A Predictive Model for Cochlear Implant Outcome in Children with Cochlear Nerve Deficiency.

The outcome of cochlear implantation (CI) in patients with cochlear nerve deficiency (CND) is variable, resulting in a wide range of speech perception performance, from degrees of environmental sound perception to conversation without lip-reading. Twenty-five cochlear implantees with CND were enrolled retrospectively to determine the factors correlated with CI outcome in patients with CND and to develop a predictive model for CI outcome. CI outcome was evaluated using the Categories of Auditory Performance (CAP) score at 2 years after CI. Patients with negative auditory brainstem response (ABR

Facial nerve stimulation produced by cochlear implants in patients with cochlear otosclerosis.

Thirty-eight adult cochlear implants have been performed at the University of Minnesota. Facial nerve stimulation by the implant in response to sound has been noted in four of these cases. Three of the four were patients whose sensorineural hearing loss was caused by cochlear otosclerosis. In each case it was possible to place the electrodes, however multiple leads had to be deprogrammed in order to avoid facial nerve stimulation. In each case characteristic radiographic findings of cochlear otosclerosis could be identified on preoperative temporal bone computed tomography scans. Although faci

Cochlear Histopathology as Observed in Two Patients With a Cochlear Implant Electrode With Positioner.

HYPOTHESIS: This study reports the cochlear histopathology of two patients who during life underwent cochlear implantation with a positioner. BACKGROUND: A silastic positioner introduced by the Advanced Bionics Corporation in 1999 was designed to position the electrode of the cochlear implant close to the modiolus. The positioner was recalled in the United States in July 2002 because of an apparent higher incidence of bacterial meningitis in patients in whom the positioner had been placed. METHODS: Four celloidin-embedded temporal bones from two patients with cochlear implants with a positio

Cochlear otosclerosis: does bone formation affect cochlear implant surgery?

This study aimed to demonstrate that new bone formation in the scala tympani of patients deaf from otosclerosis does not preclude cochlear implant surgery. In seven temporal bones from patients with otosclerosis, we measured the extent of new bone from the round window to the distal part of the new growth. We compared results to surgical data on the extent of drilling and depth and ease of placement of the electrode in 20 patients deaf from otosclerosis. We also examined clinical performance and voltage requirements for long-term implant use in patients with and patients without ossification o

Cochlear implants and deep brain stimulators.

OBJECTIVE AND IMPORTANCE: With an aging population and increasing utilization of both cochlear implants (CI) to treat hearing loss and deep brain stimulation (DBS) to treat neurodegenerative movement disorders, more patients will be sequentially implanted with the two devices. As such, understanding both presurgical evaluations and surgical procedures will be of great importance to the treating cochlear implant surgeon and neurosurgeon in optimizing outcomes for both conditions. CLINICAL PRESENTATION: Two illustrative cases of sequential CI followed by DBS are reviewed. Relevant pre-, intra-,

Cochlear Implantation in a Patient with Granulomatosis with Polyangiitis.

BACKGROUND: Granulomatosis with polyangiitis (GPA) otologic manifestations include conductive and sensorineural hearing loss (HL). Vasculitis is assumed to be the primary cause of otologic manifestations. Deaf patients and patients with HL who do not benefit from hearing aids can benefit from cochlear implants (CI). There are currently no specific guidelines for treatment of patients with GPA suited for CI. OBJECTIVES: To assess whether patients who are deaf due to GPA are good candidates for CI and if prior surgical or medical treatment of the inflammation are needed. METHODS: A case report

Effects of Tinnitus on Cochlear Implant Programming.

Clinical observations suggest that tinnitus may interfere with programming cochlear implants (CIs), the process of optimizing the transmission of acoustic information to support speech perception with a CI. Despite tinnitus being highly prevalent among CI users, its effects and impact on CI programming are obscure. This study characterized the nature, time-course, and impact of tinnitus effects encountered by audiologists and patients during programming appointments. Semistructured interviews with six CI audiologists were analyzed thematically to identify tinnitus effects on programming and re

Acute mastoiditis in children with a cochlear implant.

OBJECTIVES: Cochlear implantation is performed at a young age, when children are prone to acute otitis media. Acute mastoiditis is the most common complication of otitis media, but data on its management in the presence of a cochlear implant are sparse. The objective of this study was to assess the characteristics, treatment, and outcome of acute mastoiditis in children with a cochlear implant. METHODS: The medical files of all children who underwent cochlear implantation at a pediatric tertiary medical center in 2000-2014 were retrospectively reviewed. Those diagnosed with acute mastoiditis
