[A case of post-hemiplegic painful dystonia following thalamic infarction with good response to botulinus toxin].

We report a 67-year-old hypertensive right-handed woman who developed severe pain and dystonia in her left upper and lower extremity after a thalamic infarction. She was well until 9 months prior to the present admission to our hospital, when she had an acute onset of left hemiparesis which turned out to have been caused by a thalamic infarct. Her hemiparesis showed nearly complete recovery during the next four months. She noted an onset of severe spontaneous pain and difficulty in using her left hand four months prior to the present admission. Neurologic examination on admission revealed an a

Heterotopic ossification after complex elbow fracture-dislocations - Mid-term outcome and risk factor analysis.

BACKGROUND: Heterotopic ossification (HO) is a major complication following complex elbow fracture-dislocations and often leads to postoperative stiffness and impaired functional recovery. However, data focusing exclusively on complex elbow fracture-dislocations remain limited. The aim of this study was to determine the incidence and severity of HO, identify independent risk factors, and evaluate its impact on mid-term functional outcome. METHODS: A retrospective cohort study was performed including 169 patients who underwent surgical treatment for complex elbow fracture-dislocations between

Total elbow arthroplasty for the management of post-traumatic sequelae of distal humeral fractures : a systematic review and meta-analysis.

AIMS: Total elbow arthroplasty (TEA) has shown favourable outcomes in the management of severely comminuted distal humeral fractures in low-demand elderly patients, leading to its increased use in recent years. Less is known about the outcomes of TEA when used for the failure of either internal fixation or conservative management, particularly whether not performing TEA acutely influences the options for further management if fixation fails. This systematic review aimed to assess the clinical and functional outcomes following TEA when used for the post-traumatic sequelae of a distal humeral fr

Biomechanical Analysis of Standard Locking Compression Plate versus Dual Minifragment Locking Plates in Ulna Shaft Fracture Fixation: A Human Cadaveric Study.

PURPOSE: The main postoperative complications after fixation of ulna shaft fractures are nonunion and soft tissue irritation. Current standards recommend 3.5 mm locking compression plates, which are prone to cause soft tissue irritation, necessitating secondary implant removal in approximately 25% of cases. Therefore, an alternative approach considering a combination of two minifragment locking plates has been proposed. The aim of this study was to biomechanically compare a single 3.5 mm locking compression plate fixation versus dual minifragment locked plating of ulna shaft fractures in a hum

Surgical treatment of coronal shear fractures: short- to mid-term results and risk factor analysis.

BACKGROUND: Coronal shear fractures of the distal humerus are rare but severe injuries. Reconstruction is often challenging, especially in comminuted cases, which is why many surgeons opt for an elbow arthroplasty in those cases. However, total elbow arthroplasty is associated with a variety of potential problems itself. Therefore, the aim of this study was to present the functional and clinical outcome of coronal shear fractures treated by osteosynthetic reconstruction in a short- to mid-term follow-up, and to identify possible risk factors for an inferior outcome. METHODS: We performed a re

Clinical relevance of postoperative radial neck osteolysis after modular press-fit pyrocarbon radial head arthroplasty: a retrospective radiographic-functional correlation study.

BACKGROUND: Radial neck osteolysis is frequently reported after press-fit pyrocarbon radial head arthroplasty (RHA) and has been proposed as a marker of impaired outcome beyond a 3 mm threshold. Whether this association holds at short-to-midterm follow-up with the MoPyC implant is debated. METHODS: We retrospectively analysed 16 consecutive adults who underwent modular press-fit pyrocarbon RHA (MoPyC, FX Solutions) for unreconstructable radial head fractures between 2010 and 2023. Clinical evaluation comprised the Mayo Elbow Performance Score (MEPS), QuickDASH, visual analogue scale (VAS) for

The effect of joint angles on shot performance during the swing with different clubs among female professional golfers.

OBJECTIVE: This study aimed to investigate the differences in body joint angles during driver, 5-iron, and 7-iron swings in female professional golfers and their relationships with shot performance, providing a biomechanical basis for technical training. METHODS: Twelve female professional golfers were recruited. A Qualisys three-dimensional motion capture system (250 Hz) and a FlightScope Xi launch monitor were used to collect data at four key swing phases: half backswing (HB), top of backswing (TOP), half downswing (HD), and impact (IMP). Joint angles of the head, neck, torso, pelvis, elbow

Arthroscopically assisted repair of foveal triangular fibrocartilage complex tear using modified "double loop suture" - a retrospective cohort study.

