[Communication disorders after decline in sub-cortical aphasia: the role of fronto-sub-cortical disconnection?].

INTRODUCTION: Pragmatic analysis of Language difficulties examines the pertinence and adaptation of the spoken Language which can be disturbed even when phonologic, syntactic and semantic functions, taken separately, appear normal. Disorders of pragmatic Language function remain poorly explored and the few published papers in adult neuropsychology have been devoted to patients with traumatic frontal lesions. METHODS: In the present study we report the case of a patient admitted for right hemiplegia with "dissident" aphasia related to a capsulo-lenticular hematoma. Aphasic disorders progressiv

When does a deep dyslexic make a semantic error? The roles of age-of-acquisition, concreteness, and frequency.

Semantic reading errors are the central and defining feature of deep dyslexia. This study compared the words the deep dyslexic patient LW read correctly with those she omitted and those to which she produced semantic errors in terms of their concreteness, age-of-acquisition, frequency, and length. Semantic errors were made to less concrete, later-acquired, and shorter words than were read correctly; there was no reliable effect of word frequency. More importantly, the actual semantic errors produced were later-acquired than the stimulus words, but they were not more concrete or reliably more f

Treatment and generalization of complex sentence production in agrammatism.

The present study applies single-subject experimental design to examine (a) the acquisition and generalization of complex sentence production in agrammatism using Linguistic Specific Treatment (LST) and (b) the utility of syntactic theory in guiding hypotheses of treatment effects. LST trains construction and production of complex sentence structures. Four sentence types were selected for study: object clefts and object-extracted matrix and embedded questions (which are noncanonical with wh-movement), and embedded actives (which are canonical with no overt movement). All sentences contain over

[Bulbar-onset amyotrophic lateral sclerosis (ALS) with isolated agraphia].

We reported the two cases with bulbar-onset ALS showing isolated agraphia without overt dementia and aphasia. Patient 1 was a 69-year-old man and patient 2 was an 81-year-old woman, and both were right-handed. Each patient developed dysarthria as an initial symptom followed by dysphagia, and neurological examinations showed atrophy and fasciculation of the tongue with upper and lower motor-neuron involvement of the extremities. These characteristic features with electromyographic evidence including widespread acute and chronic denervation led to a diagnosis of bulbar-onset ALS. Around 1 year a

[Multiple meningeal and cerebral involvement revealing multifocal tuberculosis in an immunocompetent patient].

Tuberculosis is a public health problem in Morocco. Central nervous system involvement is nevertheless rare, occurring in the context of multifocal or miliary tuberculosis. However, it may be a mode of revelation even in an immunocompetent subject. We report the case of a 30-year old man with language disorder accompanied by significant impairment of general condition. Clinical examination showed Broca's motor aphasia, right-sided pyramidal syndrome and latero-cervical adenopathies. HIV serologic test was negative. Brain MRI showed lesions associating multiple intracranial tuberculomas and men

In vivo signatures of nonfluent/agrammatic primary progressive aphasia caused by FTLD pathology.

OBJECTIVE: To identify early cognitive and neuroimaging features of sporadic nonfluent/agrammatic variant of primary progressive aphasia (nfvPPA) caused by frontotemporal lobar degeneration (FTLD) subtypes. METHODS: We prospectively collected clinical, neuroimaging, and neuropathologic data in 11 patients with sporadic nfvPPA with FTLD-tau (nfvPPA-tau, n = 9) or FTLD-transactive response DNA binding protein pathology of 43 kD type A (nfvPPA-TDP, n = 2). We analyzed patterns of cognitive and gray matter (GM) and white matter (WM) atrophy at presentation in the whole group and in each pathologi

Progressive non-fluent aphasia is associated with hypometabolism centred on the left anterior insula.

Progressive non-fluent aphasia (PNFA) is a syndrome in which patients lose the ability to communicate fluently in the context of relative preservation of single word comprehension and non-linguistic cognitive abilities. Neuroimaging in case studies with PNFA has failed to identify a consistent neural substrate for the language disorder. In this study of a group of patients (n=10) whose presenting complaint was progressive dysfluency, resting cerebral metabolism was measured using [18F]fluorodeoxyglucose-PET and analysed with the technique of statistical parametric mapping (SPM). Regional atrop

Slow event-related brain activity of aphasic patients and controls in word comprehension and rhyming tasks.

Slow event-related potentials (ERP) were examined in healthy and aphasic subjects in two-stimulus designs comprising a word comprehension and a rhyming task. Aphasics, though selected to perform above chance level, made significantly more errors and responded more slowly than controls, although canonical correlations did not indicate a statistical relationship between performance measures and ERP amplitudes. A discriminant analysis of ERP amplitudes distinguished the groups for the slow wave (SW; 0.5-1.0 s post-S1 onset) in the word comprehension, for the SW and the initial contingent negative

Mortality and Pulmonary Complications of Post-stroke Dysphagia: A Casuistic Review of an Acute Stroke Unit.

INTRODUCTION: Dysphagia is a common post-stroke neurological disorder. Early screening for dysphagia can identify patients at risk of aspiration, thereby reducing the occurrence of pulmonary complications, morbidity, and mortality in this population. OBJECTIVES: This study aims to evaluate the impact of an intervention in a stroke unit, following a retrospective study carried out in the same unit in 2020, which investigated the association between dysphagia and acute cerebrovascular disease and analyzed the prevalence of readmissions due to respiratory tract infections (RTI) and mortality. Postictal mixed transcortical aphasia.

