Strategies to minimize soft tissues and septic complications in staged management of high-energy proximal tibia fractures.

BACKGROUND: Soft tissues (wound dehiscence, skin necrosis) and septic (wound infection, osteomyelitis) complications have been historically recognized as the most frequent complications in surgical treatment of high-energy proximal tibia fractures (PTFs). Staged management with a temporary external fixator is a commonly accepted strategy to prevent these complications. Nonetheless, there is a lack of evidence about when and how definitive external or internal definitive fixation should be chosen, and which variables are more relevant in determining soft tissues and septic complications risk. Management of soft tissue injuries associated with tibial plateau fractures.

Tibial plateau fractures or knee fracture-dislocations often result from trauma or sporting injuries. Given the subcutaneous position of the knee and upper tibia, without much intervening soft tissue, any such injury can be associated with significant soft tissue damage. Thus, consideration of the degree of soft tissue injury must be part of the treatment plan. Specific management decisions regarding blisters, open wounds, and compartment syndrome are discussed, as well as the timing of surgery, role of provisional external fixation, and placement of incisions for definitive internal fixation.

Circular Fixation in Charcot.

Static circular fixation is a valuable tool for patients with Charcot foot and ankle deformities. The versatility of circular fixators allows for dynamic adjustments over time and can allow for off-loading of flaps or ulcers. The circular fixator can be used as a primary fixation device in cases of osteomyelitis or to facilitate lengthening when a segmental bone defect exists such as loss of the talus. As a secondary fixation device it can protect the internal fixation or be used when there is a compromised soft tissue envelope.

Risk factors for postoperative infection after non-elective femur fracture fixation in pediatric patients.

BACKGROUND: Pediatric femoral shaft fractures are common and often require surgical fixation. Postoperative infections remain a serious complication, yet risk factors specific to this population are not well defined. METHODS: We analyzed 14,037 pediatric patients undergoing non-elective femur fracture fixation in the 2019 and 2022 Kids' Inpatient Database (KID). Multivariable logistic regression identified independent predictors of postoperative soft tissue, device-related, and overall infection. Secondary outcomes included length of stay (LOS) and hospital charges. RESULTS: Infections occur

A comprehensive protocol for simplified mouse DRG fixation, processing and F4/80 immunohistochemistry: Overcoming common challenges.

BACKGROUND: Dorsal root ganglia (DRGs) contain the cell bodies of sensory neurons and non-neuronal cells that play a role in the pathophysiology of painful inflammatory conditions, such as neuropathic pain. Immunohistochemistry (IHC) is a valuable tool for visualising and quantifying immune cell markers in DRGs, providing important insights into these mechanisms. However, isolating DRGs while preserving cell morphology for IHC staining is technically challenging due to their small size and location within the spinal column. OBJECTIVE: Using F4/80, a pan monocyte-macrophage marker, we present

Acute Limb Shortening for Major Near and Complete Upper Extremity Amputations with Associated Neurovascular Injury: A Review of the Literature.

In the setting a near or complete upper extremity amputations with significant soft tissue loss and neurovascular compromise, upper extremity surgeons are faced with the challenge of limb salvage. There are a multitude of treatment options for managing skeletal and soft tissue injuries including provisional fixation, staged reconstruction, and an acute shortening osteotomy with primary rigid internal fixation. However, many complications are associated with these techniques. Complications of provisional fixation include pin tract infection and loosening, tethering of musculotendinous units, no

Design and performance analysis of 3D-printed stiffness gradient femoral scaffold.

Studies on 3D-printed porous bone scaffolds mostly focus on materials or structural parameters, while the repair of large femoral defects needs to select appropriate structural parameters according to the needs of different parts. In this paper, a kind of stiffness gradient scaffold design idea is proposed. Different structures are selected according to the different functions of different parts of the scaffold. At the same time, an integrated fixation device is designed to fix the scaffold. Finite element method was used to analyze the stress and strain of homogeneous scaffolds and the stiffn

Lag screw only fixation for lateral malleolar fractures: a systematic review and meta-analysis.

BACKGROUND: Ankle fractures are common injuries, most typically involving the distal fibula and often requiring surgery. Traditional surgical techniques often utilise an interfragmentary compression screw (lag screw) and a neutralisation fibular plate. Due to limited subcutaneous tissue, such plates can be prominent, leading to wound complications, infections, and ankle pain. The lag screw-only (LSO) fixation has been suggested to provide adequate fixation and healing while minimizing soft tissue complications and reoperation rates. AIMS: To undertake a systematic review and meta-analysis of

Phenolic Determination in Proembryogenic Cell Complexes of Buckwheat Morphogenic Cell Culture with Osmium Tetroxide, Toluidine Blue O Dye, and Iron Chloride.

The study of the localization of secondary metabolites in both plants and the cell cultures on the intravital sections is hampered by the difficulty of obtaining thin, correctly oriented sections. Techniques for fixing tissues in resins allow these difficulties to be overcome. Properly selected tissue fixation techniques allow using different dyes to identify the compound of interest. In addition, some components of tissue fixation can act as fixatives and as a dye for identifying secondary metabolites. For example, osmium tetroxide, which fixes lipids in tissues, stains phenolic compounds bla

External fixation of the foot and ankle.

