Assessment and treatment of Spanish-speaking sex offenders: special considerations.

While there is an expanding body of literature pertaining to the special considerations related to the treatment of Hispanic patients whose primary language is Spanish, there is virtually no information reported to aid in the assessment and treatment of Hispanic and primarily Spanish-speaking sex offenders. Beyond the obvious problems posed by differences in language, there are a host of transcultural factors that may impact on the evaluation and engagement of this patient population when they are also largely psychologically unsophisticated and from a low socioeconomic level. The experiences

Are sex offenders treatable? A research overview.

OBJECTIVE: Recent legislation in several states providing for civil commitment and preventive detention of sexually violent persons has stirred legal, clinical, and public policy controversies. The mandate for psychiatric evaluation and treatment has an impact on public mental health systems, requiring clinicians and public administrators to direct attention to treatment options. It is a common view that no treatments work for disorders involving sexual aggression. The authors examine this assumption by reviewing research on the effectiveness of treatment for adult male sex offenders. METHODS

Domestic Mass Shooters: The Association With Unmedicated and Untreated Psychiatric Illness.

BACKGROUND: Given the relative lack of psychiatric information and data on the perpetrators of US mass shootings, the aim of our study was to understand who these "mass shooters" were and whether they had a psychiatric illness. If so, were they competently diagnosed, and if so, were they treated with appropriate medication for their diagnoses before the violence? METHODS: Because a prospective study of diagnosis and treatment could not, for obvious reasons, be carried out, we designed a retrospective, observational study of mass shooters, defined as those who killed 4 or more people with fire

Frequency and predictors of adverse events. PRiSM Psychosis Study. 3.

BACKGROUND: Community care has been criticised as a hazardous policy associated with high rates of adverse events. There is little research evidence as to the truth of this claim. METHOD: Best available evidence from public records, interviews, case notes, key workers and general practitioners was assembled to establish: (a) which of the 514 subjects initially identified as having psychotic illnesses had died during an average follow-up of 4.9 years; (b) care currently received by all 286 subjects originally selected for interview; and (c) rates of major adverse events and of admission for th

Contribution of a Cluster Approach to Identify the Profiles of Men Sentenced for Sexual Violence According to Their Risk of Reoffending.

INTRODUCTION: The aim of this study is to propose a typology of recidivism risk profiles based on the criminogenic needs of a population of men sentenced for sexual violence. Their socio-demographic, criminological, psychological, and psychiatric factors and vulnerabilities are compared. This classification will respond to the need for a better identification of the factors involved in the risk of recidivism of men sentenced for sexual violence, in order to develop more effective management. METHOD: Several psychological and psychiatric scales (personality traits, impulsivity, cognitive disto

Sex and sensibility in cultural history: the English governess and the lunatic asylum, 1845-1914.

This chapter is concerned with the experience of the Victorian governess and other female teachers in the lunatic asylum during the Victorian period. There is now a formidable and complex literature on the governess but little discussion of her experience behind the walls of the asylum. The chapter offers two kinds of comparison: the distinctive pattern of care of governesses in three different institutions in Victorian Devon; and secondly, the progress of those identified as 'governess' with other groups of female teachers in the same period. A number of interesting contrasts emerge. The evid

Sociocultural identification of migrant forensic psychiatric inpatients facing mandatory repatriation - a hospital ethnography.

In the Netherlands, terbeschikkingstelling (TBS) is a forensic psychiatric measure imposed on individuals who commit a criminal offence under diminished responsibility due to severe mental illness. For TBS patients without Dutch nationality, mandatory repatriation replaces resocialization as the final goal of TBS treatment, as they lose Dutch residency rights due to their offence. This article examines how this subgroup of TBS patients at a Dutch Centre for Transcultural Forensic Psychiatry reflect on and negotiate cultural identity at the intersection of criminal and migration law. Focusing o

[Socio-demographic and clinical characteristics of adolescents hospitalized in French units for inmates: Results of a cross-sectional study].

BACKGROUND: In France, on the first of January 2018, 772 underage persons were in jail or about 1.1 % of the incarcerated population. The prevalence of psychiatric disorders among those inmates is high. As a result psychiatric care (and particularly full-time hospitalization) is essential for this population. The unités hospitalières spécialement aménagées (UHSA) are full-time inpatient hospitalization units for inmates in France. Adults but also underage inmates can be admitted to UHSA for voluntary or involuntary hospitalization. However, no study about the characteristics of young patients

Intoxication and settled insanity: a finding of not guilty by reason of insanity.

This article presents a case of first-degree murder for which the defendant was acquitted as not guilty by reason of insanity, based on a defense involving the concept of "settled insanity." The literature on settled insanity is reviewed and discussed in the context of voluntary and involuntary intoxication. Statute and case law from those jurisdictions in which settled insanity is specifically allowed as an acceptable threshold condition for the insanity defense define the concept as a permanent condition resulting from substance abuse, rather than the effects of intoxication, no matter how s

The insanity defense for sex offenders: jury decisions after repeal of Wisconsin's Sex Crimes Law.

