Chordoma of skull base presenting as nasopharyngeal mass.

While the nasopharynx is most commonly regarded by the otolaryngologist as a primary site of neoplastic involvement, it is also an avenue of spread of base-of-the-skull tumors presenting as bulging nasopharyngeal masses. Chordoma is a relatively rare tumor of the skull base and sacrum thought to originate from embryonic remnants of the notochord. Chordomas arising from the skull base/clivus are typically locally aggressive with lytic bone destruction. The optimal treatment may be photon/proton radiotherapy alone or combined with a gross total resection, when feasible. We report a case of intra

Spontaneous skull base meningoencephaloceles and cerebrospinal fluid fistulas.

Cerebrospinal fluid (CSF) fistulas are characterized by the egress of CSF from the intracranial cavity through an osteodural disruption between the subarachnoid space and a pneumatized structure within the skull base. Depending on the cause, CSF fistulas are classified as acquired or congenital, and acquired fistulas are further classified as traumatic, nontraumatic, or spontaneous. Spontaneous CSF fistulas are considered to result from a multifactorial process and have been postulated to represent a variant of idiopathic intracranial hypertension. However, an anatomic predisposition involving

Intraventricular tension pneumocephalus after endoscopic skull base surgery.

BACKGROUND AND STUDY AIMS: Postoperative pneumocephalus is commonly observed after neurosurgical intracranial procedures and is usually of little consequence. Intraventricular tension pneumocephalus, however, is a rare complication and a neurosurgical emergency that requires immediate intervention. We describe the first case of intraventricular tension pneumocephalus that developed 1 week following an expanded endoscopic endonasal approach for resection of a suprasellar mass. PATIENT: A patient who underwent an endoscopic transplanum transtuberculum approach for resection of a suprasellar epi

[Fractionated stereotactic irradiation of skull-base related tumours].

The prognosis of the treatment of brain tumours depends on two main factors: biological nature and localisation of the neoplasm. Requirements of oncologic surgery can be met only partially if at all in neurological surgery of brain tumours. Resectability depends primarily on localisation of the neoplasms. The leading principle is preservation of fine neural structures, minimising morbidity from tissue resection with the goal of maximal tumour resection. As nervous structures and the target volume do not move in the intracranial space, large radiation doses unusual in traditional radiotherapy c

Anatomic skull base education using advanced neuroimaging techniques.

OBJECTIVE: The goal of the present article was to describe our dissection training system applied to a variety of endoscopic endonasal approaches. It allows one to perform a 3D virtual dissection of the desired approach and to analyze and quantify critical surgical measurements. METHODS: All the human cadaveric heads were dissected at the Laboratory of Surgical Neuro-Anatomy (LSNA) of the University of Barcelona (Spain). The model surgical training protocol was designed as follows: 1) virtual dissection of the selected approach using our dissection training 3D model; 2) preliminary exploratio

Nasopharyngeal carcinoma with skull base erosion cytologic findings.

Nasopharyngeal carcinoma (NPC) occurs more frequently in patients with south-east Asian racial backgrounds. This disease may spreads superiorly to the skull base and intracranium followed by skull base destruction. We report a 56 year-old man presented with headache and diplopia. Magnetic resonance imaging (MRI) revealed extension of destructive mass from ethmoid sinus to the parenchyma. Intraoperative touch cytology showed loose syncytial sheets of pleomorphic abnormal epithelial cells, dyskeratotic cells with abnormal chromatin clumping and irregular nuclear outlines, in a necrotic backgroun

Endoscopic resection of an anterior skull-base Schwannoma.

Sinonasal Schwannomas represent less than 4% of all head and neck Schwannomas. These neural sheath tumors arise from the ophthalmic and maxillary divisions of the trigeminal nerve, as well as autonomic nerves from sympathetic fibers of the carotid plexus and parasympathetic fibers of the sphenopalatine ganglion. Patients commonly present with nonspecific symptoms such as nasal obstruction, epistaxis, and anosmia. Nasal endoscopy usually reveals a unilateral polypoid mass. Diagnostic imaging with computed tomography (CT) and magnetic resonance (MR) is typically nonspecific. Diagnosis may be del

Endoscopic skull base reconstruction after endoscopic endonasal approach.

Recent advances in technology and techniques in neurosurgery and otolaryngology have revolutionised skull base surgery with the introduction of endoscopic endonasal approaches. One of the greatest limitations of endoscopic endonasal skull base surgery is the repair of osteodural defects. Numerous reconstructive techniques have been developed that maximise endoscopic endonasal approach efficacy and minimise complications. The choice of technique for closure depends on different factors and is based on an individual approach in each patient. In particular, size, the localisation of the defect an

Endoscope-assisted microsurgical resection of skull base meningiomas.

