Scapular upward rotation during intraoperative functional zero-position assessment in relation to postoperative outcomes and elevated alignment after reverse total shoulder arthroplasty.

PURPOSE: Scapulothoracic (ST) compensation is common after reverse total shoulder arthroplasty (rTSA); however, the clinical relevance of intraoperative scapular motion remains unclear. The functional zero-position denotes humeral shaft-scapular spine alignment during passive scaption. This study evaluated whether scapular upward rotation required to achieve this position was associated with postoperative function and elevated alignment. METHODS: We retrospectively analysed 28 shoulders with cuff tear arthropathy treated with rTSA in which the functional zero-position was fluoroscopically con

Clinical and radiological evaluation of the Bristow-Latarjet procedure in patients with 30 or more years of follow-up.

BACKGROUND: The Bristow-Latarjet surgery is the procedure most performed for recurrent anterior shoulder dislocation. There are few studies with a follow-up of more than 20 years. Because it is a nonanatomical technique, there is interest in knowing the clinical and radiological results and the rates of complications in the long term. METHODS: We performed a retrospective case series study. Patients with recurrent anterior dislocation of the shoulder who underwent the Bristow-Latarjet procedure were included and followed-up clinically for at least 30 years. We evaluated clinical scores-the Ro

Pain Management Strategies in Reverse Total Shoulder Arthroplasty.

PURPOSE OF REVIEW: Reverse total shoulder arthroplasty (rTSA) is a commonly utilized procedure for rotator cuff arthropathy, irreparable rotator cuff tears, and complex proximal humerus fractures. Although rTSA significantly improves long-term pain and function, postoperative pain remains common and is closely associated with increased opioid consumption, prolonged hospitalization, delayed recovery, and reduced patient satisfaction. Within the modern context of the opioid epidemic, optimizing pain control while minimizing narcotic usage is essential. This review summarizes recent evidence on p

Central fixation element type and length affect glenoid baseplate micromotion in reverse shoulder arthroplasty.

BACKGROUND: Reverse shoulder arthroplasty (RSA) is commonly used to treat patients with rotator cuff tear arthropathy. Loosening of the glenoid component remains one of the principal modes of failure and represents a significant complication that requires revision surgery. This study assessed the effects of various factors on glenoid baseplate micromotion for primary fixation of RSA. MATERIALS AND METHODS: A half-fractional factorial design of experiment was used to assess 4 factors: central element type (central peg or screw), central cortical engagement according to length (13.5 or 23.5 mm)

Reliability of the Classification for Assessing Hand Behind the Back Following Reverse Shoulder Arthroplasty.

BACKGROUND: Hand behind the back (HBB) after reverse shoulder arthroplasty (rTSA) is important for activities of daily living (ADLs), but the commonly used vertebral-level measurements are inconsistent and only moderately reliable. Collin et al. described three functional HBB motion patterns that capture how patients reach posteriorly (fluently vs, assisted) rather than how high they reach. We adapted this model into a bedside tool, the "HBB Classification", and compared its interobserver reliability with the traditional vertebral-level method. METHODS: In this prospective, single-centre, obs

Total shoulder arthroplasty preoperative planning: Does the patient's arm position in the CT-scan affect posterior humeral head subluxation?

BACKGROUND: Posterior Humeral Head Subluxation (PHHS) is a critical aspect to consider during the preoperative planning for Total Shoulder Arthroplasty (TSA). This study aims to investigate how variations in the initial arm position affect PHHS measurements obtained from CT scans. METHODS: A prospective analysis was conducted involving patients diagnosed with primary osteoarthritis (OA) or rotator cuff tear arthropathy (CTA) from February 2021 to February 2025. The focus was on cases planned for TSA, utilizing data sourced from a single shoulder arthroplasty planning platform (Equinoxe, Exact

Quantifying bone loss and lateralization with standardized baseplate versus augmented baseplates.

BACKGROUND: Reverse shoulder arthroplasty continues to be utilized for the treatment of cuff tear arthropathy, glenohumeral degenerative joint disease, and irreparable rotator cuff tears. With advancement in component designs, glenoid retroversion and inclination are now correctable with augmented baseplates. However, quantifying bone loss and lateralization compared to standard baseplates has not been studied. The purpose of the current study is to determine the volume of bone reamed and net lateralization with a standardized baseplate vs. augmented baseplate when glenoid inclination was corr

Reversed total shoulder arthroplasty for rotator cuff arthropathy is associated with increased scapulothoracic motion: A longitudinal two-year kinematic study.

