Repair of the Medial Ulnar Collateral Ligament of the Elbow With Internal Brace Augmentation.

BACKGROUND: The medial ulnar collateral ligament (MUCL) is the primary restraint to valgus force across the elbow during throwing. The incidence of MUCL continues to rise, and injuries are common in overhead throwing athletes often leading to elbow pain and instability. While MUCL reconstruction remains the gold standard for high-level throwing athletes with midsubstance tears, MUCL repair remains an option for select athletes and affords good functional outcomes with quicker return to sport compared with reconstruction. INDICATIONS: Indications for MUCL repair include acute injuries, proxima

Comparison of elbow valgus laxity and medial elbow stabilizer parameters during repetitive pitching between high school baseball players with and without a history of medial elbow injury.

BACKGROUND: Adolescent baseball players with a history of medial elbow injury are often reinjured during demanding practice sessions or games. The physical characteristics of baseball players with a history of medial elbow injury have been reported in several previous studies, but the nature of their medial elbow stabilizers is unknown. This study investigated the difference in characteristics of elbow valgus laxity and medial elbow stabilizer parameters between baseball players with and without a history of medial elbow injury, prior to and during repetitive pitching. METHODS: Sixteen high s

Determining the Effectiveness of a New Device for Hand Therapy (The FEPSim Device): Feasibility Protocol for a Randomized Controlled Trial Study.

BACKGROUND: Impairments of the forearm, wrist, and hand affect a sizable proportion of individuals and impose a significant economic burden on health care systems. FEPSim is a medical device for hand and wrist rehabilitation. The FEPSim device could be part of the standard of care for upper extremity rehabilitation during therapeutic activities to increase range of motion, dexterity, and strength. FEPSim has not yet been tested in a health care setting; therefore, a trial of the effectiveness of FEPSim in upper extremity rehabilitation is warranted. OBJECTIVE: This study aims to assess the fe

Accuracy of MRI and X-Ray Measurement of Displacement Distance of Humeral Lateral Condyle Fractures.

OBJECTIVE: To investigate the accuracy of X-ray and magnetic resonance imaging (MRI) measurements in evaluating the displacement of humeral lateral condyle fracture (HLCF) in different positions of the forearm based on human cadaveric HLCF models. METHODS: Three human cadaveric elbow HLCF fracture models were successfully established. The wrist joint was fixed, and the forearm was rotated forward along the mid-axis. The maximum distance between the two segments of the lateral fracture gap was defined as LFS (lateral fracture space) distance, and the maximum distance between the two segments o

Surgical Treatment of Lateral Epicondylitis: A Prospective, Randomized, Double-Blinded, Placebo-Controlled Clinical Trial.

BACKGROUND: A number of surgical techniques for managing tennis elbow have been described. One of the most frequently performed involves excising the affected portion of the extensor carpi radialis brevis (ECRB). The results of this technique, as well as most other described surgical techniques for this condition, have been reported as excellent, yet none have been compared with placebo surgery. HYPOTHESIS: The surgical excision of the degenerative portion of the ECRB offers no additional benefit over and above placebo surgery for the management of chronic tennis elbow. STUDY DESIGN: Randomi

Distal Biceps Tendon Anatomic Repair.

Distal biceps injuries, which usually occur in active middle-aged men, can result in chronic pain and loss of supination and flexion strength3,4. Surgical repair of a ruptured distal biceps tendon can reliably decrease pain and improve strength compared with nonoperative management3,4. However, even following successful healing and rehabilitation of a surgically repaired biceps tendon, full supination strength is rarely restored5-7. The expected outcome following distal biceps repair using a traditional anterior approach is a measurable loss of rotational strength, especially from neutral to s

Open Treatment of Radial Head Fractures.

Radial head fractures may commonly be treated by (1) open reduction and internal fixation (ORIF), (2) radial head excision, or (3) radial head replacement. If there is no associated elbow instability with lateral ulnar collateral ligament (LUCL) injury, the preferred approach is via a split in the extensor digitorum communis (EDC) origin. This provides a wide exposure but limits the risk of injury to the LUCL and associated instability. The radial head is fixed, excised, or replaced. Open treatment of radial head fractures begins with the following steps: (1) a laterally based incision is cent

Is it really necessary to restore radial anatomic parameters after distal radius fractures?

