The sensory and motor deficits of the CNS are varied, depending on the etiologic factors and the structures involved. Nevertheless, the clinical picture is predictable, provided one has an adequate knowledge of the neuroanatomy and the functions of the different fiber tracts, nuclei, and other specific regions of the brain and spinal cord. The purpose of this section is to provide an overall view of the sensory and motor deficits of the CNS, which will enable the clinician to treat these patients in a more objective and effective manner. Etiologically, the diseases affecting the CNS can be gro

Split cord malformations (SCM) may be discovered as isolated occult spinal dysraphism or in association with various other overt developmental spinal abnormalities or lesions. However, the occurrence of SCM with intraspinal teratoma is rare. We report 2 cases of teratoma associated with SCM, of which one is intramedullary and the other extradural within meningocele, with a brief review of the literature.

Melorheostosis was diagnosed in a 13 year old female. Two years later an x-ray of the skull showed an calcified intracerebral mass. At the age of 18 years the patient died. The autopsy confirmed the diagnosis of melorheostosis and revealed a meningioma of falx cerebri in close proximity to predominantly intraventricular calcified tissue. A neurinoma of the left acoustic nerve was also found. In the literature melorheostosis has been anecdotally described in combination with nevi, hemangiomas, pelvic lipoma, lipoma of the spinal cord, chondrofibromas of the median nerve, tuberous sclerosis and

Thymic carcinoma is a rare, aggressive neoplasm with low 5-year survival rates ranging from 28 to 67%. Initial presentation with spinal or bone metastasis in primary thymic carcinoma is extremely rare. Thymic carcinoma, compared with thymoma, has higher recurrence rates and worse survival. We report one patient, a 29-year-old African-American male, with thymic carcinoma with metastasis to the epidural space (with cord compression), multiple bony structures (T10-L1) and left supraclavicular lymph node. Immunohistochemical staining was CD5 and c-Kit positive, consistent with thymic carcinoma. Pa

BACKGROUND: The prevalence of pain after treatment of a spinal intradural tumor is remarkably high, approximately up to 40% of the patients suffer from central neuropathic pain. Publications on spinal cord stimulation (SCS) and its effect on pain caused by intradural spinal tumors are rare. We discuss the case of a patient suffering from chronic pain after removal of a Th7 level meningioma who was successfully treated with SCS and give an overview of the literature.
METHODS: MEDLINE database was searched for neuropathic pain and intradural tumors.
RESULTS: The initial search identified 35 ar

Malignant spinal cord compression is a serious complication secondary to both primary and metastatic vertebral tumours, potentially leading to permanent loss of spinal functions. The Spanish Society of Neuroradiology (SENR), Spanish Society of Emergency Radiology (SERAU), and Spanish Society of Musculoskeletal Radiology (SERME) have convened to draft this consensus document, which describes practical aspects of the radiological management of malignant spinal cord compression. The document includes guidelines on appropriate indications for imaging studies, imaging modality options, technical sp

BACKGROUND: Metastatic spinal cord compression is a potentially devastating consequence of cancer. This oncological emergency requires early recognition and treatment to prevent irreversible spinal cord injury and paralysis. Provision of information to at risk patients has been recommended in the 2023 NICE Guidelines. However, it is unclear when, how and what information should be provided.
AIM: To investigate how healthcare professionals provide safety netting information to patients at risk of metastatic spinal cord compression.
METHODS: This scoping review utilised the Arksey and O'Malley

Diagnosis of central nervous system (CNS) granulomas is challenging. The etiology may be infectious or non-infectious. The infectious causes are due to mycobacteria, fungi, parasites and rarely bacteria. The non-infectious causes include autoimmune diseases, diseases of uncertain etiology like sarcoidosis, those associated with neoplasms and reparative processes. Histologic evaluation of type of granuloma as necrotizing, non-necrotizing, fibrotic/calcific or foreign-body type, site of CNS involvement (leptomeninges/dura, brain/spinal cord) and identification of etiologic agent on histochemistr

Paraneoplastic syndromes are systemic reactions to neoplasms mediated by immunologic or hormonal mechanisms. The most well-recognized paraneoplastic neurologic syndrome, both clinically and on imaging, is limbic encephalitis. However, numerous additional clinically described syndromes affect the brain, spinal cord, and peripheral nerves. Many of these syndromes can have imaging findings that, though less well described, are important in making the correct diagnosis. Moreover, imaging in these syndromes frequently mimics more common pathology, which can be a diagnostic challenge for radiologist

