Pancreatic Calculus Causing Biliary Obstruction: Endoscopic Therapy for a Rare Initial Presentation of Chronic Pancreatitis.

INTRODUCTION: Biliary obstruction in chronic calcific pancreatitis (CCP) is often caused by inflammatory or fibrotic strictures of the bile duct, carcinoma of head of pancreas or less commonly by compression from pseudocysts. Pancreatic calculi causing ampullary obstruction and leading to obstructive jaundice is extremely rare. METHODS: The medical records of all patients with CCP or biliary obstruction who underwent endoscopic retrograde cholangiopancreatography (ERCP) over 4 years between 2010-2014 at Kasturba Medical College, Manipal were analyzed. RESULTS: Five patients of CCP with impac

[Endosonography in diagnosis of choledocholithiasis].

BACKGROUND AND STUDY AIMS: In 38-80% patients with suspicion to choledocholithiasis were no stones in common bile duct (CBD) after endoscopic retrograde cholangiopancreatography (ERCP). ERCP is not safe procedure and has a risk of complications ranging 4-15%. The aim of our study was to estimate the diagnostic value and safety of endoscopic ultrasound (EUS) in CBD stones diagnosis. PATIENTS AND METHODS: In patients with medium probability of CBD stones (i.e. CBD ≤ 10 mm and/or bilirubin ≤ 2 times upper normal) was performed BUS with Pentax EG-3870UTK and Hitachi EUB-7000HV. From September 201

Precut papillotomy using needle knife fashioned from discarded standard papillotomes.

AIM: To evaluate the results of precut papillotomy using needle-knife papillotomes fashioned from discarded standard sphincterotomes. METHODS: Case records of 50 patients undergoing precut papillotomy for access to either common bile duct or pancreatic duct during endoscopic retrograde cholangiopancreatography were reviewed. Precut was performed using needle-knife papillotomes fashioned from standard pull-type sphicterotomes that were discarded because of broken cutting wires. A diagnostic procedure was planned in all 50 patients (bile duct = 39, pancreatic duct = 9, both ducts = 2) and thera

[Pneumopericardium and retroperitoneal and scrotal emphysema after endoscopic papillotomy].

Retroduodenal perforation is considered a rare but serious complication of endoscopic papillotomy. We report on a patient suffering from a stenotic occlusion of the ductus hepatico-choledochus on whom a pre-cut via needle-knife was performed in order to place a stent. Hours after the procedure, the patient developed an extensive pneumoretroperitoneum, pneumopericardium and scrotal emphysema. The later have not been reported in relevant literature so far.

Intraoperative endoscopic papillotomy and stone removal.

Endoscopic papillotomy was attempted during cholecystectomy in 33 patients who suffered from both gallbladder and bile duct stones simultaneously (32 conventional, 1 laparoscopic cholecystectomy). Twenty-six of these patients suffered from acute obstruction of the biliary tree which required urgent operation. In addition, 10 of these patients suffered from acute biliary pancreatitis. Because of the unusual intraoperative conditions, cannulation of the papilla was modified ("orthograde papillotomy"); the instruments used were the same as for conventional endoscopic papillotomy. In 28 out of the

Pancreatic duct guidewire placement for biliary cannulation for the prevention of post-endoscopic retrograde cholangiopancreatography (ERCP) pancreatitis.

BACKGROUND: Difficult cannulation is a risk factor for post-endoscopic retrograde cholangiopancreatography (ERCP) pancreatitis (PEP). It has been postulated that the pancreatic duct guidewire (PGW) technique may improve biliary cannulation success and reduce the risk of PEP in people with difficult cannulation. OBJECTIVES: To systematically review evidence from randomised controlled trials (RCTs) assessing the effectiveness and safety of the PGW technique compared to persistent conventional cannulation (CC) (contrast- or guidewire-assisted cannulation) or other advanced techniques in people w

Advanced and salvage techniques for difficult biliary cannulation in endoscopic retrograde cholangiopancreatography: a randomized trial of outcomes and efficacy.

BACKGROUND AND AIMS: Difficult biliary cannulation (DBC) complicates 5% to 20% of ERCP procedures. Advanced techniques, double-guidewire technique (DGT), transpancreatic sphincterotomy (TPS), and precut fistulotomy, have been introduced to improve outcomes, but their comparative effectiveness remains uncertain. This study compared their efficacy, safety, and time-related performance using robust statistical analyses. METHODS: This randomized trial was conducted from March 2019 to March 2022 at 2 tertiary centers in Egypt. A total of 150 patients with DBC were assigned to DGT, TPS, or precut f

Risk Factors for Acute Cholangitis Caused by Extended-Spectrum Beta-Lactamase and AmpC-Producing Organisms.

