BACKGROUND: High-voltage electrical injuries can cause sudden cardiac arrest, most commonly due to ventricular fibrillation, and are associated with high mortality and poor neurological outcomes. Survival with complete neurological recovery following out-of-hospital cardiac arrest remains uncommon, particularly in low-income and resource-limited settings. CASE PRESENTATION: We report the case of a 22-year-old previously healthy male who sustained a high-voltage electrical injury at a construction site and collapsed immediately. On arrival at the emergency department, he was unresponsive, pulse

A 60-year-old female with a past medical history of hypertension presents to the ED with one day of throbbing left knee pain with associated numbness that worsened with ambulation. EKG shows lateral T-wave inversions with no prior for comparison. The patient had bloodwork drawn and a chest x-ray ordered. Her pain was improving with acetaminophen, and during further workup, she went into cardiac arrest. The advanced cardiac life support protocol was initiated, the patient was intubated, and point-of-care ultrasound revealed pericardial effusion. Despite all her efforts, she couldn't regain cons

Extracorporeal membrane oxygenation (ECMO) is providing an increasingly important therapy for patients in severe heart and lung failure. Care of these patients is complex, with changes in circulation that mean standard advanced life support algorithms may not always be applicable. Through collaboration between all UK ECMO centres and eight national societies, we have assessed the current evidence base and, using a modified Delphi process, produced national guidelines on the management of emergencies on ECMO. The guidelines focus on the recognition of cardiac arrest, team prioritisation, and ea

INTRODUCTION: and importance: Acute massive pulmonary thromboembolism is a potentially life-threatening condition requiring urgent management to decrease mortality. Although the standard dose of systemic thrombolysis with alteplase is 100 mg, half the dose of alteplase can be used to break up clots successfully, especially if bleeding is a concern.
CASE PRESENTATION: We report a case of massive pulmonary thromboembolism presenting with cardiopulmonary arrest, successfully managed with advanced cardiac life support, anticoagulants, and low-dose thrombolytics.
CLINICAL DISCUSSION: Management o

A 46-year-old woman demonstrated refractory Kounis syndrome (KS) after induction of anesthesia. Despite conventional management of anaphylaxis and advanced cardiac life support, her cardiovascular function continued to deteriorate until she had a cardiac arrest, and after extracorporeal membrane oxygenation (ECMO) therapy, electrical cardiac activity reappeared. A large number of patients with KS-"allergic angina syndrome"-has been known to recover well with vasodilators; however, this patient showed antibiotics-induced refractory KS during general anesthesia. Severe bronchospasms with desatur

We report the case of successful resuscitation after prolonged cardiac arrest during orthotopic liver transplantation. After reperfusion, the patient developed ventricular tachycardia, complicated by intracardiac clot formation and massive hemorrhage. Transesophageal echocardiography demonstrated stunned and nonfunctioning right and left ventricles, with developing intracardiac clots. Treatment with heparin, massive transfusion and prolonged cardiopulmonary resuscitation ensued for 51 minutes. Serial arterial blood gases demonstrated adequate oxygenation and ventilation during cardiopulmonary

INTRODUCTION: Hypothermia is an uncommon cause of cardiac arrest in the United Kingdom, and more commonly occurs in countries experiencing avalanches and significant winter climates; however, this case demonstrates that the presentation can occur in the United Kingdom. This case adds to a body of evidence that prolonged resuscitation can be successful in patients suffering a cardiac arrest secondary to hypothermia, leading to a good neurological outcome.
CASE PRESENTATION: The patient suffered a witnessed out-of-hospital cardiac arrest following rescue from a free-flowing river, and underwent

Pediatric advanced life support (PALS) teaches skills unique to pediatric resuscitation. The purpose of this study was to assess the effect of PALS training among emergency medical service (EMS) providers in out-of-hospital trauma and medical resuscitations. A physician panel evaluated all EMS run sheets of pediatric traumas and medical resuscitations brought to a tertiary children's hospital/regional trauma center over a 3-year period. In 183 responses, EMS personnel were the sole providers of medical stabilization. Evaluation included the ability to secure an airway, establish vascular acces

OBJECTIVES: To identify early cardiovascular changes in normotensive obstetricians, a high-stress group, using non-invasive haemodynamic monitoring, and to examine the association between burnout and haemodynamic parameters.
DESIGN: Cross-sectional study.
SETTING: A single tertiary hospital in China.
PARTICIPANTS: A total of 120 healthy Han Chinese adults (aged 25-45 years, both sexes) were enrolled using stratified random sampling by age and categorised into three groups: obstetricians, clinical support staff and administrative personnel. Of these, 105 (87.5%) completed the study and enter

