Emergency department diagnosis and management of skin and soft-tissue infections in children.

Children with skin and soft-tissue infections (SSTIs) commonly present to the emergency department, and while most of these infections are mild, some can be severe, with high morbidity and mortality. Emergency clinicians must be able to recognize frequently encountered SSTIs and be prepared to treat them appropriately. This issue reviews the various etiologies and patient presentations of common SSTIs, including purulent, nonpurulent, and necrotizing infections. Findings on the history and physical examination that can differentiate SSTIs from mimics are discussed, and evidence-based recommend

Association between Insurance Status and Outcomes of Hospitalizations for Necrotizing Soft Tissue Infections.

Background: Lack of insurance is associated with poorer outcomes in hospitalized patients. However, few studies have explored this association in hospitalizations for necrotizing soft tissue infections (NSTIs). This study examined the impact of insurance status on the outcome of NSTI admissions. Methods: All adult hospitalizations for necrotizing fasciitis, gas gangrene, and Fournier gangrene between 2016 and 2018 were examined using the Nationwide Inpatient Sample database. Insurance status was categorized as insured (including Medicare, Medicaid, and Private, including Health maintenance org

Recurrent Soft Tissue Infections Associated With Burosumab Therapy in X-Linked Hypophosphatemic Rickets.

X-linked hypophosphatemic rickets (XLH) is a genetic disorder characterized by elevated fibroblast growth factor 23 (FGF23), resulting in renal phosphate wasting and inadequate bone mineralization. Burosumab, a monoclonal antibody that inhibits FGF23 activity, has shown promise in improving renal phosphate reabsorption and clinical outcomes in XLH patients. However, the potential side effects of burosumab, particularly its impact on immune function and susceptibility to infections, remain a subject of concern. In this case report, we describe a 57-year-old male individual with XLH who experien

Skin and soft tissue infections in adolescent chronic myeloid leukemia under dasatinib treatment.

Although skin complications are common adverse events from tyrosine kinase inhibitors (TKIs) for the treatment of chronic myeloid leukemia (CML), no reports have focused on skin and soft tissue infections (SSTIs) associated with TKI use. We herein present five episodes of SSTIs in three CML patients under dasatinib treatment. All patients were adolescents and had been receiving dasatinib for more than 4 years. In contrast, none of 41 adult CML patients experienced SSTIs in a retrospective analysis. Our findings suggest that long-term dasatinib treatment in adolescent patients may be associated

Necrotizing Soft-Tissue Infections: A Retrospective Review of Predictive Factors for Limb Loss.

BACKGROUND: Emergent diagnosis and treatment are important for the survival of patients with necrotizing soft-tissue infections (NSTIs). Death is the most catastrophic outcome, but limb loss is also one of the most important complications that can have a significant impact on the rest of the patient's life. The purpose of this study was to identify predictive factors for limb loss caused by NSTIs. METHODS: The data of patients at our center who were diagnosed with NSTIs from May 2003 to January 2019 were analyzed retrospectively. The inclusion criteria were patients with a definite diagnosis

Imaging of Musculoskeletal Soft-Tissue Infections in Clinical Practice: A Comprehensive Updated Review.

Musculoskeletal soft-tissue infections include a wide range of clinical conditions that are commonly encountered in both emergency departments and non-emergency clinical settings. Since clinical signs, symptoms, and even laboratory tests can be unremarkable or non-specific, imaging plays a key role in many cases. MRI is considered the most comprehensive and sensitive imaging tool available for the assessment of musculoskeletal infections. Ultrasound is a fundamental tool, especially for the evaluation of superficially located diseases and for US-guided interventional procedures, such as biopsy

Nocardia takedensis: a newly recognized pathogen responsible for skin and soft tissue infections.

Nocardia takedensis was first isolated in 2005, from soil in Japan. We report here two cases of lymphangitis in France (2012-2017) caused by N. takedensis both occurring after skin injury while gardening, which enabled its inoculation. The two patients were immunocompromised and successfully treated by an antimicrobial agent active on the isolated strain, trimethoprim-sulfamethoxazole and amoxicillin-clavulanic acid for patient one and patient two, respectively. Our study along with previous ones supports the idea of a newly recognized cutaneous opportunistic pathogen and reinforces the recomm

Distinguishing bone and soft tissue infections mimicking sarcomas requires multimodal multidisciplinary team assessment.

INTRODUCTION: The aims of this study were to report the presenting characteristics and identify how best to distinguish bone and soft-tissue infections that mimic sarcomas. MATERIALS AND METHODS: A total of 238 (211 osteomyelitis and 27 soft-tissue infections) patients referred to a tertiary sarcoma multidisciplinary team with suspected sarcoma who were ultimately diagnosed with a bone or soft tissue infection were included. Data from a prospectively collated database was analysed retrospectively. RESULTS: Of all possible bone and soft-tissue sarcoma referrals, a diagnosis of infection was m

Low Frequency of Staphylococcus Aureus in Lower Extremity Skin and Soft Tissue Infections.