Postictal aphasia has been described in left temporal lobe seizures. It may be of fluent, non-fluent or global type. We present here a patient who displayed signs of mixed transcortical aphasia (MTCA). The patient was a 67 year old man who underwent excision of a left frontal parasagittal meningioma in 1987. Since then he has been treated with phenytoin for generalized tonic-clonic seizures (GTCS). He was admitted in status epilepticus. On awakening, the patient was non-fluent with palilalia and echolalia. His repetition was relatively preserved but all the other language functions were impair

Transient mutism following posterior fossa surgery in children.

We report 3 patients, ages 5, 9 and 13 years, with mutism following posterior fossa surgery (PFS). All presented with headache of 10-180 days duration, excellent premorbid learning performance and paucity of neurologic signs. Radiographic studies demonstrated large posterior fossa tumor occupying the fourth ventricle, and hydrocephalus, leading to gross total resection of the tumor. Within 24-48 hr, all patients were mute. None had problems with swallowing and coughing. All were able to nod "yes or no," follow commands, point to body parts, and make their wants known by gestures. Jaw, gag, sno

WAIS performance IQ in aphasia as a function of auditory comprehension and constructional apraxia.

This study investigated differences in nonverbal intelligence between diagnostic subgroups of aphasics using the Performance Tests of the Wechsler Adult Intelligence Scale (WAIS). Subjects were 98 right-handed hospitalized males with unilateral left-hemisphere damage who had unambiguous clinical diagnoses of Broca's, Anomic, Conduction, Mixed Non-Fluent, Wernicke's, or Global aphasia. Group differences were examined using the WAIS Performance IQ (PIQ), and subscores for spatial organization (Block Design and Object Assembly) and for verbalizability (Picture Completion and Picture Arrangement).

Identification of Phonology-Related Genes and Functional Characterization of Broca's and Wernicke's Regions in Language and Learning Disorders.

Impaired phonological processing is a leading symptom of multifactorial language and learning disorders suggesting a common biological basis. Here we evaluated studies of dyslexia, dyscalculia, specific language impairment (SLI), and the logopenic variant of primary progressive aphasia (lvPPA) seeking for shared risk genes in Broca's and Wernicke's regions, being key for phonological processing within the complex language network. The identified "phonology-related genes" from literature were functionally characterized using Atlas-based expression mapping (JuGEx) and gene set enrichment. Out of

[A peculiar speech disorder due to bomb injury of the brain].

Oslo was bombed by British bombers 6 September 1941, and a 28-year-old woman was hit on the head by a bomb fragment. She was seriously wounded with a large defect in the cranium frontally on the left side, and it was believed that she was not going to survive. She was unconscious for 3-4 days, and when she woke up she had right-sided hemiplegia and complete aphasia. She gradually recovered, and two months later she was discharged from hospital. Remarkably, the brain damage had altered her melody of language, and she spoke with a German-like accent. This led to problems for her during the war:

The influence of psycholinguistic variables on articulatory errors in naming in progressive motor speech degeneration.

We describe an analysis of speech errors on a confrontation naming task in a man with progressive speech degeneration of 10-year duration from Pick's disease. C.S. had a progressive non-fluent aphasia together with a motor speech impairment and early assessment indicated some naming impairments. There was also an absence of significant phonological or semantic impairment. In order to examine naming difficulties and the factors that influence his speech production errors, we selected 210 words varying in frequency, age of acquisition (AoA), imageability, phonemic length and syllable length and

Adult unilateral moyamoya disease with familial occurrence in two definite cases: a case report and review of the literature.

We documented an interesting case of adult "unilateral (probable)" moyamoya disease displaying familial occurrence in two "definite" cases. A 55-year-old female presented with motor aphasia, involuntary movement of the right hand and right homonymous hemianopia due to cerebral infarction. Cerebral angiography revealed typical angiographic findings on the left side and normal findings on the right side; consequently, the patient was diagnosed with probable moyamoya disease. Previously, her mother and nephew had been diagnosed with definite moyamoya disease with bilateral involvement. The patien

Lexical representation and processing of morphologically complex words: evidence from the reading performance of an Italian agrammatic patient.

The study of patients with acquired language disorders has provided crucial evidence for contemporary theories on mental lexical representation. This is particularly true for the representation of morphologically complex words. In this paper we analyzed the performance of a patient (M.B.) affected by agrammatism and dyslexia. M.B. was required to read aloud simple and morphologically complex words. The patient's pattern of errors was interpreted as the result of a predominant use of the lexical routine (phonological dyslexia). Three reading tasks were developed which allowed us to test M.B.'s

Maintenance of oral production in agrammatic aphasia: verb tense morphology training.

We used a single-subject research design to examine long-term maintenance of treatment gains in a severely aphasic patient. We used a well-defined therapy protocol [i.e., Computerized Visual Communication System (C-VIC) training] to target production of tense morphology and an assessment specifically designed to measure improvements in the targeted behavior. We first trained Subject #1, a 65-year-old man with a severe nonfluent aphasia, to produce simple subject-verb-object sentences, then tense-marked sentences. Remarkable improvement was shown and was maintained five months after training wa
