Social determinants of access to timely elective surgery in Ontario, Canada: a cross-sectional population level study.

BACKGROUND: Equitable access to surgical care has clinical and policy implications. We assess the association between social disadvantage and wait times for elective surgical procedures in Ontario. METHODS: We conducted a cross-sectional analysis using administrative data sets of adults receiving nonurgent inguinal hernia repair, cholecystectomy, hip arthroplasty, knee arthroplasty, arthroscopy, benign uterine surgery and cataract surgery from April 2013 to December 2019. We assessed the relation between exceeding target wait times and the highest versus lowest quintile of marginalization dim

Addressing scabies among street children in Ethiopia: an ethnographic study of acceptable interventions by prospective recipients and deliverers.

INTRODUCTION: Scabies is a neglected tropical disease that affects the physical, socioeconomic, and psychological wellbeing of patients. Street children, due to poor living conditions and social marginalization, are at increased risk of infestation and face significant barriers to access healthcare services. Various interventions to control scabies have been suggested and implemented, but few have been based on the needs and viewpoints of the street children themselves. Drawing on the theoretical framework of acceptability (TFA), this article explores the interventions that street children, pa

Selling sex in the context of substance use: social and structural drivers of transactional sex among men who use opioids in Maryland.

BACKGROUND: Transactional sex is an important driver of HIV risk among people who use drugs in the USA, but there is a dearth of research characterizing men's selling and trading of sex in the context of opioid use. To identify contextually specific factors associated with selling or trading sex in a US population of men who use drugs, we cross-sectionally examined social and structural correlates of transactional sex among men who use opioids (MWUO) in Anne Arundel County and Baltimore City, Maryland. METHODS: Between July 2018 and March 2020, we used targeted sampling to recruit men reporti

"We have a roof over our head, but we have to eat too:" Exploring shifting foodscapes from homelessness into Housing First in Kingston, Ontario.

BACKGROUND: People facing extended periods of homelessness exhibit a remarkable degree of agency and resilience in procuring food. The literature on foodscapes considers the sociospatial contexts of food procurement, finding that what happens within and along the way to sites of food acquisition and consumption are important considerations in fully understanding and realizing food security. PURPOSE: This study explores the shift in foodscapes of people who are transitioning from homelessness into scattered-site independent housing via a municipal Housing First program and considers implicatio

Beyond Individual Risk: A Qualitative Exploration of Intersectional HIV Vulnerabilities Among Black Cisgender Women in Baltimore.

Black cisgender women in the USA experience compounded challenges in protecting themselves from HIV. While prevention efforts often focus on individual-level behaviors, structural and contextual factors critically shape HIV vulnerability and access to care. We conducted qualitative in-depth interviews between October 2021 and April 2023 with 26 Black cisgender women aged 18 to 44 who reported inconsistent condom use and vaginal or anal sex with a male partner in the past 6 months. We used a combined inductive and deductive approach to analyze the data and presented illustrative themes across B

"When Somebody Comes into This Country and You Are Trans on Top of That Is Like You Got… Two Strikes on You": Intersectional Barriers to PrEP Use Among Latina Transgender Women in the Eastern and Southern United States.

In the United States (U.S.), Latina transgender women (LTW) are highly burdened by HIV and are prioritized for pre-exposure prophylaxis (PrEP). This study explored intersectional barriers and facilitators to PrEP uptake among LTW. Between February-November 2022, in-depth interviews were conducted with 27 LTW in the LITE Study. Participants were purposively sampled from 196 LTW in the cohort based on PrEP uptake (PrEP-naïve n = 8, PrEP-eligible and not user n = 5, current PrEP user n = 6, previous PrEP user n = 8). We conducted content analysis guided by a Modified Social Ecological Model and I

"We try to help each other out": Older immigrants' perceptions of their support resources.

The number of older migrants is steadily increasing in immigration-receiving countries. Studies show that older immigrants tend to be vulnerable to poverty, health problems, and mental distress. They often face barriers when seeking formal assistance and rely on informal support from family and community members. The present study aims to explore the experiences and perceptions of older immigrants regarding the support resources available to them as they navigate the challenges of their multifaceted needs in their host country. The study involved in-depth interviews with 21 older immigrants (a

Structural Racism, Geographies of Opportunity, and Maternal Health Inequities: A Dynamic Conceptual Framework.

Addressing the grave racial inequities in maternal health requires a new generation of research that better operationalizes root causes of these outcomes. Recent frameworks improving the conceptualization of structural racism have illuminated the need for better conceptual clarity when investigating neighborhoods as a site of structural marginalization for Black birthing populations as well. In particular, better conceptualization of dynamic feedback in how neighborhoods are constructed and experienced, especially as they embed vicious cycles of place-based racialization, is integral to produc

Drought-related cholera outbreaks in Africa and the implications for climate change: a narrative review.

