The extreme stress on the long head of the biceps tendon is defined by its specific anatomic course, with its proximity to the rotator cuff, the intertubercular groove and the acromion. We studied the morphologic and cross-sectional anatomy of the long head of the biceps tendon in 104 cadavers to analyze degenerative lesions. Degenerative changes in the tendon (disorganized collagen fibers and large mucoid deposits) occurred mainly in the distal bicipital groove and near the origin of the tendon from the superior part of the glenoid labrum. The results of histologic examination identified site

MRI is valuable in evaluation of tendons and ligaments in sports-related injuries. Tendons and ligaments show dark signal intensity on all MR sequences. Alteration in size, shape, continuity, and signal intensity of these structures can be well visualized with MRI. Impingement syndrome is the leading cause of atrophy and degeneration of the rotator cuff. Shape of the acromion, presence of subacromial spur, and involvement in certain sport activities that lead to shoulder instability may cause impingement of the rotator cuff. This article systematically reviews the injuries of tendons and ligam

BACKGROUND: Safety zones to avoid nerve injury at proximal incision of posterior minimally invasive plate osteosynthesis for humerus fracture have been scarcely studied. The purpose of this study was to describe the location of axillary and radial nerves (RN) in magnetic resonance imaging to establish safety zones.
METHODS: Fifty-two magnetic resonance imaging studies of the entire humerus were reviewed. The mean age was 50.6 ± 12.1 years, with 37 female patients. The distance of the axillary nerve (AN; distal portion, humeral midpoint) and RN (medial border, midpoint, and lateral border of t

Degenerative lesions of the subacromial structures can cause chronic anterior pain and disability of the shoulder. They are most frequently found in patients whose arms are repeatedly exposed to force while raised above head level, e.g. during tennis, volleyball or swimming. This is often due to impingement of the supraspinatus tendon, the subacromial bursa or the biceps tendon against the anterior edge and lower surface of the anterior third of the acromion, the coracoacromial ligament and, in some cases, the acromioclavicular joint. Open anterior acromioplasty as described by Neer is therefo

Background and Objectives: Anatomical variations in the head, neck and chest are common, and are observed as occasional findings on computed tomography (CT). Although anatomical variations are mostly asymptomatic and do not cause any negative influence on the body function, they may jeopardize diagnosis and may be confused with pathological conditions. The presence of variations may also limit surgical access during tumor removal. The aim of this study was to investigate the prevalence of six anatomical variations-os acromiale, episternal ossicles, cervical rib, Stafne bone cavity, azygos lobe

BACKGROUND: Portal placement is a key factor for the success of arthroscopic procedures, particularly in rotator cuff repair. We hypothesize that the acromial anatomy may strongly determine the position of the shoulder bony landmarks and limit the surgeon's freedom to position the arthroscopic approaches in direction towards the acromion. The purpose of this study was to analyze the relation between different acromial shapes and the freedom of movement of arthroscopic instruments relative to the rotator cuff from standardized arthroscopic portals in a laboratory study on 3D shoulder models.

Current wisdom holds that the acromion is a major factor in the development of rotator cuff impingement. From the examination of a large number of skeletal specimens (1232 shoulders), we conclude that this view may require some modification. It appears that internal impingement between the glenoid and the humeral head may also be a significant mechanism in the development of rotator cuff pathosis. We present here the evidence for this internal impingement, which is expressed in characteristic telltale patterns impressed into the bones on both sides of the joint. If this thesis is correct, the

BACKGROUND: Several radiologic parameters representing scapular morphology have been proposed as risk factors for degenerative posterosuperior rotator cuff tear (PSRCT). The causes of PSRCT are multifactorial; many reported risk factors are compound entities. However, no determination, made after adjustment with previously reported risk factors, of which radiologic parameters are independent risk factors for degenerative PSRCT, has been reported. Therefore, this study's purpose was to determine which radiologic parameters representing scapular morphology are independent risk factors for degene

Arthroscopy : the journal of arthroscopic & related surgery : official publication of the Arthroscopy Association of North America and the International Arthroscopy Association2019eng
Rotator cuff tears are common. When indicated, surgical repair is a highly successful procedure. There are circumstances when there is not enough tendon to perform an anatomic repair because of tear size, retraction, and/or atrophy. This clinical scenario, massive irreparable rotator cuff tear, has no perfect solution. Many options exist in treating massive irreparable rotator cuff tears: partial tendon repairs, debridement, tuberoplasty, intercalary allograft repairs, tendon transfers, superior capsular reconstruction, hemiarthroplasty, arthrodesis, and reverse total shoulder arthroplasty. BACKGROUND: The benefits of acromioplasty in treating rotator cuff disease have been debated. We systematically reviewed the literature regarding whether acromioplasty with concomitant coracoacromial (CA) release is necessary for the successful treatment of full-thickness rotator cuff tears.
MATERIALS AND METHODS: We identified randomized controlled trials that reported on patients who underwent rotator cuff repair with or without acromioplasty and used descriptive statistics to summarize the findings.
RESULTS: Four studies fulfilled the inclusion criteria. They reported on 354 patients (mea

