Temporal but not spatial dysmetria relates to disease severity in FA.

Friedreich's ataxia (FA) is an inherited disease that causes degeneration of the nervous system. Features of FA include proprioceptive and cerebellar deficits leading to impaired muscle coordination and, consequently, dysmetria in force and time of movement. The aim of this study is to characterize dysmetria and its association to disease severity. Also, we examine the neural mechanisms of dysmetria by quantifying the EMG burst area, duration, and time-to-peak of the agonist muscle. Twenty-seven individuals with FA and 13 healthy controls (HCs) performed the modified Functional Ataxia Rating S

Deep Temporal Nerve Transfer for Facial Reanimation: Anatomic Dissections and Surgical Case Report.

BACKGROUND: Facial nerve palsy is a disabling condition that may arise from a variety of injuries or insults and may occur at any point along the nerve or its intracerebral origin. OBJECTIVE: To examine the use of the deep temporal branches of the motor division of the trigeminal nerve for neural reconstruction of the temporal branches of the facial nerve for restoration of active blink and periorbital facial expression. METHODS: Formalin-fixed human cadaver hemifaces were dissected to identify landmarks for the deep temporal branches and the tension-free coaptation lengths. This technique w

Mechanisms of how sarcopenia affects functional outcomes in acute ischaemic stroke.

Sarcopenia, a common geriatric condition characterized by the loss of skeletal muscle mass and function, can negatively affect functional outcomes in acute ischaemic stroke; however, the underlying mechanisms remain unclear. We hypothesized that sarcopenia affects post-stroke outcomes, mediated through its impact on dysphagia, early neurological deterioration, or post-stroke recovery, particularly in patients with motor deficits. To explore this hypothesis, we included 600 consecutive elderly (≥65 years) patients with acute (<1 week) ischaemic stroke and assessed: (i) the relationships sarcope

Automated temporalis muscle quantification and growth charts for children through adulthood.

Lean muscle mass (LMM) is an important aspect of human health. Temporalis muscle thickness is a promising LMM marker but has had limited utility due to its unknown normal growth trajectory and reference ranges and lack of standardized measurement. Here, we develop an automated deep learning pipeline to accurately measure temporalis muscle thickness (iTMT) from routine brain magnetic resonance imaging (MRI). We apply iTMT to 23,876 MRIs of healthy subjects, ages 4 through 35, and generate sex-specific iTMT normal growth charts with percentiles. We find that iTMT was associated with specific phy

Intramuscular lipoma of the temporalis muscle extending to the infratemporal fossa: surgical pitfalls and short literature review.

Lipomas are benign tumours of mesenchymal origin, representing one of the most common tumours of the body. They are often observed between the fourth and the sixth decade of life and in 13% of the cases they occur in the head and neck region. In case of symptoms, surgical removal is the treatment of choice; when the formation involves the temporal region, the surgical approach is often challenging due to the presence of the neurovascular structures, such as the temporalis branch of the facial nerve and their potential extension to the nearby structures under the zygomatic arch to either the in

Spontaneous bilateral coronoid process fracture of the mandible after BSSO: A case report.

A patient with spontaneous bilateral fracture of the mandibular coronoid process is presented. The definite cause is unknown, but possible contributing factors include osteoporosis, bruxism and changed kinetics of the temporalis muscle after bilateral sagittal split osteotomy (BSSO) resulting in an avulsion fracture. The patient was treated surgically with bilateral coronoidectomy.

A comprehensive approach to long-standing facial paralysis based on lengthening temporalis myoplasty.

Long-standing peripheral monolateral facial paralysis in the adult has challenged otolaryngologists, neurologists and plastic surgeons for centuries. Notwithstanding, the ultimate goal of normality of the paralyzed hemi-face with symmetry at rest, and the achievement of a spontaneous symmetrical smile with corneal protection, has not been fully reached. At the beginning of the 20(th) century, the main options were neural reconstructions including accessory to facial nerve transfer and hypoglossal to facial nerve crossover. In the first half of the 20(th) century, various techniques for static

Intramuscular onchocercoma. Case report.

Onchocerciasis is characterized by the presence of numerous microfilariae in the skin, formation of subcutaneous nodules and eye lesions that can lead to blindness. We present a case, rather uncommon in our environment, in which an incomplete life cycle of the parasite, led to a single intramuscular nodule. The clinical symptoms and histological features are described, together with the treatment, using Ivermectin.

Volume and shape of masticatory muscles in patients with hemifacial microsomia.

OBJECTIVE: To test the following hypotheses in hemifacial microsomia (HFM): (1) the volumes of the masseter, lateral and medial pterygoid, and temporal muscles are reduced on the affected versus unaffected side; (2) significant differences exist between the degrees of right-left disproportion in these four masticatory muscles; (3) circumferential shapes of the masticatory muscles are more irregular on the affected versus unaffected side; and (4) the degree of masticatory muscle right-left disproportion can be judged by the degree of ear, mandibular, or dental anomalies. Augmented vertical rectus transpositions: Intraoperative measurement of torsion following sequential muscle detachment.

