Lung volume reduction after stereotactic ablative radiation therapy of lung tumors: potential application to emphysema.

PURPOSE: Lung volume reduction surgery (LVRS) improves dyspnea and other outcomes in selected patients with severe emphysema, but many have excessive surgical risk for LVRS. We analyzed the dose-volume relationship for lobar volume reduction after stereotactic ablative radiation therapy (SABR) of lung tumors, hypothesizing that SABR could achieve therapeutic volume reduction if applied in emphysema. METHODS AND MATERIALS: We retrospectively identified patients treated from 2007 to 2011 who had SABR for 1 lung tumor, pre-SABR pulmonary function testing, and ≥6 months computed tomographic (CT)

Proton stereotactic radiosurgery for the treatment of benign meningiomas.

PURPOSE: Given the excellent prognosis for patients with benign meningiomas, treatment strategies to minimize late effects are important. One strategy is proton radiation therapy (RT), which allows less integral dose to normal tissue and greater homogeneity than photon RT. Here, we report the first series of proton stereotactic radiosurgery (SRS) used for the treatment of meningiomas. METHODS AND MATERIALS: We identified 50 patients with 51 histologically proven or image-defined, presumed-benign meningiomas treated at our institution between 1996 and 2007. Tumors of <4 cm in diameter and loca

Relationship between cumulative radiation dose and salivary gland uptake associated with radioiodine therapy of thyroid cancer.

AIM: To estimate the individual absorbed dose to the parotid and submandibular salivary glands in radioiodine therapy and its dependence from the previous cumulative therapy. METHODS: Fifty-five patients with differentiated thyroid carcinoma after thyroidectomy received 1-21 GBq (131)I using single activities of 1-6 GBq. The patients were stratified according to the cumulative activities into low-activity (1-2 GBq), middle-activity (3-7 GBq), and high-activity groups (9-21 GBq). The time-activity curves over the respective salivary glands were derived from multiple static calibrated images me

A Prospective Study of Proton Beam Reirradiation for Esophageal Cancer.

PURPOSE: Reirradiation to the esophagus carries a significant risk of complications. Proton therapy may offer an advantage in the reirradiation setting due to the lack of exit dose and potential sparing of previously radiated normal tissues. METHODS AND MATERIALS: Between June 2010 and February 2014, 14 patients with a history of thoracic radiation and newly diagnosed or locally recurrent esophageal cancer began proton beam reirradiation on a prospective trial. Primary endpoints were feasibility and acute toxicity. Toxicity was graded according Common Toxicity Criteria version 4.0. RESULTS:

Modelling the impact of fractionation on late urinary toxicity after postprostatectomy radiation therapy.

PURPOSE: To fit urinary toxicity data of patients treated with postprostatectomy radiation therapy with the linear quadratic (LQ) model with/without introducing a time factor. METHODS AND MATERIALS: Between 1993 and 2010, 1176 patients were treated with conventional fractionation (1.8 Gy per fraction, median 70.2 Gy, n=929) or hypofractionation (2.35-2.90 Gy per fraction, n=247). Data referred to 2004-2010 (when all schemes were in use, n=563; conventional fractionation: 316; hypofractionation: 247) were fitted as a logit function of biological equivalent dose (BED), according to the LQ model

Combined TL and 10B-alanine ESR dosimetry for BNCT.

The dosimetric technique described in this paper is based on electron spin resonance (ESR) detectors using an alanine-boric compound acid enriched with (10)B, and beryllium oxide thermoluminescent (TL) detectors; with this combined dosimetry, it is possible to discriminate the doses due to thermal neutrons and gamma radiation in a mixed field. Irradiations were carried out inside the thermal column of a TRIGA MARK II water-pool-type research nuclear reactor, also used for Boron Neutron Capture therapy (BNCT) applications, with thermal neutron fluence from 10(9) to 10(14) nth cm(-2). Tumorigenic potential of high-Z, high-LET charged-particle radiations.

