Ultrastructural patterns of the activated cell death programs in the human brain.

The authors analyzed by transmission electron microscopy (TEM) neurosurgical samples obtained from patients with cerebral tumors, neurotrauma, cerebral ischemia, Moyamoya disease, encephalitis, etc. Their observations concern a variety of dying cell types by different programmed death pathways, including apoptosis, paraptosis, autophagy, autoschizis, programmed necrosis, as well as combined and coexisting forms. This ample work pointed out not only the role of TEM in cell death diagnosis, but the biological differences in cell behavior and beneficial or detrimental effects of suicides for home

Brain biomarkers for identifying excited delirium as a cause of sudden death.

Excited delirium (ED) syndrome is a serious medical condition associated with acute onset of agitated violent behavior that often culminates in a sudden unexplained death. While the contribution of restraint, struggle and the use of conductive energy devices (CED) to the cause and manner of death raise controversy, a CNS dysfunction of dopamine signaling may underlie the delirium and fatal autonomic dysfunction. We conducted a mortality review for a case series of ninety excited delirium deaths and present results on the association of a 2-protein biomarker signature. We conducted quantitative

A molecular approach to apoptosis in the human heart during brain death.

BACKGROUND: Brain death induces changes in tissues and organs destined for transplant at the cell, molecular, and endocrine level including cell death through apoptosis. This study was designed to examine apoptotic damage in cardiac tissue obtained from brain dead donors. METHODS: Fifty tissue specimens from the left ventricles of individual donors were processed to evaluate changes in the expression levels of five genes involved in apoptosis (BAX, BCL2, CASPASE 3, CYTOCHROME C, and FAS) using the real time-polymerase chain reaction technique. Expression levels were quantified by the relative

Brain damage and axonal injury in a Scottish cohort of neonatal deaths.

Despite the clinical and medicolegal significance attached to perinatal asphyxia, the neuropathological basis of this condition remains obscure. There are very few studies in the literature which correlate the pathological findings in neonatal brains with detailed epidemiological data, and none which are population based. In a Scotland-wide study of neonatal deaths, 70 brains have been examined. On the basis of glial and macrophage reactions, we previously identified infants with putative antepartum brain damage in this cohort and have related these reactions to signs of birth asphyxia. The pr

Unexpected death due to chloramine toxicity in a woman with a brain tumor.

It has been known for years that mixing household cleaning products can be hazardous. Nonetheless, from time to time, episodes of pneumonitis from such mixing occur. Although symptoms range from minor upper respiratory irritation to adult respiratory distress syndrome, deaths are very rare. We present the case of a woman with an undiagnosed oligodendroglioma who mixed bleach and ammonia (resulting in the formation of chloramine gas), and died while cleaning her bathroom. To our knowledge, this is the first such death reported from chloramine gas intoxication.

The influence of thyroid hormone replacement in a porcine brain death model.

This study was conducted to determine whether the administration of tri-iodothyronine (T3) to brain-dead donor pigs would improve hemodynamic instability, serum levels of thyroid hormones, or the outcome of transplantation of donor livers. Brain death was caused in young pigs (25-38 kg) by rapid inflation of an intracranially implanted balloon catheter. The animals were maintained on a ventilator and frequent measurements of acid/base balance, electrolytes, and glucose were made. At the end of 16 hr, livers were removed and implanted into prepared recipients. Serum-free tri-iodothyronine fell

Ultrastructural changes of neurons in dependence on the death cause in human brain.

Autolytic changes in animal neurons 3 hours postmortem were compared with the changes in human material taken from the patients with ischemic heart disease. Relatively homogenous neuroplasm in the animal material was observed and the nuclei and nucleoli were present as well. Light artificial spaces were visible around the neurons. Marked vacuolization was in all cases of the human material. In some cases the nucleus with the nucleolus and mitochondria were still preserved. Majority of the neurons did not reveal the nucleolus and chromatin in their nuclei; destruction of the organelles and homo

The ocular microtremor record as a potential procedure for establishing brain death.

The normal ocular microtremor possesses certain characteristics. Abnormal records are found in unconscious patients. Six patients with clinical manifestations of brain death showed complete absence of ocular microtremor when it was recorded shortly before death. A seventh patient who had taken an overdose of barbiturates had a virtually flat record which gradually improved as her coma lightened. The ocular microtremor record is proposed as a potential monitor of brain stem viability or death.

Prognostic value of soluble programmed death-1 and soluble programmed death ligand-1 in severe traumatic brain injury patients.

Patients with traumatic brain injury (TBI) frequently exhibit concomitant immunosuppression. In this study, we evaluated the predictive values of soluble programmed death-1 (sPD-1) and soluble programmed death ligand-1 (sPD-L1) in patients with severe TBI. Peripheral blood sPD-1 and sPD-L1 levels were measured within 48 h of patient admission. A total of 20 healthy volunteers and 82 patients were enrolled in this study. The levels of sPD-1 and sPD-L1 were upregulated in patients with severe TBI (P < 0.001). They were significantly increased in the post-TBI severe pneumonia group and among non-

Assessing the Brain Death/Death by Neurologic Criteria Determination Process in Korea: Insights from 10-Year Noncompleted Donation Data.