This study aims to determine the value of endoscope assistance in the microsurgical resection of skull base meningiomas. Fourty-six patients harboring a skull base meningioma underwent an endoscope-assisted microsurgical resection. In 30 patients (65%), tumor parts which could not be visualized under the microscope were detected with the endoscope. In 26 patients (56%), these tumor remnants were removed under endoscopic view. Gross total resection was achieved in 35 patients (76%) and near-total resection in 11 (24%). There was no surgical mortality. The major complication was new cranial nerv

Base-of-skull/infratemporal fossa shell fragment retrieval.

PURPOSE: Shell fragments and bullets are frequently observed to have settled in the skull base and infratemporal fossa (ITF) with the impact resulting in a new surgically complicated trauma. This compact region is one of the most difficult and dangerous locations for lodged objects. When called upon to retrieve these objects, the surgeon faces a complicated challenge. The complexity of the task depends on many factors, such as size, deformation, and surface hooks, as well as identification of the precise anatomic location. MATERIALS AND METHODS: New surgical approaches and successful methods

Extended endoscopic approaches for midline skull-base lesions.

The endoscopic transsphenoidal approach has been reported in the literature as a useful tool to treat sellar and parasellar lesions. The endoscope permits a panoramic view instead of the narrow microscopic view, and it allows the inspection and removal of the lesions of sellar, parasellar, and suprasellar compartments by angled-lens endoscopes. On the basis of the experience gained with the use of the endoscope, we have performed extended endoscopic endonasal transsphenoidal approach in 13 of 200 (total endoscopic transphenoidal approaches since September 1997) patients for the last 5 years. Surgical management of craniofacial and skull base rhabdomyosarcomas.

INTRODUCTION: Craniofacial and skull base rhabdomyosarcomas (RMSs) are rare. Surgeons are involved both in the resection and the reconstruction of these tumors and in the management of late sequelae of earlier treatment. These tumors are highly heterogeneous, and the initial presentation may be insidious. We wished to determine how diagnostic difficulties could be overcome and how management outcomes could be optimized. MATERIALS AND METHODS: All cases of RMS in patients who presented to our craniofacial program between 2004 and 2008 were retrospectively reviewed. RESULTS: Eight consecutive

Extended endoscopic endonasal approach to the skull base.

OBJECTIVE: Different approaches to the skull base have been developed through the sphenoidal sinus. Traditional boundaries of the trans-sphenoidal approach can be extended in antero-posterior and lateral planes. We review our experience with the extended endoscopic endonasal approach in the first 12 cases. METHODS: We used the extended endoscopic endonasal approach in 12 patients with different lesions of the skull base. This study specifically focuses on the type of lesions, surgical approach, outcome and surgical complications. RESULTS: The extended endoscopic endonasal approach was used i

Arrested pneumatization of the skull base: imaging characteristics.

OBJECTIVE: Arrested skull base pneumatization is a benign developmental variant that can be confused with significant skull base disease processes. This study reviews the imaging findings in 30 suspected cases of arrested skull base pneumatization. CONCLUSION: When encountering a nonexpansile lesion with osteosclerotic borders, internal fat, and curvilinear calcifications in the basisphenoid bone or adjacent skull base, radiologists should strongly consider the diagnosis of arrested pneumatization.

Paraganglioma of the skull base: a case report.

Paragangliomas are rare benign neuroendocrine tumors derived from the extra-adrenal paraganglia of the autonomic nervous system. Here, we described a new case of a 75-year-old woman with paraganglioma arising in the middle and posterior cranial fossa with extended destruction of the skull base and clivus. She was admitted to our department with the complaint of severe respiratory distress due to vocal cord paralysis. We discussed its clinical, radiological, histopathological features and treatment modalities in the light of the current literature.

Sinonasal hybrid tumour involving the anterior skull base.

OBJECTIVE: We report a rare case of sinonasal hybrid tumour within an inverted papilloma. METHOD: The patient's case history and course of management are presented. The relevant medical literature are reviewed and discussed. RESULTS: A 60-year-old man presented with unilateral nasal obstruction associated with anosmia. An ENT examination revealed a unilateral, left nasal mass. A provisional diagnosis of inverted papilloma was made. The patient underwent a transnasal endoscopic excision of the tumour, which was unfortunately complicated by a cerebrospinal fluid leak. Further treatment of this

Vagus nerve paralysis due to skull base fracture.

Lower cranial nerve paralysis originating from skull base fracture is rare and isolated vagus nerve paralysis after close head injury is extremely rare. We, in this study, present the case of a 49-year-old man sustained a right vocal cord paralysis and dysphagia in a fighting accident. Initial examination, including flexible laryngoscope, esophagogram and fine-cut neck computed tomography scan, failed to find out the cause of isolated vagus paralysis. Until high resolution computed tomography of skull base revealed the jugular foramen bony disruption. One individual cranial nerve paralysis may