A modified posterior approach to the scapula was tested on 20 cadavers. The approach also was used in 2 cases with fractures involving the scapular neck and glenoid fossa. The incision is C shaped, with the convexity directed toward the lateral angle of the scapula. The posterior muscle fibers of the deltoid are reflected laterally after detaching them from their origin. The infraspinatus is mobilized without division to expose the posterior surface of the scapular neck and glenoid. Access to the rest of the posterior and the superior surfaces of the glenoid can be achieved by osteotomizing th

BACKGROUND AND IMPORTANCE: Pterygium colli or "webbed neck" constitutes a rare congenital malformation characterized by bilateral cutaneous excess extending from the mastoid region to the acromion. Although of low incidence in the general population, this anomaly can lead to significant functional limitations and considerable psychosocial impact, particularly in children and adolescents.
CASE PRESENTATION: We report the case of an 8-year-old girl with no particular medical history, presenting bilateral cervical webbing of cutaneous appearance giving a webbed neck appearance, present since bir

Fractures of the acromion are a common complication following surgical procedures of the shoulder due to changes in joint biomechanics. To improve understanding of acromial stresses and evaluate surgical procedures, image-based finite element models (FEMs) may be used. Image-based FEMs are dependent on accurate volumetric bone mineral density (vBMD), as this relates to mechanical properties in FEMs. Image reconstruction kernel alters vBMD; however, the effect on FEM output during simulated loading of the acromion has not been reported. The objective of this study was to compare predicted force

BACKGROUND: Reverse total shoulder arthroplasty (RTSA) implants have been developed to treat patients with deficient rotator cuffs. The nature of this procedure's complications and how these complications should be managed continues to evolve. Fractures of the scapula after RTSA have been described, but the incidence and best methods of treatment are unclear.
QUESTIONS/PURPOSES: We therefore (1) determined the incidence and (2) developed a classification system intended to suggest the best choice of treatment.
PATIENTS AND METHODS: We reviewed the records of 400 patients treated with RTSA ov

Giant cell tumor (GCT) is a distinctive lesion characterized by the proliferation of multinucleate giant cells in a stroma of mononuclear cells; it is generally seen in skeletally mature individuals. GCT of bone is usually found in the long bones around the knee or in the distal radius of young adults and is unusual in the flat bones. We report a case of GCT of the acromion of the scapula, with a secondary aneurysmal bone cyst, in a 30-year-old female. Based on our review of the English language medical literature, it appears that the occurrence of a GCT along with a secondary aneurysmal bone

In relation to two rare cases of fracture-separation of the os acromiale, 26 other cases of defective epiphysial fusion were studied. A review of the literature confirmed the rarity of this anomaly of the scapula which predisposes to subacromial impingement and rotator cuff rupture. The diagnosis is radiological. It requires both an antero-posterior and an axillary view which, together with a CT scan, precisely shows the os acromiale. It should be distinguished from a fracture of the acromion to avoid unnecessary immobilisation and post-traumatic effects. Degenerative changes provoke an imping

OBJECTIVE: To compare rotator cuff muscle morphology and relevant shoulder biomechanical characteristics on MRI among patients with intact, partial-thickness, and full-thickness supraspinatus tendons.
MATERIALS AND METHODS: This study included 245 patients aged 45-65 years without tears or substantial abnormalities of the other rotator cuff tendons. Cross-sectional areas of the rotator cuff muscles and supraspinatus fossa were measured. Supraspinatus occupancy ratio, supraspinatus-based muscle ratios, acromiohumeral distance, critical shoulder angle, Goutallier grade, and acromion type were a

PURPOSE: This study compared the clinical and radiologic results of reverse shoulder arthroplasty (RSA) using either the subscapularis-sparing deltopectoral approach (SSDA) or traditional deltopectoral approach (TDA) in cuff tear arthropathy patients.
MATERIALS AND METHOD: We retrospectively evaluated 71 patients who underwent RSA for cuff tear arthropathy between July 2014 and December 2018. Patients were divided into two groups according to the surgical approach: TDA (34 cases) and SSDA (37 cases). The mean patient age was 78.6 years, and the mean (range) follow-up period was 23.5 (12-48) m

Critical shoulder angle (CSA) is the angle between the superior and inferior bone margins of the glenoid and the most lateral border of the acromion and is potentially affected during a rotator cuff tear (RCT). Acromioplasty is generally performed to rectify the anatomy of the acromion during RCT repair surgery. However, limited information is available regarding the changes in the CSA after anterolateral acromioplasty. We hypothesized that CSA can be decreased after anterolateral acromioplasty. Data were retrospectively collected from 712 patients with RCTs and underwent arthroscopic rotator
