Controversy of hormone treatment and cardiovascular function: need for strengthened collaborations between preclinical and clinical scientists.

Over the last five years, there have been several major randomized clinical trials assessing the use of hormone therapy as an adjunct treatment for primary and secondary prevention of cardiovascular disease. Results of these trials have called into question existing dogma from epidemiological and basic science studies that estrogen provides protection against development of cardiovascular disease. When studies are evaluated for design, type and duration of hormone treatment, and outcomes, directions for future research become apparent. Improved hormone formulations and selective estrogen recep

Sex steroids and the GH axis: Implications for the management of hypopituitarism.

Growth hormone (GH) regulates somatic growth, substrate metabolism and body composition. Sex hormones exert profound effect on the secretion and action of GH. Estrogens stimulate the secretion of GH, but inhibit the action of GH on the liver, an effect that occurs when administered orally. Estrogens suppress GH receptor signaling by stimulating the expression proteins that inhibit cytokine receptor signaling. This effect of estrogens is avoided when physiological doses of estrogens are administered via a non-oral route. Estrogen-like compounds, such as selective estrogen receptor modulators, p

Effects of hormonally active agents on steroid hormone receptor expression and cell proliferation in the myometrium of ovariectomized macaques.

Hormone replacement therapy and selective estrogen receptor modulators have been controversial treatment options for postmenopausal women because of their potential health benefits and/or risks. In this study, we determine the effects of the hormonally active compounds, conjugated equine estrogens (CEE), medroxyprogesterone acetate (MPA), CEE + MPA, and tamoxifen (TAM) on the myometrium of ovariectomized macaques. Immunoexpression of estrogen receptor-α (ERα), progesterone receptor (PR), and Ki-67 in the myometrium is assessed. We found no significant difference in ERα myometrial expression in

Breast cancer chemoprevention: progress and controversy.

The chemoprevention of breast cancer using pharmacologic agents has had substantial clinical success. Randomized clinical trials evaluating selective estrogen-receptor modulators (SERMs) have shown that these agents reduce the incidence of breast cancer by up to 50% in healthy women at increased risk for the development of the disease. SERMs have been of particular value in women with biopsy-proven risk factors, including atypical hyperplasia or lobular carcinoma in situ of the breast. The agents of established value are important options for women today, and efforts are under way to identify

Comparative efficacy and safety of novel oral selective estrogen receptor degraders in ER+/HER2- advanced breast cancer: An updated systematic review and meta-analysis of randomized controlled trials.

BACKGROUND: Endocrine therapy (ET) plus CDK4/6 inhibitors (CDK4/6i) remains first-line (1 L) standard for ER+/HER2- metastatic breast cancer (mBC). However, ESR1 mutations lead to ET resistance and subsequent disease progression (PD). Limitations of intramuscular fulvestrant have led to the development of novel oral selective estrogen receptor degraders (SERDs). We aim to evaluate efficacy and safety of novel oral SERDs for ER+/HER2- mBC. METHODS: PubMed, Embase, and Cochrane were systematically searched for randomized controlled trials (RCTs) comparing oral SERDs with standard treatment. Pri

Positioning oral selective estrogen receptor degraders in patients with metastatic breast cancer.

Approximately 40 % of patients with estrogen receptor (ER)-positive, HER2-negative metastatic breast cancer (mBC) who receive first-line treatment with CDK4/6 inhibitors (CDK4/6i) and aromatase inhibitors (AIs) develop acquired ESR1 mutations, leading to ligand-independent ER activation and resistance to AIs. Fulvestrant, the first selective estrogen receptor degrader (SERD) approved for clinical use, was developed to address this resistance and has remained the standard second-line endocrine therapy. However, its clinical use is limited by intramuscular administration and low bioavailability.

Oral selective estrogen receptor degraders in hormone receptor-positive, HER2-negative advanced breast cancer: a systematic review and meta-analysis.

PURPOSE: The development of endocrine resistance is frequent in hormone receptor-positive, HER2-negative advanced breast cancer (HR + /HER2-ABC), particularly after CDK4/6 inhibitor exposure. Next-generation oral selective estrogen receptor degraders (SERDs) have been developed to improve estrogen receptor blockade; however, randomized trials have yielded heterogeneous results with uncertain clinical benefit. METHODS: A PRISMA 2020 compliant systematic review and meta-analysis with PROSPERO registration was conducted. PubMed, Embase, and Cochrane CENTRAL were searched through October 2025 for

An update on drugs for the treatment of menopausal symptoms.

INTRODUCTION: Vasomotor symptoms (VMS) and the genitourinary syndrome of menopause (GSM) are recognized as the most frequent and bothersome symptoms associated with menopause. There are different treatments for both groups of symptoms, being necessary to individualize them. AREAS COVERED: There are different therapies for VMS including hormonal treatments with estrogen, with and without progestins; the new alternative, tissue-selective estrogen complex (TSEC), tibolone, phytoestrogens and only progestins. Evidence also shows efficacy with selective serotonin reuptake inhibitors. Other nonhorm

Identifying and treating sexual dysfunction in postmenopausal women: the role of estrogen.

Sexual dysfunction is a common, but frequently overlooked, problem in postmenopausal women. At menopause, dramatic decreases in circulating estrogen concentrations cause physiologic changes that may affect sexual function, most notably the development of vulvovaginal atrophy. Vulvovaginal atrophy often leads to vaginal dryness, itching, irritation, reduced lubrication, dyspareunia, and vaginal bleeding associated with sexual activity. Estrogen deficiency also can affect other aspects of sexual function, including reduced vaginal blood flow and a reduced capacity for arousal and orgasm. As estr

Available medical choices for the management of menopause.

