OBJECTIVES: We retrospectively evaluated the results of patients who underwent arthroscopic-assisted mini-open repair of rotator cuff tears.
METHODS: Twenty-three patients (7 males, 16 females; mean age 56 years; range 41 to 75 years) underwent arthroscopic-assisted mini-open repair for nonretracted rotator cuff tears. The right shoulder was involved in 15 patients and the left in eight patients; 83% being on the dominant side. The range of motion was measured with a goniometer, and muscle strength by manual examination. The patients were assessed by physical examination, the American Shoulde

BACKGROUND: Rotator cuff lesions are common in the community but reproducibility of tests for shoulder assessment has not been adequately appraised and there is no uniform approach to their use.
OBJECTIVE: To study interrater reproducibility of standard tests for shoulder evaluation among a rheumatology specialist, rheumatology trainee, and research nurse.
METHODS: 136 patients were reviewed over 12 months at a major teaching hospital. The three assessors examined each patient in random order and were unaware of each other's evaluation. Each shoulder was examined in a standard manner by reco

OBJECTIVE: The purpose of our study was to determine the interobserver variability in the sonographic evaluation of the rotator cuff.
SUBJECTS AND METHODS: Two radiologists independently scanned 61 patients with shoulder pain. Each radiologist categorized the rotator cuff as normal, partially torn, or fully torn. When a tear was present, the tendons involved were specified. All diagnoses were made prospectively without knowledge of the findings of the other radiologist.
RESULTS: The radiologists were in full agreement in the categorization of 92% (56/61) of the patients. In four of the five

Fifty-one patients treated for full-thickness rotator cuff tears were followed up an average of 4 years after surgery (range, 2-6.4 years). The shoulder function was assessed according to the Constant classification, each patient did strength measurements, and had electromyographic evaluation of the supraspinatus and infraspinatus muscles. The mean value of the Constant score for the surgically treated shoulder was 2.7 points. Forty-five patients (88.2%) had satisfactory scores and six patients (11.8%) had unsatisfactory scores. The result of treatment was significantly correlated to the size

The purpose of this study was to determine the morphologic characteristics of the glenoid osteophyte and its relationship to rotator cuff tears. Eighty-six cadaveric shoulders were studied. After inspection of cuff pathology, the glenoid was examined with soft x-rays. Histologically, degenerative changes of the articular cartilage and the labrum were assessed. There were 30 shoulders with rotator cuff tears (35%). Two types of glenoid osteophyte were identified: angular and hooked. Both osteophytes were commonly observed in the anterior to inferior portions of the glenoid. The length of hooked

Most of the papers in the literature are on rotator cuff lesions. This paper will deal with a somewhat forgotten part of the shoulder, the joint and the nontendinous structures around the joint. Since 1985, we have performed ultrasound examinations of the shoulder. We have collected many cases and some of them will be performed. The shoulder joint can be partly examined by ultrasound. We can get some information about joint effusions, humeral head cartilage, some part of the labrum, loose bodies, ganglions and fractures around the joint. The acromio-clavicular joint is also part of the examina

BACKGROUND: The clinical outcome for patients with documented rerupture after open repair of one or more rotator cuff tendons is not well known. The purpose of this study was to evaluate the clinical outcomes of a consecutive series of rotator cuff reruptures after repair and to provide information concerning the advisability of rotator cuff repair in situations in which there may be a high probability of rerupture.
METHODS: During prospective follow-up after rotator cuff repairs, we detected, with magnetic resonance imaging, structural failure of the repair in twenty patients, who had a mean

A clinical study of 200 unselected elderly subjects with an average age of 78 years living in retirement homes was performed to determine the prevalence of clinically manifest rotator cuff tears and the relevance of the tear for daily life. A comprehensive questionnaire was performed and the examination included the UCLA- and Constant-shoulder assessment scales and a multitude of special tests. The criteria for clinical diagnosis of a rotator cuff tear were a positive Jobe-test, diminished external rotation, a positive zero-degree-abduction test, the drop arm sign, pseudoparalysis and atrophy.

