Thoracoscopic clipping and ligation of a patent ductus arteriosus.

Several techniques of patent ductus arteriosus occlusion primarily by thoracotomy have been practiced. They have shown to be more reliable than nonsurgical procedures. Videothoracoscopy has been useful in several intrathoracic surgical procedures with faster recovery and minimal mortality and morbidity. We report 2 patients with clipping and ligation of a patent ductus arteriosus by a thoracoscopic approach. This technique may be useful in children with a noncomplicated patent ductus arteriosus. It allows an easier recovery and causes less pain.

[A case of esophageal cyst resected by thoracoscopic surgery].

A 59-year-old woman was admitted with back pain and an abnormal chest shadow in the left hilar region. Her chest CT scan showed a posterior mediastinal mass surrounding the descending aorta. By transesophageal ultrasonography, a diagnosis of mediastinal cyst was made. She underwent thoracoscopic resection under general anesthesia with new endoscopic devices including endo-scissors, endoscopic hemoclips and electrocautery. Pathologically, the tumor was an esophageal cyst lined by ciliated columnar and squamous epithelium overlying smooth muscle layers. She made an uneventful recovery. Her back

Thoracoscopic Heller's cardiomyotomy: a new approach for achalasia.

The authors performed Heller's myotomy using an endothoracoscopic technique on two patients with swallowing difficulties that were clinically demonstrated to be oesophageal achalasia. At 12 and 18 months' followup, respectively, both patients were asymptomatic. Extramucosal cardiomyotomy, as reported by Heller in 1913, is the most common approach to repair achalasia of the oesophagus. The advantages of the laparoscopic technique (less pain, small scars, reduced hospital stay) are given, as well as the disadvantages (possible mobilization of left lateral liver segment, possible splenic damage,

Video-assisted thoracoscopic resection of pulmonary metastases.

Pulmonary lesions in patients with malignancy often require resection to provide prognostic information and to dictate further therapy. Metastasectomy may also favorably influence survival in select patients with favorable tumor histologies. Seventy-two patients with a history of malignancy and new lung lesions identified by high resolution computed tomography underwent thoracoscopic resection. The operative approach was with standard techniques using two or three trocar sites. Preoperative needle localization was used in 13 patients with small lesions that were not immediately subpleural. Endoscopic oesophagectomy through a right thoracoscopic approach.

A technique of subtotal endoscopic oesophagectomy through the right thoracoscopic approach is described. It has been used in five patients, four with cancer and one with benign motility disorder. The operative blood loss during the endoscopic dissection stage was unmeasurable in four patients and amounted to 300 ml in one. The mean (range) duration of the endoscopic dissection was 3.3 (3.0-4.0) h and of the total procedure was 5.5 (4.5-7.5) h. After the operation, the mean (range) duration of stay in the intensive care unit was 19.5 (16-26) h. From the time of the operation, the mean (range) h

[Pleural amyloidosis: thoracoscopic diagnosis and physiopathological approach].

The authors report a case of pleural amyloidosis presenting as a generalised primary amyloidosis with immunological disturbance, in which the diagnosis was made at thoracoscopy. Bronchopulmonary disease during the course of generalised amyloidosis is frequent, but there are only thirteen cases of histologically proven pleural amyloidosis which have been reported in the literature. Using pleuroscopy the histology obtained enables a pathophysiological approach to this disease, taking account of the cells and enzymes found in the pleura, the inflammatory reactions suggests a local production at t

Asbestos-induced pleural diseases--thoracoscopic aspects.

The endoscopic morphology of asbestos-induced pleural disease is demonstrated by typical thoracoscopic findings. Among 627 thoracoscopies, lesions induced by exposure to asbestos were found in 9.9% (50 malignant mesotheliomas of the pleura, 10 pleural hyalinoses, 2 asbestos pleurisies). This comparatively high percentage is indicative of the pathogenic relevance of asbestos in the pleuropulmonary region. In addition to describing the diffuse malignant mesotheliomas of the pleura, reference is made in the discussion to the possible occurrence of benign localized fibromatous mesotheliomas that a

Outcomes of Thoracoscopic Left Atrial Appendage Occlusion With or Without Concomitant Thoracoscopic Ablation - Multicenter Retrospective Registry of 567 Patients in Japan.

