A New Horizon in Hip Health: Using an Innovative Variable Loop Curette for Core Decompression and Bone Marrow Aspirate Concentrate in the Management of Pre-collapse Avascular Necrosis of the Hip.

Avascular necrosis (AVN) of the femoral head, or osteonecrosis (ON), is a debilitating condition characterized by disrupted blood supply to the hip joint, leading to subchondral bone necrosis, joint collapse, and arthritis. Emerging evidence suggests that the long-term use of corticosteroids, particularly in the context of COVID-19 treatment, may contribute to AVN development. This case report presents a male in his 50s with bilateral hip pain and a history of corticosteroid use. The patient underwent core decompression (CD) with a bone marrow aspirate concentrate (BMAC) infusion using the inn

Acquired laryngomalacia as a cause of post-extubation stridor and extubation failure following craniotomy: a case report.

BACKGROUND: Laryngomalacia is a dynamic airway condition characterized by flaccid laryngeal tissue and inward collapse of supraglottic structures during inspiration. Although it may cause airway obstruction and requires careful management, there have been few reports regarding laryngomalacia after surgery. We report a case of adult-onset laryngomalacia occurred after craniotomy requiring reintubation. CASE PRESENTATION: A 21-year-old man was admitted to the ICU after craniotomy for a cerebellopontine angle tumor. He developed severe stridor immediately after extubation on the postoperative da

Core Decompression With Adinizer-Processed Minimally Manipulated Adipose Tissue for Osteonecrosis of the Femoral Head: A Case Report.

Osteonecrosis of the femoral head (ONFH) is a progressive disorder that may lead to femoral head collapse and eventual total hip arthroplasty, particularly in relatively younger patients. Joint-preserving strategies remain of interest, and biologic augmentation has recently been explored as an adjunct to core decompression. We report a case of right ONFH treated with core decompression combined with minimally manipulated adipose tissue (microfragmented adipose tissue, MFAT) processed using an Adinizer system. A woman in her 50s presented with a five-month history of right hip pain. Baseline im

Upper extremity nerve entrapments: the axillary and radial nerves--clinical diagnosis and surgical treatment.

BACKGROUND: Nontraumatic pain in the shoulder, arm, and hand (brachialgia) is a common complaint in the field of musculoskeletal disorders, where nerve entrapment constitutes a possible cause. The effect of nerve compression is dose-dependent; thus, a low-level compression will only result in decreased endoneurial circulation, neural edema, and a Seddon grade IV weakness, but will not be revealed in nerve conduction or magnetic resonance imaging studies. Because of technical limitations, several clinical options to diagnose compression neuropathies in the upper extremity have been proposed. Th

Sitting posture decreases collapsibility of the passive pharynx in anesthetized paralyzed patients with obstructive sleep apnea.

BACKGROUND: Obstructive sleep apnea (OSA) is an independent risk factor for difficult and/or impossible mask ventilation during anesthesia induction. Postural change from supine to sitting improves nocturnal breathing in patients with OSA. The purpose of this study was to evaluate the effect of patient position on collapsibility of the pharyngeal airway in anesthetized and paralyzed patients with OSA. The authors tested the hypothesis that the passive pharynx is structurally less collapsible during sitting than during supine posture. METHOD: Total muscle paralysis was induced with general ane

A Novel Septoplasty Technique for Patients With Nasal Fractures.

Nasal structures have both functional and cosmetic significance. These structures maintain the shape of the nose and regulate the nasal airflow. During trauma, fractures of the nasal bone are frequently associated with nasal septum deviations. This can lead to the nasal bone collapsing and nasal obstruction. The septoplasty technique is a major surgical intervention to improve nasal obstructions, with the submucosal resection of the deviated septum. In the past, septoplasty was deferred until the nasal bone fracture was healed to reduce the postoperative risk of saddle-nose and flat nose defor

Listeria Membrane Protrusion Collapse: Requirement of Cyclophilin A for Listeria Cell-to-Cell Spreading.

