The impact of a closing protocol on wound morbidity in abdominal wall reconstruction with mesh.

INTRODUCTION: Wound complications (WC) after abdominal wall reconstruction (AWR) are associated with increased cost, recurrence, and mesh infection. Operative closing protocols (CP) have been studied in other surgical disciplines but not in AWR. Our aim was to study the effect of a CP on WC after AWR. METHODS: The CP consists of antibiotic wound irrigation, glove and complete instrument exchange, and re-draping of the sterile field to cover the skin entirely prior to mesh implantation. A prospective institutional database at a tertiary hernia center was queried for patients who underwent open

Open Reduction and Internal Fixation of Both-Bones Forearm Fractures.

Compared with closed reduction or older fixation methods, open reduction and compression plate fixation has dramatically improved the outcomes of displaced diaphyseal forearm fractures. However, the procedure can be technically demanding, with implant choice, surgical approach, accuracy of reduction, and sufficient fracture stability to allow early postoperative motion all having been shown to affect outcome. The procedure is performed with the following steps:The patient is positioned supine on the operating room table with the arm on a hand-table and a tourniquet applied to the upper arm.Aft

6-Month Postoperative Magnetic Resonance Imaging Appearance of Osteochondral Allografts With Bone Marrow Aspirate Augmentation From Either the Proximal Tibia or Iliac Crest.

BACKGROUND: There has been increased interest in the biologic augmentation of osteochondral allografts (OCAs), specifically the use of autologous bone marrow, to decrease failure rates. The iliac crest is a common donor site, but its use is associated with increased pain separate from the primary surgical site and increased complexity in the operating room setup and draping. Local bone marrow aspirate (BMA) from the proximal tibia or femoral condyle addresses these limitations, but the quality of the aspirate may be lower. It has not been determined whether this difference in aspirate quality

Elbow Arthroscopy for Treatment of Osteochondritis Dissecans of the Capitellum.

BACKGROUND: Osteochondritis dissecans (OCD) of the elbow is an idiopathic lesion of subchondral bone resulting in delamination and sequestration with or without articular cartilage involvement; for patients with persistent symptoms, surgical treatment is often recommended. While traditionally performed in the supine or prone position, arthroscopic debridement and microfracture of OCD of the capitellum in the lateral decubitus position permits strategic arm positioning and placement of portals to optimize visualization and reduce the risk of damage to neurovascular structures. INDICATIONS: Art

Fixation of Displaced Medial Epicondylar Fractures in Adolescents.

UNLABELLED: Fractures of the medial epicondyle are often a marker of injury of the medial collateral ligament complex of the elbow, regardless of displacement. The medial epicondyle serves as the origin for the flexor/pronator mass superficially and the medial collateral ligament near the base. These fractures occur most commonly through the apophysis at the base of the epicondyle, making differentiation of muscular versus ligamentous avulsion difficult. Fractures associated with elbow dislocation and fractures with an intra-articular incarcerated medial epicondyle are ligamentous injuries, re

Extended negative pressure wound therapy-assisted dermatotraction for the closure of large open fasciotomy wounds in necrotizing fasciitis patients.

BACKGROUND: Necrotizing fasciitis (NF) is a rapid progressive infection of the subcutaneous tissue or fascia and may result in large open wounds. The surgical options to cover these wounds are often limited by the patient condition and result in suboptimal functional and cosmetic wound coverage. Dermatotraction can restore the function and appearance of the fasciotomy wound and is less invasive in patients with comorbidities. However, dermatotraction for scarred, stiff NF fasciotomy wounds is often ineffective, resulting in skin necrosis. The authors use extended negative pressure wound therap

Safety and Efficacy of Robotic Hysterectomy Using an Indigenous Robotic System: A Retrospective Study.

Background Minimally invasive hysterectomy approaches, including vaginal, laparoscopic, and robotic hysterectomy, offer advantages over laparotomic hysterectomy. However, vaginal hysterectomy is less feasible for patients with a large uterus, prior pelvic surgery, adnexal surgery requirements, or malignancy, making laparoscopy or robotic surgery preferable. The SSI Mantra robotic system (Sudhir Srivastava Innovations Pvt. Ltd., India) features an ergonomic open-faced console with a 32-inch 3D 4K monitor, a 23-inch 2D touch monitor for system control and DICOM applications, a head-tracking safe

Surgical Treatment of Severe Cavovarus Foot Deformity in Charcot-Marie-Tooth Disease.

