Effect of empagliflozin on ventricular arrhythmias in patients with type 2 diabetes treated with an implantable cardioverter-defibrillator: the EMPA-ICD trial.

BACKGROUND: Sodium-glucose cotransporter 2 (SGLT2) inhibitors reduce the risk of hospitalization for heart failure and cardiovascular death with type 2 diabetes; however, their effect on arrhythmias is unclear. The purpose of this study was to investigate the effects of empagliflozin on ventricular arrhythmias in patients with type 2 diabetes. METHODS: A total of 150 patients with type 2 diabetes who were treated with an implantable cardioverter-defibrillator or cardiac resynchronization therapy defibrillator (ICD/CRT-D) were randomized to once-daily empagliflozin or placebo for 24 weeks. Why is the clinical response to cardiac resynchronization better in LBBB patients?

AIMS: The improved clinical response in patients with left bundle branch block (LBBB) over LBBB-free patients treated with cardiac resynchronization therapy with a defibrillator (CRT-D) is commonly attributed to an LBBB abnormality. We aimed to find an alternative explanation. METHODS AND RESULTS: We analysed an immediate effect of selecting the LBBB group of patients in a cohort of 63 non-ischaemic cardiomyopathy (non-ICM) and 83 ischaemic cardiomyopathy (ICM) patients treated with CRT-D; 75% of non-ICM and 51% of ICM patients had an LBBB abnormality on the electrocardiogram, with a signific

Characterization of left ventricular activation in patients with heart failure and left bundle-branch block.

BACKGROUND: Conventional activation mapping in the dilated human left ventricle (LV) with left bundle-branch block (LBBB) morphology is incomplete given the limited number of recording sites that may be collected in a reasonable time and given the lack of precision in marking specific anatomic locations. METHODS AND RESULTS: We studied LV activation sequences in 24 patients with heart failure and LBBB QRS morphology with simultaneous application of 3D contact and noncontact mapping during intrinsic rhythm and asynchronous pacing. Approximately one third of the patients with typical LBBB QRS m

Prevalence and incidence of intra-ventricular conduction delays and outcomes in patients with heart failure and reduced ejection fraction: insights from PARADIGM-HF and ATMOSPHERE.

AIMS: The importance of intra-ventricular conduction delay (IVCD), the incidence of new IVCD and its relationship to outcomes in heart failure and reduced ejection fraction (HFrEF) are not well studied. We addressed these questions in the PARADIGM-HF and ATMOSPHERE trials. METHODS AND RESULTS: The risk of the primary composite outcome of cardiovascular death or heart failure hospitalization and all-cause mortality were estimated by use of Cox regression according to baseline QRS duration and morphology in 11 861 patients without an intracardiac device. At baseline, 1789 (15.1%) patients had l

Age differences in contemporary treatment of patients with chronic heart failure and reduced ejection fraction.

BACKGROUND: Elderly patients are underrepresented in clinical trials but comprise the majority of heart failure patients. Data on age-specific use of heart failure therapy are limited. The European Society of Cardiology heart failure guidelines provide no age-specific treatment recommendations. We investigated practice-based heart failure management in a large registry at heart failure outpatient clinics. DESIGN AND METHODS: We studied 8351 heart failure with reduced ejection fraction patients at 34 Dutch outpatient clinics between 2013 and 2016. The mean age was 72.3 ± 11.8 years and we divi

A Device Histogram-Based Simple Predictor of Mortality Risk in ICD and CRT-D Patients: The Heart Rate Score.

BACKGROUND: We hypothesized that survival in implantable cardioverter defibrillator (ICD) and cardiac resynchronization therapy defibrillator (CRT-D) patients is predicted by baseline Heart Rate Score. METHODS: Heart Rate Score is determined from the atrial paced and sensed histogram of a DDD ICD or CRT-D, and defined as percent of beats in the histogram in the tallest 10 beats/min range bin. It was calculated at initial remote monitoring for patients enrolled in LATITUDE® without persistent atrial fibrillation, and with pulse generators implanted in 2006-2011. Improved implantable cardioverter-defibrillator shock efficacy using programmable pulse width.

BACKGROUND: Each phase of a biphasic implantable cardioverter-defibrillator (ICD) shock waveform can be characterized by either the percentage of peak voltage that decays (tilt) or the time taken for that decay (pulse width). Although shock programming has typically relied on fixing the tilt, patient-specific fixed pulse widths may improve shock success. OBJECTIVE: The purpose of this study was to compare shock energies and success rates to treat spontaneous ventricular tachycardia/ventricular fibrillation using ICD/CRT-D (cardiac resynchronization therapy - defibrillator) devices with fixed
