The risk factors for residual juvenile nasopharyngeal angiofibroma and the usual residual sites.

OBJECTIVE: Juvenile nasopharyngeal angiofibroma (JNA) is non-metastasizing but potentially locally destructive tumor of the nasopharynx. It can destroy the skull base and invade into the cerebrum. Surgical management is the primary standard but residual disease is always a risk factor. We aimed to determine the risk factors for residual disease and usual sites for these residual tumors. METHODS: The medical records of 131 patients (mean age 17.6 ± 6.8, range 9-71 years) with histologically proven JNA were retrospectively analyzed. The surgeries were all nasal endoscopic approaches, with or wi

Facial angiofibromas treated by rapamycin 0.05% ointment and a combined laser therapy.

BACKGROUND: Tuberous sclerosis is an autosomal dominant genodermatosis characterized by nonmalignant hamartomas in multiple organs. Facial angiofibromas are most commonly located on the face and have the potential to cause disfigurement. Facial disfigurement negatively affects the quality of life of patients and their families, often leading to negative psychosocial outcomes. Nowadays there are no treatment guidelines for facial angiofibromas but due to the progressive nature of facial angiofibromas a safe technique offering good results is needed. OBJECTIVE AND RESULTS: We report the case of

Extra Nasopharyngeal Angiofibroma Simulating a Mucocele: A New Location for the Rare Entity.

Angiofibromas are rare, benign, but locally aggressive vascular tumours which account for 0.05-0.5% of all head and neck neoplasms. The common variant of angiofibromas of head and neck region is Juvenile Nasopharyngeal Angiofibroma (JNAF) which is well published in the literature. Incidence of Extranasopharygeal Angiofibromas (ENAF) is relatively rare and most of the published literature favours maxillary sinus as the most common site. We report a rare clinical entity of ENAF involving the upper lip which is first of its kind in the light of literature review owing to its anatomical location.

Facial angiofibromas of tuberous sclerosis treated with topical sirolimus in an Indian patient.

Facial angiofibromas are the most visible and unsightly of all the cutaneous manifestations of tuberous sclerosis (TSC). A 17-year-old female, a known case of TSC, presented for the treatment of cosmetically disfiguring facial angiofibromas. She was started on twice daily application of 0.1% sirolimus ointment prepared from crushed tablets of sirolimus compounded in white soft paraffin. After 3 months of use, there was visible decrease in the erythema and the size of the angiofibromas. In an attempt to accelerate the response, the concentration was further increased to 1% sirolimus which was u

Juvenile Nasopharyngeal Angiofibroma: Case report with review on role of imaging in diagnosis.

Juvenile nasopharyngeal angiofibroma is a locally aggressive benign vascular neoplasm, composed of vasogenic and myofibroblastic elements, accounts for 0.05-0.5% of all the head and neck neoplasms. There are very few case reports of nasopharyngeal angiofibroma involving the oral cavity; we report a case involving both the maxilla and mandible in a 17-year-old patient who reported with a large firm swelling on right side of face with recurrent epistaxis and headache. Magnetic resonance angiography revealed a large lobulated enhancing soft tissue mass, which was hypointense on T1-weighted image

Extensive facial angiofibromas in tuberous sclerosis treated with carbon dioxide laserbrasion.

Facial angiofibromas in systemic sclerosis cause significant cosmetic disfigurement and emotional distress. The treatment of these lesions have been a challenge because of the multiplicity of the lesions and the presence of the lesions at anatomically difficult sites. Four female patients between 12-22 years in age with extensive angiofibromas on face, nasolabial folds and chin for 2-10 years were treated with carbon dioxide (CO2) laser. The lesions in all patients were vaporized with the indigenously developed CO2 laser. All the patients were evaluated at 1 and 2 weeks and then every month fo

Differential Gene Expression and Pathway Analysis in Juvenile Nasopharyngeal Angiofibroma Using RNA Sequencing.

Juvenile nasopharyngeal angiofibroma (JNA) is a highly vascularized and locally aggressive tumor that typically presents in adolescent males. The molecular biology of this tumor remains understudied. We sought to identify differentially expressed genes in the JNA transcriptome through messenger RNA sequencing of primary fibroblasts from 2 tumor explants and tonsil tissue from tumor-free subjects. In total, 1088 significant, differentially expressed genes were identified with 749 upregulated and 339 downregulated. Pathway analysis identified a number of activated signaling pathways, most notabl

Endoscopic surgery for juvenile angiofibroma: a critical review of indications after 46 cases.

BACKGROUND: At present, transnasal endoscopic surgery is considered a viable option in the management of small-intermediate size juvenile angiofibromas (JAs). The authors critically review their 14-year experience in the management of this lesion to refine selection criteria for an endoscopic approach. METHODS: From January 1994 to May 2008, 46 patients were treated by a pure endoscopic resection after vascular embolization (87%). The lesions were classified according to Andrews (Andrews JC, et al., The surgical management of extensive nasopharyngeal angiofibromas with the infratemporal fossa

[Retrospective clinicopathological study of juvenile nasopharyngeal angiofibroma in a 20-year case load].

From the files of our institution 7 cases originally diagnosed as juvenile nasopharyngeal angiofibroma have been reviewed. Only 3 cases proved to be juvenile nasopharyngeal angiofibroma on revision, in one case inflammatory polyp, in two cases pyogenic granuloma and in one case richly vascular granulation tissue was the correct diagnosis. The clinical and histopathological aspects of the differential diagnosis of juvenile nasopharyngeal angiofibroma are discussed. All patients with true juvenile nasopharyngeal angiofibroma were males. The diagnostic usefulness of the recognition of morphologic

Managing Different Spectrums of Tuberous Sclerosis Facial Angiofibroma: A Report of Two Cases.

Tuberous sclerosis is one of the rarest genetically linked disorders that can affect a multitude of body systems in various forms. Patients with facial angiofibroma may face issues arising from the various modalities and approaches that can be applied. Moreover, surgeons also face challenges in preparing patients for specific interventions. Here, we are reporting the two spectrums of this condition that may present, along with how we managed patients in our center. The first case is a severe form of tuberous sclerosis involving the skin as well as neurological manifestation, while the second c

Mediastinal Primary Synovial Sarcoma With Postoperative Pericardial Recurrence in a Previously Treated Angiofibroma.

Synovial sarcoma is a malignant soft tissue tumor that often occurs near the limb joints. Here, we report a case of a patient with a synovial sarcoma that occurred in the mediastinum. The initial pathological diagnosis was suspected angiofibroma after surgical resection. After surgery, the tumor recurred in the pericardium and caused cardiac tamponade. Pericardial fenestration was performed and the patient was diagnosed with synovial sarcoma. The final diagnosis was the postoperative pericardial recurrence of the mediastinal synovial sarcoma. It is important to consider follow-up on the basis
