Human papillomavirus genotypes in women with invasive cervical cancer with and without human immunodeficiency virus infection in Botswana.

Cervical cancer remains a significant cause of morbidity and mortality in women worldwide and is the leading cause of cancer-related death in Botswana. It is well established that women with HIV have a higher risk of persistent HPV infection leading to cervical cancer. We assessed HPV prevalence and genotype distribution in 126 tissue specimens from confirmed invasive cervical cancer cases using Abbott real-time PCR assay. Overall, 88 (69.8%) women were HIV-infected. Fifty-seven (64.8%) of the HIV-infected women had a baseline CD4+ count ≥350 cells/μl, and 82 (93.2%) were on antiretroviral the

Reassuring Birth Outcomes With Tenofovir/Emtricitabine/Efavirenz Used for Prevention of Mother-to-Child Transmission of HIV in Botswana.

BACKGROUND: Before introduction of tenofovir/emtricitabine/efavirenz (TDF/FTC/EFV), 3-drug antiretroviral therapy (ART) was associated with increased adverse birth outcomes when used for prevention of mother-to-child HIV transmission (PMTCT) in Botswana. METHODS: We extracted obstetric records from all women at the 2 largest maternities in Botswana from 2009-2011 when Botswana National Guidelines recommended zidovudine (ZDV) from 28 weeks gestational age (GA) for CD4 ≥350 and ART for CD4 <350, and again in 2013-2014 after implementation of TDF/FTC/EFV for prevention of mother-to-child HIV tra

A root-cause analysis of maternal deaths in Botswana: towards developing a culture of patient safety and quality improvement.

BACKGROUND: In 2007, 95% of women in Botswana delivered in health facilities with 73% attending at least 4 antenatal care visits. HIV-prevalence in pregnant women was 28.7%. The maternal mortality ratio in 2010 was 163 deaths per 100,000 live births versus the government target of 130 for that year, indicating that the Millennium Development Goal 5 was unlikely to be met. A root-cause analysis was carried out with the aim of determining the underlying causes of maternal deaths reported in 2010, to categorise contributory factors and to prioritise appropriate interventions based on the identifi

Male circumcision, attitudes to HIV prevention and HIV status: a cross-sectional study in Botswana, Namibia and Swaziland.

In efficacy trials male circumcision (MC) protected men against HIV infection. Planners need information relevant to MC programmes in practice. In 2008, we interviewed 2915 men and 4549 women aged 15-29 years in representative cluster samples in Botswana, Namibia and Swaziland, asking about socio-economic characteristics, knowledge and attitudes about HIV and MC and MC history. We tested finger prick blood samples for HIV. We calculated weighted frequencies of MC knowledge and attitudes, and MC history and HIV status. Multivariate analysis examined associations between MC and other variables a

The use of HAART is associated with decreased risk of death during initial treatment of cryptococcal meningitis in adults in Botswana.

OBJECTIVE: The objective of this study was to evaluate outcomes among adults with a first episode of cryptococcal meningitis (CM), comparing those on highly active antiretroviral therapy (HAART) with those not on HAART. METHODS: We conducted a prospective cohort study among HIV-infected adults (aged 18 years and older) with a first episode of CM at the Princess Marina Hospital, in Gaborone, Botswana. The proportions surviving to discharge were compared. Logistic regression was used to evaluate the relationship between HAART use and risk of death in the hospital, adjusting for potential confou

The impact of traditional management on seasonal internal parasite burdens and productivity of indigenous Tswana goats in southern Botswana.

Data collected monthly over a period of two years were live weight, packed cell volume (PCV), nematode faecal egg counts (FECs) and coccidial oocyst counts from faecal analyses for 100 mixed age (3-7 years) indigenous Tswana does. The aims of this experiment were to determine seasonal FECs and coccidial oocysts in these goats and quantify the relationships of these burdens to liveweight and PCV. FECs significantly (P < 0.05) varied with season, with the warmer seasons viz spring, summer and autumn having higher log (x + 1) parasite burdens than the cooler winter, while seasonal trends for cocc

Stewardship configuration and medicines governance under fiscal stress: lessons from Botswana's 2025 health emergency.