INTRODUCTION: A successful adjusted treatment algorithm for the correction of cavovarus foot deformity requires soft-tissue balancing procedures, in particular total split posterior tibial tendon transfer (T-SPOTT), in combination with adjunctive corrective procedures depending on the degree of deformity. STEP 1 SURGICAL PREPARATION: Place the patient in a supine position and follow a standard aseptic surgical disinfection and draping protocol, allowing access to the iliac crest. STEP 2 STEINDLER RELEASE OF THE PLANTAR APONEUROSIS: Use a medial approach to access and transect the plantar apo

Comparison of Clinical Outcomes According to Preoperative Skin Preparation in Hip Arthroplasty for Hip Fracture: A 3-Arm Randomized Controlled Trial.

BACKGROUND: Patients with hip fractures undergoing arthroplasty are typically old and frail, and superficial surgical site infections (SSIs) can delay mobilization and prolong hospitalization. Preoperative skin antisepsis is a low-cost, universally used intervention. However, clinical practice varies, and evidence from elective arthroplasty may not fully generalize to urgent hip fracture surgery. Sequential dual-antiseptic protocols combining povidone-iodine (PVI) and chlorhexidine gluconate (CHG) are increasingly adopted; however, the optimal order for "scrub" versus "paint" remains unclear.

Four-Compartment Fasciotomy for Chronic Exertional Compartment Syndrome.

BACKGROUND: Chronic exertional compartment syndrome (CECS) is characterized by pain related to exercise that resolves with a period of rest. Along with these symptoms, CECS is commonly diagnosed based on an increase in intercompartmental pressure during exercise. A negative magnetic resonance imaging (MRI) is helpful to rule out commonly presenting symptoms. While fasciotomy remains the gold standard for patients who require more than conservative treatment, sex-specific outcome data require further investigation. INDICATIONS: This case presents a 17-year-old female adolescent with a 2-year h

Percutaneous Pinning of Pediatric Proximal Humeral Fractures.

BACKGROUND: Proximal humeral fractures are relatively common in pediatric patients. These injuries are usually treated nonoperatively in younger children or children with minimally displaced fractures. However, closed reduction or open reduction followed by percutaneous pinning is recommended for older children with displaced fractures. Percutaneous pinning has several advantages, but there are limited reports of a safe and reliable surgical technique in the literature. DESCRIPTION: Patients are positioned in a modified beach-chair position to allow orthogonal imaging. The injured extremity i

Sliding Hip Screw and Side Plate for Intertrochanteric Hip Fractures.

UNLABELLED: For stable intertrochanteric hip fractures, treatment commonly involves the use of a sliding hip screw. Intertrochanteric hip fractures are increasingly common as the population ages and lives longer. More than 250,000 hip fractures occur per year in the United States1. The mortality rate within the first year following operative treatment ranges from 14% to 27.3%2,3. Early surgical repair within 48 hours of injury is associated with a lower risk of mortality2,4,5. The goals of surgical treatment are restoration of coronal plane alignment without varus angulation and early patient

A Modified Self-Growing Rod Technique for Treatment of Early-Onset Scoliosis.

UNLABELLED: Surgical treatment of early-onset scoliosis (EOS) remains challenging as no definitive surgical technique has emerged as the single best option in this varied patient population1-3. Although the available surgical techniques may differ substantially, they all share the same goals of achieving and maintaining deformity correction, allowing physiological spinal growth, and reducing the number of operations and complications. Herein, we present a modified self-growing rod technique that represents a valid alternative to the existing surgical procedures for EOS. DESCRIPTION: The patie

Knee Arthroscopy: A Simulation Demonstrating the Imperial Knee Arthroscopy Cognitive Task Analysis (IKACTA) Tool.

BACKGROUND: Virtual reality and cadaveric simulations are expensive and not readily accessible1. Innovative and accessible training adjuncts are required to help meet training needs. Cognitive task analysis (CTA) has been used extensively to train pilots and surgeons in other surgical specialties2-6. However, the use of CTA tools within orthopaedics is in its infancy. Arthroscopic procedures are minimally invasive and require a different skill set compared with open surgery. Residents often feel poorly prepared to perform this in the operating room because of the steep learning curve associate

Open Release of Pediatric Trigger Thumb.

UNLABELLED: Open release of a trigger thumb has been shown to be the most reliable option to restore full interphalangeal (IP) joint extension and thus normal thumb-joint motion in children1. The aim of this procedure is to restore free gliding of the flexor pollicis longus (FPL) tendon in its canal in children with fixed IP joint flexion contractures or those in whom nonoperative treatment modalities have failed. DESCRIPTION: The surgical procedure is easy to perform and straightforward; however, attention must be given to several details in order to avoid surgical failure and complications.
