An analysis of changes in mobility and safety of older drivers associated with a specific older driver on-road licensing test: a population study.

BACKGROUND: From 1999 to the end of 2006, older drivers aged 80 plus in New Zealand were subject to an older driver licence test as a condition of licensing. The impact of this test has not yet been studied in terms of the safety and mobility of older people. METHODS: Three main data sources were analysed to provide evidence of changes in older driver travel mode choice, licensing rates and injuries: New Zealand Travel Survey data, licensing data and police-reported crash data. RESULTS: General trends towards higher levels of motorised mobility were apparent for this group over the 20 years

[Traffic ophthalmology. Assessment of ability to drive in road traffic].

Assessment of an individual's ability to drive in road traffic is an important part of the ophthalmologist's work. It fulfils the ophthalmologist's duty to care for the patient and increases general road traffic safety. This assessment requires testing of visual acuity, visual field, mesopic vision and glare, motility, binocular vision and colour vision. Also the optical media and the fundus have to be examined. If an optical aid is necessary, it should be suitable for use in road traffic. In some cases restrictions are necessary, which should be noted in the driver's licence. Testing of visua

Driving and off-road impairments underlying failure on road testing in Parkinson's disease.

Parkinson's disease (PD) affects driving ability. We aimed to determine the most critical impairments in specific road skills and in clinical characteristics leading to failure on a road test in PD. In this cross-sectional study, certified driving assessment experts evaluated specific driving skills in 104 active, licensed drivers with PD using a standardized, on-road checklist and issued a global decision of pass/fail. Participants also completed an off-road evaluation assessing demographic features, disease characteristics, motor function, vision, and cognition. The most important driving sk

The prevalence and nature of stopped on-the-road driving tests and the relationship with objective performance impairment.

INTRODUCTION AND OBJECTIVES: The on-the-road driving test in normal traffic is applied to examine the impact of drugs on driving performance. Although participants are accompanied by a licensed driving instructor, under Dutch law, the driver is primarily responsible for safe driving and is not permitted to continue driving when it is judged that the drug compromises safety. This review examined the prevalence and nature of stopped driving tests, and the relationship with Standard Deviation of Lateral Position (SDLP), i.e. the "weaving of the car". MATERIALS AND METHODS: A literature search wa

Survey of health and licensing policies for taxi drivers in Singapore, Hong Kong and Australia: a perspective from evidence on older drivers.

INTRODUCTION: With an ageing population, policy makers need to balance active ageing needs with older driver safety. In 2009, a survey of licensing policies for taxi drivers in Singapore, Hong Kong and Australia was undertaken for an evidence-based review of policies. METHODS: Licensing requirements collected using semi-structured questionnaires were compared descriptively and with evidence from licensing policies on older drivers. RESULTS: All the regions used medical certifications with vision screening. The frequency of medical certification varied according to the renewal cycle and age.

Prevention of traffic accidents: the assessment of perceptual-motor alterations before obtaining a driving license. A longitudinal study of the first years of driving.

UNLABELLED: A longitudinal study was designed with two objectives: first, to provide a wide cognitive, personality and social description of new drivers before they started to drive cars. Second, to examine the relationship between cognitive and other characteristics drivers had before obtaining their driving license and the number and type of accidents they were involved in during the first years as drivers. RESEARCH DESIGN: The longitudinal study started in 1997 and ended in 2002. The first assessment was made up of 241 individuals at the time they enrolled on the driving course. The follow

The relationship between performance on the standardised field sobriety tests, driving performance and the level of Delta9-tetrahydrocannabinol (THC) in blood.

The consumption of Delta9-tetrahydrocannabinol (THC) as cannabis has been shown to result in impaired and culpable driving. Testing drivers for the presence of THC in blood is problematic as THC and its metabolites may remain in the blood for several days following its consumption, even though the drug may no longer have an influence on driving performance. In the present study, the aim was to assess whether performance on the standardised field sobriety tests (SFSTs) provides a sensitive measure of impaired driving behaviour following the consumption of THC. In a repeated measures design, 40

[Revoking the driver's license, assessment of driver competence, re-education and therapy in alcohol offenses: approaches to a necessary reorientation].

After representing figures and facts about drinking behaviour and drinking-driving offences of the 45 mio. drivers in Germany counter-measures for DWI-offenders are dealt with. The proceedings practised so far are critically analysed and new approaches are discussed. Primarily reviewed are special preventive measures and here particularly the selection procedure usually applied in Germany, in which on demand of the driver licensing authorities the question is to be answered by expert opinions whether there is to be reckoned with driving under the influence in die future, too. It is pointed out

Aiding medical professionals in fitness-to-drive screenings for elderly drivers: development of an office-based screening tool.

UNLABELLED: ABSTRACTBackground:Elderly drivers are an increasing group in society. Previous research has found that functional and cognitive abilities are more important for driving abilities than biological age. In an attempt to conserve independent mobility for elderly drivers, many researchers have focused on elderly drivers diagnosed with cognitive decline (mild cognitive impairment or mild Dementia). This study is the first to focus on elderly drivers with cognitive complaints or suspected of diminished fitness to drive by an (in)formal caregiver as an at-risk group. METHODS: The main ob

Using social judgment theory to study occupational therapists' use of information when making driver licensing recommendations for older and functionally impaired adults.

OBJECTIVE: This study sought to identify the informational cues that driver-assessor-trained occupational therapists (DATOTs) consider when making driver licensing recommendations for older and functionally impaired clients. Relatively little research supports these complex decisions. METHOD: A survey using a social judgment theory framework was mailed to all practicing registered DATOTs in Victoria, Australia. RESULTS: Cue mean rank order, as calculated across the 56 responses (return rate, 78%), revealed that the four most important cues were driving instructor interventions, driver behavi

Vision testing and the elderly driver: is there a problem meriting policy change?

BACKGROUND: The number of elderly drivers is increasing. The primary purpose of public policy requiring vision screening for driver's license renewal is to identify, and, when necessary, restrict drivers with functional vision impairments. Because of age-related ocular conditions, elderly drivers, as a group, have a higher incidence and prevalence of functionally impaired vision. To date, there is no empirical evidence of a significant predictive relationship between changes in vision function and automobile crashes. METHODS: Most states require vision screening for driver's license renewal,

Inter-relationships between British drivers' visual abilities, age and road accident histories.

Statistical analysis is described of data on 1000 British drivers' accident histories and their performance on a vision screening instrument. Significant positive associations are evident between various accident variables and tests of hyperphoria, right eye visual acuity, left eye acuity and binocular acuity. The clustering of associations is strongest for older drivers. The significance of hyperphoria has received little attention previously. The age-related associations with accidents parallel the decline with age of other visual abilities. Statutory re-testing of drivers' vision at about t

[Ophthalmological experiences with automobile drivers with inadequate vision].

A total of 369 ophthalmological practices were asked to report the number of patients seen within one test week who drove their cars but did not satisfy minimum legal requirements for diurnal visual acuity applicable for applicants for a West German Class 3 driver's license. There were 471 such patients. From this number it can be calculated that approximately 570 000 individuals (+/- 9%) drive cars in the Federal Republic of Germany with inadequate visual acuity. An analysis of the questionnaire produced the following results: Fifty percent of these patients are less than 62 years of age. The
