Minimal invasive plating of distal radius fractures. A safe procedure?

BACKGROUND: Minimal invasive plate osteosynthesis (MIPO) with preservation of the pronator quadratus (PQ) muscle represents a new technique for stabilization of distal radius fractures. However, the complex anatomy of the distal radius metaphysis requires implants with features that address all morphologic specifics of this area to avoid complications, which are still reported with this technique. It was the aim of our anatomic investigation to evaluate the feasibility of plate insertion via a minimal transverse approach as well as the risk of soft- tissues compromise with the use of an implan

[Concomitant vascular complications in supracondylar humerus fractures in children].

From January 1st 1990 to December 31st 1997, 614 children were treated for supracondylar humerus fracture at the Department of Orthopedic Surgery in the Olga Hospital, Stuttgart. Ten of these children had concomitant vascular complications. The concept of treatment we had chosen was analysed retrospectively. The median patient age of the seven girls and three boys with vascular complications was six years. The vascular injuries were diagnosed after admission to the hospital by palpation of the wrist pulse, clinical appraisal of the vascularity and by registration of the Doppler signal via the

Vascularised pedicular bone grafts.

When a massive free bone graft has to be incorporated into a large bone defect in the presence of a poor vascular recipient bed, the risks of absorption and failure of the graft to revascularise are high. Experimental studies have confirmed that a bone graft transferred to its recipient site with an intact pedicle of blood supply remains viable, and unites directly with the recipient bone without having to be revascularised and replaced by creeping substitution. It also provides a live bone bridge for reconstruction of a massive bone defect, and is a ready source of vascular osteogenic tissue

Personalized training and orthopedic inserts bring significant benefits in the posturographic assessment of patients with early gonarthrosis - a randomized controlled trial.

Knee osteoarthritis (KOA) results from cartilage degeneration and abnormal load distribution, leading to impaired biomechanics and postural control. Foot abnormalities such as hypermobility, flat feet, and pronation-common in KOA-may contribute to the degeneration process of the knee joint structures. Stabilization and proprioceptive exercises, as well as orthopedic inserts, can modify these factors by improving muscle function and redistributing load. However, studies directly comparing their effectiveness using objective assessment methods are lacking. Postural control plays a key role in KO

Plantar pressure distribution during gait cycle after subtalar arthroereisis in adolescent flexible flatfoot.

BACKGROUND: Flexible flatfoot is a common foot deformity in adolescents. Subtalar arthroereisis can help reduce excessive foot pronation by placing an implant in the subtalar joint. In recent years, this method has been widely used to treat adolescent flexible flat feet. However, some postoperative complications may occur 3-6 months after subtalar arthroereisis, and few studies have explored plantar pressure and lower limb muscle activation patterns during this period. METHODS: Twenty adolescents with flexible flatfoot deformities who underwent subtalar arthroereisis were enrolled in this stu

Terrible Triad Injuries around the Elbow: It is Still a Puzzle? Prospective Study.

INTRODUCTION: The "terrible triad" injury of the elbow, consisting of fractures of the coronoid process and radial head along with posterolateral elbow dislocation and refractory instability, has historically led to poor functional outcomes. Traditional treatment focused on bony injuries, but it is now recognized that soft-tissue injuries must also be addressed. Surgical management aims to restore bony and soft-tissue stabilizers, including fixation of the coronoid process and radial head, repair of ligament complexes, and reduction of elbow dislocation. Studies emphasize the importance of ear

The Impact of Obesity and Smoking on Outcomes After Volar Plate Fixation of Distal Radius Fractures.

PURPOSE: Distal radius fractures are common fractures of the upper extremity. Whereas surgical outcomes have been extensively investigated, the impact of risk factors such as body mass index (BMI) and smoking on patient outcomes has not been explored. We hypothesized that obesity and smoking would have a negative impact on the functional and radiographic outcomes of surgically treated patients with distal radius fractures. METHODS: We performed a retrospective analysis of patients surgically treated for a distal radius fracture between 2006 and 2017 at 2 level 1 trauma centers. Patients were

Conservative Follow-up of Severely Displaced Distal Radial Metaphyseal Fractures in Children.

Introduction Distal radius fractures are the most frequent fractures seen in pediatric population and usually treated with closed reduction and casting. However, there is a risk of reduction loss and/or angulations in distal radial metaphyseal fractures. The purpose of this study is to evaluate the radiological and functional results of pediatric patients with distal radius metaphyseal fractures in which excessive displacement and/or angulations were accepted and to question upper acceptable limits in light of current literature. Methods Patients between five and 15 years of age with displaced

Complications of hinged external fixation compared with cross-pinning of the elbow for acute and subacute instability.

BACKGROUND: Elbows that are unstable after injury or reconstructive surgery often are stabilized using external fixation or cross-pinning of the joint supplemented by cast immobilization. The superiority of one approach or the other remains a matter of debate. QUESTIONS/PURPOSES: We compared patients treated with external fixation or cross-pinning in terms of (1) adverse events, (2) Broberg and Morrey scores, and (3) ROM. METHODS: Between 1998 and 2010, 19 patients (19 elbows) had hinged external fixation and 10 patients (11 elbows) cross-pinning and casting for subacute or acute posttraumat

Use of a distraction plate for distal radial fractures with metaphyseal and diaphyseal comminution. Surgical technique.

BACKGROUND: Distal radial fractures with extensive comminution involving the metaphyseal-diaphyseal junction present a major treatment dilemma. Of particular difficulty are those fractures involving the articular surface. One approach is to apply a dorsal 3.5-mm plate extra-articularly from the radius to the third metacarpal, stabilizing the diaphysis and maintaining distraction across the radiocarpal joint. METHODS: Twenty-two patients treated with a distraction plate for a comminuted distal radial fracture were included in the study. With use of three limited incisions, a 3.5-mm ASIF plate

Salvage of post-traumatic arthritis following distal radius fracture.

There are practical recommendations that can be drawn from the aforementioned results. Due to the minimal morbidity of the wrist denervation, patients with good but painful wrist motion following fracture of the distal radius should first be evaluated for wrist denervation unless formal resection of the dorsal interosseous nerve has clearly been included in the previous treatment. The evaluation is performed in a standardized manner before and after test infiltration of both interosseous nerves. This evaluation includes assessment of pain, strength, and working capacity. Whereas the grip stren