BACKGROUND: Staphylococcus aureus is often implicated in skin/soft tissue infections (SSTI). However, SSTI at sites of pressure necrosis and peripheral vascular disease (PVD) are often polymicrobial. The frequency of S aureus in these infections is uncertain. METHODS: We retrospectively reviewed culture results from adults (January 1, 2015-March 31, 2017), evaluated their records and selected SSTI in lower extremities. The patient demographics, comorbidities, characteristics and culture results were recorded. The results were stratified by S aureus status and a composite risk score (RS) was d

Management of Skin and Soft-Tissue Infections Before and After Clinical Pathway Implementation.

We evaluated if the introduction of a clinical pathway for skin and soft-tissue infections (SSTIs) would reduce methicillin-resistant Staphylococcus aureus (MRSA)-directed therapy for simple cellulitis and antibiotic use for simple abscess after drainage. We compared the treatment of SSTI during a 3-month prepathway and 11-month postpathway period. We included patients 57 days to 18 years old discharged from the emergency department (ED) with a diagnosis of cellulitis or abscess. Balancing measures included 72-hour revisit rate and ED length of stay (LOS). A total of 291 patients prepathway an

[Pharmacodynamic and pharmacokinetic of antibiotics for treatment of skin and soft tissue infections].

The pharmacodynamic and pharmacokinetic characteristics of antimicrobial agents are the two fundamental pharmacological components which provide a rational for the choice of therapy for skin and skin structure infections, and especially serious infections. The most important PK-PD parameters are well known which can potentiate therapeutic efficacy. Antimicrobial agents ca be subdivided into categories based on whether their activity is dependent on concentration or exposure time. Therefore, a correct dosing regimen for the time-dependent molecules (i.e. beta-lactams, linezolid, tigecycline) sh

[Daptomycin--a new antibiotic for the treatment of skin and soft tissue infections].

Daptomycin (Cubicin) is a bactericidal lipopeptide antibiotic that was approved in February 2006 for the European market, including Norway, for the treatment of patients with complicated skin and soft tissue infections caused by Gram-positive cocci. Daptomycin is administered intravenously and eliminated mainly by the kidneys. Dose reduction is therefore necessary in patients with impaired renal function. The drug is active only against Gram-positive cocci. Daptomycin therapy of patients with complicated skin and soft tissue infections caused by Gram-positive cocci, is as effective as standard

Diagnosis and management of Staphylococcus aureus infections of the skin and soft tissue.

Infections involving the skin and soft tissue are common and range from superficial, localized and sometimes self-limiting infections to deep, rapidly spreading and potentially life-threatening infections. Skin infections caused by Staphylococcus aureus include primary pyodermas, while those involving the soft tissues include cellulitis and pyomyositis. Surgical site infections and infections in intravenous drug users are also commonly caused by S. aureus. The severity of the infection determines the choice of treatment. There are few studies that have critically appraised the use of antibioti

Necrotizing soft-tissue infections of the perineum and genitalia. Etiology and early reconstruction.

Necrotizing fasciitis of the perineum and genitalia is a rare, rapidly progressive, often fatal infection whose unique bacterial involvement has frequently been ignored. From our experience with 14 patients, we recommend the following: early, aggressive surgical debridement and drainage to reduce morbidity and mortality; prompt institution of antimicrobial therapy directed at both aerobic and anaerobic organisms, with subsequent tailoring to the specific bacteria cultured, and early surgical reconstruction to avoid extensive scarring and cosmetic deformity and to allow earlier skin closure and

Prevalence and antibiotic susceptibility of ESKAPE pathogens in skin and soft tissue infections.

Background Skin and soft tissue infections (SSTIs) encompass a wide range of conditions, from superficial infections to life-threatening necrotising fasciitis. Enterococcus faecium, Staphylococcus aureus, Klebsiella pneumoniae, Acinetobacter baumannii, Pseudomonas aeruginosa, and Enterobacter spp. (ESKAPE pathogens) are leading causes of SSTIs and are notable for their antimicrobial resistance. Aim This study aims to determine the prevalence of SSTIs and analyse the resistance patterns of ESKAPE pathogens, providing critical insights for effective treatment strategies. Incidence of skin and soft-tissue infections in England: 11-year retrospective study.

Epidemiological estimates for the incidence of skin and soft-tissue infections (SSTIs) in England are limited. We aimed at evaluating the incidence of SSTIs and associated complications through a retrospective, observational 11-year study (2010-2020). We retrieved data on SSTI and associated complications from Clinical Practice Research Datalink, Hospital Episode Statistics, and the Office for National Statistics for England. More than four million SSTI episodes were identified, which corresponded to an overall incidence of 33.1 per 1000 person-years of observation (PYO). The incidence per 100

Severe necrotizing soft tissue infections (SENSEI) study: Protocol for a multi-centered audit.

BACKGROUND: Severe soft tissue infections are surgical emergencies typically accompanied by sepsis. Early recognition and management optimizes patient outcomes. Single-centered studies on these infections are limited due to their rare incidence. MATERIALS & METHODS: We propose a protocol for the SENSEI (Severe Necrotizing Soft Tissue Infections) study, a multi-centered audit across the United Kingdom to assess current clinical practice for treating severe soft tissue infections in adult and paediatric populations. This will be compared with standards predetermined by the Surviving Sepsis Camp