BACKGROUND: Atrial fibrillation (AF) is a major risk factor for ischemic stroke, with >90% of thrombi in nonvalvular AF originating in the left atrial appendage (LAA). Thoracoscopic LAA occlusion (LAAO), with or without ablation, is a minimally invasive alternative to endocardial devices or anticoagulation, but multicenter data are limited. METHODS AND RESULTS: The Totally Thoracoscopic Left Atrial Appendage Occlusion Study (TT-LAAOS) is a multicenter Japanese registry of thoracoscopic LAAO in nonvalvular AF. From March 2018 to December 2024, 567 patients underwent the procedure at 6 institut

Efficacy and safety of thoracoscopic-guided multiple paravertebral block for video-assisted thoracoscopic lobectomy surgery: a randomized blinded controlled study.

BACKGROUND: Paravertebral block (PVB) has been increasingly popular for postoperative analgesia. However, few studies estimated the efficacy and safety of multiple PVB using thoracoscope-assisted technique for intraoperative analgesia and postoperative pain management for video-assisted thoracoscopic lobectomy (VATS LOBECTOMY). METHODS: A total of 120 patients scheduled to undergo VATS LOBECTOMY were randomly assigned into two groups: a placebo group and a PVB group in a ratio of 1:2. Thoracoscopic-guided multi-point PVB was carried out with 0.5% ropivacaine (PVB group) or 0.9% NaCl (placebo

Does a relationship exist between the number of thoracoscopic thymectomies performed and the learning curve for thoracoscopic resection of thymoma in patients with myasthenia gravis?

This study aimed to analyze surgeons' learning curve for thymoma resection with video-assisted thoracoscopic surgery (VATS). Two hundred and eleven myasthenia gravis patients had VATS thymic resections, including 25 patients with a thymoma. Three groups of surgeries, according to the order of operations, were analyzed: Group A comprised the first 70 thymectomies, Group B comprised the second 70 thymectomies, and Group C comprised the final 71 thymectomies. We compared the groups on a set of preoperative (age, gender, body mass index, and Osserman stage), operative (number in each group, size,

[Thoracoscopic treatment for congenital diaphragmatic hernia and oesophageal atresia].

Thoracoscopic surgery is an alternative to open repair of congenital diaphragmatic hernia and oesophageal atresia. Studies show faster recovery compared with open approaches. Neonatal thoracoscopy was initially associated with longer operative times and higher complication rates. However, risks have declined with greater expertise, improved techniques, and careful patient selection. Neonatal anaesthetic management remains a demanding task and requires specialised skills. Thoracoscopy is now a safe option, though open surgery remains necessary in selected cases.

Management of serious complications in robotic-assisted thoracoscopic surgery.

BACKGROUND: Numerous studies have shown robotic-assisted thoracoscopic surgery (RATS) to be a safe alternative to video-assisted thoracoscopic surgery, and its use has been widely adopted and continues to increase. However, there are a number of potential serious complications that necessitate conversion to thoracotomy. We reviewed our experience with the management and outcomes of such complications in a high-volume robotic center. METHODS: We performed a retrospective review of all major robotic resections (single surgeon, single center) performed between May 2018 and June 2025. Serious com

Successful thoracoscopic resection of costal osteochondroma causing pneumothorax.

Costal osteochondromas are rare and infrequently cause pneumothorax and/or hemothorax by injuring the intrathoracic structures. A 13-year-old boy presented with chest pain and dyspnea. Chest radiography revealed a left pneumothorax. Computed tomography revealed a bony and pedunculated mass arising from the fifth rib, pointing directly inward into the thoracic cavity. We suspected that the mass was a costal osteochondroma that had caused a pneumothorax by injuring the left upper lobe of the lungs. The patient underwent a thoracoscopic surgery. A costal osteochondroma arising from the fifth rib

Robotic-assisted thoracoscopic oesophagotomy for impacted denture removal.

Accidental ingestion of dentures is a relatively infrequent yet clinically significant problem, often resulting in oesophageal impaction, particularly in edentulous patients who wear removable dentures. Ingestion of dentures poses a significant risk of mucosal injury or perforation during endoscopic retrieval, owing to their large size, sharp edges and metal clasps, and hence requires a low threshold for surgical intervention. This video tutorial demonstrates the technique of robotic-assisted thoracoscopic oesophagotomy and removal of the impacted denture followed by repair of the oesophagotom
