An essential role for modulation of hyperpolarization-activated current in the development of binaural temporal precision.

In sensory circuits of the brain, developmental changes in the expression and modulation of voltage-gated ion channels are a common occurrence, but such changes are often difficult to assign to clear functional roles. We have explored this issue in the binaural neurons of the medial superior olive (MSO), whose temporal precision in detecting the coincidence of binaural inputs dictates the resolution of azimuthal sound localization. We show that in MSO principal neurons of gerbils during the first week of hearing, a hyperpolarization-activated current (I(h)) progressively undergoes a 13-fold in

Constrictive pericarditis localized to left ventricle presented with left pleural effusion: a case report.

An 18-year old woman presented with progressive dyspnea and cough. Physical examination revealed decreased breath sounds at the left hemithorax and distant heart sounds with no murmurs or rub. Electrocardiogram revealed low voltage. Chest X-ray showed unilateral left-sided pleural effusion with no cardiomegaly. Transthoracic echocardiogram showed thickened pericardium localized throughout the left ventricle impairing the diastolic filling. Doppler waveforms were suggestive of localized constrictive pericarditis. A computerized tomographic scan of the chest confirmed the presence of unilateral

Cricoid Force: Anatomic, Physiologic, and Clinical Concepts.

The application of cricoid force remains controversial in modern practice. This review critically assesses the anatomic, physiologic, and contemporary clinical evidence of cricoid force application. There may be a sound anatomic basis to cricoid force application, involving occlusion of the postcricoid hypopharynx, but the physiologic basis is uncertain. Clinical evidence to date has not reliably determined efficacy of cricoid force application, largely because of inconsistent localization and application. Modern evidence suggests no clinically relevant implications to successful intubation wi

Finite element analysis of the transfer of sound in the myringosclerotic ear.

This work presents a biomechanical study of myringosclerosis (MS), an abnormal condition of the ear that produces calcification of the lamina propria of the eardrum. The study researched the transfer of sound to the stapes depending on the localization, dimension and calcification degree of the MS plaques. Results were obtained using a validated finite element model of the ear. The mechanical properties of the lamina propria were modified, in order to model MS plaques, using the rule of mixtures for particle composites considering that the plaques are made of hydroxyapatite particles in a matr

Spiking Neural Network Models of Interaural Time Difference Extraction via a Massively Collaborative Process.

Neuroscientists are increasingly initiating large-scale collaborations which bring together tens to hundreds of researchers. At this scale, such projects can tackle big challenges and engage diverse participants. Inspired by projects in mathematics, we set out to test the feasibility of widening access to such projects even further, by running a massively collaborative project in computational neuroscience. The key difference, with prior neuroscientific efforts, being that our entire project (code, results, and writing) was public from the outset, and that anyone could participate. To achieve

Effects of Training on the Use of a Manual Microphone Shutoff on a BiCROS Device.

BACKGROUND: Bilateral contralateral routing of signals (BiCROS) hearing aids function to restore audibility of sounds originating from the side of the unaidable ear. However, when speech is presented to the side of the aidable ear and noise to the side of the unaidable ear, a BiCROS arrangement may reduce intelligibility of the speech signal. This negative effect may be circumvented if an on/off switch is available on the contralateral routing of signals (CROS) transmitter. PURPOSE: This study evaluated if the proper use of the on/off switch on a CROS transmitter could enhance speech recognit

A model for sound lateralization.

Recent studies of multiple sclerosis (MS) and stroke patients suggested a correlation between two patterns of abnormal performance in lateralization tasks and two sites of pontine lesions. Most patients who had lesions below or at the superior olivary complex (SOC) perceived all interaural differences in binaural stimuli as small, while most patients who had lesions above the SOC perceived all interaural differences as large. The two abnormal performance patterns occurred for interaural time differences (ITD) and/or for interaural level differences (ILD). The present model proposes a multi-lev

[Treatment of single-sided deafness by cochlear implantation].

Single sided deafness diminishes speech understanding in noise and sound localization and thereby globally auditory performance. Most patients also suffer from tinnitus and indicate reduced quality of life. Patients have the choice to adapt to the new situation without treatment, to restore pseudostereophonic hearing by contralateral routing of signal (CROS) hearing aids or to restore binaural hearing using a cochlear implant in the deaf ear. This article summarizes the physiological base of binaural hearing and treatment options for single sided deafness with a special emphasis on the cochlea

Binaural hearing, neurological disability and brain imaging in Multiple Sclerosis.