UNLABELLED: Small variations within normal range of radiographic parameters, except ulnar variance and volar tilt, do not influence the final functional outcome in distal radius fractures. INTRODUCTION: There are many reports in the literature on the relationship between radiographic variables and their influence on the final outcome of distal radius fractures. Most authors report that a good functional result depends on anatomical restoration of the articular surface and extra-articular alignment. The aim of this study was to verify if it is really necessary to restore anatomic radiographic

Critical time period for recovery of functional range of motion after surgical treatment of complex elbow instability: prospective study on 76 patients.

INTRODUCTION AND AIM: Complex elbow instability (CEI) is one of the most troublesome pathologies that orthopaedic surgeons have to face. One of the key requirements regarding the CEI surgical treatment is an early rehabilitation programme to avoid the elbow stiffness caused by a long period of immobilisation. Although this is well known, no study has ever examined how, and to what extent, the functional range of motion (ROM) is recovered during the various stages of a prompt rehabilitation. Our aims were: (1) to prospectively analyse the pattern of ROM recovery in a series of patients with CEI

History and radiographic findings associated with clinically suspected radial head subluxations.

OBJECTIVES: To determine: 1) physician practices regarding the use of radiographs for radial head subluxations (RHS), 2) the prevalence of missed fractures in children with a clinical diagnosis of RHS, 3) the relative risk of a fracture with a nonclassic history for mechanism of injury for RHS, and 4) radiographic findings associated with RHS that are difficult to reduce. METHODS: This study began with a physician survey that addressed the integration of radiographs into the management of RHS. We subsequently conducted a prospective randomized trial with a consecutive sampling of children les

Quality of surgical technique in patients with a humeral shaft fracture in the HUMMER study (HUMMER-Survey): an international survey among expert surgeons.

BACKGROUND: It is undetermined whether the quality of surgical technique in a humeral shaft fracture (reduction and fixation) affects outcomes. The aim of this study was to score the quality of surgical technique performed in patients with humeral shaft fractures and to assess whether the quality of surgical technique was associated with complications and (functional) outcomes. METHOD: A panel of expert orthopedic trauma surgeons, experienced in upper extremity surgery, assessed the quality of surgical technique (good, sufficient, or poor) in patients who participated in the HUMMER study on p

How does a prior olecranon osteotomy impact the outcome of subsequent total elbow arthroplasty?

BACKGROUND: Olecranon osteotomy is a common surgical exposure technique used for internal fixation of distal humerus fractures. Unfortunately, retained ulnar hardware and potential osteotomy complications (malunion, nonunion, or extensor mechanism insufficiency) may negatively impact subsequent total elbow arthroplasty (TEA) if needed. Although TEA is a reasonable option for management of failed distal humerus internal fixation or posttraumatic arthritis, little is known about how prior olecranon osteotomy influences TEA outcomes. This study aimed to identify operative considerations, evaluate

Outcomes of implant removal after failed radial head arthroplasty: an average 7-year follow-up study.

BACKGROUND: Failure of radial head implants secondary to loosening, painful capitellar wear, and other reasons is not uncommon. When radial head implants fail, the relative indications for implanting a new radial head implant vs. implant removal with or without soft-tissue interposition remain unclear. Removal theoretically reduces the risk of radiocapitellar pain; however, little is known about the outcomes following removal of failed radial head replacements. The purpose of this study was to evaluate the outcomes, complications, and rates of proximal radioulnar impingement pain following rem

Ventilation-perfusion matching in early-stage of prone position ventilation: a prospective cohort study between COVID-19 ARDS and ARDS from other etiologies.

Objective.Prone positioning has been established as a therapeutic strategy for severe acute respiratory distress syndrome (ARDS). In COVID-19-associated ARDS (CARDS), the application of prone position has shown varying responses, influenced by factors such as lung recruitability and SARS-CoV-2-induced pulmonary endothelial dysfunction. This study aimed to compare the early impact of pronation on lung ventilation-perfusion matching (VQmatch) in CARDS and non-COVID-19 ARDS patients (non-CARDS).Approach.This was a two-center, prospective study comparing between CARDS and non-CARDS.
