To explore the origin of clicking sounds in the ear during deglutition or other pharyngeal movements, which are interpreted differently in the literature. Experimental study at a tertiary referral centre. Acoustic phenomena during a forced opening test of the Eustachian tube (ET) were studied in a temporal bone model. Additionally, in vivo experiments were carried out in healthy volunteers for ruling out movements of the ossicular chain or the drumhead as potential causes of clicks. Thus, acoustic recordings were performed parallel to stapedius or tensor reflex measurements or pneumatic video

The probability of system failures increases as the number of cochlear implants increases throughout the world. Whether a malfunction is a technical or physiological problem remains to be defined, particularly in very young children, while a psychogenic hearing disorder after implantation must not be excluded in adults. The battery of objective measurements used clinically at the Medizinische Hochschule, Hannover has provided useful diagnostic information for distinguishing possible causes of failure. In a normally functioning device, an electrical signal equivalent to the biphasic rectangular

A group of 40 elderly subjects were evaluated using a variety of audiological tests. Their mean age was 68 years. They had all come to the clinic because of hearing problems. The subjects were selected for this study because no cause of their hearing loss could be found other than presbycusis. They all had the typical moderately sloping pure tone audiogram configuration. The pure tone average hearing loss (PTA, average of 0.5, 1 and 2 kHz) ranged from 11 to 70 dBHL with a median of 40. The average air-bone gap was 3 dB. In addition to pure tone audiometry the following tests were performed: mo

The acoustic middle ear muscle reflex was studied in albino rats anesthetized with chloralose. The best frequency of the reflex and the threshold at this frequency were on average about 3 kHz and 57 dB SPL, respectively. The threshold increased as frequency increased above, and decreased below, the best frequency at a rate of about 20 dB/octave. Above about 12 kHz, the muscular response showed instability and habituation. Thresholds were similar between stapedius and tensor tympani reflexes and between ipsilateral and contralateral reflexes. The middle ear transmission loss due to the reflex w

HYPOTHESIS: Surgical manipulations during laser stapedotomy can produce intracochlear pressure changes comparable to pressures created by high-intensity acoustic stimuli.
BACKGROUND: New-onset sensorineural hearing loss is a known risk of stapes surgery and may result from pressure changes from laser use or other surgical manipulations. Here, we test the hypothesis that high sound pressure levels are generated in the cochlea during laser stapedotomy.
METHODS: Human cadaveric heads underwent mastoidectomy. Fiber-optic sensors were placed in scala tympani and vestibuli to measure intracochlear

The aim of this study was to examine the anatomical landmarks of the retrotympanum using two different techniques, virtual endoscopy (VE) and fiberoptic endoscopy, and to correlate the results furnished by the two methods. Ten otosclerotic patients who were due to undergo stapedectomy were scanned using high-resolution spiral CT. Selected CT datasets were processed with Navigator 2.0 software to obtain virtual endoscopic views of the retrotympanum. Subsequently, during the surgical procedure, fiberoptic endoscopy was performed with 2.7-mm 0 degrees and 30 degrees rigid endoscopes. The ability

BACKGROUND: Stapes reflex test is a method of evaluating the involuntary muscle contraction of the stapedius muscle in response to a high-intensity sound stimulus. The formation of this reflex involves the intact function of the 7th nerve, brain stem, 8th nerve, and middle ear. Due to ease of administration and information yielded, the stapedial reflex is considered one of the most powerful differential diagnostic audiological procedures. Numerous studies have remarked on the fluid communication between the intracochlear and intracranial spaces through the cochlear aqueduct. Currently, the pot

The hearing threshold level was examined in 75 cerebral palsied children (3--15 yrs.). In the case of 5 patients, we were unable to get an audiogram. 50 children had a normal audiogram on both ears. 13 patients had a bilateral, one a unilateral sensoneural loss of hearing. The rate of conductive loss of hearing was unexpectedly high: 4 patients with a bilateral pure conductive loss of hearing, 3 with a bilateral sensoneural loss of hearing and a bilateral additional airborne gap, and 2 with a monaural conductive loss of hearing. The treatment of the conductive deficiency is of primary importan

