Acute mediastinitis (AM) has become a rare disease since the introduction of antibiotics but still has an unchanged high mortality-rate if the diagnosis is not readily established, and adequate therapy is performed. Clinical signs of AM are often uncharacteristic, plain chest films do not allow a reliable diagnosis because of lack of sensitivity as well as specificity. High resolution sequential bolus dynamic computed tomography (CT) has become the modality of choice for the diagnosis, evaluation, and staging of pancreatic masses. The CT appearances in 87 patients with intraventricular mass lesion were analyzed in order to verify the characteristic finding. The pathological diagnoses included medulloblastoma in 27, astrocytoma in 16, craniopharyngioma in 15, ependymoma in 9, meningioma in 7, AVM in 4, choroid plexus papilloma and epidermoid cyst in 2 each. One each had teratoma, ependymal cyst, cysticercosis, cavernous hemangioma and hemangioblastoma. The histologic diagnosis could be suggested by the frequency, location, density, age and sex of the patients. The differential features of each diagnosis are discussed.

33 patients with clinically suspected diverticulitis of the colon were studied prospectively by CT. The predictive value of symptoms, such as thickening of the colonic wall (86.6%), inflammatory changes of the pericolic fatty tissue (87.5%), the presence of diverticula (73.3%) and abscess formation (100%), were examined separately and their significance was evaluated. We devised a rapid and sensitive computed tomography (CT) method to assess the acutely injured wrist, healing carpal fractures and post-traumatic osteonecrosis, when the plain films offer insufficient information. The CT findings of tuboovarian abscesses are described in three consecutive patients who had both CT and surgery. Computed tomography showed fluid-density adnexal masses having visible walls, septations, and indistinct borders, with anterior displacement of the mesosalpinx, hydroureter, and increased density of the fat and ligaments anterior to the sacrum. These findings are not specific but suggest the diagnosis of tuboovarian abscesses in an appropriate clinical context.

CT has become the imaging modality of choice in most acute and chronic injuries involving the articular surfaces. Today's technology allows high resolution, fast scanning, and rapid reformatting, all of which contribute to prompt orthopedic evaluation and decision making. Our orthopedic colleagues have come to rely on high-quality CT scans in order to evaluate calcaneal and tibial plateau fractures, stress fractures, osteochondritis dissecans, acute and chronic shoulder trauma, and problems of the painful wrist--especially the distal radioulnar joint.

Computed tomography is replacing contrast radiography as the method of choice in evaluating congenital posterior choanal atresia. Careful preparation of the patient, however, is necessary prior to the examination in order to obtain the maximum of information pre-operatively. We present patients with inadequate preparation and illustrate their misleading scans. The proper method of examination is discussed, with a review of the literature; it is essential to remove nasal secretions by suction and instill vasoconstrictor nose drops, shortly before scanning.

Conventional radiographs are not always able to exclude ischemic necrosis of the lunate in the early stages of the disease. Tomograms on two planes have a higher sensitivity than radiographs in two planes. Sensitivity during middle and late stages of ischemic necrosis of the lunate is approximately the same as that of magnetic resonance imaging. MRI is indicated if conventional techniques are negative or doubtful and where there are clinical signs of malacia (Decoulx stages 0 to 1). For these indications MRI is a justifiable additional method for diagnosing ischemic necrosis of the lunate.

MRI-findings in a case of parenchymal neurocysticercosis are presented. The changes of the lesions as a response to chemotherapy were monitored by MRI and CT. Problems of sensitivity (MRI vs. CT) and MRI differential diagnoses are discussed.

Twenty-three patients with atherosclerotic peripheral vascular disease underwent angiography and CT of the aortoiliac segment within a 72-hour interval. The results obtained by MR tomography in 30 patients after renal grafting are compared with the clinical and histological data in respect of corticomedullary contrast (CMC). After a review on osseous labyrinthine modifications in cochlear otosclerosis, the authors report their experience, based on four patients, in CT evaluation of the disease. CT high resolution programs detect demineralization centres, which are distinctive of cochlear otosclerosis, at an early stage. CT must be considered a more informative investigation than multidirectional tomography.

Reported are 10 patients with surgically verified acute cholecystitis and its complications. Of these 10 patients, 6 (60%) had atypical clinical presentation so that cholecystitis was not the primary diagnosis being considered before computed tomography. Common computed tomography findings included gallbladder wall thickening (100%), pericholecystic fluid (80%), gallstones (50%), and air in the gallbladder lumen and wall (20%). An awareness of the computed tomography appearances can be helpful in the diagnosis of complicated cholecystitis even when the clinical presentation is atypical.

Three cases of stress fractures of the lower extremities are presented with corresponding CT. Findings of increased medullary cavity density, endosteal sclerosis, callus formation, and soft tissue swelling are demonstrated even though they are thought to be nonspecific in differentiating stress fracture from infectious or neoplastic processes. Only when actual failure lines are demonstrated by CT can stress-related injury be specifically suggested.