INTRODUCTION: Prompt antibiotic administration is essential in the management of acute cholangitis (AC). In selecting initial antibiotics, it is important to consider both the severity of the condition and the antimicrobial activity spectrum, particularly against major resistant bacteria. Extended-spectrum β-lactamases and AmpC productions are key factors in pathogen resistance; however, knowledge of patient characteristics in cholangitis management remains limited. We aimed to analyze risk factors for extended-spectrum β-lactamase-producing Enterobacteriaceae (ESBL-E) and for chromosomally me

Standard Cannulation versus Fistulotomy for Biliary Access in Endoscopic Retrograde Cholangiopancreatography: Should We Expect the Same Success when Treating Choledocholithiasis?

INTRODUCTION: To access the common bile duct in endoscopic retrograde cholangiopancreatography (ERCP), needle-knife fistulotomy (NKF) can be associated with a shorter sphincterotomy compared to standard cannulation. We aimed to compare the success and safety of NKF versus standard cannulation in the treatment of choledocholithiasis. METHODS: A cohort of 379 naïve patients with choledocholithiasis who underwent ERCP between 2005 and 2022 was retrospectively analyzed. The patients were divided into two groups: group A (179 consecutive patients) underwent NKF, while group B (180 patients) receiv

Endoscopic features of low-phospholipid-associated cholelithiasis syndrome: A retrospective cohort study.

BACKGROUND AND OBJECTIVE: LPAC (low phospholipid-associated cholelithiasis) syndrome is a rare genetic form of cholelithiasis. ERCP (endoscopic retrograde cholangiopancreatography) is often used to remove gallstones in the bile duct. No published data is available on the role of ERCP in LPAC syndrome. PATIENTS AND METHODS: In this retrospective cohort study, we included patients diagnosed with LPAC syndrome in a single tertiary referral center between 2009 and 2021. Our aim was to assess the frequency, indications, modalities, results, and complications of ERCP, as well as predictive factors

Impact of difficult biliary cannulation on post-ERCP pancreatitis: secondary analysis of the stent versus indomethacin trial dataset.

BACKGROUND AND AIMS: Difficult biliary cannulation (DBC) is a known risk factor for developing post-ERCP pancreatitis (PEP). To better understand how DBC increases PEP risk, we examined the interplay between technical aspects of DBC and known PEP risk factors. METHODS: This was a secondary analysis of a multicenter, randomized controlled trial comparing rectal indomethacin alone with the combination of rectal indomethacin and prophylactic pancreatic duct (PD) stent placement for PEP prophylaxis in high-risk patients. Participants were categorized into 3 groups: DBC with high preprocedure risk

Utility of Urgent Endoscopic Retrograde Cholangiopancreatography in Patients with Predicted Mild Acute Pancreatitis and Cholestasis.

BACKGROUND: Endoscopic retrograde cholangiopancreatography (ERCP) within 72 h is suggested for patients presenting with acute biliary pancreatitis (ABP) and biliary obstruction without cholangitis. This study aimed to identify if urgent ERCP (within 24 h) improved outcomes compared to early ERCP (24-72 h) in patients admitted with predicted mild ABP. METHODS: Patients admitted for predicted mild ABP defined as a bedside index of severity in acute pancreatitis score < 3 and underwent ERCP for biliary obstruction within 72 h of presentation during the study period were included. Patients with p

Endoscopic transpapillary gallbladder drainage for the management of acute calculus cholecystitis patients unfit for urgent cholecystectomy.

ObjectivesEndoscopic transpapillary gallbladder drainage (ETGBD) has been proposed as an alternative to surgery or percutaneous cholecystostomy in patients with acute calculus cholecystitis (ACC). We aimed to evaluate the safety and efficacy of ETGBD via endoscopic transpapillary gallbladder stenting (ETGBS) or endoscopic naso-gallbladder drainage (ENGBD) as either a bridging or a definitive treatment option for patients with ACC when a cholecystectomy is delayed or cannot be performed.MethodsFrom July 2014 to December 2018, 171 patients with ACC in whom ETGBD were attempted were retrospective