The age at which menopause occurs is a critical factor in the magnitude of its consequences. Most of the medium-to-long-term effects of oestrogen deprivation depend on their duration. The timing of the last menstruation is therefore important, but hypoestrogenic amenorrhoea during the reproductive age is also a relevant factor in the evaluation of individual risks. In recent years, moving post-menopausal women from the lowest point of ovarian hypofunction has been the most important motivation for developing guidelines for the hormonal management of menopause. However, recent data suggest that

New products and regimens (since 2003).

The downturn in the use of hormone replacement therapy since the original publication of the Women's Health Initiative (2002) and Million Women studies (2003) has now stabilized. New products are now being developed which maintain benefits and minimise risks. However, some useful products have been withdrawn by Pharma companies through profitability decisions; other products will regrettably not be launched despite favorable data. New low- and ultra-low-dose oral preparations (containing 0.3 mg conjugated equine estrogens and 0.5 mg estradiol, respectively) appear to maintain benefits for symp

[Sexual dysfunction in the menopause. Incidence, pharmacological treatment and side effects].

The frequency of female sexual dysfunction increases with age, and the menopause has a negative influence on sexual life. Pharmacological treatment options of female sexual dysfunctions in the menopause include hormone therapy and sildenafil. Few randomised controlled studies have been done, and there is evidence suggesting that systemic hormone therapy, such as estrogen, estrogen/progesterone, estrogen/testosterone and tibolone, has a positive impact on sexual dysfunction in the menopause. There is evidence that local estrogen relieves vaginal dryness and dyspareunia. The recent discoveries o

Decision making in the context of breast cancer chemoprevention: patient perceptions and the meaning of risk.

Chemoprevention with selective estrogen receptor modulators (SERMs) is considered one of the most promising risk reduction options to date in the United States. Tamoxifen and raloxifene are both approved by the U.S. Food and Drug Administration (FDA) for breast cancer risk reduction. However, despite endorsement from the American Society for Clinical Oncology and the National Comprehensive Cancer Network, uptake remains low. Decision aids have been successful in improving women's understanding and knowledge about the risk-benefit trade-offs in decision making regarding SERMs. However, increase

The triphenylethylenes, a novel class of antifungals.

ABSTRACT New antifungals are needed, particularly in the developing world, to treat life-threatening fungal infections, such as cryptococcosis. Drug repurposing is one strategy to identify new drug-like compounds, but it is often difficult to identify a mechanism of action. Here we discuss the outstanding effort by Butts et al. to identify calmodulin as an antifungal target of repurposed estrogen receptor antagonists [A. Butts, K. Koselny, Y. Chabrier-Roselló, C. P. Semighini, Y. C. S. Brown, et al., mBio 5(1):e00765-13, 2014, doi:10.1128/mBio.00765-13]. The authors show that these compounds b

Ospemifene for the treatment of dyspareunia in postmenopausal women.

Ospemifene is a third-generation selective estrogen receptor modulator (SERM), structurally closely related to toremifene. Clinical studies in postmenopausal women with vulvovaginal atrophy demonstrated that it produced significant improvements in the structure of the vagina and its pH, and significantly reduced dyspareunia, the main complaint of the women treated. Preclinical studies demonstrated that ospemifene, unlike tamoxifen, did not produce DNA adducts, suggesting that it has less carcinogenic potential than tamoxifen. Preclinical and clinical studies showed that ospemifene has an agoni

A randomized, double-blind, placebo-controlled trial of ormeloxifene in breast pain and nodularity.

BACKGROUND: Breast pain and non-discrete breast nodularity are common in women. METHODS: We did a randomized, double-blind, placebocontrolled trial of oral ormeloxifene 30 mg, a selective oestrogen receptor modulator (SERM) or placebo twice a week for 3 months in 20-50-year-old women with breast pain with or without lumpiness. Women with a discrete benign lump or cancer were excluded from the study. Serial assessments of pain on a visual analogue scale and nodularity grade on a 5-point ordinal Lucknow-Cardiff scale were done. A total of 151 patients were randomly allocated to two intervention

[Bone quality and raloxifene].

Bone turnover affects fracture risk. Degrees of fracture risk reduction by antiresorptive agents are larger than those expected from the rates of BMD increase. Actually, fracture risk reduction by raloxifene is largely dependent on the effects of bone turnover regulation. Raloxifene stabilizes the trabecular structure, maintains the activities of micro-injury repair and healing of fracture, and increases the mean values of the degrees of tissue mineralization and the distribution of their ranges. The relationship between the fracture risk reduction and the changes in these parameters of bone q

Baseline gene expression and dynamic endocrine therapy use in ER+ breast cancer: associations with recurrence in a real-world cohort.

Endocrine therapy (ET) selection in estrogen receptor-positive breast cancer (ER + BC) is guided by menopausal status and tolerability rather than tumor biology, despite substantial heterogeneity in recurrence. We hypothesized that baseline gene expression differentially associates with recurrence depending on initial ET class. In a retrospective cohort of 74 ER+/HER2- patients treated with adjuvant ET, we profiled baseline tumor RNA and fitted adjusted Cox models for gene and ET interactions on time-to-recurrence and time-to-switch. Among selective estrogen receptor modulators-initiated patie
