A case of Pasteurella multocida bacteremia in a previously healthy hospital employee is presented. The patient had sustained a scratch from his dog four days prior to being seen in the emergency department with adequate healing and no evidence of localized infection. He presented with an acute febrile illness, and was discharged from the emergency department with a diagnosis of viral syndrome. He was asked to return to the hospital the next day when a bacteriology report of gram negative rods in both aerobic and anaerobic blood culture bottles was received in the emergency department. Pasteure

Awareness of the role of anaerobic bacteria in neonatal bacteremia has increased in recent years. The incidence of recovery of anaerobes in neonatal bacteremia varies, according to different studies, between 1.8% and 12.5%. Of the 178 cases reported in the literature, 73 were due to Bacteroides species (69 were the Bacteroides fragilis group), 57 Clostridium species (mostly Clostridium perfringens), 35 Peptostreptococci, 5 Propionibacterium acnes, 3 Veillonella species, 3 Fusobacterium species, and 2 Eubacterium species. Predisposing factors were perinatal maternal complications (especially pr

The blood culture results of all samples obtained from newborns at Grady Memorial Hospital, Atlanta, Ga., during a 6-month period were analyzed to determine the time required for a blood culture to become positive, the time at which a culture could safely be considered negative, and the spectrum of isolated organisms. During the study period, 1,248 samples were submitted from all nurseries and processed by an automated detection instrument (BACTEC NR660). Of the 1,248 samples, 98 (7.8%) were positive by the end of a 7-day processing period; 29 of the 98 were classified as definite bacterial pa

During a 3-month period 252 blood cultures were obtained on adult patients seen in our emergency room with fever and chills suggesting bacteremia. 62/252 blood cultures were positive by standard aerobic/anaerobic blood culture techniques. Buffy coat smears were positive in 76% of patients (47/62) when stained with the acridine orange technique and in 52% (32/62) when stained by the Gram method. Acridine orange was thus superior to the Gram stain for the detection of bacteremia (p less than 0.05). Gram-negative organisms were present in 70% of the buffy coat preparation stain by the acridine or

One hundred consecutive patients with blood cultures positive for microbial growth were prospectively surveyed for the presence of hepatic abnormalities and clinical evidence of infection. Complete data for 82 patients were available for analysis. Fifty-four percent had elevated bilirubin levels, and 34% had total bilirubin values of greater than or equal to 2.0 mg/dl. The levels of total bilirubin were disproportionately elevated compared with those of aspartate aminotransferase, alkaline phosphatase, and cholesterol. Nine of the 23 patients with elevated bilirubin levels had an increase in s

A total of 570 patients, aged less than 24 months, who were seen in the emergency room at Texas Children's Hospital for evaluation of acute febrile illness had a WBC count, differential count, and a blood culture obtained. Bacteremia occurred in 7.7% (n = 44) (22 Haemophilus influenzae; 17 Streptococcus pneumoniae; three Escherichia coli; one group B Streptococcus; one Staphylococcus aureus). The sensitivity, specificity, and positive predictive value of morphologic changes (vacuolization and toxic granulation) of polymorphonuclear neutrophils (segmented neutrophils plus band cells), WBC count

Four children between 5 and 12 years of age had pneumonia as a secondary complication of meningococcal bacteremia and/or meningitis. The diagnosis of meningococcal disease was established by culture of blood and/or cerebrospinal fluid. Fine inspiratory rales were found in one patient. In the other three cases pulmonary infiltrates were evidenced on chest roentgenogram. All patients recovered with penicillin G sodium therapy.