INTRODUCTION: After vertical recti transposition surgery, a torsional change may occur. We hypothesized that step-by-step monitoring of the intraoperative torsional position of the eye can avoid any unexpected complications and identify critical moments of surgery that may have adverse effects. METHODS: Nine patients with sixth nerve palsy that underwent complete transposition of the superior (SR) and inferior rectus (IR) muscles to the lateral rectus muscle by means of the technique of augmented vertical recti transposition were prospectively included. The torsional position of the eye was r

Masticatory muscle function after unilateral condylar fractures: a prospective and quantitative electromyographic study.

Electromyographic (EMG) recording of masticatory muscle activity was performed in 9 adult men with unilateral condylar fracture immediately after conservative treatment of the condylar fracture (T0) and 4 (T1) and 8 (T2) months after the trauma. From T0 to T2, maximal voluntary contraction of the anterior and posterior temporal muscles and of the masseter muscle opposite the fracture side (contralateral) increased significantly. Activity in the anterior temporal muscles also rose significantly during natural chewing, whereas the masseter muscles remained at the same level, but activity consist

Reattachment of the Temporalis Muscle After Frontoorbital Advancement in Craniosynostosis Surgery.

Craniosynostosis surgery may result in temporal hollowing occasionally. Overexpansion of the normal side and undergrowth of the affected side exacerbate the problem in unilateral cases (like unicoronal synostosis). Temporalis muscle lies in the temporal fossa, and it is usually severed or detached from its origin in order to reach the lateral aspect of the fronto-orbital bar. Reattachment of the temporalis muscle is challenging especially when the orbital bar is moved forward. In this technical note, exposure of the lateral cranial vault with zigzag incision of the temporalis muscle in 35 nons

Neural-enhanced motion-to-EMG: refining simulated muscle activity from musculoskeletal models using a Seq2Seq approach.

Electromyography (EMG) is essential for accurate assessment of motor function in rehabilitation, sports science, and robotics. However, its various time-consuming human operations (e.g., electromagnetic noise countermeasures) limit its widespread use. Meanwhile, motion capture technology has become more accessible, leading to increasing interest in musculoskeletal simulation models such as OpenSim. Although advances have been made in individualizing the model parameters, accurately estimating muscle activity remains a significant challenge. Previous efforts to optimize the parameters in muscul

EMG Validation of a Subject-Specific Thoracolumbar Spine Musculoskeletal Model During Dynamic Activities in Older Adults.

Musculoskeletal models can uniquely estimate in vivo demands and injury risk. In this study, we aimed to compare muscle activations from subject-specific thoracolumbar spine OpenSim models with recorded muscle activity from electromyography (EMG) during five dynamic tasks. Specifically, 11 older adults (mean = 65 years, SD = 9) lifted a crate weighted to 10% of their body mass in axial rotation, 2-handed sagittal lift, 1-handed sagittal lift, and lateral bending, and simulated a window opening task. EMG measurements of back and abdominal muscles were directly compared to equivalent model-predi

Changes in Muscle Activity in the Early Postoperative Period of Minimally Invasive Orthognathic Surgery.

This study aimed to assess the impact of minimally invasive orthognathic surgery on the electromyographic (EMG) activity of the masseter and anterior temporal muscles, using IMPACT, POC, ASYMMETRY, TORSION, and BARYCENTER indices. It also investigated their relationships with postoperative edema and mouth opening. Fifteen patients, with a mean age of 31.3±10.9 years, underwent minimally invasive orthognathic surgery. Surface EMG was performed during maximum dental occlusion, with measurements taken before (T0) and 7 days after surgery (T1). The results indicated a significant reduction in the

[Anatomy of the frontal region].

The forehead is an anatomic region located between the frontal hairline cranially, the eyebrow and the glabella caudally, and the anterior border of the temporal fossa laterally on both sides. Its vertical situation, due to the telencephalon growth, is specific of the human species. From surface to deep planes, the skin and sub-cutaneous fat pads are described first. The muscular plane is constituted of the frontal muscles elevators of the forehead and the eyebrow, and the depressors which are the procerus and orbicularis oculi muscles superficially, the depressor supercilii muscle, and the co

The concurrency of temporal tendinitis with TMD.

Temporomandibular disorder (TMD) is a type of orofacial pain that can originate from a number of craniofacial mandibular structures. These include the TM joints, the muscles of mastication, related nerves, tendons, ligaments, bones and teeth. Symptoms include impaired jaw function, TM joint noises and pain, limited opening, often with jaw deviations or deflections to the affected side. Temporal tendinitis is a disorder of the fibrous insertion of the temporalis muscle tendons on the coronoid process of the mandible that is characterized by both inflammation and degeneration. Sometimes, tempora

Restoration of selective facial nerve function by the nerve-muscle pedicle technique.

The nerve-muscle pedicle technique for selective reinnervation of paralyzed facial muscles, when combined with a modified muscle-fascia sling and neurotization from the temporal muscle, has been applied in 26 patients to date. The success rate has been 88 per cent for restoration of tonus and 77 per cent for restoration of voluntary motion as well. This procedure offers several significant advantages over other accepted means of reinnervation in patients in whom the seventh nerve itself is not available as a source for restoration of function. It also may have value as a method for rapid resto
