Central and peripheral adrenergic mechanisms regulating gastric secretion in the rat.

Subcutaneous injection of sympathomimetic agents reduced gastric secretion in pylorus-ligated rats, and the decreasing order of activity was: isoproterenol, norepinephrine, naphazoline and phenylephrine. The effect of naphazoline and phenylephrine was antagonized with 4 mg/kg s.c. of phentolamine, and that of isoproterenol with 4 mg/kg s.c. of propranolol. Thus there exist separate, alpha and beta adrenergic receptors which control gastric secretion in the rat. Intracerebroventricular (i.c.v.) injection of the sympathomimetic agents also reduced secretion, naphazoline being the most potent. Th

Idiopathic orthostatic hypotension from failure of noradrenaline release in a patient with vasomotor innervation.

A 26 year old man is described with life-long orthostatic hypotension unrelated to autonomic nerve degeneration and apparently due to failure of peripheral noradrenaline realese. Tests of parasympathetic and sympathetic cholinergic nerve function were normal, but sympathetic adrenergic activity was defective. Thus blood pressure regulation was abnoraml. There was no pressor response to tyramine, an indirect sympathomimetic drug, but a marked pressor response to the directly acting sympathomimetic drugs phenylephrine and noradrenaline. On standing there was a progressive fall rather than a rise

Pulmonary oedema after hexoprenaline administration in preterm labour. A report of 4 cases.

Despite the widespread use of beta-sympathomimetic agents for preterm labour there appears to be a limited appreciation of the need for cardiovascular monitoring in the mother. Four patients in whom pulmonary oedema developed during tocolysis with hexoprenaline are described and the aetiological factors and pathogenesis of this potentially lethal complication discussed. Guidelines for the safe use of hexoprenaline in preterm labour are suggested.

Neurological and cardiac events on phenylpropanolamine.

(1) Phenylpropanolamine, or norephedrine, is a sympathomimetic vasoconstrictor very similar to amphetamine, and carries a risk of potentially severe cardiac, neurological and psychiatric events. (2) One should be aware of the presence of phenylpropanolamine in many non prescription drugs. (3) Pharmacists should pay special attention to the presence of phenylpropanolamine in freshly prepared slimming mixtures.

Pharmacotherapy of respiratory muscles.

This article assesses the role of pharmacotherapy in the management of respiratory muscle dysfunction. It focuses on two classes of drugs, the methylxanthines and the sympathomimetic agents. A prospective section focuses also on the particularities of the diaphragm among the skeletal striated muscles. In addition, a new approach to respiratory pharmacotherapy, which may be beneficial in patients with respiratory muscle dysfunction, is suggested.

Reducing Cardiovascular Side Effects of DMT Using Beta-Blockers.

DMT induces rapid antidepressant effects, but acute sympathomimetic side effects, particularly hypertension and tachycardia, limit its use. This study describes coformulations with short-acting beta-blockers or nitrates, matched to DMT's pharmacokinetics, to mitigate peripheral cardiovascular risks without compromising central therapeutic action, facilitating safer psychedelic therapy in vulnerable populations.

Altered response to intravenous thiopental and succinylcholine in acute amphetamine abuse.

Substance abuse has become increasingly prevalent: illegal drugs have profound and varied physiologic effects which create a large potential for anesthetic problems and complications (1). Amphetamine is a strong sympathomimetic and may therefore influence the course of anesthesia. We report the case of a patient with acute amphetamine abuse presenting difficulties during anesthesia.

Pharmacotherapy of chronic obstructive pulmonary disease.

The pharmacologic treatment of the reversible elements of chronic obstructive pulmonary disease is discussed in this article. Discussion focuses on three classes of bronchodilator drugs--the sympathomimetic agents, the methylxanthines, and the anticholinergic agents. A section on corticosteroid use in patients with chronic airflow limitation is included. An integrated approach to pharmacotherapy is suggested that allows a treatment program to be designed to meet the needs of the individual patient.

Exploring the photodynamical landscape of biomimetic lumichrome-ephedrine-class amine complexes across femtosecond to millisecond regimes.

Flavin-amine interactions are central to neurotransmitter metabolism, yet how H-bonding and amine structure govern excited-state pathways remains unclear. Using lumichrome (LC) as a biomimetic model, we examine interactions and photobehaviour with sympathomimetic amines, norephedrine (Neph), ephedrine (Eph), and pseudoephedrine (Peph). In water, LC with amines readily forms anions. In dichloromethane, H-bonding drives ultrafast single-proton transfer (SPT) in the LC-Neph complex (~800 fs), followed by population redistribution (~14 ps) and a long-lived anion (τ ~5.4 ns), while LC-Eph and LC-Pe

The use of appetite suppressants among health sciences undergraduate students in Southern Brazil.