The potential for radiogenic neoplasia from charged-particle irradiation has been estimated using the Harderian gland of the mouse as a test system. Particles ranging in Z from Z = 1 (proton) to Z = 41 (niobium), in energy from 228 to 670A MeV, and in LET from 0.4 to 464 keV/microns were produced at the Lawrence Berkeley Laboratory BEVALAC. Expression of the tumorigenic potential of the initiated cells was enhanced by hormones from isogeneic grafts of pituitaries. The goal of the studies was to estimate the initial slope of the relationship between increased tumor prevalence at 16 months after

Estimating historical radiation doses to a cohort of U.S. radiologic technologists.

Data have been collected and physical and statistical models have been constructed to estimate unknown occupational radiation doses among 90,000 members of the U.S. Radiologic Technologists cohort who responded to a baseline questionnaire during the mid-1980s. Since the availability of radiation dose data differed by calendar period, different models were developed and applied for years worked before 1960, 1960- 1976 and 1977-1984. The dose estimation used available film-badge measurements (approximately 350,000) for individual cohort members, information provided by the technologists on their

Toward Optimal Fractionation Schemes in Nonuniform Radiation Therapy: Exploiting Helical Tomotherapy's Thread Effect for Gynecological Cancer Treatment.

PURPOSE: This study aimed to optimize dose fractionation for helical tomotherapy-based spatially fractionated radiation therapy (HT-SFRT) in gynecologic cancers by exploiting the "thread effect" to induce cohort effects. METHODS AND MATERIALS: HT-SFRT plans with varying parameters (pitch: 0.3-0.5; field width: 2.512 and 5.048 cm; modulation factor: 1.0-4.0) were applied to HeLa cells to identify the optimal configuration maximizing the peak-to-valley dose ratio. The cohort effect threshold was determined through quantification of DNA damage (γ-H2AX) and cell viability. Clinical validation inv

Biologic dosimetry of 188Re-HDD/lipiodol versus 131I-lipiodol therapy in patients with hepatocellular carcinoma.

UNLABELLED: One approach to treatment of primary hepatocellular carcinoma (HCC) is intraarterial injection of (131)I-lipiodol. Although clinical results have been positive, the therapy can be improved by using (188)Re instead of (131)I as the radionuclide. (188)Re is a high-energy beta-emitter, has a shorter half-life than (131)I, and has only low-intensity gamma-rays in its decay. The present study compared the cytotoxic effect of the radionuclide therapy in HCC patients treated with (131)I-lipiodol and (188)Re-4-hexadecyl 2,2,9,9-tetramethyl-4,7-diaza-1,10-decanethiol (HDD)/lipiodol. Oxidative stress imaging in live animals with techniques based on electron paramagnetic resonance.

Oxidative stress has been the object of considerable biological and biochemical investigation. Quantification has been difficult although the quantitative level of products of biological oxidations in tissues and tissue products has emerged as a widely used technique. The relationship between these products and the amount of oxidative stress is less clear. Imaging oxidative stress with electron paramagnetic resonance related magnetic resonance imaging, while not addressing the specific issue of quantification of initiating events, focuses on the anatomic specific location of the oxidative stre

Biological effects induced by K photo-ionisation in and near constituent atoms of DNA.

In order to assess the lethal efficiency and other biological effects of inner shell ionisations of constituent atoms of DNA ('K' events), experiments were developed at the LURE synchrotron facility using ultrasoft X rays as a probe of K events. The lethal efficiency of ultrasoft X rays above the carbon K threshold was especially investigated using V79 cells and compared with their efficiency to induce double strand breaks in dry plasmid-DNA. A correlation between the K event efficiencies for these processes is shown. Beams at 340 eV were found to be twice as efficient at killing cells than we

[Action of protons with 25 and 50 MeV energies on the chromosomes of human peripheral blood lymphocytes in vitro].

The frequency and type of chromosomal abberations in human lymphocytes induced by 25, 50, 645 MeV protons and 200 kv x-rays were studied. It was shown that the yield of one-hit acentric fragments increased linearly and that of double-break aberrations increased exponentially with the irradiation dose. It was demonstrated that under the influence of radiations with high linear energy transfer the number of paired fragments increased and that of dicentrics and rings decreased. The test of total yield of chromosomal aberrations and their specific types helped to derive coefficients of relative bi
