsphincteric approach was used in the surgery of the rectum for 18 patients. They included 7 patients with rectal cancer, 7 villous adenoma malignant change 4, 1 carcinoid, and 3 benign stenosis, parasitic granulation and rectocele. The method of the operation and its main poits were described in detail; the indications of the operation were discussed. The anastomosis was influenced by the length, blood supply and cleaning of the proximal colon. Anastomotic leakage and pelvic infection were the main surgical complications.

Faecal stream diversion may induce inflammatory changes in the defunctioned segment of the large intestine. Five studies presented at the 1992 ASCO meeting are analysed. Kligerman's study was designed to determine if pre-treatment with WR-2721 could protect normal tissues from the toxicities induced by radiation therapy (in 100 patients with advanced rectal cancer). A consecutive series of 303 patients with carcinoma of the rectum and distal sigmoid colon treated by a single surgeon over a 10-year period are reported. Of these, 202 underwent an anterior resection, 85 an abdominoperineal excision of the rectum and 16 a coloanal anastomosis. Surgery was considered palliative in 52 patients undergoing anterior resection and 24 undergoing abdominoperineal resection. A general review is given of the intussusception of the rectum, the paradoxical contraction of the pelvic floor syndrome and of the solitary ulcer syndrome. The latter is in most cases a symptom of the other two as is the descending perineum syndrome. The conditions are diagnosed by defecography, proctosigmoidoscopy and EMG. The etiologies are unknown. The treatment is either conservative or operative.

Three hundred and thirty one patients with cancer of the rectum and rectosigmoid junction, operated on during the period 1968-1985 are here examined. The study shows the prognostic value and the impact that stage, histopathological grade, number and location of nodal metastases, morphology of the primary tumour, occurrence of obstruction, perforation, and haemorrhage have on the natural history of the tumour. Free perforation of the colon and rectum is an acute surgical condition. Possible factors affecting the prognosis include age, sex, underlying disease, surgical procedures, duration from onset to surgical treatment, general condition before surgery and degree of contamination. Colon and rectal endometriosis is a relatively rare entity that may have a wide array of clinical symptomatology and radiographic findings. Thirty-two patients with a diagnosis of colon or rectal endometriosis were seen and treated at the Ferguson Clinic between 1960 and 1986.