Multiple sclerosis is an inflammatory autoimmune disease characterized by focal destruction of myelin within the central nervous system. Auditory deficits are a relatively frequent, but often underdiagnosed symptom. This study examined sound detection threshold and functional hearing ability in a cohort of patients with multiple sclerosis. Thirty adults with relapsing-remitting multiple sclerosis and 30 age, gender and hearing-level matched controls underwent peripheral and central auditory assessments. These included audiometric (sound detection threshold) measurement, binaural speech percept

Surface-Based Amplitude of Low-Frequency Fluctuation Alterations in Patients With Tinnitus Before and After Sound Therapy: A Resting-State Functional Magnetic Resonance Imaging Study.

This study aimed to investigate abnormal tinnitus activity by evaluating brain surface-based amplitude of low-frequency fluctuation (ALFF) changes detected by resting-state functional magnetic resonance imaging (RS-fMRI) in patients with idiopathic tinnitus before and after 24 weeks of sound therapy. We hypothesized that sound therapy could gradually return cortical local brain function to a relatively normal range. In this prospective observational study, we recruited thirty-three tinnitus patients who had undergone 24 weeks of sound therapy and 26 matched healthy controls (HCs). For the two

Psychophysiological effects of low-frequency sound mediated by room size, room shape, and stimulus duration.

Music and sounds elicit a wide range of emotions and activate numerous psychological and physiological effects associated with the functioning of the autonomic nervous system (ANS) and involved in the maintenance of homeostasis. As such, sound interventions can play an important role in supporting human wellbeing and improve outcomes in medical patients. Psychophysiological responses are dependent on the stimulus type, frequency, duration, and spatial conditions of the acoustic environment. To arrive at a more articulate understanding of these dependencies, the effects of singing bowl sounds a

[Cochlear Implantation in Single-Sided Deafness - Six Years of Follow-Up from Diagnosis and Consistent Use, Through Dedicated and Creative Auditory Learning, to the Development of Bilateral Central Processing Abilities].

This article describes the rehabilitation of a girl who passed newborn hearing screening but was diagnosed with left single-sided deafness at age 2 years and 10 months following assessment for speech and language delay with poor speech intelligibility. At the time, experience in auditory rehabilitation after cochlear implantation for single-sided deafness was limited, and the mother encountered various approaches. Rehabilitation goals, set with the treatment team, focused on developing bilateral hearing, enhancing directional perception, improving speech understanding in noise, and developing

A contribution to the debate on tinnitus definition.

Tinnitus is generally defined as an auditory perception in the absence of environmental sound stimulation. However, this definition is quite incomplete as it omits an essential aspect, the patient's point of view. This point of view constitutes, first and foremost, a global and unified lived experience, which is not only sensory (localization, loudness, pitch and tone), but also cognitive (thoughts, attentiveness, behaviors) and emotional (discomfort, suffering). This experience can be lived in a very unpleasant way and consequently have a very negative impact on quality of life. This article

Association of different neural processes during different emotional perceptions of white noise and pure tone auditory stimuli.

Sound is a sensory stimulant ubiquitously found throughout our environment. Humans have evolved a system that effectively and automatically converts sound sensory inputs into emotions. Although different emotional responses to sounds with different frequency characteristics are empirically recognized, there is a paucity of studies addressing different emotional responses to these sounds and the underlying neural mechanisms. In this study, we examined effects of pure tone (PT) and white noise (WN) inputs at ordinary loudness levels on emotional responses. We found that WN stimuli produced more

Development of the head, pinnae, and acoustical cues to sound location in a precocial species, the guinea pig (Cavia porcellus).

The morphology of the head and pinna shape the spatial and frequency dependence of sound propagation that give rise to the acoustic cues to sound source location. During early development, the physical dimensions of the head and pinna increase rapidly. Thus, the binaural (interaural time and level differences, ITD and ILD) and monaural (spectral shape) cues are also hypothesized to change rapidly. Complex interactions between the size and shape of the head and pinna limit the accuracy of simple acoustical models (e.g. spherical) and necessitate empirical measurements. Here, we measured the cue

Acquired resistance to acoustic trauma by sound conditioning is primarily mediated by changes restricted to the cochlea, not by systemic responses.

Hearing loss caused by intense sound exposure can be significantly reduced by pre-exposing subjects to moderate-level acoustic stimuli. This phenomenon occurs in a variety of mammals. We investigated whether sound conditioning provides acquired resistance to acoustic trauma through local mechanisms selectively in the conditioned ears or if systemic mechanisms are involved that would yield contralateral protection in unconditioned ears. Guinea pigs (group I) in which one external ear canal was occluded were exposed to conditioning sound (2-20 kHz, 85 dB SPL, 5 h/day, 10 days). After removing th
