Dyshidrotic eczema (DE) is a chronic and/or recurrent vesicular eczema of the palmar sides of the hands and sometimes the soles of the feet. The major aetiologies to be considered are atopic DE, contact DE, systemic contact DE and DE due to fungal or microbial allergy. To clarify the role of occupational contact allergens, 59 workers with DE were examined to determine whether they had atopy, environmental contact sensitization and/or contact allergy coupled with work activity. For all, patch testing was carried out using the European standard series and the material brought in by the patients.

Gloves are now worn routinely by most general dental practitioners and dental students while treating patients, with latex being the most commonly used glove material. By means of a questionnaire, this study surveyed 160 students in one dental school and 300 randomly selected general dental practitioners in respect of their exposure to latex. Response rates were 74.4 per cent and 57.3 per cent for dental students and practitioners respectively. Epicutaneous tests do not permit resolution of all the problems set by drug allergy, but they are an important asset. Their value is sure when the pathology does not correspond precisely with a Gell and Coombes hypersensitisation classification, and especially when there is an interaction between several mechanisms. This presentation of five cases shows the value of these tests.

Ectodermal dysplasias are diseases with abnormal development of ectodermally derived tissues such as skin, hair, teeth, and nails. Mutations in the transcription factor p63 have been linked to several syndromes characterized by ectodermal, orofacial, and limb defects. We describe the case of a 69-year-old man with malignant lymphoma who developed granuloma annulare on his fingers. The number of cutaneous nodules increased when a recurrence of lymphoma was demonstrated during the course of disease. Granulomatous lesions disappeared after the fifth cycle of chemotherapy with CHOP (cyclophosphamide, Adriamycin, vincristine, prednisolone). The coexistence of these two diseases is relatively rare. The clinical features seen in this case, however, might suggest that granuloma annulare is a generalized granulomatous reaction induced by malignant lymphoma.

Management of hand eczema is complex because of the broad range of different pathogeneses, courses, and prognoses. Furthermore, the efficacy of most available treatments is not well established and the more severe forms can have a major impact on the patient's quality of life. Digital ulcers in systemic sclerosis are painful ischemic necrotic lesions of the acra. Optimal treatment consists of conventional wound management and medication: iloprost infusions promote primary healing of the ulcers, while the dual endothelin receptor antagonist bosentan is used for secondary prophylaxis of new ulcers. Erythema elevatum diutinum is a rare chronic inflammatory skin disease of unknown etiology with a histopathology of a leukocytoclastic vasculitis. Clinical findings involve symmetrical livid-erythematous to red-brown papules and plaques or nodules which are initially soft but become firmer. The skin lesions are located over extensor surfaces and often over joints. Early treatment with dapsone, corticosteroids and antibiotics is sometimes effective. A patient with multiple myeloma displayed unusual lesions of erythema elevatum diutinum.