BACKGROUND: The triangular fibrocartilage complex (TFCC) plays an important role in distal radioulnar joint (DRUJ) stabilization, and is frequently torn. In particular, when conservative treatment fails, surgical treatment is needed. METHODS: In this retrospective study, fourteen individuals with TFCC foveal tears and DRUJ instability were admitted to the department and treated with arthroscopic-assisted modified "double loop suture" transosseous repair between January 2021 and 2023. During surgery, an osseous tunnel was established, and two nickel-based alloy loops and a 2-0 polydioxanone II

Is Dorsal Vertical Double Plating an Effective Alternative to Volar Plating for Distal Radius Fractures With Dorsal Collapse?

OBJECTIVE: Dorsal articular collapse in distal radius fractures presents unique fixation challenges. While volar locking plating (VLP) dominates current practice, dorsal vertical double plating (DVDP) offers direct biomechanical support but carries perceived tendon risks. This study compares DVDP versus VLP for dorsally collapsed comminuted fractures. METHODS: A retrospective cohort of 106 patients (2022-2024) with AO type C2/C3 fractures received either VLP (n = 50) or DVDP (n = 56). General information encompassed gender, age, injured side, injury mechanism, AO classification, time from inj

Outcomes of Below-Elbow Versus Above-Elbow Immobilization in the Management of a Partial Triangular Fibrocartilage Complex Tear Without Distal Radio-Ulnar Joint Disruption: A Retrospective Study.

Background The triangular fibrocartilage complex (TFCC) is a crucial biomechanical structure that supports the distal radioulnar joint (DRUJ), forearm rotation, and load transfer along the ulnar aspect of the wrist. Conservative treatment is the preferred approach for managing stable TFCC injuries, but the techniques of immobilization remain a topic of debate. This study aims to evaluate whether above-elbow immobilization provides better pain relief, functional recovery, and fewer complications than below-elbow immobilization in the conservative treatment of partial TFCC tears without DRUJ dis

Unsatisfactory radiographic findings do not correlate with functional impairment in patients with coronal shear fractures of the distal humerus treated with internal fixation: a long-term retrospective study according to Dubberley's classification.

HYPOTHESIS: Coronal shear fractures of the distal humerus are uncommon elbow fractures involving the capitellum and trochlea. Dubberley's classification is the most commonly used and open reduction with internal fixation is the standard of care. This study aims to evaluate the long-term functional and radiographic findings of coronal articular shear fractures of the distal humerus treated with internal fixation and to determine the incidence of clinical complications according to Dubberley's classification and to correlate radiographic and functional outcomes. MATERIALS AND METHODS: A surgica

Minimum 10-year follow-up after open reduction and internal fixation of radial head fractures Mason type II and III.

BACKGROUND: The treatment of complex radial head fractures remains controversial with open reduction and internal fixation (ORIF), radial head arthroplasty, and radial head excision being the most common treatment options. While ORIF is the preferred treatment strategy for Mason type II fractures, the optimal treatment of Mason type III fractures is debated. The purpose of this study was to report minimum 10-year outcomes after ORIF of Mason type II and type III radial head fractures. We hypothesized that both Mason Type II and Type III fracture patients would demonstrate satisfactory clinical

Transolecranon fracture dislocation and transolecranon basal coronoid fracture dislocation; results of standardized treatment in a retrospective cohort.

BACKGROUND: Our understanding of transolecranon fracture dislocation (TOFD) has evolved to include associated ligament, coronoid, and radial head lesions impacting stability, rather than solely bone stability. Similarly, the understanding of coronoid fracture patterns has progressed. O'Driscoll et al state that basal subtype 2 coronoid fractures correspond to a TOFD, with a fracture passing through the base of the coronoid. In the literature, there is no clear differentiation between basal coronoid TOFD (BC-TOFD) and pure-TOFD outcomes. The main objective of this study is to evaluate the funct

A Novel Technique of Arthroscopic-Assisted Four-Corner Fusion and Robot-Assisted Fixation for Scaphoid Nonunion Advanced Collapse Wrist: A Single Case Study.

OBJECTIVE: Scaphoid nonunion advanced collapse (SNAC) is a relatively common and debilitating wrist disorder yet its treatment remains challenging and controversial. We aim to describe a novel technique of a dual arthroscopic and robotic assisted four-corner fusion (4CF) in the treatment of SNAC wrist. METHODS: In this study, we describe an original arthroscopic and robotic assisted 4CF, which is novel and currently unpublished in literature. The surgical approach included these predefined steps: arthroscopic resection of the scaphoid, radial styloid and of diseased cartilage between the capi
