Primary Souter-Strathclyde total elbow prosthesis in rheumatoid arthritis.

BACKGROUND: Total elbow arthroplasty is a well-established treatment for the painful elbow joint in patients with rheumatoid arthritis. We present the results of what we believe to be the first prospective study of the Souter-Strathclyde total elbow prosthesis. METHODS: Between June 1982 and December 2000, 204 primary total elbow prostheses were inserted in 166 patients who had rheumatoid arthritis. No patient was lost to follow-up. The mean duration of follow-up was 6.4 years. All patients were examined preoperatively, at one and two years postoperatively, and at regular intervals thereafter

Nursemaid's Elbow in Children: Proposal of a Preventive Strategy in the Digital Era.

Background Nursemaid's elbow, or painful pronation of the elbow, is a common orthopedic condition in children aged 0-5 years. It is typically caused by sudden traction on the forearm, leading to subluxation of the radial head. Despite its benign nature and straightforward treatment, the condition remains frequent, largely due to insufficient awareness among parents and caregivers. Preventive strategies, including health education, are essential to reduce incidence and recurrence. Objective The primary objective of this study was to evaluate the level of knowledge and awareness of nursemaid's e

[On-Table Reconstruction of Radial Head Fractures].

PURPOSE OF THE STUDY: Fractures of the proximal radius constitute a significant proportion of elbow joint injuries, representing approximately one-third of all such fractures and 75% of proximal forearm fractures. In adults, they account roughly for 4% of all fractures. Treatment of these fractures varies depending on their severity. No uniform guidelines have been published to date for the treatment of severely comminuted fractures. MATERIAL AND METHODS: The study included 16 patients who underwent on-table reconstruction, with a mean age of 49 years. Half of the cases involved Mason type II

Locked plating of geriatric olecranon fractures leads to low fixation failure and acceptable complication rates.

HYPOTHESIS: The purpose of this study was to report the rate of major complications in patients with geriatric olecranon fractures managed operatively with a locking plate. Secondary objectives included minor complications, as well as pain and range of motion at the final follow-up. We hypothesized that these patients have a low rate of complications as well as low pain and satisfactory elbow range of motion at the final follow-up. MATERIALS AND METHODS: A retrospective review of isolated geriatric olecranon fractures presenting from 2006 to 2019 was performed at a single level I trauma cente

Predictors for outlier results after radial head replacement for acute complex elbow instability.

BACKGROUND: Radial head replacement (RHR) is a well-established treatment for comminuted fractures (radial head fracture). In most cases, it provides satisfactory long-term functional results. However, some patients have pain and significant functional limitations. Our aim was to identify patients who underwent an RHR for acute trauma and had a clinical outcome that was considered an outlier. We assessed specific factors that contributed to an unfavorable result. MATERIALS AND METHODS: From a cohort of 134 patients undergoing RHR for acute complex elbow instability, the Oxford Elbow Score (OE

Response to the first awake prone positioning relates with intubation rate in SARS-CoV-2 patients suffering from acute respiratory failure with moderate to severe hypoxaemia: a retrospective study.

AIMS OF THE STUDY: Awake prone positioning (aPP) in non-intubated patients with severe SARS-CoV-2-related pneumonia improves oxygenation and reduces the intubation rate, but no early predictors for success or failure of the strategy have been described. The main objective of this study was to assess whether response to the first aPP in terms of PaO2/FiO2, alveolar-arterial gradient (Aa-O2), respiratory rate and PaCO2 could predict the need for intubation. As secondary objective, we assessed the effects of aPP on the same parameters for all the sessions considered together. METHODS: Retrospect

Short-term results of arthroscopic treatment of osteochondritis dissecans in skeletally immature patients.

BACKGROUND: Osteochondritis dissecans (OCD) of the capitellum occurs in skeletally immature athletes, and most likely results from repetitive trauma during overhead activities. Treatment may consist of activity modifications, internal fixation, abrasion chondroplasty, microfracture, antegrade drilling, retrograde drilling, osteochondral autograft or allograft implantation, chondrocyte implantation, and rib autograft through arthroscopy or arthrotomy. One treatment modality has not been proven to be clearly more effective than the others. This study was undertaken to evaluate our treatment regi

Prevention of non-contact anterior cruciate ligament injuries in soccer players. Part 1: Mechanisms of injury and underlying risk factors.

Soccer is the most commonly played sport in the world, with an estimated 265 million active soccer players by 2006. Inherent to this sport is the higher risk of injury to the anterior cruciate ligament (ACL) relative to other sports. ACL injury causes the most time lost from competition in soccer which has influenced a strong research focus to determine the risk factors for injury. This research emphasis has afforded a rapid influx of literature defining potential modifiable and non-modifiable risk factors that increase the risk of injury. The purpose of the current review is to sequence the m

ARM: a modular hinged joint for the AO tubular external fixator.

Certain complex traumatic elbow lesions challenge the orthopaedic and trauma surgeon. If they are not treated correctly, they cause a high rate of disability, arising from elbow instablility and stiffness, either by fibrosis or joint incongruity. Injuries such as complex fractures of the proximal third of the ulna, coronoid fractures associated with radial head fractures (the "terrible triad"), are even worse if they are accompanied by soft tissue lesions. Hinged external fixators, complemented by other surgical procedures, are, for many, a recommended alternative when dealing with irreparable

Lacertus Syndrome: an Outcome Analysis After Lacertus Release.

PURPOSE: The lacertus fibrosus or bicipital aponeurosis is a sheet of ligamentous tissue just distal to the elbow joint and can be a compression point for the median nerve. Essentially, lacertus syndrome is a subset of pronator syndrome and an uncommon diagnosis by itself. Surgical release of the lacertus consists of a small 2-cm incision that can be performed under local anesthesia. This study aimed to evaluate the outcome of lacertus release in resolving median nerve symptoms. METHODS: This retrospective study was performed at Prince Court Medical Centre, Kuala Lumpur, Malaysia, from Januar

[Our Initial Experience with Radial Head Replacement].

PURPOSE OF THE STUDY A single treatment procedure for multi-fragmented non-reconstructible radial head fractures has not been established as of yet. One of the available treatment methods can be the implantation of an endoprosthesis, but there is no consensus in available publications. We therefore decided to trial their use and to obtain our own experience. This study includes the evaluation of the outcomes of treatment at one year postoperatively. MATERIAL AND METHODS Our group included eight patients (six men and two women with the mean age of 46.1 years, ranging from 24 to 80 years) with a

The results of conservatively treated simple elbow dislocations.

OBJECTIVES: Closed reduction followed by short-time immobilization is the generally accepted treatment for simple elbow dislocations. However, the number of studies concerning the results of this method is limited. In this study, the clinical and radiographic results of conservatively treated simple elbow dislocations were retrospectively evaluated. METHODS: The study included 21 patients (16 males, 5 females) who underwent closed reduction and immobilization for simple elbow dislocations. Simple dislocations were defined as non-fracture dislocations and dislocations accompanied by minor avul

Wrist arthrodesis.

Wrist arthrodesis results in a high degree of patient satisfaction and predictable pain relief in most patients. Most patients are able to return to gainful employment, many without impairment. Some patients require restrictions and employment in a less strenuous occupation. Successful fusion rates have been reported in the vast majority of patients overall. Although the functional outcome is acceptable for most patients, some adaptation is necessary, because certain activities such as perineal care and manipulating the hand in tight spaces are difficult. Activities that require forceful grippi