BACKGROUND: Botswana's health system has been characterized in terms of its handling of HIV/AIDS and the comparability of public-sector delivery. These characterizations have pretty good bedrock. The 2025 declaration of an official public health emergency, after disruptions of the supply of almost all essential medicines, threw a spanner in the works. Public discourse changed in response. The conversation now became primarily about budgetary and supply chain procurement challenges. While these were concerns, there were several other issues, the most troubling being the absence of a systemic re

Cardiovascular risk factors and cardiac dysfunction in people with HIV and breast cancer: an observational cohort study in Botswana.

BACKGROUND: HIV, cancer, and their respective treatments are independently associated with cardiovascular risk, but limited data exist on the intersection of these conditions. The purpose of this study was to gain insights into the cardiovascular risk factor burden and cardiac function in people with HIV (PWH) treated for breast cancer. METHODS: In a cohort of PWH and breast cancer treated with anthracyclines and/or trastuzumab (2017-2022) in Botswana, we assessed pre-treatment (baseline) left ventricular ejection fraction (LVEF), and prospectively obtained an echocardiogram at least one year

Neonatal admission hypothermia and early postnatal growth in a tertiary neonatal unit in Botswana: a prospective observational study.

INTRODUCTION: Neonatal hypothermia is independently associated with higher morbidity and mortality and remains under-recognized and inconsistently managed in many low-resource neonatal units. Although thermal-care interventions are well established, the relationship between admission temperature and early postnatal growth has been insufficiently described, particularly in sub-Saharan African tertiary neonatal units. METHODS: We conducted a secondary retrospective analysis of a prospective observational study of neonates admitted to a tertiary public neonatal unit in Gaborone, Botswana, from 1

Perceptions of young people living with HIV and caregivers in Botswana on analytical treatment interruption studies: a qualitative approach.

BACKGROUND: Current research suggests that post-treatment HIV control may be possible for some people following antiretroviral treatment (ART) interruption. Although closely monitored analytic treatment interruption (ATI) studies can be performed safely and are crucial to determine who may no longer require ART, currently it is unknown how acceptable these studies will be in Botswana. AIM: To determine the knowledge, attitudes and acceptability of ATI-inclusive studies in Botswana. METHODS: We employed a non-randomized purposive design to recruit study participants. We conducted face-to-face

A Booklet Self-Help Intervention for People Living with HIV and Depressive Symptoms in Botswana: A Randomized Controlled Trial.

In low- and middle-income countries (LMICs), there is a scarcity of psychological treatment options for people living with HIV (PLWH) with depressive symptoms. Self-help programs for depressive symptoms, in particular, are cost-effective and scalable, and therefore a promising tool in the treatment of depressive symptoms for people in low-resourced countries. This paper presents the results of a study that examined the effectiveness of a guided self-help program in reducing depressive symptoms in PLWH in Botswana. A Randomized Controlled Trial (RCT) was conducted on a sample of PLWH who were s

The profile and outcomes of patients in a newly established acute surgical care unit in a teaching tertiary hospital in Botswana.

BACKGROUND: The scope of emergency general surgical care services varies among health institutions and countries. The burden, profile, and outcomes of patients in many low- to middle-income countries is not adequately investigated. METHODS: Medical records of patients admitted to acute surgical care (ASC) team were reviewed for a year. Demographics, diagnosis, dates of admission and discharges, comorbidities, operative procedures and operative care providers, outcomes, and factors associated with outcomes were analysed. RESULTS: During the study period, 278 ASC admissions were made. The medi

Comparison of central obesity prevalence among adults living with and without HIV in Botswana: a cross-sectional study.