Two areas of the damage localization in the organs of hearing after streptomycin use were found in the experiments with guinea pigs, i.e. (1) the Corti's organ of the lower part of the basal helix, the zone of perception of the maximum frequency sounds and (2) the tympanic muscles. Administration of streptomycin subcutaneously in a daily dose of 300 mg/kg for 50 days resulted in pronounced atrophic changes in the fibres of m. tensor tympani and some atony of m. stapedius. On the basis of the histological examination of the tympanic muscles after prolonged use of streptomycin and comparison of

Results of impedancemetry have been compared with operative findings in 1 867 cases of otosclerosis, 1 583 of which were pure otosclerosis. This comparison indicated that: (1) with present techniques, the results of impedancemetry are only of a relative value in otosclerosis; it is thus necessary to asses the various connected anatomical elements in the middle ear with extreme rigor; (2) impedancemetry data have an absolute value in all disorders of the middle ear with closed eardrum associated with otosclerosis; (3) relative impedancemetry often gives informative results when it concurs with

This study examined the precision of the bivariate method in subjects with high-frequency sensorineural hearing loss. The current bivariate data effectively separated normal hearing subjects from those with pure tone averages of greater than 32 dB HL, in a manner consistent with the results of Popelka and Trumpf (1976) and Margolis and Fox (1977b). However, for persons with high-frequency losses the prediction of hearing levels from acoustic reflex thresholds (ARTs) appears to be complicated. Moderate hearing losses involving 500, 1000 and 2000 Hz ("speech frequencies") as well as higher frequ

The persistence of the stapedial artery is a rare vascular anomaly. It is mostly asymptomatic but sometimes cause conductive hearing loss, pulsatile tinnitus, or vertigo. The estimated prevalence of this rare postembryonic persistence ranged from 0.02% to 0.48%. Four different anatomical forms have been identified, and their preoperative diagnostic is essential. We report the case of an incidental discovery of pharyngo-hyo-stapedial artery, the most uncommon form of persistent stapedial artery. Its per-operative finding has become rare because tomodensitometry is performed in case of conductiv

Vestibular responses (vertigo, nystagmus-like eye movements) to acoustic stimuli are known as the "Tullio phenomenon". Detailed electro-oculographic analysis of this reaction, as observed in a 30-year-old patient, revealed the following: a maximum amplitude of eye movement (mainly vertical) was achieved by sine wave bursts of high intensity, a frequency of 500 to 1000 Hz and a duration of 100 ms. The ocular deviation was composed of a fast initial component, followed by a slower resetting movement that was often divided into two parts of different velocities. At longer stimulus durations (more

BACKGROUND: Cat scratch disease is a common infectious disorder caused by Bartonella henselae that is transmitted primarily by kittens. It typically exhibits a benign and self-limiting course of subacute regional lymphadenopathy and fever lasting two to eight weeks. The most severe complication of cat scratch disease is involvement of the nervous system, such as encephalitis, meningitis, and polyneuritis. Peripheral facial nerve palsy associated with Bartonella infection is rare; few reported pediatric and adult cases exist and the precise pathogenesis is unknown.

HYPOTHESIS: To study the detailed anatomy of the incudostapedial joint (ISJ).
BACKGROUND: Detailed study of the anatomy of the ISJ has been surprisingly neglected and continues to be controversial.
METHODS: We studied the joint in 86 temporal bones from 51 subjects, aged 12 to 104 years, by light microscopy and three-dimensional images of computer-based reconstructions.
RESULTS: In the course of this study, we found that the lenticular process is extremely delicate, consisting of thin trabeculae and spaces filled with areolar tissue. The ISJ is at least partially a bichambered diarthrodial

Progress in new audiometric techniques, chiefly in impedance-audiometry, and better knowledge of the innermost mechanism of the otospongiotic disease, have led the authors to plan prevention of the otospongiotic disease, which appears to be a genetic deafness with an autosomal dominant inheritance and about 40% penetrance of genes. The bases of this prevention are: first, the early detection of stapedial fixation by means of systematic audiometric investigations, particularly elicitation of the on-off effect by impedance-audiometry; second, the treatment of oto-spongiotic children with very lo