The use of Hoffmann external fixation in the treatment of ankle and foot trauma in 26 patients is reviewed. There were 21 males and five females, ranging in age from 14 to 56 years. There were 22 fresh fractures and four arthrodeses. Fourteen of the 22 injuries were open fractures. Eight of 22 involved foot joint disruptions. This form of treatment required a complete set of external fixation equipment, sound knowledge of foot and ankle anatomy, and technical skill in frame construction and application. This fixateur allowed three plane stabilization of complex fractures, mobilization of unaff

Modern External Ring Fixation Versus Internal Fixation for Treatment of Severe Open Tibial Fractures: A Randomized Clinical Trial (FIXIT Study).

BACKGROUND: Modern external ring fixation has been hypothesized to reduce complications requiring hospital readmission compared with internal fixation when treating patients with high-energy open tibial shaft fractures. In this study, the 1-year probability of a major limb complication was compared between external and internal fixation of severe open tibial fractures. METHODS: This multicenter randomized clinical trial included patients 18 to 64 years of age with severe open tibial shaft fractures randomly assigned to either modern external ring fixation (n = 127) or internal fixation (n = 1

Antibodies validated for routinely processed tissues stain frozen sections unpredictably.

Background: Antibody validation for tissue staining is required for reproducibility; criteria to ensure validity have been published recently. The majority of these recommendations imply the use of routinely processed (formalin-fixed, paraffin-embedded) tissue. Materials & methods: We applied to lightly fixed frozen sections a panel of 126 antibodies validated for formalin-fixed, paraffin-embedded tissue with extended criteria. Results: Less than 30% of the antibodies performed as expected with all fixations. 35% preferred one fixation over another, 13% gave nonspecific staining and 23% did no

Enhancing Histological Techniques for Small Crustaceans: Evaluation of Fixation, Decalcification, and Enzymatic Digestion in Neocaridina Shrimp.

Histological techniques are essential for studying small crustaceans', such as Neocaridina shrimp, anatomy and physiology. However, their small size and rapid tissue autolysis present challenges for fixation and processing. This study aimed to optimize histological methods for Neocaridina shrimp by evaluating different protocols for fixation, decalcification, and enzymatic digestion. Shrimp were fixed using 10% neutral-buffered formalin (NBF) and Bouin's or Davidson's fluid with or without modifications such as trypsin digestion, decalcification, or abdomen removal. Transcriptomic profiling of nuclei from paraformaldehyde-fixed and formalin-fixed paraffin-embedded brain tissues.

BACKGROUND: Paraformaldehyde (PFA) fixation is necessary for histochemical staining, and formalin-fixed and paraffin-embedded (FFPE) tissue archives are the largest repository of clinically annotated specimens. Single-cell gene expression workflows have recently been developed for PFA-fixed and FFPE tissue specimens. However, for tissues where intact cells are hard to recover, including tissues containing highly interconnected neurons, single-nuclear transcriptomics is beneficial. Moreover, since RNA is very unstable, the effects of standard pathological practice on the transcriptome of sample

[Surgical treatment of tibial nonunion after wounding by high velocity missile and external fixators: a case report].

INTRODUCTION: The missiles of modern firearms can cause severe fractures of the extremity. High velocity missile fractures of the tibia are characterized by massive tissue destruction and primary contamination with polymorphic bacteria. Treatment of these fractures is often complicated by delayed healing, poor position healing, nonhealing and bone tissue infection. CASE OUTLINE: We present the management of tibial nonunion after wounding by high velocity missile and primary treatment by external fixation in a 25-year-old patient. The patient was primarily treated with external fixation and re

Arthroscopic Treatment of Schatzker Type III Tibial Plateau Fractures Using a Combination of Bioabsorbable Interference Screws and Cannulated Screws.

Schatzker type III tibial plateau fractures, characterized by a pure depression of the lateral articular surface, predominantly affect osteoporotic, middle-aged, and elderly patients. Successful management requires anatomical reduction and stable fixation to minimize complications like post-traumatic arthritis. Conventional open reduction and internal fixation, which involves extensive soft-tissue dissection and relies on fluoroscopic assessment, often fails to address concomitant intra-articular injuries. Utilizing advancements in arthroscopy and bioabsorbable implants, we propose a technique

Talus Fractures: An Update on Current Concepts in Surgical Management.

Talus fractures can be challenging injuries to treat because of complex talar shape, an abundance of articular cartilage, a potentially unforgiving soft-tissue envelope, and an easily injured blood supply. In addition, the spectra of energy involved, soft-tissue injury, and the fracture pattern are wide. Temporizing treatment is sometimes required, including débridement of open fractures, reduction of dislocations, and occasionally spanning external fixation. Definitive treatment first requires an understanding of the fracture pattern, including location and fracture line orientation. Multiple

A new antigen retrieval technique for human brain tissue.

Immunohistochemical staining of tissues is a powerful tool used to delineate the presence or absence of an antigen. During the last 30 years, antigen visualization in human brain tissue has been significantly limited by the masking effect of fixatives. In the present study, we have used a new method for antigen retrieval in formalin-fixed human brain tissue and examined the effectiveness of this protocol to reveal masked antigens in tissues with both short and long formalin fixation times. This new method, which is based on the use of citraconic acid, has not been previously utilized in brain