Both clinical and laboratory studies have demonstrated an increased risk of fatal sepsis after splenectomy. The importance of the spleen in clearing a bacterial challenge from pneumococci or other encapsulated organisms is now well accepted. The role of the spleen in handling a bacteremia due to nonencapsulated (Gram negative) organisms is not well established. Rats were subjected to either sham operation or splenectomy. Two weeks after surgery, all animals received 10(6) E. coli by intraperitoneal injection. Tail vein blood samples for quantitative culture were obtained at multiple time inter

Transient, usually asymptomatic bacteremia occurs in a wide variety of procedures and manipulations, particularly those associated with mucous membrane trauma. It may also occur with such daily functions as tooth brushing and bowel movements. These brief bacteremias are especially common in tooth extraction and other dental procedures. Although numerical risk is uncertain, these bacteremias can occasionally give rise to infective endocarditis in the susceptible patient. While no proof exists that antibiotics given prior to procedures causing bacteremia prevent endocarditis in humans, experimen

Aeromonas hydrophila (A. hydrophila)is a facultative anaerobic, Gram-negative, water-dwelling bacterium that can cause a wide range of infections, including bacteremia, especially in immunocompromised patients. This report presents the case of a 68-year-old female patient with a history of renal transplant and chronic immunosuppression, who developed bacteremia caused by A. hydrophila after being exposed to raw crab. Initial presentation included an acute kidney injury and gastrointestinal symptoms, but she had an abnormal clinical course, as she did not have a fever or leukocytosis. Empiric p

We describe an uncommon case of a six-week-old male infant with Pasteurella multocida bacteremia. He presented to a tertiary care pediatric hospital emergency department (ED) in the American southeast (Arkansas, USA) with a new onset of fever and decreased oral (PO) intake. On exam, he was irritable, febrile, and tachycardic with no significant findings. Infectious workup was initially remarkable for an elevated procalcitonin of 7.25 ng/mL (reference range: 0.00-0.10 ng/mL) and a low white blood cell count of 3.2 K/cmm (reference range: 5.0-19.5 K/cmm). He was admitted and treated empirically

Rhodococcus hoagii is a gram positive actinomycete found in horses and cattle. Humans can be infected by ingestion or inhalation through contaminated food or soil. The organism usually infects immunosuppressed hosts with pneumonia being the common presentation. We present a case of an 89 years old, apparently immunocompetent host presenting with fever, encephalopathy and arthritis who grew Rhodococcus hoagii in blood and synovial fluid, The patient responded well to a combination of vancomycin, azithromycin and imipenem-cilastatin. Our case demonstrates that extra-pulmonary manifestations such

Brevibacillus species are environmental organisms that are rarely implicated as human pathogens. We present the case of postsurgical Brevibacillus brevis bacterial meningitis and an associated bacteremia after debulking surgery for a newly diagnosed pilocytic astrocytoma in a 19-year-old woman. The patient experienced clinical cure with a 4-week course of vancomycin, but her postinfectious course was complicated by the development of a pseudomeningocele that required surgical repair. To our knowledge, this is the first described case of a central nervous system infection caused by Brevibacillu

Of the non-Helicobacter pylori Helicobacter (NHPH) species, Helicobacter cinaedi is an emerging cause of infection in humans. Here we report a novel clinical presentation of H. cinaedi infection: a case of fever in a returning traveler. A 31 year old previously fit and well male presented with onset of fever 24 h after returning from travel in Singapore and Indonesia. Associated symptoms consisted of sore throat, mild shortness of breath, generalized myalgia and arthralgia, headache, and four episodes of loose stools. The patient recovered spontaneously without treatment and was discharged. Af

OBJECTIVE: The aim of this study was to determine the predominant bacterial species causing bacteremia among febrile cancer patients, and their antibacterial resistance profiles at the Uganda Cancer Institute.
RESULTS: We enrolled in-patients with a documented fever (≥ 37.5 °C). Bacteria from positive blood cultures were identified using standard methods biochemically. Antibacterial susceptibility testing was performed with the Kirby-Bauer disc diffusion method. From a total of 170 febrile episodes, positive blood cultures were obtained from 24 (14.1%). A positive culture was more likely to b

Of the 200 Clostridium spp. known to exist, approximately 30 have been associated with human disease. Commonly found in soil, marine sediment and mammalian intestinal tracts, these gram-positive bacilli are known to cause infections ranging from cellulitis to septicemia. Isolates that are identified by clinical microbiology laboratories include Clostridium perfrigens species in 20-40% of cases. However, when Clostridium sporogenes is identified, is rarely considered to be pathogenic. We present a case of Clostridium sporogenes bacteremia secondary to lower limb cellulitis and osteomyelitis in
