Triceps surae activation is altered in male runners with Achilles tendinopathy.

UNLABELLED: Achilles tendinopathy is a common injury in running sports however the exact etiology of Achilles injury is still unclear. In recent years, altered neuromotor recruitment patterns of the triceps surae have been hypothesized to create differential intra-tendinous loads leading to pathology; however, this hypothesis has not been investigated. Further, the effect foot orthoses may have on neuromotor recruitment of the triceps surae in Achilles tendinopathy has not been investigated. METHODS: The electromyographic activity of the triceps surae was recorded during an over-ground runnin

Skin-derived tenocyte-like cells for the treatment of patellar tendinopathy.

BACKGROUND: Recent research of lateral elbow tendinopathy has led to the use of laboratory-amplified tenocyte-like cells. HYPOTHESIS: Ultrasound-guided injection of autologous skin-derived tendon-like cells are effective compared with other injectable therapies for the treatment of refractory patellar tendinosis. STUDY DESIGN: Randomized controlled trial; Level of evidence, 1. METHODS: From 60 patellar tendons in 46 patients with refractory patellar tendinopathy, a 4-mm skin biopsy was sampled to grow tenocyte-like collagen-producing cells. Patients were allocated into 2 groups: (1) injecti

The prevalence of neovascularity in patients clinically diagnosed with rotator cuff tendinopathy.

BACKGROUND: Shoulder dysfunction is common and pathology of the rotator cuff tendons and subacromial bursa are considered to be a major cause of pain and morbidity. Although many hypotheses exist there is no definitive understanding as to the origin of the pain arising from these structures. Research investigations from other tendons have placed intra-tendinous neovascularity as a potential mechanism of pain production. The prevalence of neovascularity in patients with a clinical diagnosis of rotator cuff tendinopathy is unknown. As such the primary aim of this pilot study was to investigate i

Rotator cuff tendinopathy: is there a role for polyunsaturated Fatty acids and antioxidants?

Despite the lack of robust evidence, there has been a steady increase in the use of dietary supplements, including Omega 3 fatty acids and antioxidants, in the management of musculoskeletal conditions. One reason for this is that unsatisfactory outcomes with conventional treatments have lead sufferers to seek alternative solutions including the use of nutritional supplements. In the United Kingdom alone, the current supplement market is estimated to be over 300 pounds million per annum. One target market for nutritional supplements is tendinopathies including conditions involving the rotator c

An isokinetic eccentric programme for the management of chronic lateral epicondylar tendinopathy.

BACKGROUND: Lateral epicondylitis represents a frequent overuse injury. In spite of many conservative treatment procedures, prolonged symptoms and relapse are frequently observed. OBJECTIVE: To compare the outcome of patients performing an isokinetic eccentric training with that of age-, gender-, activity-matched patients receiving a non-strengthening classical rehabilitation. METHODS: Ninety-two patients with unilateral chronic lateral epicondylar tendinopathy (mean duration of symptoms 8+/-3 months) were assigned either to a control group (n = 46) or to an eccentrically trained group (n =

The influence of the lower patellar pole in the pathogenesis of chronic patellar tendinopathy.

Resection of the lower patellar pole provides good results in the treatment of jumper's knee. Therefore we hypothesized that the length of the lower patellar pole is increased in patients with chronic patellar tendinopathy. Cohort study, level of evidence 2. Between 2000 and 2005, 25 patients with chronic patellar tendinopathy underwent conservative and surgical treatment in our clinic. All of them had preoperative MRI were three independent examiners measured the Caton Index, the length and the ratio of the articular and non-articular patellar surface, tendon length and thickness and the thic

MRI and US of gluteal tendinopathy in greater trochanteric pain syndrome.