After repeal of a Wisconsin statute permitting hospitalization of defendants convicted of sexual crimes, the authors noted an increase in the percentage of sex offenders among persons hospitalized after being found not guilty by reason of insanity. They also found that a greater proportion of hospitalized sex offenders than of other kinds of offenders were diagnosed as nonpsychotic. Illustrating with three case studies, the authors argue that despite trends away from the therapeutic model of corrections, juries continue to make decisions that provide treatment for defendants perceived to need

Sex offending among adolescents and young men with history of psychiatric inpatient care in adolescence.

BACKGROUND: Some mental disorders have been associated with increased likelihood of sexual offending in adolescents (and adults), but relevant studies tend to be of established sex offenders. AIMS: To examine relationships between adolescent mental disorders and subsequent involvement in sex offending and to explore any predictive value of primary diagnoses for subsequent interpersonal offending, whether sexual or violent. METHODS: We analyse national register-based longitudinal data on males in Finland admitted for their first psychiatric inpatient treatment between the ages of 13-17 in the

Personality traits as predictors of inpatient aggression in a high-security forensic psychiatric setting: prospective evaluation of the PCL-R and IPDE dimension ratings.

The Dangerous and Severe Personality Disorder (DSPD) initiative in England and Wales provides specialized care to high-risk offenders with mental disorders. This study investigated the predictive utility of personality traits, assessed using the Psychopathy Checklist-Revised (PCL-R) and the International Personality Disorder Examination, with 44 consecutive admissions to the DSPD unit at a high-security forensic psychiatric hospital. Incidents of interpersonal physical aggression (IPA) were observed for 39% of the sample over an average 1.5-year period following admission. Histrionic personali

[Treating patients on the court-ordered therapy programme].

Psychologists and a psychiatrist testify to the clinical condition of people sentenced to socio-judicial supervision with court-ordered therapy. Despite the extension of the indication of the programme, this complex supervision often concerns the perpetrators of intra and/or extra familial sexual abuse. Preventing reoffending through the global treatment of the patient is a major concern both for the doctor-coordinators as well as the doctor and psychologists delivering the therapy.

[Acceptance and commitment therapy].

INTRODUCTION: Acceptance and commitment therapy (ACT) is a third generation of cognitive-behavioral therapies. The point is to help patients to improve their psychological flexibility in order to accept unavoidable private events. Thus, they have the opportunity to invest energy in committed actions rather than struggle against their psychological events. OBJECTIVES OF THE STUDY: (i) To present the ACT basic concepts and (ii) to propose a systematic review of the literature about effectiveness of this kind of psychotherapy. METHOD: (i) The core concepts of ACT come from Monestès (2011), Scho

[Psychosocial characteristics of victims of suicide in prisons].

BACKGROUND: Besides some cases reported by the media in recent years about prison inmates who committed suicide, hardly anything is known concerning social, criminological and psychiatric characteristics of suicide victims in jails and prisons in Austria. It therefore seems necessary to evaluate jail and prison suicides in order to establish effective means of prevention. METHODS: A study was conducted evaluating all suicides in Austrian prisons between 1975 and 1999. In addition to nationality, education and working status, other indicators of social integration and psychiatric history of su

[In-patients suicide: epidemiology and prevention].

BACKGROUND: Few studies have been devoted to in-patients' suicides. This covers all suicides that occurred during hospitalisation, whatever the place (inside or outside the institution) and often, for psychiatric in-patients, suicides carried out within 24 hours after leaving the institution. LITERATURE FINDINGS: However, the incidence of suicide in hospital is high, higher than that observed in the general population. It is 250 per 100,000 admissions in psychiatric hospitals and 1.8 per 100,000 admissions in general hospitals, which is four to five times more than in general population. Five

Offending and clinical characteristics of adults with autism spectrum disorder: Experience at forensic psychiatry center in Türkiye between 2012 and 2022.

The forensic and clinical need for better understanding of criminal offending in adults with ASD is increasingly recognized. To date, few studies have examined the differences and similarities between criminal offenders with and without ASD with respect to demographics, offending profiles, and clinical characteristics. This study, conducted in Turkey, is the first to conduct such as comparison using a national database of forensic files. Computerized search of the forensic records of 11,853 adults assessed between January 1, 2012, and January 1, 2022, for criminal responsibility by the Turkish

Personality, smoking and suicide: a prospective study.

From a random sample of Swiss army conscripts, 28 cases of suicide, 36 deaths due to accident and 2754 controls were analyzed for social and psychological features, in particular for personality factors at age 19 years; further information was collected during a follow-up study to age 36 years. Among the subjects who committed suicide low school achievement, smoking, and psychiatric disorders were overrepresented. In both groups of deaths aggressiveness (excitability, reactive aggression, spontaneous aggression) and depression scores were higher than among controls. The findings are compatible

Patients released after psychiatric commitment evaluation: comparison with the committed.

The authors retrospectively analyzed two groups of patients who were evaluated for involuntary commitment. Based on the psychiatrist's evaluation, one group (N = 39) was committed and the other group (N = 39) was released. The committed patients were significantly younger and diagnosed psychotic. No significant differences between groups were found in sex, race, petitioner, treatment history and presenting behavior. Psychiatrists regardless of experience tended not to commit the non-psychotic patient, with the released patients constituting a unique subgroup of the outpatient population.
