Posterior scleritis. A cause of diagnostic confusion.

The referring diagnoses in seven women with posterior scleritis included intraocular neoplasm, retrobulbar tumor, choroiditis, and idiopathic central serous choroidopathy. In all cases, a localized area of intense posterior scleritis was responsible for the misdiagnosed ocular findings. Features that helped to correctly identify posterior scleritis were as follows: female sex; a history of anterior scleritis; a fundus mass the same color as normal adjacent pigment epithelium; choroidal folds; serous retinal detachment with cloudy fluid; early pinpoint leaking spots from fluorescein angiography

Exudative retinal detachment and scleritis in polyarteritis.

A 64-year-old white man, treated with systemic corticosteroids for five years, developed polyarteritis. He then developed a severe scleritis with an exudative retinal detachment in the right eye, which became blind and painful and was enucleated 23 months after onset of the scleritis. Histopathologic examination of the enucleated eye revealed granulomatous scleritis, chronic nongranulomatous uveitis, exudative retinal detachment, and perivasculitis of intrascleral, iris, ciliary body, and retinal blood vessels. Systemic findings were minimal and limited to elevated sedimentation rate, weight l

Unilateral Syphilitic Nodular Scleritis: A Case Report.

Syphilis is a systemic infectious disease known as the "Great Masquerader" for its ability to mimic a wide range of ocular conditions, potentially involving any ocular structure. We report the case of a 26-year-old man with a history of unprotected sexual intercourse who presented with a two-week history of a painful, red right eye without loss of visual acuity. Examination revealed nodular lesions on the temporal sclera and concomitant reddish-brown palmar spots. Laboratory tests were performed to determine the etiology of the anterior nodular scleritis. Serologic testing (treponema pallidum

Determinants of Clinical Outcomes After Infectious Scleritis.

PURPOSE: The aims of this study were to describe the clinical course of microbial infectious scleritis and identify factors associated with poor visual outcomes. METHODS: Data from 26 eyes of 26 patients with culture-proven bacterial or fungal scleritis presenting at a single tertiary center from January 1, 2007, to July 1, 2021, were reviewed. Thirty-six variables were analyzed for associations with poor vision [best-corrected visual acuity (BCVA) <20/200] or loss of vision (no light perception vision or requirement for enucleation or evisceration) at final visit. RESULTS: The mean age at i

Fungal panophthalmitis presenting as severe posterior scleritis.

PURPOSE: To report a highly unusual and fulminant case of infectious fungal panophthalmitis that initially presented as angle closure in the setting of posterior scleritis, culminating in the loss of the affected eye. OBSERVATIONS: A 57-year-old woman with a history of poorly controlled diabetes mellitus and autoimmune disease presented with a unilateral flat anterior chamber, highly elevated intraocular pressure (50-65 mmHg) and severe chemosis of the right eye. Initial VA was NLP in the affected eye. An ultrasound B-scan revealed a very pronounced T-sign and severely thickened posterior scl

Nodular anterior scleritis associated with Berger's disease.

A 45-year-old female patient presented with a complaint of right eye redness and pain for 7 days. She was under investigation for urinary abnormalities and reported a previous history of recurrent oral ulcers and ocular hyperemia in both eyes. Best-corrected visual acuity was 20/30 and 20/20 in the right and left eyes, respectively. Slit-lamp biomicroscopy of the ocular surface of the right eye revealed nasal scleral hyperemia that persisted after instillation of topical phenylephrine 10%, reinforcing the diagnosis of anterior scleritis. Renal biopsy showed immunoglobulin A immune complexes an

Choroidal involvement in non-infectious posterior scleritis.

PURPOSE: To provide a comprehensive overview of choroidal involvement in non-infectious posterior scleritis; including different imaging modalities and their clinical usefulness. METHODS: Narrative review. RESULTS: Posterior scleritis is an uncommon yet potentially sight-threatening inflammation of the sclera. During the disease process, inflammation can spread to the adjacent choroid, causing different manifestations of choroidal involvement: (1) increased choroidal thickness, (2) choroidal vasculitis, (3) presentation as a choroidal or subretinal mass in nodular posterior scleritis, and (4

Latent tuberculosis-related scleritis: a case report.

BACKGROUND: Scleritis is a painful inflammatory process centered in the sclera that may involve the cornea and the underlying uvea. The etiology is commonly idiopathic or autoimmune but some cases are associated with systemic infection such as tuberculosis. CASE PRESENTATION: In this report, we describe an unusual case of a female Moroccan patient who had a long history of bilateral recurrent scleritis associated with peripheral keratopathy and anterior uveitis. The patient was diagnosed with latent tuberculosis and responded to antitubercular therapy administrated after exclusion of other ae

Five-year outcome in immune-mediated scleritis.

BACKGROUND: Immune-mediated scleritis is a rare condition, and the information on the clinical course and complications is scarce. The aim of this study was to identify prognostic factors, complications, and therapeutic effects in patients with immune-mediated scleritis. METHODS: Patients with diagnosis of scleritis and a follow-up time of 5 years were identified. Systemic disease, laboratory investigations, type of scleritis, disease activity, therapy, and complications were recorded. The study design was a retrospective, non-comparative, interventional case series. RESULTS: Systemic diseas

Subconjunctival dexamethasone implant for non-necrotizing scleritis.

BACKGROUND: The purpose of this study is to report the management of non-necrotizing anterior scleritis with a single-dose subconjunctival 0.7 mg dexamethasone implant (Ozurdex®, Allergan, Inc., CA, USA). Six patients with clinical diagnosis of non-necrotizing anterior scleritis (diffuse, sectorial, and nodular) were submitted to subconjunctival injection of dexamethasone implant. The injection was performed under topical anesthesia at the slit lamp. All patients reported only mild discomfort related to the procedure. Five patients had subconjunctival hemorrhage. Follow-up was performed 1, 7,

Scleritis after proton therapy in uveal melanoma.

BACKGROUND: Sclera is a very radioresistant tissue and scleritis after proton therapy has not been described so far. HISTORY AND SIGNS: Four female patients, aged between 31 and 74 years, were treated with proton therapy for uveal melanoma (height range: 2.2 - 3.5 mm), located in the macula, the superior equator and 2 in the ciliary body. All patients had a history of a previous or active inflammatory disease and developed scleritis after radiotherapy. THERAPY AND OUTCOME: Two patients had infectious scleritis and were treated with adequate antibiotic therapy. After systemic corticotherapy,

Posterior scleritis resembling acute retinal necrosis syndrome.

PURPOSE: To report a challenging diagnosis of posterior scleritis in a patient with features of retinal necrosis. METHODS: Single observational case report. RESULTS: A 74-year-old man presented with atypical anterior scleritis in the left eye. Funduscopy showed subretinal inflammatory infiltration resembling acute retinal necrosis. The patient was treated with indomethacin, with resolution of the process. CONCLUSION: Typical signs of posterior scleritis include serous retinal detachment and choroidal folds in the posterior pole. Nonetheless, an atypical presentation can also occur, with per