Rotator cuff deficiency with associated glenohumeral arthritis poses a formidable clinical challenge. Humeral head replacement with maintenance of the coracoacromial arch is the current treatment of choice. Properly performed arthroplasty can provide pain relief and modest gains in motion.

Shoulder function was assessed in 37 patients with a mean age of 57 years after acromioplasty and repair of full-thickness rotator cuff tears were performed. The cuff tears were closed without transfer of other muscles or use of synthetic materials. At review 2 years after surgery 32 patients had significant pain relief and improved shoulder function. Range of motion, muscle strength, and endurance were increased. The mean Constant score was 77, and 80% of the patients were graded as having mild or disability. Radiographic measurements disclosed an increased supraspinatus outlet area after sur

A modified Neer acromioplasty, subacromial decompression, and débridement of massive, irreparable lesions of the supraspinatus and infraspinatus tendons was performed in fifty-seven patients. Fifty patients (fifty-three shoulders) were followed for an average of six and one-half years. The average age of the patients was sixty years (range, thirty-eight to seventy-four years). The results, as rated on the basis of pain, function, range of motion, strength, and satisfaction of the patient, were satisfactory in forty-four shoulders (83 per cent) and unsatisfactory in nine (17 per cent). Thirty-seven patients older than 40 years of age were seen after sustaining primary anterior dislocations of the shoulder. An associated rupture of the rotator cuff in each patient had been missed, often being mistaken for an axillary neuropathy. Eleven of these patients developed recurrent anterior instability that was due to rupture of the subscapularis and anterior capsule from the lesser tuberosity. In no patient was there a Bankart lesion. Repair of the capsule and subscapularis restored stability in all of the patients with recurrence.

Thirty-one patients with arthroscopically documented partial thickness rotator cuff tears treated by arthroscopic debridement of the lesion were retrospectively reviewed. The patients had had symptoms for an average of 20.5 months prior to surgery. Twenty-two of these 31 shoulders also had bursoscopy, with 18 having arthroscopic subacromial decompression. The results were graded by both the UCLA Shoulder Rating Scale and Neer's criteria. Twenty-six (84%) of the patients had satisfactory results with the remaining 5 (16%) patients having unsatisfactory results. A classification system for the s

Rotator cuff tears (RCT) are frequent and increase with age. Why do only a relatively small percentage cause a permanent severe handicap justifying surgery? Is there a relationship between size and site of the tear and the clinical symptoms? How do they influence the postoperative result? These questions were answered in a prospective study of 76 operated patients. We adopted D. Patte's classification of the RCT into four groups according to the site and size of the lesion. Our figures were compared with those obtained by Patte in an analogous study of 256 cases. Of the tears in our series, 56

BACKGROUND: Massive rotator cuff tears (MRCTs) involving the rotator cable are associated with increased tendon retraction, fatty infiltration, and high retear rates after repair. Biceps anterior cable reconstruction (ACR) is a surgical technique that augments rotator cuff repair by reconstructing the anterior cable with the long head of the biceps tendon (LHBT), improving greater tuberosity coverage and biomechanical stability.
INDICATIONS: Biceps ACR is indicated for full-thickness MRCTs with anterior cable involvement in patients with an intact LHBT and no advanced glenohumeral arthritis.

BACKGROUND: Bariatric surgery (BS) is an increasingly utilized intervention for the treatment of obesity. However, BS is also associated with postoperative nutritional deficiencies that may affect healing rates after orthopaedic procedures.
PURPOSE: To compare failure rates and patient-reported outcomes after arthroscopic rotator cuff repair (RCR) between patients with and without a history of BS.
STUDY DESIGN: Cohort study; Level of evidence: 3.
METHODS: Patients in a single institution with a history of BS who underwent arthroscopic RCR for full-thickness supraspinatus tears were identifi