The peripheral nervous system (PNS) relays messages between the central nervous system (brain and spinal cord) and the body. Despite this critical role and widespread distribution, the PNS is often overlooked when investigating disease in diagnostic and experimental pathology. This review highlights key features of neuroanatomy and physiology of the somatic and autonomic PNS, and appropriate PNS sampling and processing techniques. The review considers major classes of PNS lesions including neuronopathy, axonopathy, and myelinopathy, and major categories of PNS disease including toxic, metaboli

A direct causal relationship between a human virus and malignant transformations in target cells (sensory neuronal precursors) was suggested by the development of a medulloepitheliomatous neoplasm in the central nervous system. Twenty-six newborn Sprague-Dawley rats were given a single intracerebral inoculation of 0.05 ml of adenovirus fluid, 10(3.5) to 10(4.5) TCID(50) HeLa cells/0.1 ml, in the left frontal lobe. Within 37 to 151 days after the virus inoculation, 23 (88.7%) rats autochthonously developed an adenovirus-typical neoplasm in the central nervous system. Nine animals developed a mu

A 2 1/2-year-old female with a sphenooccipital-vertebral chordoma presented with neck pain, torticollis, fever, a lytic lesion of C2 vertebra, and bilateral nodular infiltrates in the lung. The lung biopsy revealed multiple tumor emboli by an enigmatic epithelioid-appearing neoplasm with immunohistochemical staining for vimentin, cytokeratin, and epithelial membrane antigen. A thorough roentgenographic evaluation disclosed a destructive, prepontine mass in the region of the clivus, erosion of the odontoid process, and compression of the cervical spinal cord. The patient died after a clinical c

AIM: Our intention was to examine the value of ultrasonography in the case of spinal stenosis caused by bony fragments or a tumour. Pre- and postoperative computed tomographies were compared with intraoperative ultrasound images.
METHOD: Intraoperative sonography during spinal surgery was used after laminotomy or laminectomy in twenty patients with a stenosis of the spinal canal, caused by fragments or tumours.
RESULTS: The posterior vertebral wall as well as tumour-tissue and myelon can be well distinguished by sonography. The dural sheath remains sonographically visible as a white line bet

A 74-year-old woman had acute cervical transverse myelopathy from a neurilemmoma. Although extremely rare, the possibility of tumor should be recognized in patients with apparently spontaneous spinal subdural hematoma. As in other cases of compressive myelopathy, early operation is essential for recovery of neurological deficit.

Capillary haemangioma is a benign tumour frequently encountered in the skin and other soft tissues. Histologically, these vascular lesions are characterised by nodules of capillary-sized vessels lined by flattened endothelium, each of which is subserved by a feeding vessel. Capillary haemangioma of the central and peripheral nervous system is extremely rare. Less than 20 of these lesions have been described as occurring within the confines of the spinal dura mater, in close relation to the conus medullaris and nerve roots of the cauda equina. The presenting symptoms are similar to those of mor

Longstanding, severe hyperparathyroidism (HPT) can lead to the formation of "brown tumors". A brown tumor is a radiolucent bone lesion that is locally destructive; it is not a neoplasm, but rather a stromal mass consisting of fibrous tissue, poorly mineralized woven bone, and supporting vasculature. These tumors are a rare complication of advanced primary or secondary HPT. We present a young female with chronic kidney disease (CKD) on hemodialysis with uncontrolled secondary HPT (SHPT). The patient presented with progressive lower extremity weakness and back pain. CT imaging showed multiple ly

BACKGROUND: Cervical spinal manipulation therapy is a common non-invasive treatment for neck pain and stiffness, and has been widely used in the population. However, most people do not pay attention to the potential risks of neck manipulation, such as ligament damage, fractures, and spinal cord injuries. Epidural hematoma is a disease in which blood accumulates in the epidural space of the vertebral body. This disease is usually caused by trauma or iatrogenic surgery, and may be associated with blood coagulopathies, neoplasms, or degenerative spinal disease. Reports of epidural hematoma caused

Actinomycosis is a slowly progressing infection caused by Actinomyces species, which consists of filamentous gram-positive bacteria. Intraspinal actinomycosis is very rare and most of the previous cases presented with epidural lesions. Only two cases of intrathecal actinomycosis have been described. We reported a case of intrathecal actinomycosis in a 46-year-old woman. Our patient presented with multisegmental root failure, which was different from previous intrathecal cases mainly involving the spinal cord. The manifestations, cervical MR imaging results, biopsy and histopathological feature