Africa has historically seen several periods of prolonged and extreme droughts across the continent, causing food insecurity, exacerbating social inequity and frequent mortality. A known consequence of droughts and their associated risk factors are infectious disease outbreaks, which are worsened by malnutrition, poor access to water, sanitation and hygiene and population displacement. Cholera is a potential causative agent of such outbreaks. Africa has the highest global cholera burden, several drought-prone regions and high levels of inequity. Despite this, research on cholera and drought in

When Resilience Is Not Enough: Enacted Stigma and Mental Health of Trans and Nonbinary People.

Trans and nonbinary (TNB) people are subject to social and institutional marginalization, and simultaneously report varied experiences of resilience. We developed and validated a novel measure of TNB-specific resilience and were following our participants longitudinally when our NIH grant was unjustly terminated. Resisting the government's efforts to derail science, the current study uses our baseline data to understand whether the positive effects of resilience on mental health hold across various levels of enacted stigma. Our sample included 608 participants between the ages of 18 and 77 yea

Talent has no race, has no face - but it has a skin colour: The 'appropriate femininity' with the case of Kurdish-Swedish Actress Evin Ahmad.

Sexist and misogynist attitudes towards actresses in the mainstream film industry and other media have been the target of feminists for decades. Stigmatized and stereotyped images of immigrants on the screen have also been scrutinized. However, very little attention has been given to the ways in which actresses with foreign background, not necessarily from Black communities, are portrayed and narrated. Addressing this issue would reveal how non-Western body images on screen are racialized according to certain Western beauty standards. Sweden is often described as the most gender-equal film ind

Children from a rural region in The Chiapas Highlands, Mexico, show an increased risk of stunting and intestinal parasitoses when compared with urban children.

BACKGROUND: The state of Chiapas has held the first place of extreme poverty in Mexico. The majority of Chiapas' municipalities are inhabited by marginalized, indigenous populations, who usually present diarrhea of unknown etiology. We evaluated the nutritional status, intestinal parasites, and common bacterial pathogens, including DEC (diarrheagenic Escherichia coli) strains, in 178 children under five years of age with a high (rural) and a moderate (urban) degree of marginalization. METHODS: Z-scores for anthropometric indexes from the children were obtained, whereas intestinal parasites we

From principles to practice: Description of a novel equity-based HCV primary care treatment model for PWID.

BACKGROUND: Knowledge is increasing regarding effective models of HCV care for people who inject drugs (PWID). However, examples implementing such models in primary care are lacking, leaving a gap in our applied understanding of how practically we best scale-up such care: this is critical and urgent if the benefits of treatment advances are to be realized for PWID. A CASE STUDY: The Cool Aid Community Health Centre (CHC) provides HCV programming for PWID, putting recent advances into practice. A case study of the CHC's HCV programming describes the practice experience and outcomes of its nove

Estimating HIV prevalence and risk behaviors of transgender persons in the United States: a systematic review.

Transgender populations in the United States have been impacted by the HIV/AIDS epidemic. This systematic review estimates the prevalence of HIV infection and risk behaviors of transgender persons. Comprehensive searches of the US-based HIV behavioral prevention literature identified 29 studies focusing on male-to-female (MTF) transgender women; five of these studies also reported data on female-to-male (FTM) transgender men. Using meta-analytic approaches, prevalence rates were estimated by synthesizing weighted means. Meta-analytic findings indicated that 27.7% (95% confidence interval [CI],

Differential relationships between social adversity and depressive symptoms by HIV status and racial/ethnic identity.

OBJECTIVE: Historically marginalized groups are likely to be exposed to social adversity, which predicts important mental health outcomes (e.g., depression). Despite the well-established relationship between adversity and poor health, few studies have examined how adversity differentially predicts mental health among people living with multiple, co-occurring marginalized identities or statuses. The current study fills this gap by examining whether relationships between social adversity and depressive symptoms differed between those living with or without a stigmatized disease (i.e., HIV) and/o

Equity and Resilience in Higher Education: An Examination of Environmental Injustices Affecting College Students from Marginalized Populations Amid Climate Crises.

As climate crises become more severe, higher education institutions face unprecedented challenges in protecting and supporting their students. This conceptual paper explores the intersection of equity and resilience in higher education, emphasizing how structural inequities increase climate-related vulnerabilities among marginalized student groups, especially low-income, first-generation, and BIPOC students. Using the Social Ecological Model and the Whole Community Approach, this paper suggests that climate resilience in higher education must go beyond traditional emergency management to inclu