BACKGROUND: Listeria generate actin-rich tubular protrusions at the plasma membrane that propel the bacteria into neighboring cells. The precise molecular mechanisms governing the formation of these protrusions remain poorly defined. METHODS: In this study, we demonstrate that the prolyl cis-trans isomerase (PPIase) cyclophilin A (CypA) is hijacked by Listeria at membrane protrusions used for cell-to-cell spreading. RESULTS: Cyclophilin A localizes within the F-actin of these structures and is crucial for their proper formation, as cells depleted of CypA have extended actin-rich structures t

A sensor-adaptor mechanism for enterovirus uncoating from structures of EV71.

Enterovirus 71 (EV71) is a major agent of hand, foot and mouth disease in children that can cause severe central nervous system disease and death. No vaccine or antiviral therapy is available. High-resolution structural analysis of the mature virus and natural empty particles shows that the mature virus is structurally similar to other enteroviruses. In contrast, the empty particles are markedly expanded and resemble elusive enterovirus-uncoating intermediates not previously characterized in atomic detail. Hydrophobic pockets in the EV71 capsid are collapsed in this expanded particle, providin

Surgical Management of Nasal Airway Obstruction.

The management and diagnosis of nasal airway obstruction requires an understanding of the form and function of the nose. Nasal airway obstruction can be structural, physiologic, or a combination of both. Anatomic causes of airway obstruction include septal deviation, internal nasal valve narrowing, external nasal valve collapse, and inferior turbinate hypertrophy. Thus, the management of nasal air obstruction must be selective and carefully considered. The goal of surgery is to address the deformity and not just enlarge the nasal cavity.

C7 fracture treated with a pedicle screw system under navigation guidance.

A 51-year-old woman with traumatic fracture-subluxation of C6-C7 vertebrae was treated by pedicle screw fixation. Among several methods for surgical treatment of the cervical spine, this technique provides a more rigid anchor to prevent collapsing or instability of the spinal column, particularly at the cervicothoracic junction. However, the risk of injuring the adjacent neurovascular structures cannot be completely eliminated. The characteristics of the cervicothoracic junction, surgical approaches and pedicle screw fixation techniques are emphasised.

Orofacial complications associated with forward repositioning of the mandible in snore guard users.

Snoring and obstructive sleep apnea (OSA) result from a collapse of the tongue, soft palate, and pharynx, causing temporary airway blockage. Acrylic mandibular advancement splints and Herbst appliances are used to relieve snoring and mild OSA. Repositioning the mandible forward may have an adverse effect on the orofacial and dentoalveolar structures of susceptible individuals. This article reports two cases in which orofacial complications developed following the utilization of snore guard appliances.

The regeneration of soft tissue and bone around implants with and without membranes.

The popularity of osseointegration for the replacement of anterior teeth has placed an increased demand on the preservation of normal gingival anatomy. This has led to the development of a variety of soft-tissue and bone-augmentation techniques that either prevent tissue collapse or restore previously damaged areas of the alveolus. With bone grafts, barrier membranes, and autogenous connective-tissue grafts used in combination with each other or separately, lost structures can be reconstructed into a normal configuration for optimum esthetic restorations.

Glutamine maintains the stability of alveolar structure and function after lung transplantation by inhibiting autophagy.

Excessive autophagy may lead to degradation and damage of alveolar epithelial cells after lung transplantation, eventually leading to alveolar epithelial cell loss, affecting the structural integrity and function of alveoli. Glutamine (Gln), a nutritional supplement, regulates autophagy through multiple signaling pathways. In this study, we explored the protective role of Gln on alveolar epithelial cells by inhibiting autophagy. In vivo, a rat orthotopic lung transplant model was carried out to evaluate the therapeutic effect of glutamine. Ischemia/reperfusion (I/R) induced alveolar collapse,

Urbanization impacts the environmental microbial community and its antibiotic phenotypic resistance potential by reshaping the diversity and network complexity.

