Long working hours and health in Denmark: a post hoc analysis of three cohort studies.

BACKGROUND: To protect the safety and health of workers, national legislations often stipulate an upper limit to weekly working time. The purpose of the present study was to provide information that may facilitate the obtainment of evidence-based limits to weekly working time. OBJECTIVE: To estimate incidence rate ratios (RRs) of (i) hospital treatment or death due to accidental injuries, (ii) redeemed prescriptions for psychotropic drugs and (iii) redeemed prescriptions for antihypertensive drugs, respectively, as continuous functions of weekly working hours among employees in the general po

Randomised controlled trial of the efficacy of a blue-enriched light intervention to improve alertness and performance in night shift workers.

OBJECTIVES: Night workers often experience high levels of sleepiness due to misalignment of the sleep-wake cycle from the circadian pacemaker, in addition to acute and chronic sleep loss. Exposure to light, in particular short wavelength light, can improve alertness and neurobehavioural performance. This randomised controlled trial examined the efficacy of blue-enriched polychromatic light to improve alertness and neurobehavioural performance in night workers. DESIGN: Participants were 71 night shift workers (42 males; 32.8±10.5 years) who worked at least 6 hours between 22:00 and 08:00 hours

[Morbidity of irregular work schedules].

This paper provides a review of literature on the effects of shift work on physical health, mental health and well-being. In Europe, 20% of the workforce is involved in irregular work schedules. Up to 70% of workers report problems, with increasing age, associated with more difficulties in adjusting to shift work. Epidemiologic studies on large populations have suggested a relation between employment in shift work and the incidence of sleep disorders, cancer, cardiovascular disease, diabetes, obesity, metabolic syndrome, reduced fecundity, preterm births, low birth weight, spontaneous abortion

A clinical approach to circadian rhythm sleep disorders.

Circadian rhythm sleep disorders are characterized by complaints of insomnia and excessive sleepiness that are primarily due to alterations in the internal circadian timing system or a misalignment between the timing of sleep and the 24-h social and physical environment. In addition to physiological and environmental factors, maladaptive behaviors often play an important role in the development of many of the circadian rhythm sleep disorders. This review will focus on the clinical approach to the diagnosis and management of the various circadian rhythm sleep disorders, including delayed sleep

Inca Trail porters: the health of local tourism employees as a challenge for travel medicine.

BACKGROUND: Trekking is an activity that forms part of the increasing adventure and outdoor tourism. High altitude trekking in the Himalayas or Andes has been popular for some time. For longer treks, porters are employed to carry necessary equipment. Porters' working conditions are unfortunate and subsequent health problems considerable. Although Himalayan porters have received some attention in the press and research literature, porters on the popular Inca Trail in Peru have been neglected. In light of the growing awareness of health problems of local tourism employees, the purpose of this st

Critical incident exposure and sleep quality in police officers.

OBJECTIVE: Police officers face many stressors that may negatively impact sleep quality. This study compares subjective sleep quality in police officers with that in control subjects not involved in police or emergency services. We examined the effects of critical incident exposure (trauma exposure) and routine (nontraumatic) work environment stressors on sleep quality after controlling for the effects of work shift schedule. METHODS: Subjective sleep disturbances were measured by the Pittsburgh Sleep Quality Index in police officers (variable-shift workers, N = 551; stable day-shift workers,

Does night-shift work induce apnea events in obstructive sleep apnea patients?

The aim of the present study was to determine the direct effect of night-work on the occurrence of obstructive apneas during sleep after a night shift in fast-rotating shift workers with sleep-related breathing disorders. Eight obstructive sleep apnea patients were examined with the use of a polysomnograph during sleep under two conditions: after day-shift work and after night-shift work. Both sleep studies were conducted within 2 to 3 weeks of each other. In four of the 8 subjects, during sleep after a night-shift, an increase in apnea/hypopnea index was found. Night work significantly increa

Decision volume and temporal workload structure as dissociable correlates of fatigue and mood in intensive care physicians: a real-time observational study.

BACKGROUND: Intensive care physicians make hundreds of decisions per shift under sustained cognitive and emotional demands. Decision fatigue, the tendency towards less effortful choices as cumulative decision load increases, may affect clinician wellbeing and decision processes. However, the associations of decision volume and temporal workload structure with within-shift fatigue and mood in intensive care unit (ICU) physicians remain unclear. This study explored whether decision volume and temporal clustering show distinct associations with fatigue, sleepiness, mood, and post-shift decision r

Vacation effects on behaviour, cognition and emotions of compulsive and non-compulsive workers: do obsessive workers go 'cold turkey'?

Compulsive workers often face psychological and physical health disturbances because of working long hours and an obsessive preoccupation with work during off-job time. Prolonged respite episodes may either relief these employees from their daily stressors to recover or trigger withdrawal symptoms. Our research question was as follows: How do (1) work hours, (2) rumination and (3) affective well-being unfold for compulsive workers compared with non-compulsive workers across long vacations? In a longitudinal field study, work hours, rumination and affective well-being were assessed in 54 employ

Association of job strain with working hours, shift-dependent perceived workload, sleepiness and recovery.

We explored the relationship of job strain with working hours, shift-dependent perceived workload, sleepiness and recovery. Nurses/nursing assistants (n = 95) were recruited from wards that belonged to either the top (high-strain group, HJS) or the bottom (low-strain group, LJS) job strain quartiles of a Job Content Questionnaire survey of employees in five health care districts and four cities in Finland. Three-week field measurements during naturally occurring shift schedules and a subset of pre-selected shift arrangements consisted of the Karolinska Sleepiness Scale, perceived workload and

Night work and breast cancer: a population-based case-control study in France (the CECILE study).

Night work involving disruption of circadian rhythm was suggested as a possible cause of breast cancer. We examined the role of night work in a large population-based case-control study carried out in France between 2005 and 2008. Lifetime occupational history including work schedules of each night work period was elicited in 1,232 cases of breast cancer and 1,317 population controls. Thirteen percent of the cases and 11% of the controls had ever worked on night shifts (OR = 1.27 [95% confidence interval = 0.99-1.64]). Odds ratios were 1.35 [1.01-1.80] in women who worked on overnight shifts,

Diagnosis of circadian rhythm sleep disorders.

Insomnia and excessive sleepiness are common in the investigation of sleep-disordered breathing. Circadian rhythm sleep disorders are perhaps the most often overlooked conditions in the differential diagnosis of these symptoms. Circadian rhythm sleep disorders manifest as misalignment between the sleep period and the physical/social 24-h environmental cycle. The two most prevalent circadian rhythm sleep disorders are delayed sleep phase (common in adolescents) and advanced sleep phase (common in the elderly), situations in which the sleep period is displaced to a later or earlier time, respect

Disruption of the light cycle ablates diurnal rhythms in gastric vagal afferent mechanosensitivity.

BACKGROUND: Gastric vagal afferents (GVAs) respond to mechanical stimulation, initiating satiety. These afferents exhibit diurnal fluctuations in mechanosensitivity, facilitating food intake during the dark phase in rodents. In humans, desynchrony of diurnal rhythms (eg, shift work) is associated with a higher risk of obesity. To test the hypothesis that shift work disrupts satiety signaling, the effect of a rotating light cycles on diurnal rhythms in GVA mechanosensitivity in lean and high-fat diet (HDF)-induced obese mice was determined. METHODS: Male C57BL/6 mice were fed standard laborato