BACKGROUND: Reversed total shoulder arthroplasty (RTSA) is a standard surgical procedure for the treatment of rotator cuff tear arthropathy (CTA), aimed at restoring active arm elevation. Shoulder elevation relies on both scapulothroacic (ST) and glenohumeral (GH) motion, but RTSA computer planning primarily focuses on the GH joint due to challenges in visualizing scapulothroacic (ST) motion. RESEARCH QUESTION: Does the scapulohumeral rhythm, by means of the relative contributions of ST rotation and GH elevation per degree of arm elevation, in a longitudinal setting for up to two years postop

Arthroscopically Assisted Lower Trapezius Transfer Using Peroneus Longus Autograft for Irreparable Posterosuperior Rotator Cuff Tears.

BACKGROUND: Massive retracted rotator cuff tears are disabling in physically active patients. In patients with persistent pain in whom nonoperative treatment has failed, multiple surgical treatment options are available. Lower trapezius tendon transfer is a promising surgical procedure that can decrease pain, improve external rotation strength, and recreate more normal glenohumeral kinematics. In the present video article, we describe the surgical technique for successful arthroscopic ("scopy")-assisted lower trapezius transfer (SALTT) with use of an easily accessible peroneus longus autograft

Artificial intelligence-based quantitative computed tomography assessment of shoulder muscle and its association with outcomes after reverse total shoulder arthroplasty.

BACKGROUND: Muscle atrophy and fatty infiltration of the shoulder muscles are known to influence clinical outcomes after reverse total shoulder arthroplasty (rTSA). Although quantitative evaluation of rotator cuff muscles has been increasingly reported, most previous studies have focused on intrinsic muscles and relied on two-dimensional or manual assessment methods. The purpose of this study was to evaluate shoulder muscle volume and composition using an artificial intelligence-based automated segmentation method on pre-operative computed tomography (CT) images and to investigate their associ

Effect of arm lengthening on functional outcomes and nerve injury after reverse shoulder arthroplasty: A prospective electrodiagnostic study.

BACKGROUND: The original Grammont Reverse Total Shoulder Arthroplasty (RTSA) increases the deltoid moment arm by medializing the centre of rotation, which is biomechanically advantageous. However, excessive tension can also produce complications, being one of the main risk factors for nerve injury. The purpose of the present study was to correlate arm lengthening after a medialized RTSA implant with functional outcomes and neurological injuries in the setting of rotator cuff tear arthropathy (CTA). METHODS: Prospective cohort study enrolled 40 consecutive patients who underwent RSA for CTA. E

Acromion and Clavicle Stress Fractures After Reverse Total Shoulder Arthroplasty Reflect Failure to Address Osteoporosis: A Case Report and Literature Review.

The exponential increase in the rate of reverse total shoulder arthroplasty (RTSA) has been accompanied by a rise in complication rates of this procedure. Of these, peri-prosthetic stress fractures can be particularly problematic due to their potential to cause significant impairment of shoulder function. Despite the association between these stress fractures and osteopenia/osteoporosis, pre-operative bone density assessment is not standard practice for elective RTSA. We report the case of a 68-year-old female patient who, at eleven weeks after elective RTSA (for rotator cuff-tear arthropathy)

Intraoperative functional zero-position is associated with superior outcomes after reverse total shoulder arthroplasty for cuff tear arthropathy: a retrospective comparative study.

BACKGROUND: Optimal implant positioning in reverse total shoulder arthroplasty (rTSA) remains challenging because of the lack of standardized intraoperative criteria. The "zero-position," defined as alignment of the humeral axis with the scapular spine, represents a balanced state in native shoulders. In rTSA, a similar alignment, termed the functional zero-position, may reflect balanced glenohumeral motion. This study evaluated its association with post-operative outcomes in patients with cuff tear arthropathy (CTA). METHODS: A retrospective review of 45 shoulders (43 patients) with CTA and