Urbanization imposes severe anthropogenic pressures on ecosystems, yet its impacts on the structural cohesion of environmental microbiomes and the co-occurrence of antimicrobial resistance (AMR) remain poorly understood. Here, we combined metabarcoding (16S/ITS2) with in vitro AMR screening of cultivable isolates across aquatic and terrestrial ecosystems in Nouvelle-Aquitaine (France). Random Forest models (RFM), co-occurrence and bipartite networks were used to quantify ecological drivers and evaluate network resilience via percolation simulations. While urbanization categorical diversity ind

Length-Dependent Structural Transformations of Huntingtin PolyQ Domain Upon Binding to 2D-Nanomaterials.

There is a strong negative correlation between the polyglutamine (polyQ) domain length (Q-length) in the intrinsically disordered Huntingtin protein (Htt) exon-1 and the age of onset of Huntington's disease (HD). PolyQ of Q-length longer than 40 has the propensity of forming very compact aggregate structures, leading to HD at full penetrance. Recent advances in nanobiotechnology provided a new platform for the development of novel diagnosis and therapeutics. Here, we explore the possibility of utilizing 2D-nanomaterials to inhibit the formation of supercompact polyQ structures through the so-c

Accelerated serum sickness in the rabbit. II. Glomerular ultrastructural lesions in transient proliferative and progressive disorganizing glomerulonephritis.

Electron microscopic analysis was performed on the development of irreversible glomerular distortion in experimental serum sickness nephritis in the rabbit. In the animals showing transient albuminuria, glomerular hypercellularity was due to the accumulation of monocytes and polymorphonuclear leukocytes and was seen to recover to nearly normal glomerulus. In this condition the glomerular structure was observed to be well preserved throughout the inflammation. In contrast, in the animals showing persistent proteinnuria, a disorganizing process was found in their glomeruli. Mesangial disintegrat

Molecular insights into structure and solvation of temperature-responsive PNIPAM-grafted gold nanoparticle.

Efficient design of thermoresponsive polymer-grafted metal nanoparticles necessitates a microscopic understanding of polymer conformational reorganization, interfacial interactions and the role of hydration in response to a change in temperature. This study focuses on investigating the structure and solvation of poly(N-isopropylacrylamide (PNIPAM)-grafted gold nanoparticles (AuNPs) with varying temperature, using molecular dynamics simulations. Our findings demonstrate that the temperature-dependent reduction in the effective size of the nanoparticles can be achieved when the polymer chains ei

Extracellular Vesicles in Osteonecrosis of the Femoral Head: An Integrated Review of Experimental and Bioinformatic Evidence.

Background/Objectives: Osteonecrosis of the femoral head (ONFH) is a progressive condition characterized by bone necrosis, impaired vascularization, and immune dysregulation, often resulting in femoral head collapse. Effective strategies to halt disease progression are limited. Extracellular vesicles (EVs), including exosomes and microvesicles, mediate intercellular communication and influence osteogenesis, angiogenesis, and immune responses. This review summarizes current evidence on EVs in ONFH and their translational potential. Methods: A structured narrative review of PubMed, Scopus, Web o

Prelaminated Nose Reconstruction: Comparison of Forehead and Radial Donor Site and Review of the Literature.

INTRODUCTION: Total nose reconstruction is demanding as it is a 3-dimensional structure that needs lining, support and external coverage. Usually, several stages are needed to achieve a satisfactory result. The authors present 2 cases of prelaminated radial forearm and 2 prelaminated forehead nose reconstructions and compare both methods. According to our review of the literature, this is the first report of prelaminated forehead for total nose reconstruction. MATERIALS AND METHODS: The last 5 years the authors have treated 4 patients with prelaminated flaps for total nose reconstruction. The
