Post-Transplant HCC Recurrence and Survival: Impact of Bridging Therapy and Tumor Biology in 185 Consecutive Liver Transplants.

Background: Hepatocellular carcinoma (HCC) is a leading indication for liver transplantation (LT), representing a curative treatment option for selected patients. A remaining clinical challenge is the recurrence of HCC after transplantation, impacting long-term graft and patient survival. The impact of different bridging therapies (BTs) such as transarterial chemoembolization (TACE), local ablation or liver resection on recurrence rates remains unclear. We assessed post-transplant HCC recurrence and survival focusing on the role of pre-transplant bridging therapies. Methods: Adult recipients u

Bridging the gap: meta-epidemiological analysis on the clinical translation of stem cell-based therapies in women's reproductive diseases.

STUDY QUESTION: What is the current landscape of randomized controlled trials (RCTs) evaluating stem cell-based therapies for women's reproductive diseases, and how effectively has preclinical research informed their clinical translation? SUMMARY ANSWER: The current clinical trial landscape for stem cell-based therapies in women's reproductive diseases is characterized by rapid growth in trial registration, particularly for premature ovarian failure (POF) and intrauterine adhesions (IUA), yet remains predominantly in early-phase, single-center, single-country studies with significant heteroge

Bridging the gap between neuroscience and clinical practice: a review of the development of Temperament Based Therapy with Support (TBT-S).

Temperament Based Therapy with Support (TBT-S) is an emerging, personalized treatment approach for eating disorders (EDs) that integrates patients' temperamental traits and neurobiological research into therapeutic strategies to reduce symptoms. While research on the efficacy of TBT-S is still in its infancy, the development of TBT-S as a treatment for EDs spans over two decades. The approach is unique in that it was formulated from neurobiological research, and utilizes treatment tools which were created collaboratively with individuals with lived experience. Hence, the aim of this review of

Non-coding RNAs in congenital heart disease and placental development: Bridging molecular mechanisms to clinical biomarkers and therapies.

Non-coding RNAs (ncRNAs) have emerged as pivotal regulators of gene expression, orchestrating embryonic development and disease pathogenesis. This review synthesizes current knowledge on the origin, biogenesis, and functional diversity of ncRNAs, with a focus on their regulatory crosstalk in congenital heart disease (CHD) and placental development. The fetal heart-placenta axis, a bidirectional signaling network essential for cardiogenesis and placental morphogenesis, is spatiotemporally modulated by ncRNAs through epigenetic and post-transcriptional mechanisms. Through precise regulation of c

Model-informed development of bacteriophage therapy: bridging in vitro and in vivo efficacy against multidrug-resistant Pseudomonas aeruginosa.

Bacteriophages are emerging as promising alternatives to antibiotics for multidrug-resistant (MDR) infections. However, their unique pharmacokinetic and pharmacodynamic (PKPD) properties arising from host-dependent amplification present challenges for dose selection and clinical translation. Here, we present a mechanistic PKPD model informed by in vitro kinetic assays and in vivo mouse studies of phage therapy targeting MDR Pseudomonas aeruginosa. The model extends the classical predator-prey model by addressing dormancy-related bacterial persistence and partitioning bacterial subpopulations b

Bridging Antiplatelet Therapy With Cangrelor in Spinal Cord Stimulator Trial and Implant for Patient With Refractory Angina: A Case Report.

BACKGROUND: Antiplatelet medications increase the risk of neuraxial bleeding during spinal cord stimulator (SCS) trials and implants, necessitating adequate discontinuation. However, interrupting antiplatelet therapy is undesirable in patients at high risk for thromboembolism. Cangrelor, a novel nonthienopyridine adenosine triphosphate analog, has a rapid onset and offset that can be used to bridge antiplatelet therapy prior to procedures involving neuraxial access, minimizing the risk of subtherapeutic anticoagulation. CASE REPORT: We present the case of a 44-year-old man with an extensive c

Efficacy and safety of radiotherapy as a bridging strategy to CD19-targeted CAR-T therapy: systematic review and meta-analysis.

Radiotherapy (RT) is increasingly used as bridging therapy before CD19-targeted CAR-T infusion in large B-cell lymphoma, but its clinical role remains uncertain. We conducted a systematic review and meta-analysis of 16 studies involving 488 patients with large B-cell non-Hodgkin lymphoma who received bridging RT. Pooled estimates showed an overall response rate (ORR) of 74.6% and complete response (CR) rate of 55%. The 1-year progression-free survival (PFS) and overall survival (OS) rates were 57.7% and 72.1%, respectively. Cytokine release syndrome (CRS) and immune effector cell-associated ne

Yttrium-90 Transarterial Radioembolization as a Bridging Therapy to Liver Transplant in a Toddler With Pretreatment Extent (PRETEXT) IV Hepatoblastoma.

Hepatoblastoma (HB) is a rare but highly malignant tumor that commonly arises in utero. The mainstay treatment is surgical resection, with adjuvant treatment options including chemotherapy, hepatic artery chemoembolization, and liver transplantation. Yttrium-90 (Y90) transarterial radioembolization (TARE Y90) is a developing treatment option that utilizes radioactive beads delivered specifically to blood vessels supplying the tumor. This treatment method has previously been utilized in the treatment of adult tumors but has rarely been utilized in the pediatric population. In this report, we di

Bridging therapy versus direct endovascular thrombectomy in basilar artery occlusion stroke: a systematic review and meta-analysis.