Greater trochanteric pain syndrome is commonly due to gluteus minimus or medius injury rather than trochanteric bursitis. Gluteal tendinopathy most frequently occurs in late-middle aged females. In this pictorial review the pertinent MRI and US anatomy of the gluteal tendon insertions on the greater trochanter and the adjacent bursae are reviewed. The direct (peritendinitis, tendinosis, partial and complete tear) and indirect (bursal fluid, bony changes and fatty atrophy) MRI signs of gluteal tendon injury are illustrated. The key sonographic findings of gluteal tendinopathy are also discussed

Clinical Reasoning-Led Physiotherapy Intervention for Biceps Tendinopathy: A Case Report.

Long head of biceps tendinopathy (LHBT) is a common source of anterior shoulder pain, often misdiagnosed due to overlapping symptoms with other shoulder disorders. This case report presents a 56-year-old female with chronic shoulder pain managed through a clinical reasoning-guided physiotherapy program. Assessment using clinical reasoning, including the International Classification of Functioning, Disability, and Health (ICF) framework, confirmed LHBT. The intervention included pain management, manual therapy, range of motion exercises, strengthening exercises, and patient education over 6 wee

Patients with Achilles Tendinopathy Show Reduced Intratendinous Sliding during Dynamic Exercises.

PURPOSE: Intratendinous sliding, that is, nonuniform motions between the superficial and deep layers of the Achilles tendon, is reduced in individuals with Achilles tendinopathy compared with healthy controls during isometric contractions. Previous work has shown that intratendinous sliding can be increased by performing isometric contractions with a horizontally outward-rotated ("toes-out") foot position. The purpose of this study was to compare intratendinous sliding between healthy individuals and patients with Achilles tendinopathy during dynamic exercises, and to determine the effect of a

A remote ultrasound imaging approach for monitoring rotator cuff calcific tendinopathy.

Rotator cuff calcific tendinopathy is a common pathological condition characterized by calcium deposits within the rotator cuff tendons, leading to severe pain and restricted shoulder mobility. This pathology progresses through distinct stages, during which the hardness of the calcification gradually decreases. The gold-standard treatment is ultrasound-guided percutaneous irrigation; however, this procedure can only be performed once the calcification has softened sufficiently to allow calcium deposit removal via needling and saline solution lavage. For this reason, the timing between diagnosi

Calcific Tendinopathy Atypically Located Outside the Rotator Cuff: A Systematic Review.

BACKGROUND AND PURPOSE: Calcific tendinopathy is a common cause of painful shoulder easily identified with ultrasound or conventional radiography. Although the rotator cuff is by far the most common location of the disease and diagnostic or treatment strategies are well known in clinical practice, a lack of awareness characterizes the assessment of the other sites affected by this condition; consequently, the risk of underestimating the prevalence of atypical non-rotator cuff calcific tendinopathy is high. This may lead to expensive or invasive diagnostic exams and/or inappropriate treatment,

Flexor hallucis longus tendon morphology in dancers clinically diagnosed with tendinopathy.

PURPOSE: The unique demands of dance technique make dancers more prone to certain pathologies especially of the foot and ankle. Flexor hallucis longus (FHL) tendinopathy, colloquially known as "dancer's tendinopathy," is common in dancers and not well studied. The purpose of this study was to assess if morphological alterations in tendon structure occur as an adaptive response to dance activity by comparing the FHL tendon in dancers to non-dancers, and if pathology further alters tendon morphology in dancers clinically diagnosed with tendinopathy. METHODS: Three groups of ten participants wer

Editorial Commentary: Platelet-Rich Plasma Shows Promise for Improving Shoulder Tendinopathy.

Platelet-rich plasma (PRP) injections continue to be used at increasing rates to treat common musculoskeletal conditions. PRP has a low-risk profile and emerging in vitro evidence to support its positive effects on soft-tissue healing. PRP has been shown to be of benefit for knee osteoarthritis, but less has been published regarding the shoulder. PRP delivers a high concentration of growth factors, cytokines, and other important inflammatory modulators. Its use is appealing for treating partial-thickness rotator cuff tears, subacromial bursitis, and rotator cuff tendinopathy since rotator cuff