OBJECTIVES: The aim was to establish the community prevalence of central obesity in Botswana and assess its association with HIV status. DESIGN: We performed a one-time central obesity assessment nested within a community-based cluster-randomised controlled HIV treatment and prevention trial (Botswana Combination Prevention Project (BCPP)) conducted in Botswana. SETTING: The BCPP enrolled consenting adults from a random sample of 20% of households in 30 rural/peri-urban communities. PARTICIPANTS: A subset of participants from 22 communities was selected for a nested central obesity study.

Pathways to care with HIV-associated cryptococcal meningitis in Botswana and Uganda: Findings from a qualitative methods study.

HIV-associated cryptococcal meningitis remains a key driver of AIDS-related mortality. Mortality is twice as high in those who present later to care and with severe symptoms such as confusion. We embedded a qualitative methods study within a randomised controlled trial in Gaborone, Botswana and Kampala, Uganda with the aim of understanding pathways to care. We conducted in-depth interviews with trial participants and surrogate decision makers and analysed data thematically. Between January 2020 and June 2021 we interviewed 58 individuals. Pathways to care were prolonged because headaches were

Prevalence and control of hypertension in a high HIV-prevalence setting, insights from a population based study in Botswana.

In a population-based representative sample of adults residing in 22 communities in Botswana, a southern African country with high HIV prevalence, 1 in 4 individuals had high blood pressure. High blood pressure was less prevalent in adults with HIV than without HIV. Sixty percent of persons with high blood pressure had not previously been diagnosed. Among individuals with a prior diagnosis of high blood pressure who reported being prescribed anti-hypertension medications, almost half had elevated blood pressure, irrespective of HIV-status. One-third of adults in this setting (mainly men) decli

Evaluating the geographic distribution of cervical cancer patients presenting to a multidisciplinary gynecologic oncology clinic in Gaborone, Botswana.

OBJECTIVE: In Botswana, cervical cancer is the leading cause of cancer death for females. With limited resources, Botswana is challenged to ensure equitable access to advanced cancer care. Botswana's capital city, Gaborone, houses the only gynecologic oncology multi-disciplinary team (MDT) and the one chemoradiation facility in the country. We aimed to identify areas where fewer women were presenting to the MDT clinic for care. METHODS: This cross-sectional study examined cervical cancer patients presenting to the MDT clinic between January 2015 and March 2020. Patients were geocoded to resid

The barriers and facilitators of HIV-exposed infant testing as perceived by HIV-positive mothers in Botswana: A qualitative study.

BACKGROUND: Despite high rates of HIV testing and enrolment of HIV-positive pregnant women on antiretroviral therapy in Botswana, coverage for HIV-exposed infant (HEI) testing remains suboptimal. Many factors can contribute to suboptimal HEI testing rates, but they have seldom been thoroughly investigated in Botswana. Therefore, the aim of this study was to explore the experiences and perspectives of HIV-positive mothers on the barriers and facilitators of HEI testing to inform interventions to promote HEI testing in Botswana. METHODS: We conducted focus group discussions (FGDs) with HIV-posi

Role of Support Reciprocity in HIV Viral Suppression Among People Living with HIV and Their Treatment Partners in Botswana.

BACKGROUND: Social support reciprocity (the extent to which people exchange mutual support) is associated with long-term health. METHOD: We examined whether overall social support and reciprocity of support between people living with HIV and their treatment partners (informal caregivers selected from patients' social networks to support adherence) are associated with HIV viral suppression. A total of 130 patients living with HIV and their treatment partners were recruited from a clinic in Gaborone, Botswana, from May 2016 to April 2017. Participants completed surveys assessing sociodemographi

Preliminary survey on Anopheles species distribution in Botswana shows the presence of Anopheles gambiae and Anopheles funestus complexes.

BACKGROUND: Botswana is one of the four front line malaria elimination countries in Southern Africa, with malaria control activities that include routine vector control. Past and recent studies have shown that Anopheles arabiensis is the only known vector of Plasmodium parasites in the country. This report presents a preliminary evaluation on Anopheles species composition in seven districts of Botswana with some inferences on their vectorial role. RESULTS: Overall, 404 Anopheles mosquito females were collected, of which 196 were larvae collected from several breeding sites, and 208 were adult