At the end of 2005 the new guidelines for the treatment of cardiac arrest were published. The diagnostic criteria of cardiac arrest were simplified and priority is given to thoracic compressions. The ratio of thoracic compressions to insufflations is 30/2. The frequency of thoracic compression is 100/min. In ventricular tachycardia (VT) without pulse or in ventricle fibrillation (VF), defibrillation is attempted with a single external electric shock per cycle. The reanimation cycles are divided in periods of 2 minutes. The two drugs, used to treat VF and VT without pulse, are amiodarone and ad

INTRODUCTION: Defibrillation is effective and the most common treatment for ventricular fibrillation (VF) and pulseless ventricular tachycardia in patients with cardiac arrest.
PATIENT CONCERNS: Recently we experienced 3 cases refractory ventricular fibrillation (RVF) which was successfully terminated with double sequence defibrillation (DSD) in our emergency department, so we'd like to report and discuss it.
DIAGNOSIS: Cardiac arrest.
INTERVENTIONS: A single defibrillation 200J was performed twice for patients with ventricular fibrillation in the initial rhythm of the emergency room. At th

New American Heart Association Guidelines 2010 emphasize the need for high-quality CPR, which can be seen in initiating chest compressions sooner (before 2 ventilations) and with slightly modified compression depth and rate. Fundamental change in CPR sequence is abandoning A-B-C steps for C-A-B (all age groups excluding newly born) to minimise the delay in initiating chest compressions. Dispatchers should help bystanders recognize cardiac arrests and provide instructions on Hands-Only CPR. New guidelines add fifth link to the Adult Chain of Survival - "post-cardiac arrest care" and underline t

Extracorporeal membrane oxygenation (ECMO) use in poisoned patients is increasing, but is rare post cardiac arrest. We report a case of ECMO use with complete recovery in a patient who arrested twice after a cardiotoxicant overdose. A 17-year-old male presented after an unknown overdose. He rapidly became hypotensive and bradycardic and received aggressive supportive care without improvement. He was transferred to our institution and suffered a cardiac arrest shortly after arrival. Six minutes of advanced cardiac life support resulted in return of spontaneous circulation. High-dose insulin, li

Background: Cardiac metastases, though more common than primary cardiac tumors, remain under-recognized due to their often subtle clinical presentation. These tumors can lead to life-threatening complications, and their diagnosis is typically delayed. Objective: This paper is aimed at reviewing two distinct cases of metastatic cardiac tumors, shedding light on diagnostic challenges, clinical presentations, and management approaches. Methods: We present two cases of patients with metastatic melanoma and undifferentiated malignant spindle cell neoplasm, respectively. Diagnostic imaging, includin

BACKGROUND AND OBJECTIVE: Extracorporeal membrane oxygenation (ECMO) has historically been utilized as a temporary life support option for patients with severe cardiac and pulmonary dysfunction. Recent advancements have enabled the safe application of ECMO in a wider variety of patients; we present a review of its use in patients undergoing general thoracic procedures supported by a case series at our institution.
METHODS: We review current literature focusing on ECMO applications in thoracic surgery outside of the traditional use. Additionally, we offer three cases of ECMO utilization to ill

The clinical presentation of pulmonary embolism (PE) and acute coronary syndrome can be similar. We report a case of a patient presenting with antero-septal ST-segment elevation after cardiac arrest, found to have acute-PE-mimicking ST-segment elevation myocardial infarction (STEMI), treated with aspiration thrombectomy and catheter-directed thrombolysis (CDT). A 78-year-old man was admitted with dyspnea, chest pain and tachycardia. During evaluation, cardiac arrest in pulseless electrical activity was documented. Advanced life support was started immediately. ECG post-ROSC revealed ST-segment

UNLABELLED: We present the case of a 15-year-old boy with hypertrophic cardiomyopathy who suffered out-of-hospital cardiac arrest. After failure of conventional defibrillation to terminate refractory ventricular fibrillation, double sequential external defibrillation was used successfully, resulting in return of spontaneous circulation. Ultimately, the patient survived neurologically intact.
BACKGROUND: Hypertrophic cardiomyopathy is the most common monogenic cardiac disorder. Paediatric hypertrophic cardiomyopathy patients have a higher risk of ventricular fibrillation and sudden cardiac dea