OBJECTIVE: To investigate the prevalence of appetite suppressant use among health sciences students in Southern Brazil. METHODS: Undergraduate students (n=300) from seven health science undergraduate courses of the Universidade de Caxias do Sul completed a questionnaire about the use of substances to suppress appetite. RESULTS: A significant percentage (15%; n=45) of research participants used appetite suppressants at least once in their lives. The most commonly used substances were sympathomimetic stimulant drugs (5%), including amfepramone (3.3%) and fenproporex (1.7%). The lifetime use of

Ritodrine hydrochloride infusion in pregnant baboons. I. Biophysical effects.

Pulmonary edema is a potentially fatal complication of beta-sympathomimetic therapy for premature labor. Isolated case reports have supported either primary pulmonary capillary membrane injury or left ventricular failure as the cause of pulmonary edema. By simultaneously monitoring cardiac function and extravascular lung water in six control and six ritodrine hydrochloride-treated pregnant baboons, we attempted to define this pathophysiology. The pulmonary capillary wedge pressure increased in treated animals, reaching significance at 16 hours of ritodrine treatment (p less than 0.0001) concur

[Sympathetic activity of S-(+)-ketamine low doses in the epidural space].

BACKGROUND AND OBJECTIVES: S-(+)-ketamine is an intravenous anaesthetic and sympathomimetic with properties of local anaesthetic. It has an effect of an analgetic and local anaesthetic when administered epidurally, but there are no data whether low doses of S-(+)-ketamine have sympathomimetic effects. The aim of this study was to determine whether low doses of S-(+)-ketamine, given epidurally together with local anaesthetic, have any effect on sympathetic nervous system, both systemic and below the level of anaesthetic block. METHODS: The study was conducted on two groups of patients to whom

Xylometazoline abuse induced ischemic stroke in a young adult.

INTRODUCTION: substance abuse is an important cause of ischemic stroke in the young. This includes over-the-counter dietary supplements and cough and cold remedies, which were reported to be an independent risk factor for hemorrhagic stroke. CASE REPORT: this article describes a young male patient with acute ischemic infarctions in the posterior inferior cerebellar and posterior cerebral artery territories bilaterally, the right cerebral peduncle, the left pontine tegmentum, and lateral pons following abuse of xylometazoline-containing nasal decongestant for 10 years. CONCLUSION: this is the

Update on the management of glaucoma.

Most cases of chronic open-angle glaucoma can be treated medically. When medical management is ineffective, laser trabeculoplasty is usually performed. If medical management and laser treatment fail, surgery is the next step. Recently, ultrasound has been employed successfully for control of glaucoma when medical, laser, and surgical treatments were not successful. Acute angle-closure glaucoma is an ophthalmic emergency. This condition can be treated at an early stage by relieving the pupillary block with peripheral iridectomy by laser or surgery. Four major classes of drugs are currently used

Terbutaline (intravenous bolus) for the treatment of acute intrapartum fetal distress.

beta 2-Sympathomimetics have been used in acute intrapartum fetal distress to abolish uterine contractions and thus enable the fetal metabolism to recover before delivery. Because some serious complications were reported when a terbutaline intravenous bolus (0.25 mg) was used as a tocolytic, we assessed its safety and efficacy when used in patients not affected by cardiovascular disease, tachycardia greater than 100 beats/min, thyrotoxicosis, fluid overload, corticoids, atropine, or severe abruptio placentae. No maternal or fetal complications occurred in the 36 patients studied; a well-tolera

Clinical experience using appetite suppressants and SSRIs.

The use of sympathomimetic appetite suppressants and serotonin-selective reuptake inhibitors (SSRIs) has been questioned due to anecdotal reports of serotonin syndrome. This survey of bariatric physicians using these medications in clinical practice did not find any cases of serotonin syndrome among 1174 patients. The monitored use of the combination of these medicines by trained practitioners is justifiable.

Drug-induced hyperthermia.

Drug-related causes of hyperthermia can often be overlooked in the setting of elevated body temperature. This article reviews the pathophysiology, presentation, and treatment of several drug-induced hyperthermia syndromes: malignant hyperthermia, neuroleptic malignant syndrome, sympathomimetic poisoning, and anticholinergic toxicity. Although the general approach is similar, specific management strategies may be required for each syndrome.

Interpreting an ECG. Atrial premature complex.

When APCs are symptomatic or are a potential trigger for supraventricular tachycardia or atrial fibrillation, suppressive therapy with beta-blocking drugs or type I antiarrhythmic agents is indicated. If APCs are infrequent and produce no symptoms, attention to possible provocative factors--excess tobacco or caffeine, or drugs (eg, sympathomimetic amines, monoamine oxidase inhibitors, tricyclic antidepressants)--is all that is required.

Sleeping bradycardia during recovery from childhood status asthmaticus.

Seventeen of 67 children recovering from status asthmaticus developed sleeping bradycardia. They were compared to a control population of asthmatic children who did not develop sleeping bradycardia and differed only in the low heart rate. The authors speculate that this phenomenon is due to a combination of withdrawal of sympathomimetic stimulation and catch-up in REM (rapid eye movement) sleep.