BACKGROUND: This study aimed to analyze the current status of brain death/death by neurologic criteria (BD/DNC) determination in Korea over a decade, identifying key areas for improvement in the process. METHODS: We conducted a retrospective analysis of data from the Korea Organ Donation Agency spanning 2011 to 2021, focusing on donors whose donations were not completed. The study reviewed demographics, medical settings, diagnoses, and outcomes, with particular emphasis on cases classified as nonbrain death and those resulting in death by cardiac arrest during the BD/DNC assessment. RESULTS:

Hormone replacement before brain death increases organ donation rate after catastrophic brain injury: An EAST multicenter trial.

BACKGROUND: One third of organ donors die due to catastrophic brain injury (CBI). CBI causes massive physiological derangements that can damage organs and prevent organ donation. Hormone replacement therapy (HRT) can mitigate this damage when given in the critical care setting, but no standardized protocol for use at trauma centers exists. Single-center studies show that HRT increases organ donation, but there has not been an investigation on which hormone combinations are most beneficial. METHODS: This prospective, observational EAST multicenter trial included adult CBI patients at 38 Level

Early Heart Rate Variability and Electroencephalographic Abnormalities in Acutely Brain-Injured Children Who Progress to Brain Death.

OBJECTIVES: Heart rate variability is controlled by the autonomic nervous system. After brain death, this autonomic control stops, and heart rate variability is significantly decreased. However, it is unknown if early changes in heart rate variability are predictive of progression to brain death. We hypothesized that in brain-injured children, lower heart rate variability is an early indicator of autonomic system failure, and it predicts progression to brain death. We additionally explored the association between heart rate variability and markers of brain dysfunction such as electroencephalog

Assessing Medication Confounders Before Determination of Brain Death/Death by Neurologic Criteria: A Primer, with Opioids as a Worked Example.

PURPOSE: To provide a primer on how clinicians, particularly pharmacists, can systematically assess whether a medication may confound determination of brain death/death by neurologic criteria (BD/DNC), using pharmacokinetic and pharmacodynamic principles that can be applied to any agent with the aid of tertiary references, and using opioids as a worked example. SUMMARY: All major guidelines require exclusion of central nervous system (CNS) depressant effect before BD/DNC determination, yet none provides a general method for deciding when a given medication has cleared. Rather than cataloging

Improving outcomes in donation after circulatory death vs. donation after brain death liver transplantation for acute-on-chronic liver failure.

Acute-on-chronic liver failure (ACLF) carries high short-term mortality, and liver transplantation (LT) remains the curative treatment. Donation after circulatory death (DCD) grafts expand the donor pool but carry higher risks of dysfunction compared to donation after brain death (DBD) grafts, and data directly comparing their outcomes in ACLF are limited. Using the Scientific Registry of Transplant Recipients (SRTR), we conducted a retrospective cohort study of 67,201 adult LT recipients from 2004 to 2023, stratified into three eras (Era 1: 2004-2012, Era 2: 2013-2018, Era 3: 2019-2023). Heart Allograft Utilization Rates Following Donation After Circulatory Death Compared to Donation After Brain Death in the Ideal Heart Donor.

BACKGROUND: The number of heart allografts obtained from donation after circulatory death (DCD) remains low, especially compared to donation after brain death (DBD). Our study aimed to identify factors associated with the underutilization of hearts in ideal donors. METHODS: Patient-level data were obtained from the United Network for Organ Sharing (UNOS) database for all adult deceased donors who underwent organ procurement and subsequent transplantation between January 2020 and December 2023. "Ideal" DCD donors were analyzed as a separate subset and compared with ideal DBD donors. Transplant

Effects of Cold Preservation on Intestinal Grafts from Donation after Circulatory Death and Brain Death Donors in an Experimental Model.

INTRODUCTION: Donation after circulatory death (DCD) has been successfully implemented in several solid organs, but its biological impact on intestinal grafts remains underexplored. Understanding how DCD grafts tolerate cold ischemia compared to those from brain-dead (BD) donors is crucial to refine preservation strategies and expand the donor pool. METHODS: Adult male Wistar rats were assigned to three groups according to donor condition: ventilated living donor, brain death (BD), and DCD. Jejunal segments were flushed with histidine-tryptophan-ketoglutarate solution and stored at 4°C for 0,

Infectious Complications in Living Donation Versus Donation After Brain Death Kidney Transplantation.

BACKGROUND: Kidney transplantation from living donors (LDs) offers benefits over donation after brain death (DBD). Whether LD independently protects against infection incidence remains unclear. METHODS: We compared posttransplant infection between LD (n = 102) and DBD (n = 401) groups in a prospective observational cohort of 503 KT recipients at a tertiary-care center. Crude and adjusted associations between donor type and infection risk were assessed using propensity score (PS) modeling and multivariable Cox regression. RESULTS: Cumulative incidence rates of overall and bacterial infection

Shamanic Kambô Frog Hyponatremic Toxicity Leading to Brain Death: A Case Report.

The South American shamanic Kambô ritual involves applying the skin secretions of Phyllomedusa bicolor (giant monkey frog) to superficial burns for purported spiritual and therapeutic benefits. These secretions contain a complex mix of bioactive peptides, such as phyllocaerulein, phyllomedusin, phyllokinin, sauvagine, dermorphins, and deltorphins, that interact with diverse neurotransmitter and hormone receptor systems. Specifically, these peptides engage cholecystokinin (CCK) receptors, neurokinin-1 (NK1) receptors, bradykinin B2 receptors, corticotropin-releasing factor (CRF) receptors, and