The benefits and safety of bridging therapy in basilar artery occlusion remain unclear, although current guidelines recommend it based on weak evidence and analogy to anterior circulation strokes. This study compares bridging intravenous thrombolysis and endovascular thrombectomy versus direct endovascular thrombectomy in a larger-than-ever basilar artery occlusion Population. Embase, PubMed, Scopus, Web of Science, and Cochrane Library were systematically searched. Studies that reported 90-day functional independence (mRS: 0-2), 90-day independent ambulation (mRS: 0-3), successful recanalizat

Infectious complications after cardiac transplantation in patients bridged with mechanical circulatory support devices versus medical therapy.

BACKGROUND: Mechanical circulatory support (MCS) is increasingly used as a bridge to heart transplantation. It is not known whether patients who receive MCS as bridge to transplantation (BTT) have more frequent and severe infectious complications in the first transplant year. METHODS: Using a retrospective cohort in a single large transplant center from 2009 to 2014, we compared rates of post-transplant infections among patients bridged to transplantation with medical therapy (n = 134) or MCS (n = 178) over the first post-transplant year. Serious infections necessitated >14 days of continuous

Acute mechanical prosthetic valve thrombosis after initiating oral anticoagulation therapy. Is bridging anticoagulation with heparin required?

Prosthetic valve thrombosis (PVT) represents a serious and potentially lethal complication. It can be attributed more frequently to inadequate anticoagulant therapy. We present a case of acute aortic mechanical valve thrombosis six months after implantation. The patient discontinued oral anticoagulation after being discharged following the primary operation. Two days after reinitiating warfarin as an outpatient, he developed acute valve thrombosis presenting with symptoms and signs of cardiac failure. He was managed with intravenous thrombolysis with a recombinant plasminogen activator which r

Severe left ventricular dysfunction secondary to primary pulmonary hypertension: bridging therapy with bosentan before lung transplantation.

When right ventricular failure develops secondary to primary pulmonary hypertension, right-left ventricular interaction may lead to severe impairment of left ventricular function. In such cases, many experts favor combined heart-lung transplantation by fear that the left ventricle may not recover after transplantation of the lungs alone. We report a case of primary pulmonary hypertension with severely diminished right and left ventricular function. The patient was rendered amenable to isolated pulmonary transplantation with the endothelin-receptor antagonist bosentan. The medication improved r

Current evidence and strategies for bridging therapy in CD19-directed chimeric antigen receptor T-cell therapy for relapsed/refractory large B-cell lymphomas.

CD19-directed chimeric antigen receptor T-cell (CAR T-cell) therapy has markedly improved the prognosis of patients with relapsed or refractory large B-cell lymphoma (R/R LBCL). However, disease progression during the manufacturing period remains a major barrier to successful treatment. Bridging therapy (BT), defined as anti-lymphoma treatment administered between leukapheresis and lymphodepleting chemotherapy, serves two primary purposes: to prevent disease progression ensuring eligibility for CAR T-cell infusion, and to modulate the immune microenvironment to potentially enhance CAR T-cell e

Predictors and responses to varying durations of BTK inhibitor bridging therapy before anti-CD19 CAR-T cell therapy in patients with relapsed/refractory DLBCL.

BACKGROUND: Anti-CD19 chimeric antigen receptor (CAR)-T cell therapy has demonstrated clinical potential in treating relapsed or refractory (R/R) diffuse large B-cell lymphoma (DLBCL); however, enhancing its therapeutic efficacy remains a significant challenge. To this end, bridging therapy with Brutonyg tyrosine kinase inhibitors (BTKi), such as ibrutinib or zanubrutinib, is being investigated as a strategy to improve treatment outcomes. PATIENTS AND METHODS: In this retrospective analysis, we assessed the impact of different durations of BTKi bridging therapy prior to anti-CD19 CAR-T cell i

Adjuvant Scrotal Radiation Therapy As Bridging Therapy to Chimeric Antigen Receptor T-Cell Following Extramedullary Relapse in B-Cell Acute Lymphoblastic Leukemia.

Chimeric antigen T-cell (CAR T) therapy is a promising emerging treatment option for patients with relapsed/refractory acute lymphoma. The role of bridging radiotherapy prior to CAR T infusion is an area of increasing interest with a sizable body of literature regarding its use in non-Hodgkin lymphoma, but reports of its use in leukemia are limited. Furthermore, available literature on bridging radiotherapy is limited to the treatment of bulky, often symptomatic disease, as opposed to its role in treating high-risk regions and sanctuary sites. Here, we present an adult male with multiply relap

Temporary mechanical circulatory support as a bridge to durable left ventricular assist device as destination therapy in fulminant giant cell myocarditis:A case report.

Fulminant giant cell myocarditis is a fatal form of acute myocarditis leading to a rapid-onset clinical presentation with lethal arrhythmias, acute heart failure, or cardiogenic shock requiring mechanical circulatory support. We report the case of a 52-year-old female diagnosed with fulminant myocarditis requiring veno-arterial extracorporeal membrane oxygenation (V-A ECMO) and intra-aortic balloon pump(IABP) support. Due to hemodynamic instability, she was transferred to our hospital by helicopter on day 4. On arrival at our hospital, she underwent percutaneous balloon atrial septostomy to de
